2.Analysis of current status of diagnosis, treatment and referral of pulmonary tuberculosis in general hospital

Objective To investigate current status of diagnosis, treatment and referral of pulmonary tuberculosis in general hospital. Methods Statistical reports of hospital work, notification forms of tuberculosis (TB) cases on the internet, registration book of laboratory sputum mgcobacteria examinations, statistical reports of X-ray examinations for outpatient department, referral forms for mycobacteria patients, medical records for hospitalized patients at the respiratory ward of the People's Hospital, Peking University during August 2005 to July 2006 were reviewed and analyzed. Results In outpatient pulmonary department, there were 45 055 visitors during the period of August 2005 to July 2006,and 4960 of them (11.0%) had their chest X-ray examined, 1 512 (3.4%) had sputum mycobacteria examined with smear-positive in 24 cases, 189 (0.4%) were referred to specialized TB dispensaries for further diagnosis and 183 with notification forms with a notification rate of 96.8%, and 30 (0.1%) were finally diagnosed as TB after hospitalization with an interval less than 14 days between onset of symptoms and diagnosis in 27 of the 30 (90.0%) and 26 were referred to a specialized TB dispensary (86.7%) for treatment. Conclusions On general, detection, diagnosis and referral of TB in People's Hospital of Peking University are in a good situation.However,more aUenfion should be paid to the following aspects:① quantity and quality of sputum myeobacteria examinations for outpatients should be improved further,②chest radiograph reading by radiologists and clinicians need to be improved,and ③consciousness of early detection for TB need to be enforced in physicians by bronchoscopy and pathological diagnosis,especially in those with complicated and difficultly diagnosed smear-negative TB.

Combined with the partial least square regression, the oc tane number (RON), density, temperature evaporate (50%,90%), aromatics and olefi n concentration of gasoline were determined by the Fourier transform infrared spe ctroscopy at the wavenumber region of 1550~660cm-1. The experimenta l results showed that the actual values measured by the standard method were agre e with the prediction values by the proposed method.

5.Culture of vascular smooth muscle cells from coronary arteries of diabetic rats

Aim To investigate the method of culturing vascular smooth muscle cells (VSMC) of coronary arteries derived from experimental diabetic rats and to establish the model of VSMC for the study of chronic cardiovascular diseases of diabetes. Method After the establishment of the model of diabetic rats, the coronary arteries of rats were separated carefully to culture VSMC using the method of explanting culture. Results The primary culture of VSMC was performed successfully by the method mentioned above. The cells could be isolated by different kinds of digestive enzyme in three steps. 7 ~10 days afterward, the cells overlapped in layers and displayed the typical ‘hill and valley’ pattern and positive immunochemical staining for smooth muscle actin. Conclusion VSMC from experimental diabetes proliferates faster than normal VSMC and its cultivating condition is strictly limited. The morphology of diabetic VSMC is as same as that of normal rats.
6.Effect of tetrahydroxystilbene-glucoside on lipid and inflammatory factor levels of experimental atherosclerosis in rats

AIM: To investigate the preventive effect of tetrahydroxystilbene-glucoside (TSG) on experimental atherosclerosis rats. METHODS: Sixty one male rats were randomly divided into six groups. normal control; model control; TSG low dose(30 mg?kg-1?d-1);TSG middle dose(60 mg?kg-1?d-1);TSG high dose(120 mg?kg-1?d-1); simvastatin(2 mg?kg-1?d-1). The AS model of rats was made by feeding high grease food and injecting VitD3 .All the rats were fed for 12 weeks, blood samples were drawn from carotid artery of rats, the levels of TC, TG, HDL-C, LDL-C ,CRP,IL-6 and TNF-? in serum were measured with biochemical method. After blood samples were collected , the aorta samples were separated from the bodies, then they were placed 4% paraformaldehydea and were through Sudan Ⅳ and HE staining. STATA7.0 software was used to evaluate the differences between groups. RESULTS: Data of the study demonstrated that the levels of TC、TG、LDL-C、TNF-?、IL-6 and CRP were decreased remarkably, the level of HDL-C was increased by TSG 60, 120 mg?kg-1?d-1 group in the high cholesterol-fed rats, which showed a dose-dependent effect. The result of Sudan Ⅳ and HE staining suggested that the lipid deposits in aortic endothelium in TSG group were less than those in model group. CONCLUSION: TSG has preventive effect on the experimental atherosclerosis among the high cholesterol-fed rats. The anti-atherogenic effect of TSG seems to be closely related to regulating plasma lipid profile, and inhibiting inflammation.
7.Effects of 2,3,4',5-tetrahydroxystilbene-2-O-?-D glucoside preventative administration on expression of nitric oxide synthase in atherosclerotic rats

