1.The Detection of Ureaplasma urealyticum in Cervix of Gynecologic Check-up Population and Female Sex Workers and Its Biovars and Serotypes

Objective To investigate Ureaplasma urealyticum in cervical specimens of gynecologic check-up population and female sex workers(FSW)and its biovars and serotypes.Methods The gynecologic check-up patients and female sex workers were screened using Uu liquid selective medium.The positive cases were then divided into biovars and serotypes by the means of PCR.Results Ninety-eight FSW and164gyne-cologic check-up patients were investigated.In the FSW group,89cases(90.8%)were positive in culture,in-cluding biovar164cases(71.9%),biovar1+27cases(7.9%),biovar218cases(20.2%).In71biovar1positive cases,biovar1+2and co-infection of serotypes of biovar1were defined as mixed infection.Infection with single serotype of biovar1was defined as simple infection.So in71biovar1positive cases of FSW group,there were17mixed infections(23.9%)and54simple infections(76.1%).Simple infections were then subdivided into serotypes,including serotype117cases(31.5%),serotype322cases(40.7%),and serotype615cases(27.8%).In gynecologic check-up group,104cases(63.4%)were culture positive,in-cluding biovar197cases(93.3%),biovar27cases(6.7%).In97biovar1positive cases,there were11mixed infections(11.3%)and86simple infections(88.7%).In86simple infections,there were serotype122cases(25.6%),serotype337cases(43.0%),and serotype627cases(31.4%).Conclusions The posi-tive rate of Uu in FSW is higher than that of gynecologic check-up population(P
2.Efficacy of mirabilite and rheum officinale combined with pulse high-volume hemofiltration for treatment of patients with severe acute pancreatitis

Objective To observe the clinical efficacy of mirabilite and rheum officinale combined with pulse high-volume hemofiltration(PHVHF)in treatment of patients with severe acute pancreatitis(SAP). Methods A total of 11 patients with SAP in Department of Critical Care Medicine of Qiannan Traditional Chinese Medicine Hospital of Guizhou Province received the combined treatment of external application of mirabilite,gavage of rheum officinale decoction and PHVHF. The changes in abdominal symptoms,vital signs,blood gas analysis,blood biochemical indicators,and the acute physiology and chronic health evaluation(APACHEⅡ)score,Ranson score and prognosis were observed and compared before and after treatment. Results After treatment,the patients' fever〔body temperature(℃):37.31±0.13 vs. 39.12±0.12〕,tachycardia〔heart rate(beats/min):106±17 vs. 123±22〕, respiratory distress〔respiratory frequency(times/min):23±6 vs. 31±5〕and other symptoms were ameliorated in different degrees,and the APACHE Ⅱ score(9.1±2.2 vs. 21.2±8.2),Ranson score(3.2±1.1 vs. 5.8±1.3)were decreased significantly ,all the above indexes compared before and after treatment being of statistical significant differences(all P<0.05). The indexes of hemodynamics of 10 survival patients were stable,heart rate(beats/min:106±17 vs. 123±22)was decreased,and mean arterial pressure〔MAP,mmHg(1 mmHg=0.133 kPa):73±6 vs. 41±5〕was increased gradually(both P<0.05). After treatment,serum amylase〔AMY(U/L):367.3±102.3 vs. 923.5±351.7〕,alanine aminotransferase〔ALT(U/L):63.3±23.2 vs. 201.5±123.2〕,total bilirubin〔TBil (μmol/L):22.1±20.1 vs. 56.1±63.2〕,serum creatinine〔SCr(μmol/L):132.1±23.5 vs. 392.4±12.2〕,urea nitrogen〔BUN(mmol/L):9.5±4.9 vs. 19.2±5.9〕, K+(mmol/L:4.2±0.2 vs. 5.6±2.1) were significantly decreased,arterial partial pressure of carbon dioxide〔(PaCO2,mmHg):35.1±7.1 vs. 27.2±5.5〕,arterial partial pressure of oxygen〔PaO2(mmHg):93.2±13.2 vs. 49.1±7.2〕and oxygenation index(mmHg:187.1±28.5 vs. 148.2±32.7),Na+(mmol/L:132.1±19.1 vs. 127.1±42.1)were significantly increased compared with those before treatment, there were statistically significant differences(P<0.05 or P<0.01). Conclusion The combined treatment of mirabilite,rheum officinale and PHVHF has significant effects on the treatment of patients with SAP,and it can be one of the assistant therapies of SAP.
3.Analysis of Antimicrobial Therapy Participated by Clinical Pharmacists for a Patient with MRSA Infection

