3.Clinical study on treatment of acute urinary retention after anorectal surgery by comprehensive treatment of TCM

Objective Clinical observation of the effect of comprehensive treatment of TCM on the patients with Acute Urinary Retention after Anorectal Surgery. Methods A total of 90 patients with acute urinary retention after anorectal surgery were selected. The patients were divided into control group and treatment group by random number table method. The control group was treated with intramuscular injection of neostigmine. The treatment group was treated with comprehensive treatment of TCM. Both groups were observed over 24 h. The voiding patency score, abdominal fullness score, urination time, the average effective time, the first urinary bladder residual urine output and 2 h after treatment the total urination were detected and compared. Results The total effective rate was 88.89% (40/45) in the treatment group and 64.44% (29/45) in the control group. The total effective rate of the two groups was statistically significant (Z=-2.131, P=0.033). After treatment, the levels of urine patency score (0.94 ± 0.66 vs. 1.87 ± 0.74, t=-2.433), belly fullness score (0.72 ± 0.26 vs. 1.39 ± 0.35, t=-2.185), first urination time (35.72 ± 10.43 min vs. 58.69 ± 12.85 min, t=-2.210), average effective time (60.23 ± 14.60 min vs. 97.57 ± 17.33 min, t=-2.980), the urinary bladder after the first urination (23.87 ± 15.29 ml vs. 65.14 ± 17.53 ml, t=-2.125) in the treatment group were significantly lower than those in the control group (P<0.05). And total urination within 2h after treatment (839.77 ± 106.35 ml vs. 0.46 ± 0.55 ml, t=-2.003) in the treatment group were significantly higher than that in the control group (P<0.05). Conclusions Comprehensive treatment of TCM can significantly reduce the degree of urination and abdominal fullness fullness of the score, shorten the first timeand the average duration of urination, reduce the amount of urine after the first urination of the patients with Acute Urinary Retention after Anorectal Surgery.

The effective ingredients such as curcumin, gambogic acid, neogambogic acid, matrine, Oxymatrine, cinobufagin, sanggen baicalin, Scutellaria barbata extract, etc., and the compound ingredients such as Sijunzi Decoction, Jiawei- Wumei Pill, Huangqin Decoction, etc, which could inhibit the proliferation and invasion of tumor cell through multiple channels and multiple targets, and inducing tumor cell apoptosis, and enhance the function the body's immune system and chemotherapy drugs.
6.Clinical observation of Qingjie-Painong decoction combined with debridement drainage in the treatment of perianal necrotizing fasciitis

Objective To observe the clinical effect of Qingjie-Painong decoction combined with debridement drainage in the treatment of perianal necrotizing fasciitis.Methods A total of 78 patients with perianal necrosis fasciitis were randomly divided into two groups,39 patients in each group.The two groups received the basic treatment of anti-infection,and the control group was treated with debridement drainage,and the treatment group applied the Qingjie-Painong decoction combined with debridement drainage treatment.Both groups were treated for 14 days.Before and after treatment,systemic symptom score,lesion range score,perianal pain score were assessed.The levels of WBC,hs-CRP,IL-6 and TNF-α were measured and the clinical efficacy was evaluated after treatment.Results The total effective rate was 94.88% (37/39) in the treatment group and 82.05% (32/39) in the control group.The total effective rate of the two groups was statistically significant (Z=-2.099,P=0.036).After treatment,the Systemic symptom (0.21 ± 0.05 vs.0.98 ± 0.17,t=-2.601),Lesion range (0.57 ± 0.23 vs.1.08 ± 0.40,t=-2.270),Perianal pain (0.92 ± 0.35 vs.1.77 ± 0.49,t=-3.493) in the treatment group were significantly lower than those in the control group (P<0.05).After treatment,the WBC (6.25 × 109/L ± 2.08 × 109/L vs.10.98 × 109/L ± 2.72 × 109/L,t=-3.829),CRP (1 1.20 ± 2.41 mg/L vs.23.34 ± 4.39 mg/L,t=-2.162),IL-6 (13.03 ± 3.36 pg/ml vs.24.69 ± 4.52 pg/ml,t=-2.297),TNF-αt (8.33 ± 3.16 ng/ml vs.14.93 ± 3.80 ng/ml,t=-3.340) in the treatment group were significantly lower than those in the control group (P<0.05).Conclusions The Qingjie-Painong decoction combined with debridement drainage in the treatment of perianal necrotizing fasciitis can significantly reduce the body's inflammatory status,improve systemic symptoms,reduce the scope of the lesion,reduce perianal pain,and improve clinical efficacy.
7.Clinical study of Wuwei-Xiaodu decoction lavage assisted vacuum sealing drainage (VSD)in the treatment of perianal abscess pus period

