The volumetry by cineangiocardiography is known to have the most diagnostic and prognostic value in theevalutaion of various valvular heart diseases and ischemic heart diseases. Athough many authors favor thearea-length method for the voulmetry of left ventricle, 45degrees RAO projection seems to be more simple,inexpensive and acurate technique, considering the positional relationship of cardiac valves and obliquity of thelong axis of left ventricle within the chest cage. Authors present the anatomical, geometrical and radiologicalbasis for the superiority of 45degrees RAO projection by analyzing 20 normal heart specimen and 115cineangiocardiograms of valvular heart disease, and the results as follows; 1. Blood flow and motility of themitral and aortic valves can be more clearly demonstrated by 45degrees RAO projection than by AP view. 2. The longdiameter of left ventricular silhouette made 45degrees RAO projection reflects 90% or more of real diameter. 3. InRAO 45degrees position, patient's left nipple is optimal and convenient level for the ruler offering accuratemagnification coefficient of left ventricle. 4. Ejection fration after the extrasystole is exaggerated regardlessof the left ventricular function, so it is desirable to exclude the 2 or 3 beats after extrasystole.

2.An experimental study on vascular changes in renal biopsy injury

An experimental study on the vascular alternations of the kidney following biopsy procedure was carried out in 47 kidneys from 28 rabbits to clarify their nature and frequency by renal arteriography and microangiography together with histopathologic investigation. Renal arteriography and microangiography were perforemd immediately 2 days, 1 week, and 2 weeks after percutaneous biopsy and the findings were correlated with histological nature. The results are summerized as follows; 1. Important biopsy injuries verified by renal arteriography and microangiography were arterial spasm, perfusion defect, arteriovenous fistula, injury to vasa recta and renaltubules, intrarenal and extrarenal extravasation of contrast media, and arterial obstruction, in order of frequency. 2. Artierial spasm observed in majority of the cases were relieved during the period of 2 weeks. 3. Detectability of perfusion defect was 57% and 72% by angiography and microangiography, respectively, and this perfusion defect seemed to be mostly caused by renal infarction due to vascular injury, such as arteriovenousfistula, arterial obstruction and other vascular injuries. 4. Arteriovenous fistula was detected in 28% byangiography and 50% by microangiography. Many of the arteriovenous fistula appeared to be closed spontaneously within a week. Above findings suggest that renal biopsy procedure results in various degree of vascular injuries with their sequential modification, and that microangiography is assumed the most effective approach in analysisof biopsy injuries such as small arteriovenous fistula, perfusion defect, injury to vasa recta and renal tubules, overcoming the limitation of traditional angiography.

3.Renal metastasis from adenoid cystic carcinoma of salivary gland: report of two cases.

Adenoid cystic carcinoma of the major salivary gland is a relatively rare tumor which is well known for its high local recurrence rate and frequent distant metastasis. Metastasis of this tumor to kidney has not been reported previously to our knowledge. We report two cases of renal metastasis from adenoid cystic carcinoma of the salivary gland.

4.Radiological differentiation of neurogenic tumors in the thorax with plain film and CT

35 neurogenic tumors of the thorax were analyzed with plain film and CT scan. Radiological analysis emphasizedshape, location calcification, bone change, pleural change in plain film and calcification, cystic change, bonechange, pleural change, presence of contrast enhancement in CT scan. The results were as follows; 1. Age may bethe most important clinical factor for differential diagnosis. 2. Plain film findings(35 cases); 1) The mostcommon shape in the ganglion series tumor (ganglioneuroma, ganglioneuroblastoma, neuroblastoma), was elongation(80%) and these tumors generally had tapered borders(50%). In contrast with those of the genglion series, nervesheath tumors(schwannoma, neurofibroma, malignant Schwannoma) showed a definite tendency to be roudn (95%) and thesulcus effect was more frequetnly seen(67.5%). Most of the masses were smooth. The lobulated masses were commonlymalignant ganglion series tumors. 2) Though overall incidence of calcification was low(8.6%), it was observed morefrequently in ganglion series tumors(20%). 3) Associated bony abnormality were generally observed in large tumorsabove 5cm in long axis, especially in neuroblastoma and ganglioneuroblastoma. 4) Tumors showed pleural effusionwere all malignant. 3. CT findings (17 cases); 1) The overall incidence of cystic change of the mass was 23.6%. Itwas most common in Schwannoma(60%). 2) The contrast enhancement was noted in 64.7%. It's degree was variable andshowed no predilection to any specific tumors. 3) The incidence of calcification, associated bony abnormality andpleural effusion were similiar to plain film. Confident specific diagnosis can be made in most of the neurogenictumors of the thorax by shape of the mass in plain film and internal architecture in CT combined with patient'sage as clinical information.

5.A Comparison of 10% Lidocaine Spray and Intravenous 2% Lidocaine on Mean ArterialPressure and Heart Rate Following Tracheal Intubation.

