1.An Experimental Study of Self-made Directional Arcuate Bone Drill in Percutaneous Vertebroplasty

Objective To explore the application value of self-made directional arcuate bone drill during vertebroplasty by observing its operative effects for osteoporosis in vitro . Methods EDTA-Na2 was used to soak the prepared vituline osteoporotic vertebral bodies in vitro .A total of 40 osteoporotic vertebral bodies were randomly divided into Group A and B , with each group having 20 vertebral bodies for vertebroplasty .The Group A underwent a puncture by routine straight bone drill , while the Group B received a puncture by self-made directional arcuate bone drill with the arc directing toward the contralateral , which terminated when the drill reached the first 1/3 of vertebral body .Finally, statistical analysis was conducted on the distance between the bone drill bit and exterior margin of contralateral vertebral body , as well as whether bone cement reached or went over the vertebral sagittal midline . Results The osteoporotic vertebral bodies were successfully prepared in vitro by using EDTA-Na2 immersion decalcifying for 9 days. In the Group A, the drill bit was (2.50 ±0.32) cm away from contralateral exterior margin of vertebral bodies , which was significantly different from that in the Group B (0.90 ±0.26) cm (t=17.354, P=0.000).The bone cement reached or went over the vertebral sagittal midline in 11 vertebral bodies in the Group A and in 19 vertebral bodies in the Group B , with statistical difference ( Fisher’ s test,P=0.004).The intraspinal bone cement leakage occurred in 9 vertebral bodies in the Group A and 4 in the Group B, without significant difference between the two groups (Fisher’s test,P=0.176). Conclusion The self-made directional arcuate bone drill can build an osseous channel that reach or go over the vertebral sagittal midline and guide bone cement distributed to contralateral puncture , which avoids the disadvantages of bilateral vertebral pedicular puncture .The result shows that the self-made directional arcuate bone drill has more adventages in vertebroplasty than that of traditional straight bone drill but doesn ’ t have significant advantages in preventing bone cement leakage .

4.Influence of Tongluo Tangtai Recipe on Inflammatory Cytokines in Rats with Diabetic Peripheral Neuropathy

Objective To investigate the effect of Tongluo Tangtai Recipe(TTR)on inflammatory cytokines in rats with diabetic peripheral neuropathy(DPN). Methods Rat model of spontaneous DPN was established by feeding high fat diet. The rats were divided into 5 groups , namely model group , western medicine(metformin 100 mg·kg-1·d-1+ methycobal 0.2 mg·kg-1·d-1) group, and low-, middle- and high-dose TTR groups (TTR in the dosage of 0.72, 1.44, 2.88 g·kg-1·d-1 respectively). Rats of the normal group and model group were treated with normal saline orally, and the medication groups were given the corresponding medicine according to the experimental design. After intragastric administration for 4 weeks, sciatic motor/sensory nerve conduction velocity was detected with electrophysiological examination method, and tumor necrosis factor alpha(TNF-α), interleukin-6(IL-6), C-reactive protein(CRP), myelin basic protein(MBP) levels in the serum were examined by enzyme-linked immunosorbent assay(ELISA). Results TTR in various dosages could reduce the serum levels of MBP and inflammatory cytokines of TNF-α, IL-6 and CRP, and increase sciatic nerve conduction velocity (P<0.05 or P<0.01 compared with those in the model group), the high dose showing the strongest effect. Conclusion TTR could inhibit inflammation reaction and nerve demyelination, and improve nerve structure and conduction function, which may contribute to the preventive and therapeutic mechanism of TTR for DPN.
5.Effect of combination of dexmedetomidine and sufentanil on postoperative analgesia for highly nicotine dependent patients undergoing esophagectomy