Objective: To evaluate the clinical value of clinical pharmacists in clinical treatment team through participating in the treatment of one case of MRSA infection.Methods: According to the infection site, MRSA infection treatment principle and the characteristics of drug treatment, clinical pharmacists assisted physicians in optimizing the therapy plan and provided the pharmaceutical care.Results: Physicians adopted the clinical pharmacist's suggestions, and the symptoms of patient were improved with effectively reduced ADR.Conclusion: The participation of clinical pharmacists in the optimization of anti-infective therapy plan can improve efficacy and security.
4.Arthroscopic reconstruction of anterior cruciate ligament with hamstring tendon graft using bioabsorbatble rigidfix cross pins

Objective To describe the surgical technique in reconstructing anterior cruciate ligament (ACL) with six strands harmstring tendon graft fixed by bioabsurbable rigidfix cross pins under arthroseopy and to e-valuate its efficacy. Methods From March 2005 to June 2008,39 patients with ACL injury were treated with ACL reconstruction by transplantation of six strands autogenous harmstring tendon , fixed by bioabsorbable rigidfix cross pins in femoral side. There are 22 male and 17 female,ages from 22 to 55 (the average age is 37). 19 cases were hurt in traffic accident, and another 20 cases in accidental injury. The state of illness is 7 days to 38 months. 13 cases merge the meniscus rupture, and 4 cases of meniscus suture,8 partial meniscectomy, 1 meniscectomy were performed simultaneously ;4 cases associating with the medial collateral ligaments Ⅲ degree injure underwent medial collateral ligament neo-plasty or reconstruction ;no cases merge posterior cruciate ligament injury, the patients were followed up 12 to 51 months , Pre-and post-operative knee joint function and stability were evaluated according to the Lysholm scoring scale system and the results of KT-2000 arthrometer , the clinical results and the reliability of the fixation were analyzed. Results 32 patients were followed-up and there is no limitation of the extention in the knee joints. The flexation of the knee joint is greater than 120°,and the anterior drawer test in 90° of flexation were negative in all patients. The postoperative Lachman test was strong positive in 1 case, negative in 26 cases and positive in 5 cases. The Lysholm scores was (92.6±4.2) points. The results of KT-2000 arthrometer: 31 cases 0-4.5 nun, average 3.2 mm;1 case 6. 5 mm. Conclusions It is a safe and reliable method to reconstruct ACL with six strands harmstring tendon graft fixing by bioabsorbable rigidfix cross pins under arthroscopy, and this procedure can obtain primary stabilization and long term stabilization of the autografts.
5.Treatment strategies of pancreatic neuroendocrine neoplasms with liver metastases

Liver metastases is the most common distant metastases of pancreatic neuroendocrine neoplasms, and it is also one of the main factors affecting the prognosis of patients. With the development of treatment technology and the rise of a multidisciplinary treatment model, the treatment plan for patients with liver metastases has gradually evolved into a comprehensive treatment plan including surgery, intervention, medicine, and radiation therapy. Radical surgery is still the only treatment currently available to cure patients with pancreatic neuroendocrine tumors and liver metastases. Very few selected patients can also benefit from liver transplantation. For most patients who are unable to undergo surgery can choose intervention, medicine and radiation therapy which can improve the patient′s prognosis to a certain extent. Targeted therapy and immunotherapy have good development prospects, and further research is still needed. With multidisciplinary treatment, individualized treatment can help patients obtain the best treatment results.

7.The value of Gd-EOB-DTPA-enhanced MRI in patients with major resection of liver cancer

