1.Curative effect of methylprednisolone in the treatment of children with nephrotic syndrome and its influence on function of adrenal cortex

Objective To evaluate the efficacy and safety of methylprednisolone in the treatment of children with nephrotic syndrome ,and to observe its adverse reaction .Methods 62 cases with primary nephrotic syndrome were randomly divided into the two groups .32 cases in the treatment group were given methylprednisolone combined with prednisone ,30 cases in the control group were given prednisone therapy .Adreno-corticotropic hormone ( ACTH) and cortisol (Cr) in serum were detected by radioimmunoassay (RIA);plasma albumin,urine protein,creatinine,ala-nine aminotransferase ( ALT) ,blood uric acid were measured by conventional laboratory methods;bone mineral densi-ty was measured by bone mineral density machine .Results The curative effect of complete remission and partial remission between the two groups had no significant difference (χ2 =1.56,P>0.05).After treatment for 10 months, the concentrations of ACTH and Cr between the two groups had no significant differences (t=5.45,P>0.05).After treatment,the contents of plasma albumin in the two groups were (36.5 ±5.5)g/L,(36.7 ±5.9)g/L),the contents of urinary protein in the two groups were (1.5 ±1.1)g/24h,(1.6 ±1.5)g/24h.Before treatment,those in the two groups were albumin (25.7 ±3.3)g/L,(26.3 ±3.5)g/L,urinary protein (5.5 ±3.0)g/24h,(5.8 ±3.5)g/24h, the differences were significant (t=11.45,12.15,all P<0.05);The time of urinary protein changed to negative in the treatment group was 6 days,which was significantly shorter than 8 days in the control group ( Log Rank=10.56, P<0.05);The incidence rate of adverse reactions in the treatment group was 40%(13 cases),which was significantly higher than 26.6%(8 cases) in the control group (χ2 =23.11,P<0.05).Conclusion The combined treatment can shorten the time of urinary protein changed to negative ,but during the treatment ,it may increase adverse reactions in children with primary nephrotic syndrome .
2.Analysis of influencing factors of hemoglobin A1c in type 2 diabetic patients

Objective To analyze the influencing factors of hemoglobin A1c in type 2 diabetic patients,including blood glucose,anti-diabetic therapeutic regiments,disease course,age,diabetic education,degree-educated,self-monitoring blood glucose.Methods Seventy-eight type 2 diabetic patients were divided into two groups by different degree of blood glucose,anti-diabetic therapeutic regiments,disease course,age,diabetic education,degree-educated,self-monitoring blood glucose.The levels of hemoglobin A1c in two groups were compared.Results There were significant changes in the levels of hemoglobin A1c between fasting blood glucose (FBG) ≤ 6.1 mmol/L group (30 cases) and FBG > 6.1 mmol/L group (48cases),2 h postprandial blood glucose (2hBG) ≤8.0 mmol/L group (32 cases) and 2hBG > 8.0 mmol/L group (46 cases ),insulin treatment group (27 cases) and no-insulin treatment group ( 51 cases ),combined drugs treatment group ( 36 cases ) and single drug treatment group ( 15 cases),age ≤ 65 years old group (41cases) and age >65 years old group (37 cases),disease course≤ 10 years group (39 cases) and disease course > 10 years group( 39 cases),degree-educated above high grade group( 34 cases) and degree-educated below high grade group (44 cases),diabetic education ≥2 times/month group (20 cases) and diabetic education < 2 times/month group (58 cases),self-monitoring blood glucose≥2 times/week group ( 19 cases)and self-monitoring blood glucose < 2 times/week group (59 cases)[ (6.7 ± 1.5 )% vs.(7.9 ± 1.3 )%,(6.8 ±1.1)% vs.(7.8 ± 1.2)%,(6.7 ± 1.5)% vs.(8.1 ± 1.4)%,(6.8 ± l.0)%vs.(8.0 ± 1.6)%,(6.9 ± 1.7)% vs.(7.4 ± 1.6)%,(6.5 ± 1.2)% vs.(8.2 ± 1.3)%,(6.9 ± 1.0)% vs.(7.6 ± 1.4)%,(6.1 ± 1.7)% vs.(8.0 ±1.1 ) %,(6.7 ± 1.1 )% vs.(7.6 ± 1.2)% ] (P < 0.01 or < 0.05 ).Conclusion Blood glucose,anti-diabetic therapeutic regiments,disease course,diabetic,degree-educated,self-monitoring blood glucose are the important factors influencing hemoglobin A1c.
3.Status and training of preoperative anesthetist visit in anesthesia residents