BACKGROUND: Laryngoscopy and tracheal intubation for general anesthesia are potent stimuli to the cardiovascular system due to sympathetic stimulation. The aim of this study was to compare the hemodynamic responses with different administration routes of lidocaine on blood pressure and heart rate changes associated with tracheal intubation. METHODS: Sixty patients were randomly assigned to spray group (Group 1, n=20) received 10% lidocaine 1.5 mg/kg spray to intratracheally immediately before tracheal intubation, IV group (Group 2, n=20) received 2% lidocaine 1.5 mg/kg intravenously before 90 sec tracheal intubation and control group (Group 3, n=20). Anesthesia was induced with thiopental 5 mg/kg IV, vecuronium 0.1 mg/kg and 50% N2O-O2- 2vol % enflurane. After 5 minutes, tracheal intubation was performed. Mean arterial pressure and heart rate were measured at preintubation and immediately postintubation, 1, 3, 5 mins after tracheal intubation. RESULTS:The differences of mean arterial pressure which were measured at preintubation and immediately postintubation and 1 minute after intubation in group 1 were significantly lower than those in other group (p<0.05). The differences of heart rate which measured at preintubation and 1 minute after intubation were lower than those in group 3 (p<0.05). CONCLUSIONS: For the suppression of sympathetic stimulation following tracheal intubation, 10% lidocaine spray to the laryngotrachea is an effective method to suppress cardiovascular response.

6.Histopathologic changes of the craniomandibular joint according to the amount of distraction after 6 weeks of distraction osteogenesis in rabbits.

The purpose of this study is to observe histopathologic changes in the bilateral craniomandibular joints after allowing 6 weeks of consolidation by varying the amount of distraction in rabbit mandible. Eight rabbits weighing about 2 to 3kg were used. After corticotomy was performed on the left mandibular body between the first premolar and the second premolar region, a unilateral fixation device was placed. Then, a 7-day period was allowed without distraction of the device. The mandible was lengthened 0.5mm/day. Corticotomy and lengthening of mandible were not performed in control group. After the completion of the lengthening process, a 6-week-consolidation period was allowed. Then, the rabbits were sacrificed, and histologic examination of the craniomandibular joints was performed. Proliferative changes were observed in the craniomandibular joints in all groups. With the increasing amount of distraction, hypertrophy of the cartilage layer became more severe, bone formed was dense and enchondral ossification was clearly shown in subchondral bone. Hypertrophy of the cartilage layer was also seen in the non-distracted side as the distracted side in the experimental group. These results indicate that when physical force is applied constantly to joints, the proliferation of articular cartilage and bone formation are present. When more than 6 weeks of consolidation period is allowed at the time of performing distraction for more than 5mm, articular changes, especially, in the contralateral side should also be noted.

7.A case of giant cell carcinoma in pancreas

A case of a pleomorphic giant cell tumor of the pnacreas in a 54 year old man is presented. It produced hugecystic mass in the pancreatic area ultrasonographically. Also multiple loculated pancreatic pseudocyst andmultiple low densities in liver were presented in CT. Pathologically this case was a form of giant cell carcinomawhich was called as epulis-osteoid type, but the osteiod component in this case was not conspicuous.

8.Report of two cases of double outlet left ventricle

Double outlet left ventricle (DOLV), even defined as origins of both great vessels from left ventricle,implies varieties of morphology in concern with loop pattern, location of ventricular septal defect, conotruncalanatomy, and presence or adsence of pulmonary stenosis. With the improvement of surgical tenchnique, correctivesurgery of DOLV became possible, and preoperative diagnosis and determination of the accurate anatomicalrelaltionship between great vessels, ventricular septal defect, and ventricles is very important in surgicalimplication. Authors have recently experienced two cases of DOLV. The one is with subpulmonic conus, subpulmonicventricular septal defect, patent ductus arteriosus, and preductal type coarctation of aorta. The other is withbilateral coni and subaortic ventricular septal defect. Loop patterns are [S,D,D] in both cases.

9.Percutaneous transcatheter embolization

Transcatheter embolization was done in 15 patients wtih various diseases at departement of radiology, SeoulNational University Hospital from Sept. 1978 to July 1983 using absolute ethanol, autologous blood clots andgelfoam particles as embolic agents. The results were as follows; 1. Successful embolizations were done in 14 of15 cses. (Success rate:93%). 2. Most of complication was post-embolization syndrome such as fever, nausea, painbut minimal. Serious complicaiton was very little. 3. Transcatheter embolization appears to be highly valuableadjuvant therapy in debulking of preoperative and inoperable hypervascular mass or treatment of bleeding fromvarious etiology.

10.Percutaneous transhepatic biliary drainage in 107 patients

Percutaneous transhepatic biliary drainage has played a major role in treatment of patients with bilairy tractdisease, especially obstruction by malignant disease. Percutaneous transhepatic biliary drainage was performed 128 times in 107 patients of obstructive jaundice for recent 2 years and 8 months from July, 1981 to March, 1984 at departement of radiology, Seol National University Hospital. The resuls were as follows; 1. The male to femaleratio was 2:1 and the age ranged from 19 to 88 with average of 54. 2. The causes of obstructive jaundice included 94 malignant diseases and 13 benign diseases. Malignant diseases were 48 cases of bile duct cancer, 20 cases of metastasis, 20 cases of pancreatic cancer, 4 cases of gallbladder cancer, 1 case of ampulla Vater cancer, and 1case of duodenal cancer. Benign diseases were 8 cases of comon bile duct stone and 3 cases of benign stricture and2 cases of cholangitis. 3. The most common indication was palliative drainage of obstruction secondary to malignant tumor in 78 cases. 4. The overall success rate was 93.7%. Internal drainage was acheved in 34 (26.5%)and external drainage was accomplished 86(67.2%). 5. Decline in serum bilirubin level was found in 100cases(5.6%). Percutaneous transheatpic biliary drainage is a proven technique for non-operative biliarydecompression and established alternative to surgery.
