1.Crural Ulcer After Accidental Application of 5 - Fluoroutacil Cream.

Topical fluorouracil therapy is an advanced treatment for actinic keratoses, Bowen's disease, multiple basal cell epithelioma and other precancerous skin lesions. Because of extensive and frequent use of 5-fluorouracil(5 F-U) cream, some untoward effects, mostly allergic contact dermatitis and irritation dermatitis, can be anticipated. We have experienced a case of crural ulcer after accidental application of 5 F-U cream for treating the tinea cruris at drug store. The patient was 21 year oid man who had suffered from erythema, erosion and shallow ulceration with oozing, serosanguinous discharge, buming and stinging sensation on the both groins and its adjacent area of the scrotum. The skin lesion was not well responded to topical corticosteroid but controlled by wet dressing and zinc oxide lotion within 10 days of hospitalization. At the 20th hospital day, the skin was healed with remained follicular hyperpigmentation. A few days later, the tinea cruris was confirmed by direct microsopic examination and culture. Generally, topical 5 F-U spares normal cell, while selectively destroying the more rapidly proliferating celI, but there are some possible factors to induce destruction of non-proliferating cell in the highly penetrating area The skin penetration can be different depending upon the anatomical location.

2.Short- and Long-segment Pedicle Screw Fixation with Anterior Interbody Fusion with Short Bone Graft in Denis type B Burst Fractures.

STUDY DESIGN: The operative treatment methods of twenty-one patients with Denis type B burst fractures were evaluated according to the level of pedicle screw fixation. OBJECTIVE: To compare the results of short segment pedicle screw fixation with long segment pedicle screw fixation in Denis type B burst fractures treated anteriorly with short bone graft. SUMMARY OF BACK GROUND DATA: Relatively good results(Short fusion time, good union rate, less loss of lordosis postoperatively etc.) were obtained in long segment fixation group during follow-up period. METHODS: Twenty-one patients with Denis B type burst fractures of thoracolumbar and lumbar spine were divided into two groups depend on the level of pedicle screw fixation in operative treatment methods, one group consisted of 10 cases which were treated by posterior short segment pedicle screw fixation and anterior interbody fusion with short bone graft(Group I : one level above and below posteior pedicle screw fixation of the fracture site and upper end plate of fracture site was fused with autogenous iliac bone or rib to adjacent vertebral body anteriorly through involved disc space) and the other groups consisted of 11 cases which were treated by posterior long segment pedicle screw fixation and anterior interbody fusion with short bone graft (Group II : more than two level above and below posterior pedicle screw fixation of the fracture site and additionally, anterior interbody fusion with short bone graft performed same as Group I). Bony union rate, union period, changes of height of bone and disc, kyphotic angle loss, interscrew angle and complications were measured using pre-, post-operative and follow-up radiographs and chart review. RESULTS: Comparing the two groups, there were no significant postoperative and follow-up results in both groups, especially, change of body & disc height, change of kyphotic angle and change of interscrew angle. However, relatively short fusion time, good union rate, less kyphotic angle loss, and low complication rates were obtainecl and maintained during follow-up period in long segment fixation lion group(Group II). CONCLUSIONS: Long segment pedicle screw fixation and anterior interbody fusion with short bone graft group could effectiyely treat Denis B type burst fractures of thoracolumbar & lumbar spine and maintain their postoperative results during follow up peroid.

4.Serum Peak Growth Hormone and Insulin like Growth Factor-I(IGF-I) Level After Insulin, L-dopa Provocation Test in Children with Constitutional Delay of Growth and Puberty.

PURPOSE:There were many controversies whether constitutional delay of growth and puberty(CDGP) is simple varient of normal growth pattern, or one of the cause of growth disturbance induced by the disturbance of growth hormone secrtion or its function. So we studied about the difference in serum peak growth hormone level after insulin, L-dopa provocation test, and serum IGF-I leve between constitutional delay of growth and puberty(CDGP) and familial short stature(FSS). METHODS:Measurement of serum peak growth hormone and insulin like growth factor-I(IGF-I) level after insulin, L-dopa provocation test were performed in 33 children with costitutional delay of growth and puberty (CDGP). Two groups of children with familial short stature (FSS) whose height were below 10 percentile for chronologic age of Korean national height standards were included as control groups. RESULTS: 1)There were no significant difference of serum peak growth hormone level between children with CDGP and children with FSS and these results were similar in both sex. 2)The mean serum IGF-I level of children with CDGP were 125.69+/-4.06 ng/ml(71.53-189.34ng/ml) in male, 157.7+/-3.17ng/ml(81.9-279.2ng/ml) in female. Both results were significantly lower to those of FSS children by chronologic age group because the mean serum IGF-I level of FSS children were 190.19+/-7.97ng/ml (87.64-297.6ng/ml) in male, 205.47+/-15.87ng/ml(61.7-433.1ng/ml) in female. But compared to FSS children by bone age of 72-96 months, there were no significant difference noted because the mean serum IGF-I level of children with FSSwere130.47+/-0.27ng/ml(63.24-198.2ng/ml)inmale,162.35+/-9.43ng/ml(54.9-217.53 ng/ml) in female. CONCLUSIONS:The results of this study showed that the serum peak growth hormone level after insulin, L-dopa provocation test with children of CDGP revealed no significant difference with those of FSS children in both sex. Serum IGF-I level of CDGP children was lower significantly to those of FSS children by chronologic age group, but no much difference with FSS children of bone age group.

