BACKGROUND:As the complexity of human anatomic structure, the abnormity of tissue shape and the difference among individuals, the structure of multifractal is adapted.OBJECTIVE:To investigate medical image segmentation based on multifractal.METHODS:Image segmentation was performed by algorithm based on capacity measurement and probability measure. The experimental images were segmented using traditional region growing, max capacity measurement, sum capacity measurement, and probability measure.Following adding noise, the images were identically segmented and compared.RESULTS AND CONCLUSION:In the two algorithms based on multifractal, the key of the algorithm based on capacity measurement is that appropriate measure μα is defined, and the key of the algorithm based on probability measure is that appropriate normalized probability Pi is defined. The different measures (probability) and thresholds bring greater effect. The method based on probability measure is sensitive to noises, but after filtration noise, segmentation effect is greater for the images whose pixels vary comparatively great and very complicated. The results show that it is feasible that appropriate measure (probability) and threshold is chosen based on medical image segmentation.Especially greater advantage exists for the distinction of texture and edge in the complicated image processing, which can reserve details while precisely dividing. It has very significant practical significance. At the same time, multifractal can also be characteristics of images, which provide powerful data for feature extraction.

Objective To investigate the effect of vacuum sealing drainage (VSD) technology on prevention and treatment of infection wound and to repair the infectious fracture wound in earthquake. Methods Twenty-two patients with limb fractures and open infection wound received VSD from May 12, 2008 to June 19, 2008 in West China Hospital of Sichuan University. Before the VSD, we debrided all wounds and gave effective systemic antibiotics. A -18 ~ - 14 kPa pressure was exerted to the wound, and the VSD was used for 8 ~ 10 days. We took a germiculture regularly. The capacity, color, and nature of negative pressure drainage, the regression of limb swelling, and systemic inflammatory responses were observed. Results There was no active bleeding wound or transparent film off in all patients. Three patients had drainage clogging, and were kept flowing freely using the sterile saline pipe to remove the blockage of necrotic tissues. During the VSD, granulation tissues grew well in the 13 patients with bone exposure of the wounded. Two patients whose symptom of inflammatory was not obviousy eased had another debridement to completely remove the necrosis, and the symptom was relieved. After 3-5 days of VSD, swelling and fever in the other 20 patients significantly subsided. Conclusion VSD can alleviate the wound inflammation, facilitate the growth of the fresh granulation tissue from the surrounding to the center, and reduce the flap transfer area for the Stage Ⅱ coverage of the exposed bone.

This study was aimed to make a systematic and comprehensive evaluation on clinical efficacy and safety of vertigo treatment with Tian-Ma Gou-Teng (TMGT) Decoction. Articles had been searched in the Chinese Journal Full-text Database, Chongqing VIP Chinese Scientific Journals Database, China National Knowledge Infrastructure (CNKI), Chinese Biomedical Literature Database Online, Wanfang Data and Chinese Medicine Journal Literature Database for randomized controlled trials (RCT) on vertigo treatment with TMGT Decoction. The data retrieval time was from the establishment time of the database until present. Two investigators screened literatures, extracted data and assessed quality according to inclusion and exclusion criteria independently. RevMan5.0.2 was used in the meta-analysis. The results showed that a total of 33 trials were involved, which covered 2922 vertigo cases. The meta-analysis showed that from the aspect of clinical effectiveness, the comparison between Chinese medicine treatment group and modern medicine treatment group (OR = 3.67, 95% CI [2.66~5.07]), comparison between integrative medicine treatment group and modern medicine treatment group (OR = 3.28, 95% CI [2.33~4.62]), the comparison between Chinese medicine combined with other method (such as acupuncture, Tuina) treatment group and other Chinese medicine treatment group (OR = 2.29, 95% CI [1.34~3.91]), therapeutic effect in the experimental group was better than the control group. For the aspect of relapse rate, TMGT Decoction in the treatment of vertigo was better than the control group (OR = 0.27, 95% CI [0.12~0.65]). In the aspect of adverse reactions, TMGT Decoction in the treatment of vertigo was better than the control group (OR = 0.40, 95% CI [0.13~1.24]). For aspects of other indicators such as quality of life, no further analysis was conducted since there was no complete reported literature. It was concluded that the system evaluation results showed that compared with other methods, TMGT Decoction has better clinical efficacy and low recurrence rate, but safety needs to be further eualuated. However, due to the quality of included studies was not high, current evidences cannot be determined to be fully applicable in the clinical practice. But its efficacy advantages are worthy of further research. It especially requires high quality multi-center, large sample, double-blind, RCTs to verify.
