1.Mucha - Habermann's Disease ( Pityriasis Lichenoides et Varioliformis Acuta ): Report of A Case.

Muchz-Habermann disease is cutaneous disease of unknown etilogy and characterized by a, polymorphous eruption consisting of papulosquamous, bemorrhagic, ulceronecrotic lesion eventually into varioliform scars. It is also classified into parapsoriasis group according to Broq with parapsoriasis en guttata (pityriasis lichenoides chronica), parapsoriasis en plaques, parapsoriasis en lichenoides. The 61-year-old male patient visited to Dermatologic Department of Severance Hospital due to sudden onset of generalized eruption. He presented pea to fingr tip sized erythematous, silvery maculo-papulo-squamous eruptions on trunk, extre- mities and buttock, but didnt complain of any subjective symptoms except markel fatigue. Skin biopsy was done on right forearm and was confirrr.ed to pityriasis lichenoides et varioliformis acuta by histopathologic finding. The lesion was completely controlled by oral tetracycline 2. Ogm per daily for first 4 wecks and reduced dosage l. Ogin per day for second 2 weeks.

2.Immunohistochemical Application of Leukocyte Common Antigen and Epithelial Membrane Antigen in the Diagnosis of Large Cell Undifferentiated Tumors.

A total of 42 cases undifferentiated large cell tumors were stained by immunoperoxidase techniques using antibodies against leukocyte common antigen (LCA) and epithelial membrane antigen (EMA). In 18 of the 21 cases studied, initially diagnosed as malignant lymphoma or Hodgkin's disease, reactivity with monoclonal anti-LCA (and noreactivity with monoclonal anti-EMA)indicated that the tumor was a lymphomas. The remaining 3 cases gave the reverse reaction pattern and therefore were classified as carcinoma. One out of 16 cases diagnosed as undifferentiated carcinoma proved to be a case of mialignant lymphoma in 5 patients in whom the original diagnosis was uncertain, a definite diagnosis was possible in all cases and 3 of these proved to be large cell lymphoma; the remainders, undifferentiated carcinoma. It is suggested that the staining of undifferentiated human neoplasms using combinations of antibodies reactive with epithelial and lymphoid cells may result in much greater diagnostic accuracy.

4.The Influence of TURP on Detrusor Instability in Patients with BPH: A Clinical and Urodynamic Analysis.

PURPOSE: Detrusor instability (DI) is a main cause of persistent voiding difficulty after TURP in patients with benign prostatic hyperplasia (BPH). So we retrospectively estimated the effect of TURP in BPH patients with DI. MATERIALS AND METHODS: Of the 81 patients who had undergone TURP due to prostatism, 35 patients with BPH were followed at 3 weeks and 6 months postoperatively (21 without DI cases vs. 14 with DI; 6 persistent DI, 7 resolved DI, 1 missing case at 3 weeks and 4 persistent DI, 7 resolved DI, 2 missing cases at 6 months). We compared each group using symptom score (IPSS) and urodynamic parameters. RESULTS: Each irritative and obstructive symptoms were significantly improved in both group (p<0.05). The maximal flow rate (from 12.2 to 25.4ml/sec), average flow rate (from 8.9 to 15.2ml/ sec), residual urine volume (from 135.9 to 39.1ml) in patients without DI and maximal bladder volume (from 150.0 to 203.7ml), maximal flow rate (from 12.5 to 18.6ml/sec), residual urine volume (from 65.7 to 26.0ml) in patients with DI were improved significantly (p<0.05) after TURP, but other parameters were not improved (p>0.05). There were no significant differences in all parameters between 3 weeks and 6 months postoperative period (p>0.05). The amplitude of improvement had no significant difference between two group. There were no significant differences in IPSS and urodynamic parameters between persistent and resolved DI group. CONCLUSION: We suggest that TURP is good therapeutic option even in BPH patients with DI at short term postoperative period, but attempt to predict which patients would have persistent DI following TURP was failed.

5.Chronotropic Actions of Higenamine in the Isolated Right Atrium of the Rabbit.

