2.The Comparison of the Ankle Arthrodesis According to Surgical Procedures.

The aim in arthrodesis of ankle is to gain a painless, normal walking gait of destroyed ankle joint caused by various origins. Despite more than 30 different fusion techniques have been described in the literature, but the results were not always satisfactory. Blair introduced tibial sliding graft, rectangular hone block donated at distal tibia, to the neck of talus. His method has advantages of a normal apperance of the foot, no shortening, and the weight bearing thrust on normal tissue. The purpose of this study was to compare the ankle arthrodesis according to surgical procedures & introduce the method which could produce excellent results for ankle arthrodesis. We reviewed 23 patients(24 cases) undergone ankle arthrodesis from Jan. 1989 to Dec. 1996 in Kyung Hee Medical Center. Male were twelve(13 cases) and female were eleven. Mean age was 42.6(l8-75)years. The Blair fusion method was used in ten cases. Iliac bone graft followed by internal fixation using Steinmann pin in 4 cases, using cancellous screw in I case, and using both Steinmann pin and staple in 1 case, while tibiotalar coaptation without bone graft followed by internal fixation using Steinmann pin in 3 cases, using staple in 1 case, and using cancellous screw in 1 case. In 3 cases, arthroscopic abrasion and cancellous screw was used. The mean duration of follow-up was 46(6-99) inonths. Time to union was average l4.6(8-23) weeks in Blair fusion and 17.5(8-40) weeks in other methods. Delayed union was observed in three cases, but no case was found in Blair fusion. Pain was ohserved in 5 cases, 3 in Blair fusion and 2 in other methods. Nonunion or pseudoarthrosis was not ohserved. On the clinical criteria for Boston Childrens Hospital ankle-scoring system, we ohserved nineteen excellent and five good cases. The difference in results according to fusion method was not ohserved. The results in tibiopedal motion using cineroentgenography were seven good and three fair cases in Blair, while two good, ten fair and two poor in other methods. By using Blair fusion, we expect early bony union and better clinical results.

3.Multifocal Reticulum Cell Sarcoma of Bone: A Case Report

A Case of multifocal reticulum cell sarcoma in a 61 year old male is reported. The patient had multiple foci of sarcomatous lesion in the olecranon of the right ulna, medial candyle of the left tibia and in both ankle joints. His initial complaints on admission were pain and swelling in his both ankles and the right elbow of three months duration. Biopsy was carried out on his right ankle joint and diagnosis of reticulum cell sarcoma was made. Surgical excision of the right olecranon and below Knee amputation of the right lower extremity were performed. Following the surgical intervention, Cobalt 60 irradiation therapy was given for 20 days with total amount of 4,000 rads. The follow-up study three months after surgery revealed much improvement and no evidence of recurrence.

4.Measurement of the Oblique Diameter of the Lumbar Spinal Canal in Korean Army-aged Group by Echographic Method

The size and configuration of the lumbar spinal canal constitute one of the important factors in the production of symptoms referable to cauda equina and nerve roots of the lumbar spine and the narrowness of the spinal canal. Numerous attempts have been made to measure the size of the lumbar spinal canal, but most of those are not sufficient to measure the oblique diameter of the lumbar spinal canal. Also, echographic diagnosis is much simple, safe, less expensive and non-invasive and furthermore demonstrates much more accuracy than other alternative and radiographic procedures. The purpose of this study is to establish the range of normal values of the oblique diameter of the lumbar spinal canal in Korean Army-aged group by echographic method in the interest of facilitating clinical evaluation of the lumbar spinal canal stenosis. The author measured oblique diamenter of the lumbar spinal canal in thirty healthy persons of both sexes of ages between nineteen and twenty-five years by ultrasound using sonolayergraphy model SSL-21A. Distance from the ligamentum flavum to the posterior longitudinal ligament was measured in millimeters with dial vernier caliper. Actual distance of oblique diameter of the lumbar spinal canal is obtained by multiplication of the distance of echogram by calibration factor 2.22. The results were as follows; 1. The mean values of the oblique diameter of the lumbar spinal canal in male and female between nineteen and twenty-five years of age were 14.77±0.85mm and 14.87±0.82mm in Ll, 13.99±0.87mm and 14.13±0.96mm in L2, 13.40±0.69mm and 13.71±0.75mm in L3, 12.88±0.69mm and 12.93±0.68mm in L4, 14.58±0.79mm and 14.42±0.84mm in L5 respectively. 2. The oblique diameter of the lumbar spinal canal was the widest at the first lumbar vertebra, the next at the fifth lumbar vertebra and the narrowest at the fourth lumbar vertebra. 3. There was no difference of diameter of the lumbar spinal canal between both sexes.