Objective:To investigate the value of Gd-EOB-DTPA-enhanced MRI in evaluating liver function and predicting the risk of post-hepatoectomy liver failure in patients with major resection of liver cancer.Methods:A total number of 212 patients were included from June 2016 to June 2019 at Department of General Surgery, Peking University Third Hospital with a retrospectively collected data.All patients underwent Gd-EOB-DTPA-enhanced MRI for diagnosis and preoperative evaluation of liver function.There were 135 males and 77 females, with age of (63.1±10.3) years old (range: 18 to 86 years old) . Relative enhancement ratio (RER) of the region of interest on Gd-EOB-DTPA-enhanced MRI was acquired by two independent researcher and then conducted the comparison of RER among the patients with or without post-hepatoectomy liver failure (PHLF) .Preoperative evaluation demonstrated that 141 cases infected by hepatitis virus, 128 cases with hepatitis B alone and 11 cases with hepatitis C alone, 2 cases had both of hepatitis B and C, and all patients were grade A judged by Child-Pugh score. The relationship between RER and PHLF was evaluated by Pearson correlation analysis and the diagnostic value of RER in predicting PHLF was test by receiver operating characteristic curve.Results:PHLF occurred in 42 patients according to ISGLS standard. Among them, 31 cases had level A liver failure, 9 cases had level B liver failure and 2 had level C failure. There was a significant correlation between RER and overall level of PHLF and RER was also significantly associated with severe B to C level of PHLF ( P<0.05) .The further receiver operating characteristics curve analysis showed that the diagnostic accuracy of RER on overall PHLF was 0.818 (sensitivity 72.9%, specificity 83.3%, cut-off value 73.5%, 95 %CI: 0.75 to 0.887) and on severe PHLF was 0.924 (sensitivity 97.0%, specificity 90.9%, cut-off value: 61.5%, 95 %CI: 0.79 to 0.90) . Conclusion:For patients who planned to undergo major resection of liver cancer, preoperative Gd-EOB-DTPA-enhanced MRI can help with the assessment of liver function and predicting the risk for post-hepatectomy liver failure.

The incidence of pancreatic ductal adenocarcinoma(PDAC) is increasing year by year among digestive system tumors.It has a high degree of malignancy and a poor prognosis.Its five-year survival rate is less than 8%.Traditional radiotherapy and chemotherapy are not sensitive to pancreatic cancer,so finding a new drug is the future hope for pancreatic cancer.In recent years,molecular targeted therapy has made obvious progress in diseases such as hematological malignancies,non-small cell lung cancer and melanoma,and a large number of targets have been tried in pancreatic cancer, for example, gene mutation targets, important signaling pathway targets, receptor targets, etc. The use of these targeted drugs alone or in combination has shown good results in preclinical models and some clinical trials, and some treatment have been standardized for patients with pancreatic cancer, while other options that have not yet been tested in large-scale clinical trials also offer a variety of potential possibilities for future targeted treatments for pancreatic cancer.

10.Dose response relationship between elevated blood pressure and body mass index in primary and secondary school students in Shibei District, Qingdao

Objective:
To investigate the doseresponse relationship between the risk of elevated blood pressure and body mass index (BMI) in primary and secondary school students in Shibei District, Qingdao, so as to provide a reference for precise interventions of elevated blood pressure.
Methods:
Statistical analysis was conducted on the health examination data of 92 091 primary and secondary school students in Shibei District, Qingdao, in 2022. Overweight and obesity were assessed using the standards from the Screening for Overweight and Obesity among Schoolaged Children and Adolescents, and blood pressure levels were evaluated using the Reference of Screening for Elevated Blood Pressure among Children and Adolescents Aged 7-18 Years. The relationship between BMI and elevated blood pressure was examined using analysis of variance, Chisquare test, multifactorial Logistic regression, and a combination of restricted cubic spline after data cleaning.
Results:
Based on the standardized scores under different age and gender, BMI was classified into 5 categories. Compared with the group of BMIZ scores 0-<1, the risk of developing high blood pressure gradually decreased with BMIZ scores (OR=0.55, 0.53, P<0.05). Conversely, the risk of developing high blood pressure increased with increasing BMIZ scores (OR=1.90, 3.71, P<0.05). Stratified analyses showed that BMI was positively associated with elevated blood pressure by gender (male, female), age (aged 7-8, 9-11, 12-14, 15-16), and waisttohip ratio (≤0.83, >0.83) (OR=1.18, 1.19, 1.15, 1.22, 1.19, 1.18, 1.19, 1.18, P<0.01). There were multiplicative interactions between BMI and gender, between BMI and age, between BMI and waisttohip ratio (OR=1.53, 1.08, 2.31, P<0.01). Restricted cubic spline analysis showed that as BMI levels increased, the risk of developing elevated blood pressure showed a nonlinear increasing trend in both the 7yearold and the 10 to 16yearold (χ2=27.56, 10.69, 6.10, 27.26, 18.32, 25.71, 10.53, 6.14, P<0.05).
Conclusions
The risk of elevated blood pressure in primary and secondary school students increases with BMI, showing a nonlinear doseresponse relationship. The blood pressure should be monitored regularly, and comprehensive and effective measures should be implemented to control elevated blood pressure in children and adolescents.
