1.Analysis of current status of diagnosis, treatment and referral of pulmonary tuberculosis in general hospital

Objective To investigate current status of diagnosis, treatment and referral of pulmonary tuberculosis in general hospital. Methods Statistical reports of hospital work, notification forms of tuberculosis (TB) cases on the internet, registration book of laboratory sputum mgcobacteria examinations, statistical reports of X-ray examinations for outpatient department, referral forms for mycobacteria patients, medical records for hospitalized patients at the respiratory ward of the People's Hospital, Peking University during August 2005 to July 2006 were reviewed and analyzed. Results In outpatient pulmonary department, there were 45 055 visitors during the period of August 2005 to July 2006,and 4960 of them (11.0%) had their chest X-ray examined, 1 512 (3.4%) had sputum mycobacteria examined with smear-positive in 24 cases, 189 (0.4%) were referred to specialized TB dispensaries for further diagnosis and 183 with notification forms with a notification rate of 96.8%, and 30 (0.1%) were finally diagnosed as TB after hospitalization with an interval less than 14 days between onset of symptoms and diagnosis in 27 of the 30 (90.0%) and 26 were referred to a specialized TB dispensary (86.7%) for treatment. Conclusions On general, detection, diagnosis and referral of TB in People's Hospital of Peking University are in a good situation.However,more aUenfion should be paid to the following aspects:① quantity and quality of sputum myeobacteria examinations for outpatients should be improved further,②chest radiograph reading by radiologists and clinicians need to be improved,and ③consciousness of early detection for TB need to be enforced in physicians by bronchoscopy and pathological diagnosis,especially in those with complicated and difficultly diagnosed smear-negative TB.

Combined with the partial least square regression, the oc tane number (RON), density, temperature evaporate (50%,90%), aromatics and olefi n concentration of gasoline were determined by the Fourier transform infrared spe ctroscopy at the wavenumber region of 1550~660cm-1. The experimenta l results showed that the actual values measured by the standard method were agre e with the prediction values by the proposed method.

5.Content Determination of 5 Anthraquinones Constituents in Sanhuang Drop Pills by HPLC

OBJECTIVE:To establish a method for the determining contents of 5 anthraquinones constituents in Sanhuang drop pills. METHODS:HPLC method was adopted. The column was Diamonsil C18 with the mobile phase of methanol-0.23% phosphoric acid(85∶15,V/V)at the flow rate of 1.0 ml/min,the detection wavelength was 440 nm and the volume temperature was room tem-perature. The sample size was 10 μl. RESULTS:The linear range was 0.070-0.700 μg for aloeemodin(r=0.999 9),0.646-6.460 μg for rhein(r=0.999 9),0.130-1.300 μg for emodin(r=0.999 9),0.150-1.500 μg for chrysophanol(r=0.999 9)and 0.074-0.740 μg for physcion(r=0.999 9),respectively. The RSDs of precision,stability and repeatability tests were no more than 1.96%. The av-erage recoveries were respectively 100.92%(RSD=2.80%,n=9),97.39%(RSD=1.39%,n=9),99.29%(RSD=1.81%,n=9), 100.73%(RSD=2.60%,n=9)and 99.81%(RSD=2.06%,n=9). CONCLUSIONS:The method is simple,accurate and specific, and can used for the content control of 5 constituents in Sanhuang drop pills.
6.Effects of 2,3,4',5-tetrahydroxystilbene-2-O-?-D glucoside preventative administration on expression of nitric oxide synthase in atherosclerotic rats

Aim To investigate the effects of TSG on the content of nitric oxide and the expression of endo-thelium nitric oxide synthase in artery vessels of experimental atherosclerotic rats.Methods Sixty male rats were randomly divided into six groups.GroupⅠ,Normal control;GroupⅡ,Model control;Group Ⅲ,TSG low dose;Group Ⅳ,TSG middle dose;GroupⅤ,TSG high dose;group Ⅵ,Simvastatin.After 12 weeks,blood samples were collected from carotid arteries of rats,and the levels of NO in serum were measured.After that,the aortas were separated from the bodies and placed in-70℃,then the content of nitric oxide synthase was detected,and gene expression of eNOS and iNOSmRNA in artery vessels were respectively measured with RT-PCR method.STATA 8.0 software was adopted in date analysis..Results compared with those of the model group,the level of NO,the activity of NOS and the gene expression of eNOS were increased,and the gene expression of iNOS was reduced by simvastatin and TSG 30,60,120 mg?kg-1?d-1 in the high cholesterol-fed rats,which showed a dose-dependent effect.Conclusion TSG can enhance the expression of eNOS gene and reduce the expression of iNOS gene in aorta vessels of experimental atherosclerotic rats,which may be one of the anti-atherosclerosis mechanisms of TSG.
7.Optimizing the Ultrasonic Extraction of Gardenia Oils from Gardenia Jasminoides Ellis by Response Surface Methodology