Higenamine is known to possess stimulatory activity on beta-receptor of the heart. Chronotropic actions of higenamic were studied in spontaneously beating right atrial muscle isolated from rabbits. The frequency of spontaneous beating and the relative threshold voltage of the right atrium were examined. The relative threshold voltage was defined as the minimal voltage of the given impulse above which the right atrium could be paced at the frequency of 20% of higenamine was also observed. Higenamine caused the postive chronotropic effect. This response became prominent as the ca2+ concentration in the bathing solution lowered. When tetrodotoxin was added to the bathing solution, the effect of higenemine altered and became similar to that of epinephrine. Higenamine reduced the relative threshold voltage of the right atrium in the bathing solution with [ca2+] of 0.5mM. Such effect was abolished by tetrodotoxin. The effects of verapamill on the spontaneous rate and the relativel threshold voltage were inhibited by higenemine. The above results suggest that, aithough the main action og higenamine is on the Ca channel, higenamine also have a minor effect of augmenting the Na channel.

6.The Topical Effects of Antibiotics and Aprotinin in Experimental Pseudomonas Ketaititis.

The effects of the topically applied antibiotics and Aprotinin were evaluated in the experimentally induced pseudomonas aeruginosa keratitis in rabbits. 40 albino rabbits were used for this study and were divided into 8 groups according to the treatment protocols as follows: Aprotinin 40u/ml, 1,OOOu/ml, 1O,000u/ml, antibiotics (Ciprofloxacin and Tobramycin), and antibiotics and Aprotinin combined-treated group. The drugs were instilled in the rabbits, cornea 6 times per day for 2 weeks. The clinical evaluation was performed using a hand-held slit lamp every day. For the histopathologic examination, we enucleated every samples at different time intervals and prepared specimens for light microscope. The results were Aprotinin-treated group showed progression of ulcer infiltrates regardless of its concentrations. There was a statistically significant difference between the antibiotics-treated group and the control group (p<0.05). However, the combination of Aprotinin and antibiotics were not able to gain statistical significance when compare with antibiotics alone (p>0.05). There was no clinical and histopathological differences in the rabbit cornea between ciprofloxacin and tobramycin treated group (p>0.05).

8.Longitudinal Study of Motor Evoked Potential in Patients with Acute Ischemic Stroke.

BACKGROUND: The prognostic value of motor evoked potentials(MEP) by transcranial magnetic timulations(TMS) in acute stroke has not been well established. To determine the value of MEP as predictors of motor and functional recovery we examined correlations of neurophysiological measurements of central motor conduction time(CMCT) and functional assessments. METHOD: A longitudinal study was performed on 32 first-ever acute ischemic stroke patients. The cortical and cervical motor evoked potentials(MEPs) along with the degree of extremity weakness, the NIH stroke scale, and the Barthel index were performed in the period immediately following stroke and after 3-4 months of the onset. RESULTS: Five groups of patients were divided on the basis of MEP responses following the cortical stimulation; initial normal response following the normal one (group I; 11), initial delayed response following a normal or still delayed one (group II; 3/group III; 0), and an initial absent response with the reappearance of CMCT or still absent one (group IV; 8/group V; 10). Patients in group I,II and IV had consistently lower scores in motor weakness degree and NIH scale and higher scores in the Barthel index both at initial and follow-up studies, and achieved significantly better functional recovery(p<0.01). CONCLUSIONS: Discordant results of MEP in predicting the prognosis of stroke may result from the disregard of group IV, which had previously been considered as a poor outcome group solely because no cortical MEP responses were recorded at the initial study. As the MEP alone during the acute stage could not differentiate group IV from group V which had a persistent absence of cortical MEP responses, the evaluation of the neurological deficit at the same time is needed for better prediction of prognosis.

9.Liver in Systemic Lupus Erythematous: Clinicopathological analysis of 8 cases.

Hepatic changes in eight cases of systemic lupus erythematosus(SLE), were studied to evaluate the nature of hepatic injury of SLE and its relation to lupoid hepatitis. Common histologic changes of the liver in SLE included intralobular necro-inflammatory activities, fatty change, canalicular bile stasis and mild perivenular and Disse space fibrosis. The fatty change and intralobular fibrosis were thought not to be due to SLE itself but associated with steroid administration or chronic alcoholic consumption. The necro-inflammatory changes were usually mild and features of chronic active hepatitis with periportal destruction occured in one case. Symptoms related to the involvement of many other organs than the liver appeared much frequently in patients with SLE than lupoid hepatitis, suggesting that the SLE involving liver and the lupoid hepatitis are different diseases.