5.Clinical Studies of Acute Poisoning in Children.

This is the clinical studies of 171cases of acute poisoning in pediatric age which were visited at the emergency room of Han-gang Sacred Heart Hospital during the past 2 years and 5 months (Apr., 1974~Aug., 1976). The results were summerized as followings; 1. The incidence of acute poisoning among the emergency pediatric patients were 2.3%(171/7,310). 2. The sex ratio of male to female was 1.5:1. 3. The peak age group was 2 to 5 years of age (41.5%). 4. The peak incidence in a year was shown in August (17.5%) with prominence in summer season (47.3%). 5. The accidental poisoning was occured the most frequently from 4: 00 P.M. to 8:00P.M. during the day. 6. The motive of poisoning was mainly accidental causes (85.9%). 7. The most frequent causative substances were insecticides (29.8%) and then rodenticides (14%), D.D.S. (8.8%), reserpine (4.1%) were the next causes. 8. RH-787 poisoning were occured in 4 cases with the complication of diabetes mellitus 2 cases. 9. In poisonous plants, there were azalea (Rhododendron) poisoning (4 cases) and Gingko biloba Linne fruit poisoning (1 case).

6.Cardiac Structure and Function and It's Alterations after Growth Hormone Treatment in Children with Growth Hormone Deficiency.

PURPOSE:This study was performed to investigate the status of cardiac structure and function and to assess their alterations after growth hormone(GH) treatment in children with growth hormone deficiency(GHD). METHODS:Interventricular septal thickness and left ventriclular posterior wall thickness, ejection fraction(EF), fractional shortening(FS), systolic time interval(STI) of left ventricle were measured by two-dimensional and M-mode echocardiography in sixteen children with GHD and age, sex matched sixteen children with GH normal short stature as control. The measure were done before GH treatment and at 6 and 12 months of GH treatment, respectively. RESULTS: 1)Left ventricular posterior wall thickness in GHD group was significantly thinner than that of control group(P<0.05). 2)Interventricular septal thickness and left ventricular posterior wall thickness were increased with GH treatment from 10.4+/-1.7mm, 8.1+/-1.8mm before GH treatment to 11.0+/-0.9mm, 8.7+/-0.7mm and 11.2+/-1.7mm, 9.7+/-1.8mm at 6 and 12 months of GH treatment, respectively. The increment of left ventricular posterior wall thickness after 12 months GH treatment revealed statistic significance(P<0.05). 3)There was no significant alterations of EF, FS, STI of left ventricle after GH treatment at 6 months and 12 months, respectively. CONCLUSION: Left ventricular posterior wall thickness in GHD group was significantly thin compared to that of control group(P<0.05). GH treatment in GHD children for 12 months, resulted statistically significant increase(P<0.05) in posterior wall thickness. There is no evidence of hypertrophic cardiomyopathy after GH treatment. But we could not exclude the possibility of these alterations were induced by an increased overall body size and body surface area after GH treatment. To clarify the exact alterations of cardiac structures and function in children with GHD after GH treatment, long term follow-up studies should be necessary.

7.Extramammaty Paget's Disease: Electron Microscopic Study for Observation of Bleomycin Effect.

Extramammary Paget's disease is a dermatosis which pathologically and clinically resembles Paget's disease of the breast. The disease occurs in regions with apocrine gIands; most often in the anogenital area, seldom in the axilla. umbilicus, mucous membrane of the mouth, nose, ear and Moll's gland of tne eyelid. In this work the effect of bleomycin for extramammary Paget's disease was studied on ultrastructural aspect. The material was a case of extramammary Paget's disease of 7 years standing occuring in the scrotum and the pubic area by serial sections after IV injection of 60mg, 120mg and 240mg of bleomycin, and examined under the electron microscope. The results obtained are summarized as follows: 1. The size of Paget's cell and its nucleus was gradually decreased. 2. The numbers of lysosome, dense body, autophagic vacuolization and tonofilament was decreased. From these fidings, it is suggested that bleomycin induces retrograde degenerative change of Pagct's cell, and cell necrosis finally.