Objective To investigate the resident performance on preoperative anesthetist visit in Beijing hospitals,thereby providing introduction for further training.Methods A self-designed questionnaire was distributed among the anesthesia residents who were receiving anesthesia residency training in Beijing through WeChat.The questionnaire covers geographic data of the residents,information on preoperative visit and existing training program.Results160 questionnaires were reclaimed.History-taking and physical examination were not comprehensive in many residents,the nature of surgery was not evaluated by most residents.The capacity of risk assessment and risk informing were not competent in many residents.The most desired training methods for preoperative visit were scenario simulation and bedside observation.Conclusions Scenario simulation with standard patient may have a promising prospect in preoperative anesthetist visit training.
4.Optimizing the Ultrasonic Extraction of Gardenia Oils from Gardenia Jasminoides Ellis by Response Surface Methodology

This study aimed to optimize the ultrasonic extraction of gardenia oils from Gardenia jasminoides Ellis.by response surface analysis methodology (RSM).The duration of extraction,ratio of liquid to materials and extracting times were selected as impact factors based on single-factor experiment.RSM was adopted to learn the effects of the three factors on the extraction rate of Gardenia oil from Gardeniajasminoides Ellis.As a result,it was found that the duration of extraction was 28.7 min,while the ratio of liquid to materials was 11.62:1,and the number of extracting times were 3.36.Under this extracting condition,the average extraction rate of the oil was 13.5% according to three validation experiments,fitting the predicted value well.In conclusion,it was demonstrated that the ultrasonic extraction method effectively improved the extraction rate of gardenia oil,providing a certain basis for the development and utilization of gardenia oil.
5.Clinical significance of circulating tumor cells detection in small cell lung cancer patients

Objective To investigate the clinical significance of circulating tumor cells (CTCs) in pa-tients with small-cell lung cancer patients (SCLC). Methods 42 SCLC patients, 10 patients with benign lung disease(hospitalized from June 2012 to June 2013), and 10 healthy volunteers from the First Affiliated Hospital of Shihezi University Medical College were studied and their clinical information was collected. Negative immuno-magnetic separation method and immunocytochemistry method were applied to collect the Pan-CK +/CD45 - of CTCs, in order to analyze the relationship between CTCs and clinical manifestation. Results CTCs were found in 32 cases of SCLC group, with a positive rate of 76.19%. CTCs was not found in peripheral blood in control group. Difference in the two groups is significant (P = 0.000). The presence of CTCs was correlated positively with the patients′ PS score , clinical stage , M stage , the number of metastatic sites , NSE value and LDH value (P < 0.05). Conclusion The CTCs number is correlated with patients′ PS score, clinical stage, M stage, the number of metastatic sites, NSE value and LDH value, respectively. CTCs detection is useful for more accurate risk assessment of patients.
6.The relationship between circulating tumor cells and clinicopathologic characteristics of primary lung cancer patients

