1.Effect of Aerobic Exercise on the Maximal Oxygen Uptake in Sedentary Healthy Adults: A Meta Analysis

Objective Through this meta analysis of controlled clinical trials of the effect of aerobic exercise on the maximal oxygen uptake(VO_2max)in sedentary healthy adults,we attempt to provide evidence for exercise prescription.Methods MEDLINE search for controlled trials that involved in the effect of aerobic exercise on the VO_2max from 1976 to 2008 was conducted.The literatures were selected according to inclusion criteria and exclusion criteria.Review Manager 5.0 software provided a systematic evaluation for calculating VO_2 max Weighted Mean Difference with random effects model.Assessment of the sub-group of aerobic exercise intensity and literature publication bias were conducted.Results(1)Thirty five literatures were included in this review;involving 889 adults met the inclusion criteria.VO_2max Weighted Mean Difference is 3.61ml/kg/min,95%confidence interval 2.96~4.25ml/kg/min,with an increase of 13.2%,comparing with the average VO_2max values(27.26 ml/kg/min)before aerobic exercise.(2)The most effective exercise intensity was 70~80%VO_2max,71~80%HRmax and 61~70% HRR according to the sub.group of aerobic exercise intensity meta analysis.Conclusion VO_2max increased significantly due to participating in aerobic exercise in sedentary healthy adults.The results will provide a theoretical basis for me development of aerobic exercise prescription with appropriate intensity.

3.Extended pancreaticoduodenectomy for carcinoma of head of pancreas

Objective To study the effect of extended Whipple′s resection for carcinoma of head of pancreas.Methods Seventy-five patients who underwent surgery for carcinoma of head of pancreas were divided into control group(traditional Whipple′s operation,n=38) and study group(extended Whipple′s operation,n=37).The survival,complication,mortality and recurrence rates were compared between the 2 groups.Results In control group and study group,the complication rate was 60.53%and 29.73%;the mortality rate was 5.26% and 2.70%;the recurrence rate was 78.95% and 35.14%;and 3-year survival rate was 15.79% and 35.14%,respectively.Conclusions Extended Whipple′s resection for carcinoma of head of pancreas can decrease postoperative recurrence rate and increase 3-year survival rate.
4.Preliminary study of effects on dogs' testis by microbubbles excitated by ultrasound

Objective To explore the effects on dogs' testis by microbubbles excitated by ultrasound.Methods Sixteen male dogs were divided into 3 groups randomly.Pulsed ultrasound irradiation and intravenous microbubbles injection were both applied in the microbubble enhanced ultrasound group (MEUS),pulsed ultrasound irradiation and intravenous microbubbles injection were individually applied in the ultrasound group (TUS) and the simple microbubbles group (MB).Results In MEUS group,the layers of seminiferous tubules decreased and the germ cells were arranged disorderly and bubbles appeared in the cells.The appearances of focal ultra-structural damage such as intercellular space widening,baseline broken and tight-connection of sustentacular cells disappearing were observed by transmission electron microscope,which were slightly or hardly demonstrated in other groups.Conclusions Microbubbles excitated by ultrasound can affect testicular germ cells and the integrity of bloold-testis harrier,which providing new train of thought of target treatments of testis diseases.
5.The role of mucin gene 1 mediated anti-MKN-45 cell invasion

Objective To create Mucin gene 1 (MUC1) antisense peptide nucleic acid (PNA),and to observe its effects on MKN-45 cell invasion and explore the mechanism. Methods The sequence of antisense PNA was designed according to MUC1 gene sequence and transfected into human gastric cancer cells (MKN-45) by liposome,and the empty vector group (randomized control group)and blank control group (negative control group) were involved. The expression of MUC1 was detected by real time quantitative PCR and the changes of E-cadherin expression were also observed.The effects on gastric cancer cell invasion were tested with transwell chamber assays.Results The expression of MUC1 gene was effectively suppressed by the 3 created antisense PNA,and their expression level (0.62±0.18,0.49±0.12 and 0.60±0.21) was significantly lower than that of negative control group (1.18 ± 0.03,P < 0.01). There was no significant difference between radomized control group and negative control group (1.00±0.04,P=0.657).After MUC1 PNA transfected,the capability of gastric cancer cell invasion decreased significantly (P=0.005).And the expression of E-cadherin at mRNA and protein level was up-regulated.Conclusions There is negative correlation between MUC1 and E-cadherin expression in gastric cancer cell MKN-45.The capability of tumor cell invasion is significantly inhibited by suppressing MUC1 gene expression.
6.Progress in new-type vaccines against classical swine fever.

Classical swine fever (CSF), an acute and highly contagious disease of swine, is caused by classical swine fever virus. CSF is one of the most devastating diseases to the pig industry worldwide and results in serious economic losses. Currently prophylactic vaccination is still an important strategy for the control of CSF. Live attenuated vaccines (such as C-strain) are safe and effective. However, there are significant changes in the clinical features of CSF, displaying concurrent typical and atypical CSF, and simultaneous inapparent and persistent infections. Immunization failure has been reported frequently and it is difficult to distinguish between wild-type infected and vaccinated animals (DIVA). So there is an urgent need to develop more effective and safer DIVA or marker vaccines for the control of CSF. In this review, some of the most recent advances in new-type vaccines against CSF, including DNA vaccines, live virus-vectored vaccines, protein or peptide-based vaccines, gene-deleted vaccines and chimeric pestivirus-based vaccines, are reviewed and discussed.