This study aimed to optimize the ultrasonic extraction of gardenia oils from Gardenia jasminoides Ellis.by response surface analysis methodology (RSM).The duration of extraction,ratio of liquid to materials and extracting times were selected as impact factors based on single-factor experiment.RSM was adopted to learn the effects of the three factors on the extraction rate of Gardenia oil from Gardeniajasminoides Ellis.As a result,it was found that the duration of extraction was 28.7 min,while the ratio of liquid to materials was 11.62:1,and the number of extracting times were 3.36.Under this extracting condition,the average extraction rate of the oil was 13.5% according to three validation experiments,fitting the predicted value well.In conclusion,it was demonstrated that the ultrasonic extraction method effectively improved the extraction rate of gardenia oil,providing a certain basis for the development and utilization of gardenia oil.
8.An Analysis on Nosocomial Infection after Surgical Operation

0.05);the main infection site was lower respiratory tract(61.97%),followed by surgical site infection(SSI)(23.94%).CONCLUSIONS In order to reduce postoperation nosocomial infection rate,we should take following measures:Improve pulmonary function,remove tracheal intubation as early as possible,shorten lasting time of surgery and days before operation,keep aseptic technique,strengthen nursing,enhance immunity,and use antimicrobial agents reasonably.
9.Effects of parecoxib sodium combined with continuous femoral nerve block on the balance of Th1/Th2 in elderly patients after total knee arthroplasty

Objective To study the effects of parecoxib sodium combined with continuous femoral nerve block on the balance of Th1/Th2 in elderly patients after total knee arthroplasty. Methods Fifty elderly patients,33 males and 1 7 females,aged 65 to 80 years,with ASA Ⅰ or Ⅱtreated with total knee arthroplasty were randomly divided into two groups (n =25 each):parecoxib sodium combined with continuous femoral nerve block group (group A)and parecoxib sodium com-bined with intravenous analgesia group (group B).Quick induce laryngeal mask anesthesia and intra-venous parecoxib sodium 40 mg at 30 min before skin incision were adopted in both groups.After the first 8 h,parecoxib sodium of 40 mg was intravenously injected again.The patients in group A re-ceived femoral nerve puncture and continuous electronic analgesia pump.The patients in group B re-ceived postoperative intravenous electronic analgesia pump.Visual analogue score(VAS)during rest and movement at 6,12,24,36,48 h after operation,side effects in two groups were recorded,venous blood samples were taken before anesthesia (T0 ),at the end of operation (T1 ),and 24 h(T2 ),48 h (T3 ),72 h (T4 )after operation for determination of plasma IFN-γ,IL-10 and cortisol (Cor). Results The values of VAS were significantly lower in group A compared with group B in rest at 6, 12 h after operation,the values of VAS were significantly lower in group A compared with group B in moving at every time points after surgery (P <0.05 ).The plasma Cor concentrations were signifi-cantly increased at T1-T3 compared with baseline value at T0 in two groups(P <0.05 ).The plasma Cor concentrations in group A were significantly decreased compared with group B at T1-T3 (P <0.05).The plasma IFN-γconcentrations were significantly lower at T2 and T3 compared with baseline value at T0 in group A,the plasma IFN-γ concentrations were significantly lower at T2-T4 compared with baseline value at T0 in group B (P <0.05),the plasma IFN-γ concentrations were significantly increased in group A compared with group B at T2-T4 (P <0.05 ).The plasma IL-10 concentrations were significantly increased compared with baseline value at T1-T4 in two groups (P < 0.05 ),the plasma IL-10 concentrations were significantly decreased in group A compared with group B at T1-T4 (P <0.05).Compared with group B,the incidence of postoperative nausea,vomiting and pruritus in group A was lowered significantly (P <0.05).Conclusion Parecoxib sodium combined with continu-ous femoral nerve block on postoperative pain in elderly patients after total knee arthroplasty can a-chieve good effect of postoperative analgesia and fewer complications, lower cortisol secretion, slowing down the decrease of IFN-γ secretion and the increase of IL-10 secretion.This method could effectively protect the immune function of patients undergoing arthroplasty.
10.Treatment adherence and correlated factors of patients with hypertension: a longitudinal study

Objective This article aimed to investigate the treatment adherence of patients with hypertension and examine determinants of adherence.Methods A longitudinal design was adopted to test the treatment adherence and correlated factors of patients with hypertension twice from 2009 to 2012.The influencing factors of treatment adherence in patients with hypertension were analyzed.Results A total of 520 patients finished the first investigation,and 331 patients completed the second investigation.The comparison of demographic characteristics between patients who participated the second investigation or not showed no significant difference except for the education degree (x2=9.38).The incidences of symptoms and complications,systolic blood pressure and diastolic blood pressure of hypertensive patients and the laboratory indexes such as serum creatinine (SCr),total cholesterol (TC),low density lipoprotein (LDL),high density lipoprotein (HDL),fasting blood-glucose and postprandial blood sugar in the second investigation were lower than those of the first investigation,but the difference showed no statistical significance.The scores of treatment adherence,self-efficacy,social support,quality of life were higher in the second investigation than those of the first investigation.The multiple regression analysis found that treatment adherence,social support,education degree and duration of disease course were significant predictors in the first investigation entered the equation,accounting for 26% of the total variance,among which treatment adherence explained 15% of the variance,social support 7%,education degree 3% and duration of disease course 1%.Conclusions The treatment adherence of hypertensive patients improved over one year follow-up.Healthcare providers should pay attention to adherence behavior at initial phase of disease development,and effective strategies targeted patients at risk are suggested to be necessary and should be designed according to the factors affecting adherence.
