Acute severe manifestations of inherited metabolic diseases include disorders of the intoxication,disorders with disturbed energy metabolism and disorders of neurotransmission.Therefore,early diagnosis and treatment must be initiated in order to decrease risk of mental injury and damages or acute death.Emergency treatment need speedy diagnosis of inherited metabolic diseases and diagnostic emergency first line laboratory evaluation should cover all differential diagnosies.All of the first line laboratory results are indispensable for planning and conducting the first steps of metabolic emergency treatment and should be available within 30 min.According to the clinical situation and biochemical derangement,secondary special metabolic investigations must be initiated in parallel.The results of all laboratory investigations relevant to the diagnosis of metabolic disorders for which specific emergency therapy exists should be available within 24 h.
3.Effect of acupuncture combined with function training on patients with acute stroke

ObjectiveTo study the effect of acupuncture combined with function training on patients with acute stroke.Methods87 patients with acute stroke were divided into 2 growps randomly:the rehabilitation group(44 cases),in which the patients accepted acupuncture and rehabilitation training,and the control group,in which the patients accepted the normal medical therapy only.All the patients were evaluated with Clinical Nervous Functional Deficiency(CNFD),Fugl-Meyer Assessment(FMA)and Modified Barthel Index(MBI).ResultsThe scales of CNFD,FMA and MBI of the patients in the rehabilitation group was increased significantly than that of the control group.ConclusionAcupuncture compared with the rehabilitation training is effective on the recovery of patients with stroke.
4.Standard and evaluation of mental health in undergraduates

OBJECTIVE: Recent studies of mental health status in undergraduates are reviewed by literatures in mental health standard, measurement and influential factors. It is aimed to analyze the current status of mental health in undergraduates and provide references for related research of mental health, psychological counseling and psychological health in undergraduates. DATA SOURCES: The China Journal Full-text Database (CJFD) and Wanfang database were undertaken to id entify related articles published from January 1995 to August 2005 with the key words of "psyche of undergraduate, mental health, health standard" in Chinese. References of retrieved articles were searched. Mental health related books were retrieved such as Health Psychology, Psychic Health Measurement and so on.STUDY SELECTION: The data were selected primarily, and related articles of mental health standard, measurement and influential factors were included. Studies unrelated to undergraduates were screened out, and fulltexts of the other literatures were looked up.DATA EXTRACTION: A total of 40 articles of mental health standard,measurement and influential factors in undergraduates were selected, of which 17 articles accorded with topic and content were collected. There were 4 articles of mental health standard, 6 literatures (listing 6 types and 20 kinds of questionnaires and tests) of measurement and 7 literatures (6articles) of influential factors.DATA SYNTHESIS: Main content of mental health was intellective health, emotional health and spiritual health. ①No mental health standard was authorized and generally accepted in internal and external studies at present. The four signs of mental health designed by World Psychiatric Association (WPA) were praised and identified, which were harmony of body,intelligence and emotion; adaptation of the environment and modesty in each other; well-being; bringing ones ability into full play and living effective living. Mental health standard of Chinese undergraduates was composed of normal cognition, good emotion, perfect personality, coordinated interpersonal relation and appropriate self-consciousness. ②Present commonly used-mental health measurement for undergraduates was various, but there still was some disadvantage. Big Five Questionnaire (BFQ) and University Personality Inventory (UPI) were commonly used at abroad. Symptom Checklist-90 (SCL-90) was commonly used at home. ③Major factors influencing mental health of undergraduates were family, environment, age,sex, school, subject, religion and so on, and speciality had obvious effect on mental health of undergraduates.CONCLUSION: The importance of mental health of undergraduates is realized gradually by scholars, and more and more scholars at home and abroad study the mental health of undergraduates. How to design exact mental health standard and precise mental health measurement for undergraduates deserve further investigation. 5.Expression changes of intestinal proinflammatory cytokines in response to glutamine following traumatic brain injury in rats