Objective To evaluate the clinical effect of Wuwei-Xiaodu decoction lavage assisted VSD in the treatment of perianal abscess pus period. Methods Ninety patients with perianal abscess who met the inclusion criteria were divided into 2 groups by random number table, 45 in each group. The control group was treated with 0.9% sodium chloride injection assisted VSD. The treatment group was treated with Wuwei-Xiaodu decoction lavage assisted VSD treatment. Both groups were treated for 7 days. WBC was detected by automatic hemorheology tester, CRP, IL-6 and TNF-α were detected by ELISA. The anus bulge, anal self-made function, wound exudation, wound pain were scored and the incision bacteria were detected after treatment. The rate and number of colonies were evaluated. Results The total effective rate was 88.89% (40/45) in the treatment group and 73.33% (33/45) in the control group. The total effective rate of the two groups was statistically significant (Z=-2.584, P=0.010). After treatment, the levels of WBC, CRP, IL-6, and TNF-α in the treatment group were significantly lower than those in the control group (t=-3.730, -2.385, -2.170, -3.509, P=0.007, 0.023, 0.034, 0.011). The anal bulge, wound exudation, and wound pain scores in the treatment group were significantly lower than those in the control group (t=-2.760, -2.549, -21.151, P=0.005, 0.017, 0.038),. The rate of incision bacteria was significantly lower than that in the control group (χ2=4.406, P=0.036), the number of colonies were significantly lower than that of the control group (t=5.375, P<0.01). Conclusions The Wuwei-Xiaodu decoction lavage assisted VSD can effectively reduce the number of colonies in the perianal period of patients with perianal abscess, reduce postoperative anal bulge, pain and wound exudation, and improve clinical efficacy.
8.Effect of Neibu Huangqi Decoction combined with Kangfuxin Liquid on wound healing after hemorrhoid fistula

Objective:To investigate the effects of Neibu Huangqi Decoction combined with Kangfuxin Liquid on wound healing after hemorrhoid fistula.Methods:Randomized controlled trial. A total of 90 patients with hemorrhoid fistula surgery in Tangshan Hospital of Traditional Chinese Medicine from January 2020 to June 2021 were selected as the observation objects and divided into 2 groups by random number table method, with 45 cases in each group. The control group was treated with Kangfuxin Liquid after surgery, and the observation group was treated with Neibu Huangqi Decoction. Both groups were treated continuously for 14 days. Wound symptom score was performed before and after treatment. The levels of TNF-α, IL-6 and IL-8 were determined by ELISA. The wound healing time was observed and the wound healing rate was calculated. Adverse reactions were recorded and clinical efficacy was evaluated.Results:The total effective rate was 93.33% (42/45) in the observation group and 66.67% (30/45) in the control group, with statistical significance ( χ2=9.89, P=0.002). After treatment, the scores of pain [(0.63±0.14) vs. (0.97±0.27), t=7.50], exudation [(0.67±0.12) vs. (1.09±0.31), t=8.48], edema [(0.78±0.17) vs.(1.25±0.36), t=7.92], pruritus [(0.78±0.20) vs. (1.32±0.33), t=9.39] were lower than those in the control group ( P<0.01); serum TNF-α [(33.46±2.86) μg/L vs. (45.78±3.92) μg/L, t=25.39], IL-6 [(41.86±5.84) μg/L vs. (56.12±6.75) μg/L, t=15.98], IL-8 [(27.40±3.58) ng/L vs. (36.16±3.84) ng/L, t=16.69] were lower than those in the control group ( P<0.01). The wound healing time of the observation group was shorter than that of the control group ( t=8.60, P<0.01), and the wound healing rate was higher than that of the control group ( t=24.65, P<0.01). During treatment, the incidence of adverse reactions was 11.11% (5/45) in the observation group and 6.67% (3/45) in the control group, without statistical significance ( χ2=0.14, P=0.711). Conclusion:Neibu Huangqi Decoction combined with Kangfuxin Liquid can promote wound healing, reduce inflammatory cytokines, relieve pain and exudation, improve clinical efficacy, and have few adverse reactions.
