2.A Case of Tuberous Sclerosis with Psoriasis.

A 10-year-old boy had pin-head sized, discrete, yellowish waxy, translucent papules on the face and also, various sized round, circumscribed, erythematous patches with silvery white scales on the trunk. The clinical and histopathologic findgs were compatable with diagnosis of tuberous sclersis associate with psorlasis. There were strong familial history during 3 generation in psoriasis(Fig. 1) Both psoriasis and tuberous sclerosis are heritable skin diseases. Some heritable dermatologic conditions found in tuberous sclerosis including ichthyosis, hypertrichosis and palmar and plantar keratoses but there is no report of tuberous sclerosis associated with psoriasis in published literatures.

3.Treatment of Alopecoa Areata with Dinitrochlorobenzene.

This study was designed to investigate the therapeutic effect of dinitrochlorobenzene (DNCB) on alopecia areata. Forty patients with alopecia areata were sensitized by applying 0.1ml of 2% DNCB dissolved in aceton solution on the voiar aspect of left forearm. Patients were subsequently chaIlenged at weekly mtervals with 0. 1% DNCB solution appIied to one side of affected area for periods of 2 to 8 months. The other side of affected area was reserved as a contol site. The results were as follows. 1. In 29 out of 40 patients (72.5%), hair regrowth occurred either excIusiveIy on the treated site (18 cases) or considerably faster and denser on treated site(11 cases)(Table 1) 2. In the majority of cases, the regrowth of hair was observed, within eight weeks of the treatment. 3. The response to DNCB depended on the severity of the aIopecia. The response rate were 80.9% (17/21) in moderate degree (<25% bald area) cases, 68.7% (ll/16), in the extensive degree(>25% bald area) caes and 33% (1/3) in alopecia totalis(Tale 1) 4. The rate of response depended on the duration of the alopecia. Satisfaetory results were obtained in 19 out of 25 patients with a history of less than one year(76%), in 7 out of 10 patients with a history of one to three year(70%) and in 3 out of 5 patients with a history of more than three years (60%) (Table 2) 5. The main side reactions were burning sensation (3 cases), severe eczema (2 cases), folliculitis (2 cases) and cervical lymphadenopathy(7 cases). 6. Liver function test and complete blood cell count were in normal range during the course of treatment in 5 randomly selected patients. 7. Although the treatment of alopecia areata with DNCB still remains to be an experimental trial, this therapy could be substituted for corticosteroids in selected cases.

4.Concentration of beta- Endorphin in Plasma of Patients with Stress - Associated Dermatoses.

BACKGROUND: Stress has long been known to play a role in many dermatologic disorders and can affect the onset and course of the disorder in some patients. Stress-induced exacerbation or onset of symptoms has been reported in chronic urticaria, alopecia areata, herpes simplex, herpes zoster, and psoriasis vulgaris, and these diseases can be classified as stress-associated dermatoses. Beta beta-endorphin is one of the most important mediators of stress, which is known to be generated upon stimulation of the pituitary-adrenal axis, and its secretion increases during periods of stress. OBJECTIVE: In order to see wheather beta-endorphin might be related to the onset or recurrence of stress-associated dermatoses, we compared the plasma concentration of beta-endorphin in patients with stress-associated dermatoses with those of healthy subjects. METHODS: The concentration of beta-endorphin. In sera was quantified by radioimmunoassay, using the INCSTAB 125I RIA Kit for plasma beta-endorphin, Each patient was asked to indicate if they believed that their skin problem began after an important stressful event in their lives. RESULTS: There was no significant difference in plasma beta-endorphin levels between patients with chronic urticaria, alopecia areata, herpes simplex, and herpes zoster and healthy subjects(p>0.05), whereas in patients with psoriasis vulgaris, plasma level of beta-endorphin was significantly increased (p<0.001). There was no relationship between the stressful events and plasma beta-endorphin concentrations. CONCLUSIONS: The plasma beta-endorphin level is not correlated with the onset or recurrence of stress-associated dermatoses such as chronic urticaria, alopecia areata, herpes simplex, and herpes zoster. The increase in beta-endorphin in psoriasis vulgaris is more likely that this peptide is generated by the lymphocyte infiltrated in the skin and/or by lymphocytes when they recirculate rather than by the activation of the pituitary-adrenal axis by stress.