Objective To study the role of the cytokine-mediated inflammation in the pathogenesis of acute intestinal mucosa damage and explore the potential mechanisms of glutamine reducing intestinal inflammatory factors on the intcstines and protecting the structure and barrier of intestinal mucosa after traumatic brain iniury(TBI).Methods A total of 54 male Wistar rats were divided equally into nine groups(6 rats in each group),ie,control group,TBI groups(sacrificed at days 1,3,5 and 7 postinjury,respectively)and Glutamine group.Levels of TNF-α and IL-1β in the intestinal tissues were determined by using ELISA and activity of NF-κB by immunohistochemical method.Results Activity of NF-κB in ileum was significantly increased at day 1 following TBI,reached peak at day 3 and remained hjgh level till at day 7 postinjury.TNF-α and IL-1β levels in the intestinal tissues were also significantly increased after TBI,peaked at day 1 and remained high level at day 3 pestinjury.NF-κB activity and the expressions of TNF-α and IL-1β were all obviously decreased posterior to glutamine administration through stomach tube after TBI,compared with normal foodintake group after TBI.Conclusions TBI can induce a rapid and persistent up-regulation of NF-κB and proinflammatory cytokines in the intestine.Inflammation response may play an important role in the pathogenesis of acute intestinal mucosa injury.Glutamine administration throush stomach tube can inhibit NF-κB activity in ileum and reduce TNF-α and IL-1β levels after TBI,indicating that the intestinal barrier function at stress state may be protected by glutamine administration.
6.The effects of visual feedback in retraining upper limb motion after stroke

Objective To explore the effectiveness of visual feedback in retraining upper limb function after stroke.Methods Sixty-one hemiplegic stroke patients were randomly assigned to a treatment group (31 cases)which received motion training using visual feedback in addition to routine neurodevelopment treatment and a motor relearning program,and a control group (30 cases) which received only the neurodevelopment treatment and a motor relearning program.The Fugl-Meyer assessment (FMA) and the modified Ashworth scale (MAS) were used for as-sessment before treatment and after 4 weeks of treatment.Results After 4 weeks of treatment,the FMA and MAS scores of both groups had improved significantly,as had the strength of the forearm flexors.The improvements were significantly greater in the treatment group than in the control group.Conclusion Motion training with visual feed-back can alleviate upper limb spasticity after stroke significantly better than routine rehabilitation alone.

8.Clinical analysis of acute arsenic poisoning treated by early plasma exchange in 11 patients.

Objective To investigate the effect of plasma exchange early on treatment for acute arsenic poisoning and prevention for acute renal failure.Methods Eleven patients of acute arsenic poisoning were treated by plasma exchange early.Their clinical features and biochemistry indexes were observed before and after treatment.Results After plasma exchange,the clinical features of all patients improved.Nine patients had normal renal function and 2 patients had impaired renal function.Conclusion Early plasma exchange is an effective measure for prevention for acute renal failure caused by acute arsenic poisoning.
9.Analysis of the Effect of Approaching Healthy Promotion for Hypertension in Rural Area in Chongqing

Objective To explore methods for hypertension intervention healthy promotion, improve villagers 'healthy consciousness and facilitate their behavior change in country. Methods Adopted "point to lead side and point combine with side" to develop community diagnosis and mobilization, establish system for measuring blood, organize villagers abstain from tobacco and supervise those who with hypertension in experimental village. Use self comparison to evaluate intervention effects. Results Understanding rate(full understanding means knowing all parts of the project ) about the clinical manifestation and harmful influence of hypertension have increased from 2.0% to 10.4% and 0.2% to 8.7% respectively just after intervening, but preventive knowledge level hasn't improved obviously between 2003(before intervening) and 2006(after intervening).Before 2003, There were no villagers have ever measured their blood pressure on their own, and the rate of medical workers who have measured blood pressure for patients on their own was 28.5%. But after 2006, the rate of villagers who know ought to measure blood pressure periodical was 97.6%, and among them, 83.0% have measured their blood pressure on their own. Medical workers who have measured blood pressure for patients increase to 80%~90%. Through medical institution launching a program named " measuring blood pressure for above 35 years free charge", 137 822 hypertensive were found in 3 years in 6 item country 256 health institutions of township and village levels. 10.Henoch-Sch(o)nlein purpura nephritis in children: an update on the diagnosis and treatment

Henoch-Sch(o)nlein purpura (HSP) is the common form of systemic vasculitis in children,which characterized by the deposition of immunoglobulin E within the small vessels.The long-term prognosis of HSP depends on the severity of renal involvement.Renal involvement in HSP affects 10%-100% patients and the clinical manifestations vary from microscopic hematuria to nephritic/nephritic syndrome even renal dysfunction.Early diagnosis,early treatment and long-term follow-up affect the prognosis of HSP patients.As for the treatment of HSP nephritis (HSPN),there have been many reports dealing with the immunosuppressive agents.Now glucocorticoid is the first-line treatment for HSPN,and other immunosuppressive agents offer more choices to treat HSPN especially for severe HSPN.But,the use of immunosuppressive agents still needs large sample multicenter randomized controlled trials and evidence-based study.