Objective To investigate the relationship between the presence of circulating tumor cells (CTC) and the clinicopatho-logic characteristics of primary lung cancer patients .Methods Samples of peripheral blood(7 .5 mL) from 65 cases of patients with lung cancer(lung cancer group) and 10 cases of patients with lung benign diseases and 10 healthy volunteers (control group) ,and then those CTC were enriched by negative immunomagnetic separation with anti-CD45 antibody and then dectected by immunocyto-chemistry with anti-pan-cytokeratin antibody .The relationship between the positive rate of CTC and clinicopathologic characteristics of lung cancer patients were analyzed .Results The positive rate of CTC in lung cancer patients was 69 .23% (45/65) ,while CTC was not detected in any patients with lung benign disease or healthy volunteers .The positive rate of CTC in patients at TNM stageⅠ - Ⅱ ,Ⅲ and Ⅳ was 23 .08% (3/13) ,50 .00% (6/12) and 90 .00% (36/40) ,respectively .There was significant difference in posi-tive rate of CTC between patients at different TNM stages (P= 0 .000) .The positive rate of CTC in metastatic (stage M1 ) lung cancer patients(83 .33% ,35/42) was significantly higher than that in non-metastatic(stage M0 ) lung cancer patients(43 .48% ,10/23)(P=0 .002) .With the increasing number(0 ,1 ,2 ,3) of metastatic sites ,the positive rate of CTC increased (53 .48% ,83 .33% , 87 .50% ,75 .00% ) ,and there was significant difference in different positive rates of CTC (P=0 .024) .There was statistically sig-nificant difference in the positive rate of CTC between Cyfra 21-1 normal group(53 .33% ,16/30) and high value group(92 .00% ,23/25)(P=0 .021) ,the same with LDH normal group(55 .00% ,22/40) and high value group(82 .86% ,29/35)(P=0 .004) .Conclu-sion CTC were presented in some lung cancer patients .The presence of CTC in peripheral blood is correlated positively with the patients′clinical stage ,M stage ,the number of metastatic sites ,Cyfra21-1 value and LDH value .CTC detection is useful for more accurate risk assessment of patients with lung cancer .
7.Clinical application of immediate latissimus dorsal myocutaneous flap in breast reconstruction after partial re-moval of breast

Objective To study the immediate conservative breast surgical reconstruction by trans-ferring the same lateral latissimus dorsal myocutaneous flap (LDM) for the treatment of stage Ⅱ and stage Ⅲ breast cancer, combined with neoadjuvant chemotherapy. Methods Breast cancer patients in stage Ⅱ and Ⅲ with pathological diagnosis by core needle biopsy, had undergone immediate conservative breast reconstruction surgery with transferring the latissimus dorsal myocutaneous (LDM). We scored the reconstructed breast twice at one month after operation and completion of radiation. The final score was decided using the mean value of the twice evaluating score. Kaplan-Meier survival was used to analyze the survival of 18 cases with traditional mastectomy. Results The tumor size ranged from 30 mm to 55 mm before neoadjuvant chemotherapy, and ranged from 25mm to 45 mm after neoadjuvant chemotherapy. The median weight of the tumor specimens in breast conservative operation was 140 g (90 g to 220 g). A-mong the 18 patients, 16 cases had more than 3 scores with satisfied cosmetics (93.33 % ). Dorsal subcu-taneous seroma in donation area was observed in 5 of 18 patients (27.78 %) and dorsal incision dehiscence was observed in 2 of 18 patients (11.11%). None of the patients developed recurrence except that 1 pa-tient (5.55 %) had distance metastasis (bone metastasis} in a mean 22-months follow-up. The Kaplan-Meier survival curve in breast conservation reconstruction patients had no different from that of traditional mastectomy patients at the same time. Conclusions Immediate conservative breast surgical reconstruction by using the same lateral latissimus dorsal myocutaneous flap is an effective method for stage Ⅱ and stage Ⅲ breast cancer patients combined with neoadjuvant chemotherapy, which can increase satisfactory breast so that cosmetic outcome can be reached.
8.Relationships of alcohol consumption to the prevalence of metabolic syndrome and its related components in Yichang professional population