5.Simultaneous Bilateral Spontaneous Pneumothorax.

The simultaneous bilateral spontaneous pneumothorax is very dangerous, so the chest drain insertion should be done urgency. Contrary to the unilteral pneumothorax, the patient with simultaneous bilateral pneumothorax complains a severe respiratory distress, cyanosis and chest pain without the tension pneumothorax. Now, we present two cases of the simultaneous bilateral pneumothorax. The prompt insertion of chest drains relieved symptoms. One case is resolved by the bilateral thoracotomy, the other by the insertion of chest drains only.

6.Crural Ulcer After Accidental Application of 5 - Fluoroutacil Cream.

Topical fluorouracil therapy is an advanced treatment for actinic keratoses, Bowen's disease, multiple basal cell epithelioma and other precancerous skin lesions. Because of extensive and frequent use of 5-fluorouracil(5 F-U) cream, some untoward effects, mostly allergic contact dermatitis and irritation dermatitis, can be anticipated. We have experienced a case of crural ulcer after accidental application of 5 F-U cream for treating the tinea cruris at drug store. The patient was 21 year oid man who had suffered from erythema, erosion and shallow ulceration with oozing, serosanguinous discharge, buming and stinging sensation on the both groins and its adjacent area of the scrotum. The skin lesion was not well responded to topical corticosteroid but controlled by wet dressing and zinc oxide lotion within 10 days of hospitalization. At the 20th hospital day, the skin was healed with remained follicular hyperpigmentation. A few days later, the tinea cruris was confirmed by direct microsopic examination and culture. Generally, topical 5 F-U spares normal cell, while selectively destroying the more rapidly proliferating celI, but there are some possible factors to induce destruction of non-proliferating cell in the highly penetrating area The skin penetration can be different depending upon the anatomical location.

8.Studies on the VP4 and VP7 Genes of Bovine Rotaviruses from Field Samples Using RT-PCR and RFLP Analysis.

Characterizations of the VP4 (P type) and VP7 (G type) genes of Korean isolates of bovine rotavirus were performed using RT-PCR/RFLP and nucleotide sequencing analysis. After RT-PCR amplification of partial length (1094bp) of the VP4 and full length (1062bp) of the VP7 genes, amplified PCR products were digested with restriction endonucleases and digestion patterns were compared with those of reference rotaviruses. With the VP4 genes, four RFLP (AD) profiles were observed; three (A, B and C) were the same as those of bovine rotavirus NCDV (P[1]), IND (P[5]) and B223 (P[11]), respectively, Profile D was the same as that of porcine rotavirus OSU (p[7]). With the VP7 genes, five RFLP profiles (I-V) were observed; three of them (1, II and III) were the same as those of bovine rotavirus NCDV (G6), Cody I-801 (G8), and B223 (G10), respectively, Profile IV and V were atypical to those of reference bovine rotaviruses used in this study. These two profiles were identified as G6 and G5, respectively, after analyzing and comparing the nucleotide sequences. The G typing analysis revealed that 61.9% (26/42) were G6, which included G6 subtype; 28.6% (12/42) were G5; 7.1% (3/42) were G10; 2.4% (1/42) were G8. The P typing analysis revealed that 54.8% (23/42) were P(5); 28.6% (12/42) were P(7); 11.8% (5/42) were P(11); 4.8% (2/42) were P(1). Our results showed that G6/P(5) were the most prevalent rotaviruses in diarrheic calves in Korea. Also, this is the first report that G5P(7) rotaviruses were identified from cattle with diarrhea.

10.Disseminated Superficial ACtinic Porokeratosis Occuring in 3 Generations.

Disseminated superficial actinic porokeratosis (DSAP) has an autosomal dominant mode of inheritance with reduced penetration at young age, characterized by multiple superficial keratotic lesions surrounded by a slightly raised border. The aggrevation of the lesion by exposing UV light indicates that phenotypic expression of gene regluating DSAP is related to sun light. We observed a family of which 13 members had DSAP in 3 successive generations. They were grandrnother, two sons of her 3 offsprings, 2 grandsons and 8 granddaughters. Lesions of all cases were found on the sun exposed areas, especially, face, extremities and upper trunk but exceptionally one had a lesion on the unex posed area, abdomen below the iliac crest. The number of lesions of the individual patient were 12-116, and there appeared to be increasing number of lesions with age.