Objective To observe the effect of combination of dexmedetomidine and sufentanil on postoperative analgesia for the highly nicotine dependent patients undergoing esophagectomy. Methods Ninety highly nicotine dependent patients undergoing esophagectomy were randomly allocated into three groups (n = 30 each): low-dose sufentanil group (Group S1), high-dose sufentanil (Group S2), dexmedetomidine and sufentanil group (Group DS). All patients of three groups received postoperative PCIA with following setting: 2 mL/h;bolus: 2 mL; lock time: 5 min; 4 hours limited: 40 mL. The scores of VAS and Ramsay, effective presses/total presses of PCIA, the consumption of sufentanil during 72 h after operation, side effects and the satisfaction degree of patients were recorded. Results Compared with those of group S2, the scores of VAS (both at rest and movement) decreased significantly in group DS from 1 h to 8 h (P<0.05). Compared with those of group S1, the scores of VAS (both at rest and movement) decreased significantly in group S2 and DS from 1 h to 72 h (P<0.05). Compared with that in group S1 and S2, the consumption of sufentanil during 72 h after operation decreased significantly in group DS (P < 0.05). Compared with those in group DS, The numbers of nausea and vomiting were significant decreased in group S1 and S2(P < 0.05). Compared with those in group S1, the remedial cases were significantly decreased in group S2 and group DS (P < 0.05). Conclusion The effect of combination of dexmedetomidine and sufentanil is superior to that of sufentanil in terms of postoperative analgesia in highly nicotine dependent male patients undergoing esophagectomy.
6.The expression of PKM2 and HIF-1α in laryngeal squamous cell carcinoma and its clinical indication.

OBJECTIVE:
The purpose of this study was to investigate the expression of PKM2 and HIF-1α in laryngeal squamous cell carcinoma and to analyze their correlation in laryngeal squamous cell carcinoma.
METHOD:
Total 37 laryngeal carcinoma samples and para-carcinoma tissues were taken from the patients who accepted operation treatment in the Second Hospital of HeBei Medical University from 06/2013 to 06/2014. The protein expression levels of PKM2 and HIF-1α were detected with SP immunohistochemical methods. The data were analyzed by the SPSS 13.0 statistical software.
RESULT:
The positive expression of PKM2 in laryngeal carcinoma tissues and adjacent tissues were 62.16% and 13.15%. The difference was statistically significant (P < 0.01). The positive expression of HIF-1α in laryngeal carcinoma tissues and adjacent tissues were 64.86% and 21.62%. The difference was statistically significant (P < 0.01). The positive expression of PKM2 and HIF-1α in well differentiated laryngeal squamous cell carcinoma were both 47.83%, while in medium and poorly differentiated laryngeal squamous cell carcinoma were 85.71% and 92.86% respectively. The difference was statistically significant (P < 0.05). The positive expression of PKM2 and HIF-1α in patients with lymph metastasis were 90.00% and 100.00% respectively, 51.85% in those without lymph metastasis, the difference was statistically significant (P < 0.05). The rate of HIF-1α positive expression in I-II stage was 53.85%, 90.91% in III-IV stage. The difference between the two groups was statistically significant (P < 0.05). The expression of PKM2 and HIF-1α had no relationship with the age and smoking (P > 0.05). The expression of PKM2 was positively related with HIF-1α in laryngeal squamous cell carcinoma (P < 0.01).
CONCLUSION
The expression of PKM2 and HIF-1α are related with the proliferation, invasion and metastasis of laryngeal squamous cell carcinoma. It provides a certain theoretical basis for laryngeal cancer diagnosis and screening to measure the expression of PKM2 and HIF-1α as biological indicators.

7.The expression of Nrf2 and Keap1 in laryngeal carcinoma and its clinical indication.

OBJECTIVE:
To investigate the role of Keap1-Nrf2-ARE pathway in laryngeal carcinoma occurrence and development.
METHOD:
Thirty-five laryngeal carcinoma samples and para-carcinoma tissues were taken from the patients who accepted operation treatment in our hospital from Feb 2012 to Sep 2013. The expression levels of Nrf2 and Keap1 were detected in 35 cases of laryngeal carcinoma with SP immunohistochemical methods. The data were analyzed by the SPSS 13.0 statistical software.
RESULT:
The expression of Nrf2 in the 35 cases of laryngeal carcinoma was 77.14%, which in the para-carcinoma tissues was 8.57%, the difference between these two groups was statistically significant (P < 0.01). The expression of Keap1 in the 35 cases of laryngeal carcinoma was 71.43%, which in the para-carcinoma tissues was 31.43%, the difference between these two groups was statistically significant (P < 0.01). The expression of Nrf2 in I -II stage was 65.00% and in III-IV stage was 93.33%, the difference was significant (P < 0.05). The expression of Keap1 in I-II stage was 55.00% and in III-IV stage was 93.33%, the difference was significant (P < 0.05). Of the 35 cases, the positive expression rate of Nrf2 and Keap1 in laryngeal carcinoma with lymphnode metastasis were both 100.00%, compared with those without lymph nodes metastasis 68.00% and 60.00%, the difference was significant (P < 0.05). The expression of Nrf2 and Keap1 had no relationship with tumor differentiation, smoking and patient age (P > 0.05).
CONCLUSION
Keap1-Nrf2-ARE pathway may play an important role in progression of laryngeal squamous cell carcinoma.