Objective To study the relationships of alcohol consumption to the prevalence of metabolic syndrome and its re‐lated components in Yichang professional population and provide decision making basis for lifestyle intervention to MS crowd in the city .Methods We examined baseline data from 3 640 participants (2 285 men and 1 355 women) aged 20 to 70 years who under‐went a careful drinking habits survey ,then physical examination and blood chemical examination .They were divided into three groups by average ethanol intake [non ,occasional (<10 g/day) ,moderate (≥10 and <30 g/day) ,and heavy (≥30 g/day) drink‐ers] .Results Compared with no drinking and occasional drinking group ,the prevalence of MS and abnormal rate of BMI ,DBP ,TG , UA increased significantly (P< 0 .05) while SBP decreased significantly in moderate drinking group;compared with moderate drinking group ,the prevalence of MS and abnormal rate of BMI ,DBP and SBP ,GLU ,TCH ,TG ,LDL C ,UA increased significantly (P<0 .05) in heavy alcohol consumption group;whether moderate or heavy drinking ,abnormal rate of CRP and HDL C did not change significantly(P>0 .05) .Conclusion Even moderate drinking has a significant effect to BMI ,DBP ,TG ,LDL C ,UA ,but not obvious to CRP ,GLU ,TCH ,HDL C .Excessive alcohol consumption has a very bad effect on BMI ,DBP ,SBP ,GLU ,TCH ,TG , HDL C ,LDL C ,UA .Both the prevalence of MS and the abnormal rate of its related components are proportional to the alcohol in‐take .Although moderate drinking could lower SBP and GLU ,but can not reduce the incidence of MS .
9.Comparison of PG-SGA, NRS 2002 and BIA in nutritional assessment and screening of patients with gynecologic cancers

Objective:To evaluate the application of PG-SGA,NRS 2002 and BIA in nutritional assessment and screening of patients with gynecologic cancers.Methods:118 patients were randomly selected.Nutritional status were evaluated by PG-SGA,NRS 2002 and BIA,and consistency between each tools were compared.Results:The prevalence of malnutrition or nutritional risk of patients were 64.4% (PG-SGA),57.6% (NRS 2002),and 33.9% (BIA) respectively.In all patients,the consistency of PG-SGA and NRS 2002 was high (P < 0.001),while there were not significantly consistent between BIA and PG-SGA,or between BIA and NRS 2002 (P < 0.001).Conclusion:According to the evaluation of PG-SGA or NRS 2002 in gynecologic patients,the prevalence of malnutrition or nutritional risk is high,and these two scales are suitable for nutritional assessment and screening of gynecologic cancer patients,especially in ovarian cancer patients.In addition,BIA may be a promising tool to evaluate cervical cancer patients' nutritional status.
10.Association of EB virus-induced pharyngitis with mesenteric lymphadenitis in children

Objective To analyze the clinical correlation between EB virus-induced pharyngitis with gastrointestinal symptoms and mesenteric lymphadenitis in children , and to explore the value of EBV-VCA-IgM dectection and color Doppler ultrasound in the diagnosis of EB virus-induced mesenteric lymphadenitis. Methods 356 children with pharyngitis complicated by gastrointestinal symptoms were prospectively analyzed. 162 patients who had a postive result of EBV-VCA-IgM detection by ELISA were assigned to a study group , while another 194 patients who had a negative result were assigned to a control group. The size , number and blood flow of mesenteric lymph nodes were determined by color Doppler ultrasound in both groups. The data were counted , compared and analyzed. Results 43 children had mesenteric lymphadenitis in the study group , so had 11 in the control group. There was a significant difference between the two groups (P < 0.01). Conclusions Children with pharyngitis complicated by gastrointestinal symptoms may suffer from mesenteric lymphadenitis. ELISA for EBV-VCA-IgM detection and color Doppler ultrasound have certain clinical value in the diagnosis of mesenteric lymphadenitis indcued by EB virus infection.