Aim To investigate the effects of TSG on the content of nitric oxide and the expression of endo-thelium nitric oxide synthase in artery vessels of experimental atherosclerotic rats.Methods Sixty male rats were randomly divided into six groups.GroupⅠ,Normal control;GroupⅡ,Model control;Group Ⅲ,TSG low dose;Group Ⅳ,TSG middle dose;GroupⅤ,TSG high dose;group Ⅵ,Simvastatin.After 12 weeks,blood samples were collected from carotid arteries of rats,and the levels of NO in serum were measured.After that,the aortas were separated from the bodies and placed in-70℃,then the content of nitric oxide synthase was detected,and gene expression of eNOS and iNOSmRNA in artery vessels were respectively measured with RT-PCR method.STATA 8.0 software was adopted in date analysis..Results compared with those of the model group,the level of NO,the activity of NOS and the gene expression of eNOS were increased,and the gene expression of iNOS was reduced by simvastatin and TSG 30,60,120 mg?kg-1?d-1 in the high cholesterol-fed rats,which showed a dose-dependent effect.Conclusion TSG can enhance the expression of eNOS gene and reduce the expression of iNOS gene in aorta vessels of experimental atherosclerotic rats,which may be one of the anti-atherosclerosis mechanisms of TSG.

Aim To observe the effects of 2,3,4',5-tetrahydroxystilbene-2-O-?-D glucoside on expression of ICAM-1、VCAM-1 and VEGF in atherosclerotic rats,and approach the influence of TSG in atherogenesis.Methods Sixty SD rats were randomly divided into six groups(Control,Model,TSG 30、60、120 mg?kg-1?d-1 and Simvastatin 2 mg?kg-1?d-1 groups),given high-fat-diet for 12 weeks,the level of SOD and MDA then were determined.ICAM-1、VCAM-1 and VEGF protein were determined with Western blot assay,ICAM-1 mRNA and VCAM-1 mRNA were measured with RT-PCR,and VEGF expression was assessed by immunohisto-chemistry.Results In comparison with the model,the level of SOD increased markedly and the level of MDA decreased markedly after administering TSG;TSG could inhibit the expression of ICAM-1、VCAM-1 and VEGF in atherosclerotic arteries.Conclusions TSG could show its antiatherosclerotic effect by anti-oxydation and inhibiting the excretion of adhesion molecules.

10.Treatment adherence and correlated factors of patients with hypertension: a longitudinal study

Objective This article aimed to investigate the treatment adherence of patients with hypertension and examine determinants of adherence.Methods A longitudinal design was adopted to test the treatment adherence and correlated factors of patients with hypertension twice from 2009 to 2012.The influencing factors of treatment adherence in patients with hypertension were analyzed.Results A total of 520 patients finished the first investigation,and 331 patients completed the second investigation.The comparison of demographic characteristics between patients who participated the second investigation or not showed no significant difference except for the education degree (x2=9.38).The incidences of symptoms and complications,systolic blood pressure and diastolic blood pressure of hypertensive patients and the laboratory indexes such as serum creatinine (SCr),total cholesterol (TC),low density lipoprotein (LDL),high density lipoprotein (HDL),fasting blood-glucose and postprandial blood sugar in the second investigation were lower than those of the first investigation,but the difference showed no statistical significance.The scores of treatment adherence,self-efficacy,social support,quality of life were higher in the second investigation than those of the first investigation.The multiple regression analysis found that treatment adherence,social support,education degree and duration of disease course were significant predictors in the first investigation entered the equation,accounting for 26% of the total variance,among which treatment adherence explained 15% of the variance,social support 7%,education degree 3% and duration of disease course 1%.Conclusions The treatment adherence of hypertensive patients improved over one year follow-up.Healthcare providers should pay attention to adherence behavior at initial phase of disease development,and effective strategies targeted patients at risk are suggested to be necessary and should be designed according to the factors affecting adherence.