8.Quantitative detection of minimal residual leukemia cells in peripheral blood and clinical application

Objective To explore the clinical application significance of quantitative detection of minimal residual leukemia cells in peripheral blood in acute leukemia patients.Methods Using PCR amplification of quantitative method of limited dilution,the FLT3 gene was detected in blast cells of peripheral blood from 25 newly diagnosed cases of acute leukemia,and the number of minimal residual leukemia cells in newly diagnosed cases and the cases after one course of chemotherapy was calculated respectively.10 healthy subjects were used as control group.Results ①The positive rate of FLT3 gene in 25 newly diagnosed cases of acute leukemia sample was 80%(20/25),the positive rate in acute myeloblastic leukemia(AML) patients was 88.2%(15/17)and in acute lymphocytic leukemia(ALL)was 62.5%(5/ 8).②The mean DNA content in peripheral blood in 20 cases with FLT3 positive expression in newly diagnosed group was(2.36?1.25)?108 ?g?L-1,being equal to(18.66?8.79)?106 leukemia cells in every microliter peripheral blood.After one course of chemotherapy ,there was 1 case without remission,the DNA content in peripheral blood was 1.69?107?g?L-1,being equal to 1.01?106 leukemia cells in every microliter peripheral blood;there were 3 cases with partial remission,the mean DNA content in peripheral blood was(0.57?0.24)?106 ?g?L-1,being equal to(1.82?0.19)?103 leukemia cells in every microliter peripheral blood.There were 9 cases with complete remission with FLT3 negtive expression,and 7 cases with complete remission with FLT3 positive expression,the mean DNA content in peripheral blood was(0.16?0.06)?106 ?g?L-1,being equal to(1.86?1.31)?102 leukemia cells in every microliter peripheral blood.Conclusion The quantitative and periodic detection of minimal residual leukemia cells would help to evaluate leukemia chemotherapy efficiency and to adjust treatment scheme in time.
9.Increased expression of 5-hydroxytryptamine transporter gene in monocrotaline-induced pulmonary hypertension in rats

Aim To study the relationship between pul-monary hypertension (PHT) and 5-hydroxytrapta-mine transporter (5-HTT) in the pulmonary arteries (PAs) from normal and monocrotaline (MCT) induced pulmonary hypertension rats. Methods MCT-treated rats were used as a model for chronic PHT.Concentration-response curves of 5-hydroxytraptamine induced pulmonary vasoconstriction were established. The medial wall thickness of pulmonary arteries was measured. Semi-quantitative RT-PCR was performed to identify the expression of 5-HTT mRNA in rat PAs.Results 5-HT induced vasoconstriction response of PAs from MCT rats was significantly increased.The thickness of pulmonary vascular medial walls was increased in MCT rats. A significantly higher level of 5-HTT mRNA expression was detected in PAs from MCT rats. The ratio of the PCR products of 5-HTT gene to those of ?-actin gene was higher in MCT rats than in control rats. 5-HTT mRNA expression of pulmonary arteries correlated with the thickness of pulmonary vascular medial walls in rats.Conclusions MCT-induced pulmonary vascular remodeling and increased vascular contractile response to 5-HT were accompanied with enhanced level of 5-HTT mRNA expression and there existed correlation between the wall thickness of pulmonary arteries and 5-HTT mRNA expression,indicating 5-HTT is an important mechanism involved in pulmonary hypertension.