6.Multiple Glomus Tumor in Brachial Plexus: A Case Report.

Glomus tumor is a kind of vascular tumor that arises from the glomus body, which regulates skin temperature and is placed in the skin and the subcutaneous area. It is a benign tumor that usually presents in the subungal area. It is relatively common in areas other than the fingers, but its occurrence in peripheral nerves is known to be comparatively rare. We report our experience with a case of glomus tumor arising from the brachial plexus, a rare site of occurrence for glomus tumors.

7.Use of the flexible fiberoptic laryngoscope to change endotracheal tube.

The fiberscope is no longer an experimental instrument, and it is a state-of-the-art technique for airway management in the operating room, recovery room, and intensive care unit. Changing the oral tube to a nasal one is often necessary if the trachea is to be intubated for a long period. A nasal tube is better tolerated by the patient, is easier to secure when the patient is moved, and is easier to attach to the respiratory care equipment. We describe three cases report changing the oral tube to a nasal one in neurosurgical pa- tient post operatively under flexible fiberoptic laryngoscope, while the interruption of venti- lation is minimized.

8.Malignant Hemangiopericytoma of the Chest Wall.

Primary hemangiopericytoma of chest wall is very rate and only a few cases have ever been reported. The tumor aries from the capillary pericyte of Zimmerman. It is a highly vascular slow growing tumor which can be calssified as both malignant and benign varieties. We report a case of a 66 year-old man in whom recurrent hemangiopericytoma was treated by complete surgical excision. In October 1993 he had received surgical excision of hemangiopericytoma on posterior chest wall. For more than 6 years after the operation he was in good condition until a recurrent mass was found on the chest X-ray. The patient was discharged 9 days after the operation and is receiving radiotherapy.

9.A Case of Lichen Aureus.

Lichen aureus is a rare disorder classified as a pigmented purpuric dermatosis. This case report is of a 46-year-old female in whom a coin sized, gold tinted, erythematous maculopapule developed on the right lower quadrant of the abdomen l0 years ago which showed characteristic histological findings of lichen aureus consisting of lymphohistiocytic infiltration in the upper dermis and extravasation of red blood cells.

10.The psychological characteristics of functional dyspepsia patients by MMPI.

BACKGROUND: The functional dyspepsia gives rise to such a symptom similar to peptic ulcer without showing a noticeable lesion(e.g, gastric or duodenal ulcer) by endoscopy or upper GI serises, and it has been known that this is about twice as many as the peptic ulcer. However, there is no exact theory about its pathophysiology and it is suggested that multiple factors including genetic, physiologic, psychologic, environmental factors are engaged. Especially, in respect to the occurrence of functional dyspepsia and psychological factors, there have been many clinical studies over the world. But, we still need more studies in this country, therefore, this study has been initiated to give help patients of functional dyspepsia, invest.igating their characteristic traits through MMPI. METHODS: From March to September in 1996, the patients who visited the family medicine out-patients department of St. Marys hospital of Catholic Medical College were sampled in this study. There were 46 patients having a similar symptoms to peptic ulcer without showing pathologic lesions by endoscopy. Through an interview and some basic tests, the 41 controls who had no dyspeptic symptoms and were not affected to a noticeable physical or physiological disorders were selected for the normal reference group. The standardized MMPI of our country was applied to the whole object groups to get a result. Then each average value of the measure(mean T-score) was calculated, and the differences of the average value were analyzed by the t-test and chi-square test. And the psychological pattern analysis was done. RESULTS: In demographic aspects, there were no significant differences between the two groups. The mean T score for each measure of the two groups falls within the normal ranges with their values being of between 30 to 70. And the average of L, F, and K measure, known as the validity measure, did not give a significant differences between the two groups. The measure of Hs, D, and Hy have appeared significantly high in the patients group(p<0.01) and the Pd measure, too(p<0.05). While Mf, Pa, and Ma measure have not shown a significant differences, Pt, Sc, and Si measure have very significant differences(p<0.01). CONCLUSIONS: In the patient group with the functional dyspepsia, the measures of Hs, D, and Hy where they indicate a neurotic pattern have shown a very significantly high point as well as those of Pd, Pt, Sc, and Si where they indicate a psychotic pattern. This reveals that in the group of the functional dyspepsia neurotic or psychotic trends to attribute. However, it is very hard to define the characteristics of a group with just one test, so further study with more variable test will be necessary.
