Objective To investigate the effect of plasma exchange early on treatment for acute arsenic poisoning and prevention for acute renal failure.Methods Eleven patients of acute arsenic poisoning were treated by plasma exchange early.Their clinical features and biochemistry indexes were observed before and after treatment.Results After plasma exchange,the clinical features of all patients improved.Nine patients had normal renal function and 2 patients had impaired renal function.Conclusion Early plasma exchange is an effective measure for prevention for acute renal failure caused by acute arsenic poisoning.

OBJECTIVETo investigate the influence of three primers on the shear bond strength between cast titanium (Ti) and composite resin.

METHODSThe disks (n = 40) were cast by commercially pure (CP) Ti, which diameter were 8 mm and thick were 3 mm. The titanium surfaces were polished with silicon carbide sand papers under running water and then treated by sandblasting and acid (4%HF) etching. They were divided into four groups: control group (group A), treated with KH-570 (group B), treated with Alloy Primer (group C), treated with Metal photo primer (group D). After treatment, the specimens were evaluated for their shear bond strengths by universal testing machine. The values were statistically analyzed. The fractured surfaces were observed by scanning electron microscope (SEM).

RESULTSThe shear bond strengths of group A, B, C, D were (9.773 +/- 0.67), (11.463 +/- 0.82), (14.224 +/- 0.75), (13.157 +/- 0.73) MPa. There were significant differences in bond strength between A and B, C, D (P < 0.01). B and C, D had significant differences (P < 0.01). C and D had no significant differences (P>0.05).

CONCLUSIONKH-570, Alloy Primer, Metal photo primer significantly improve the bond strength of ceramage composite resin to cast titanium. KH-570 group bonding strength is lower than the the Alloy Primer group and Metal photo primer group.

Objective To observe the therapeutic effect of Di'ao Xinxuekang (DAXXK) on myocardial ischemia-reperfusion injury in rats, and to explore its mechanisms. Methods The myocardial ischemia-reperfusion injury model was established by the ligation of descending coronary artery in rats. Then animals after the modeling were randomized into model group, DAXXK-d (31.5 mg/kg) group, DAXXK-g (63.0 mg/kg) group and Diltiazem (24.8 mg/kg) group. A separate sham group was used as control. The treatment group was given DAXXK once a day for 7 days. Cardiac function and cardiac configuration were measured by color Doppler ultrasound diagnostic method. Hemodynamics was measured by Millar catheter method. The arterial oxygen saturation and blood oxygen pressure were measured by i-STAT 300 blood gas analyzer. Inflammatory cytokines interleukin (IL)-1β, IL-6, tumor necrosis factor (TNF)-α, adhesion molecules VCAM-1, ICAM-1, and apoptosis-related protein Bcl-2, Bax were detected by ELISA. Myocardial apoptosis was measured using TUNEL method. Results Compared with model group, the left ventricular fractional shortening (FS), the systolic and diastolic function were improved, and the left ventricular pressure maximum rise/ fall rates (± LVdp/dtmax) were increased, in DAXXK group. DAXXK improved lung function, increased arterial oxygen pressure and oxygen content. The inflammatory cytokines IL-1β, IL-6, TNF-α and adhesion molecules ICAM-1 and VCAM-1 were decreased in DAXXK group. The myocardial swelling and inflammatory infiltration were relieved, myocardial apoptosis was reduced, the expression of Bcl-2 protein was increased and the expression of Bax protein was decreased in DAXXK group. Conclusion DAXXK can protect myocardial ischemia-reperfusion injury, which involved in the inhibition of apoptosis and reduction of inflammatory cytokines.

Objective The astragaloside Ⅳ(ASI)has been proved to play an important role in protecting against cell death on cardiovascular diseases.This study aims to investigate the effect of the astragaloside derivate.(ASId)on confronting oxidative stress and hypertrophy in myocardial cells.Methods Following exposure embryonic rat cardiac H9c2 cells to hydrogen peroxide(H2O2)and angiotensin Ⅱ for developing oxidative stress and hypertrophy,ASId at final concentrations(0.1,1,and 10 μmol/L)was added to study its role in protecting cardiomyocytes by biochemical detection and cell size measurement In addition,the mitochondrial permeability transition pore(mPTP)opener atractyloside(20 μmol/L)and inhibitor cyclosporin A(CSA)(1 μmol/L)were employed to investigate the possible mechanisms for anti-oxidation.Results ASId at 1 and 10 μmol/L in cultures suppressed oxidative stress at different degrees,which induced the decrease in LDH activity and MDA content,and also the increase in SOD activity in comparable with the model group; The mPTP opener atractyloside and inhibitor CSA weakened and strengthened the role of ASId,respectively.ASId at 10 μmol/L inhibited cell hypertrophy,and the cell diameter,surface area,and protein content were all decreased in comparable of those cells in model group.Conclusion ASId is involved in the cytoprotective effects on oxidative stress through a pathway mediated by mPTP,and also has a protective effect against hypertrophy.