7.Expression and clinical significance of mismatch repair gene in colorectal cancer

Objective: To investigate the expression and clinical significance of mismatch repair genes (MMR) MLH1, MSH2, MSH6, and PMS2 in colorectal carcinoma. Methods: Colorectal cancer tissues, collected from 607 patients enrolled in Sichuan Provincial People's Hospital from January 2015 to September 2016, were assigned into two groups based on whether the samples were positive or nega-tive for MMR expression to determine the relationship between MMR expression and clinicopathology. We then evaluated the diag-nostic value of MMR expression in the screening of Lynch syndrome and sporadic colorectal cancer. Results: The deletion rate of MMR protein was 35.58%. No statistically significant difference in age, sex, tumor size, P53, CD34, and D2-40 expression was detected be-tween the negative group with MMR protein deficiency and the positive group with normal expression (P>0.05). Differences in tumor location, differentiation, TNM stage, lymph node metastasis, and VEGF and Ki-67 expression between the two groups were statistically significant (P<0.05). The combined detection of MLH1, MSH2, PSM2, and MSH6 proteins may serve as a simple and economical meth-od for screening patients with Lynch syndrome. Conclusions: The risk of colorectal cancer can be reduced by MMR detection of surgi-cal specimens from colorectal cancer patients, screening of patients with Lynch syndrome and their family members, and assisting with proper management and intervention.

: Rehabilitation medicine is a medical branch which focused on functional recovery. Function Evaluation is very important in assessing the function of patients, the effect of treatment and the efficiency of rehabilitation. Comprehensive Function Evaluation includes evaluation of physical, psychological, speech and society. Vocal, mental and social evaluation have deep cultural and national background. Therefore every country must have its own Function Evaluation Method. Now we present our design for Comprehensive Evaluation. Based on the cooperation of Department of Rehabilitation, Department of Neurology, Department of Spinal Cord Injury, Department of Speech Therapy and Department of Psychology. The advantages of this method are as follows: 1. The style of ADL, speech and thinking are suitable for the condition of our country. 2. The evaluation result adopts hundred work system, it is easy for medical staff, patients and their family to understand and communicate the result. 3. We make it more accurate, comprehensive and reliable by some simple tests on speech pathology and psychology. 4. We overcome some disadvantages of evaluation indexes because it is not correct and is difficult to be understood before. Now every evaluation index has quantity standard. 5. It is simple and practical. Each subtest takes 20 minutes or more. 6. It has been tested by normal people. The norm and severity grade had been developed. 7. The reliability is tested and is proved to be dependable.

康复医学是以恢复患者功能为中心的医学分支。因此,功能评定无论是在客观地评定患者的功能方面,还是在最终评定治疗结果和康复效率方面都是极为重要的。全面的功能评定包括躯体、精神、言语和社会四个方面。其中,言语、社会功能和认知功能中的思维方式,都具有强烈的民族文化色彩。因此,每个国家都应该有切合自己国情的功能评定方法。但由于我国康复医学发展较晚,至今尚无一套既切合国情,又全面、实用和可靠的功能评定方法。有鉴于此,我们在“中心”顾问室、神经康复科、脊髓损伤康复科、言语治疗科、心理科和老年病科的通力合作下,经过近2年的研究,在吸收国际先进经验的基础上,密切结合国情,设计了本文所述的综合功能评定法。其优点有:1.在饮食、起居等生活方式方面以及在言语、社会、思维等方面,均切合我国国情。2.评定结果采用群众熟悉的100分制,使医务人员、患者和患者家属均易于理解,便于交流和沟通。3.在言语、认知等功能的评定方面,直接由言语和心理学家选择一些简易的言语和心理学测试项目,提高了量表的准确性、全面性和可靠性。4.各项评定指标的量化程度高,在言语、认知和社会方面尤其如此,克服了一些量表中对此类项目的评定指标不够具体和不易掌握的不足。5.简便实用,一次检查对正常人仅需20分钟左右,对患者则无负担。6.本法已在128名正常人中应用,并求出了正常值,据此拟定了功能障碍严重程度的等级,可供参考和应用。7.信度经过检验,证明可靠。 综合功能评定法的正常值、功能障碍严重程度分级及信度研究结果,将陆续报道。
10.Relatedfactors of blood transfusion during total hip arthroplasty

【Objective】 To investigate the related factors of allogeneic blood transfusion (ALBT) in total hip arthroplasty. 【Methods】 Thebasic information, surgical details and laboratory data of 258 patients who underwent total hip arthroplasty in Orthopedics Department of our hospital were collectedfrom the electronic medical record system and laboratorytest system. The factors concerningALBT were obtainedby single factor and multivariate logistic regression analysis. 【Results】 The ALBT rate in this study was 19%, and the differencesin such important factors affecting ALBT as gender, age, intraoperative blood loss, drainage volume, operation duration, preoperative hemoglobin (Hb), preoperative activated partial thromboplastin time (APTT), preoperative prothrombin time (PT) andhypertension between the two groupswere notable (P<0.05). Multivariate regression analysisrevealed that the independent related factors ofALBT were age ≥ 60 years (OR3.577), intraoperative blood loss (OR1.003), drainage volume (OR1.004)and preoperative PT (OR1.888). Preoperative Hb (OR0.94) was a protective factor. 【Conclusion】 Specific and individualized evaluation of ALBT, aimed atreducingunnecessary blood transfusion, can be provided through the analysis of relevant factors.