Objective To investigate the effect of percutaneous eoronary intervention (PCI) on the prognosis of acute ST-segment elevation myocardial infarction (ASTEMI) in the elderly.Methods The 1318 ASTEMI patients in our hospital from June 1998 to June 2008 were retrospectively analyzed. Among them, 338 (25.6%) elderly patients were over 60 years old, and 316patients consistent with inclusion and exclusion criteria were consecutively enrolled in our research.Then they were divided into two groups: PCI group (136 cases, 43.0%) and conservative drug treatment group (180 cases, 57. 0%). The clinical data of study objects were collected. Then they were followed up regularly for two years. Results There were no statistically significant differences between the two groups in mean age, gender, hypertension, diabetes, dyslipidemia, excess smoking,wine and family history (all P＞ 0.05). And there were no statistically significant differences in anterior wall STEMI, Killip Ⅲ-Ⅳ class, thrombolysis therapy and malignant ventricular arrhythmia (all P＞0. 05). Most of the objects proceeded therapeutic lifestyle improvements, such as giving up smoking, restricting wine, regulating diet, losing weight and insisting on exercises, and so on.Secondary prevention drugs of acute myocardial infarction including angiotensin converting enzyme inhibitor, angiotensin receptors blockers, beta receptor, aspirin and statins were regularly administrated in the two follow-up years. In the retrospective research, incidence rates of reinfarction, NYHA (New York Heart Association) Ⅲ-Ⅳ class heart function and one-month mortality were much higher in conservative treatment group than in PCI group (17.2% vs. 2. 2%, OR=9. 224,95% CI: 2. 756-30. 857; 31.1% vs. 8.1%,OR=5.132, 95%CI: 2. 568-10. 257; 8. 3% vs. 1.5%,OR= 6. 091, 95% CI: 1. 369-27. 105, respectively; all P ＜ 0. 01). Above all, one and two-year mortalities were much higher in conservative treatment group than in PCI group (21.1% vs. 2. 2 %,OR=11.864, 95%CI: 3.577-39.349; 32.2% vs. 4.4%, OR=10.301, 95%CI: 4.289-24.736,respectively; all P＜0. 01). Conclusions PCI may reduce the re-infarction, NYHA Ⅲ-Ⅳ class heart function and one-month mortality, especially so in view of the one and two-year mortality. PCIcan significantly improve the prognosis of ASTEMI in the elderly.

Objective To study the relationship between blood cell count and cardiac events in acute ST elevation myocardial infarction patients treated with reperfusion in the early stage. Methods In this study, we assigned 151 patients to whom reperfusion therapy had been delivered within 3 hours of STEMI symptom into two groups:primary percutaneous cor-onary intervention group and thrombolysis therapy group. Differences of cardiac events and white blood cell count in these two groups were analyzed in the first 4 days. Results In the second day and the forth day, cardiac events rate and white blood cell count were significantly lower in the primary percutaneous coronary intervention group than those in the thromboly-sis therapy group (P＜0.05). In the second day, white blood cell count has a positive correlation with cardiac events rate (r=0.226, P＜0.05). Conclusion Primary percutaneous coronary intervention decreased blood cell count and cardiac events rate. In patients with acute myocardial infarction, increasing white blood cell was related to cardiac events in the near future.

Objective To develop a programmable neuromuscular electrical stimulation system as a potential countermeasure for muscle atrophy in long-term microgravity.Methods The system was mainly composed of a controllable constant current pulse generator,wires and skin electrodes.To meet the requirements of stimulating recipes selection,the stimulating parameters could be adjusted independently and the combinatorial output of different stimulating phases should also be achieved.Results The system worked well with high accuracy outputs.The sensation,contraction,tetany and pain thresholds of all the 5 waveforms stimulating signals were measured.For different stimulating waveforms,thresholds of each state were different and the subjective sensations induced by same stimulating amplitude were dissimilar.Conclusion The developed system providing powerful programmable and accurate stimulating signals,is convenient for the selection and verification of stimulating recipes and can be used as an instrument platform for study of electrical stimulation to prevent muscle atrophy in microgravity.Volunteer experiments have proved its validation for function of muscle contraction and provide primary basses for the optimization of stimulating recipes.

Objective: To observe the clinical effect of thumbtack needles combined with pediatric Tuina (Chinese therapeutic massage) for constipation in children caused by liver depression and Qi stagnation.Methods: A total of 60 children with constipation caused by liver depression and Qi stagnation were randomly divided into a combined group and a Tuina group, with 30 cases in each group. The cases in the Tuina group were treated with pediatric Tuina, while those in the combined group were treated with additional thumbtack needle therapy. We observed the clinical efficacy and the change in the number of spontaneous bowel movements per week before and after treatment. Results: After treatment, the cure rate and the total effective rate in the combined group were higher than in the Tuina group, showing statistical significance (P<0.05). Compared with the same group before treatment, the number of spontaneous bowel movements per week in both groups increased after treatment (P<0.05). With the progression of treatment, the number of spontaneous bowel movements per week increased in both groups, and the number in the combined group was more than that in the Tuina group, showing statistical significance (P<0.01). Conclusion: Thumbtack needles combined with pediatric Tuina is more effective than pediatric Tuina alone in the treatment of constipation in children caused by liver depression and Qi stagnation.

To investigate the knowledge, attitude and practice (KAP) on rabies among a mass of the exposed population in Beijing so as to provide scientific evidence for development of measures on the prevention and protection of rabies, the descriptive studies were employed, in which patients were interviewed face to face with a standard questionnaire in hospital or the center for disease control and prevention (CDC), including history of exposure, knowledge on rabies, post-exposure treatment etc. Of 478 respondents, 76% of them were bitten on the upper limbs with the exposure proportions in category Ⅰ, Ⅱ and Ⅲ of 24%, 70% and 6% respectively. The proportions of awareness of knowledge in rabies before biting, its prevention, the correct measure to treat their wounds themselves, rabies vaccine inoculation, and injections with rabies antiserum constituted in 82%, 55%, 59%, 99% and 22% respectively. Result of this investigation indicates that most of the people investigated had a lower awareness of knowledge on rabies, the correct measures to treat rabies after biting as well as the measures to prevent rabies, suggesting that community-based prevention and control measures should be emphasized, especially targeting the population with high risk and the heath education on rabies should performed promptly by several governmental departments.
