1.Three Cases of Familial Benign Chronic Pemphigus.

Familial Renign Chronic Femphigus(FRCP) is rare, persistent and vesicobullous genodermatoai. Three casee of FRCP with typical clinical and histapathological findings are reported. The first case of FBCP was 27-year-oId man who had recurrent episodes of grou vesicles and rust with miId pruritue at the posterior neck, in both axillae and in inguinal area for 10 years. The second case was 49-year-old woman with skin lesions of recurrent persistent. vesicles, pustules on the erythematous skin, crust and lichenification on the flexural area of extrimeties, axillae, ingumal and neck area for 10 yeara. The third case was 49-year-old male patient who had recurrent pruritic grouped vesicles with crust on the neck and axillary area. In all three patients, skin eruptions usually began and or aggravated in the spring or summer and tend to heal spontaneously in the autumn and winter and showed positive Nikolsky sign. The only one case of the reported three casea had familiaI history of skin lesions similar to FRCP. Histopat,holag;ic finding of biopsy specimens from the vesicular eruption showed auprabasal vesiculation with acantholysis, lacunae and villi formation in all 3 cases.

2.Relationship Between Visual Acuity and Refractive Error in Myopia.

The purpose of this study is to present quantitative data showing the relationship between visual acuity and refractive error in the various types of myopia: simple myopia, simple myopic astigmatism and compound myopic astigmatism. 1. The study covered 681 patient (1259 eyes) examined by having refractive error in myopia. (-0.25 ~ -20.00D) the Department of Ophthalmology, Yonsei University Medical Center and Wonju Christian Hospital. 2. Plotting the logarithm of visual acuity on the logarithm of the degree of myopia gives a coefficient correlation of -0.8028. The statistical data showed significant in correlation coefficient (r) in various types of myopIa: simple myopia r=-0.7789(p=0.001), simple myopic astigmatism r=-0.7877(p=0.001), compound myopic astigmatism r=-0.7537(p=0.001). 3. The coefficient of determination (R2) was 0.6445. 4. The mathematical mean of the various types of myopia: a. myopIa; simple myopia: 0.29(=20/60), simple myopic astigmatism: 0.87(=20/25), compound myopic astigmatism: 0.29(=20/60). b. visual acuity; simple myopia: -2.53D, simple myopic astigmatism: -0.50D, compound myopic astigmatism: -3.04D. 5. The formulas for the two regression line were: log V=-0.7385 log M-0.4976 (A)(F=2175.7303, p=0.001), log M= -0.8727 log V -0.3641 (B)(F=2175.7303, p=0.001). 6. Confidence limits were determined for the regression lines and table and set up for predicting either the degree of myopia or the visual acuity if the other is known.

4.A Case of Malignagr Lymphoma.

A 28-year-old male patient is described who developed multiple erythematous nodules and papules on the face, upper chest, back, upper and lower extrimities, leading to extensive ia.filtration of the skin and lymph nodes. Histology from the involved lymph node and skin nodule revealed widely scattered patchy infiltration of abnormal. hyperchromatic polymorphous histiocytes in the dermis. The patient died at about 8 months after onset of the disease, in spite of antineoplastic treatment.

5.Two Cases of Acute Febrile Neutrophilic Dermatosis (Sweet's Syndrome).

We present two cases of acute febrile neutrophilic dermatosis developed in women aged 56 and 52. One of the two patients deveIoped the lesions during longterm antituberculosis chemotherapy for her advanced pulmonary tuberculosis, The other case developed the lesions, initially at the site of acupuncture done for relief of her arthralgia, followed by the involvement of the other sites. The etiology of acute febrile neutrophilic dermatosis is not clear, however, hypersensitivity and association with systemic diseases such as upper respiratory infection, malignancies, ulcerative colitis and pyoderma gangrenosum are postulated.

6.A Case of Xanthoma Tuberosum.

A 43 year-old Korean male has had multiple, grouped, asymptomatic, yellowish brown flat papules and nodules on the knees, elbows, buttocks, palms and soles for 3 years. Small yeIlowish, discrete papules first appeared on the right palm and left elbow, and gradually increased in size and number and spread to knees, buttocks and soIes. Physical examination revealed normal except for the skin lesions and no familial occurrence was noted. Gross finding of the serum was turbid, more or less creamy and paper electrophoresis revealed marked increase of prebetalipoprotein, increase of betalipoprotein and alphalipoprotein and absence of chylomicron, suggesting type III or type IV hyperlipoproteinemia. Serem choleterol was 200mg% and fasting blood sugar was 115.0mg%. with normal glucose tolerance test. Histopathology showed many aggregates of foam cells on H-E stain and many lipid droplets in the dermis on Sudan red stain. Based on the clinical and histopathological findings and lipoprotein analysis by the paper electrophoresis, the patient was diagnosed as type IV hyperlipoproteinemia with xanthoma tuberosum.

7.Assessment of Sprengel Deformity Using Three - Dimensional Computed Tomography.

We evaluated the scapular shape, dispiacement and rotation in 10 cases of Sprengel deformity using three-dimensional computed tomography in order to investigate its clinical usefulness. Standard views, including trunk posterior view, scapular true posterior view and medial view, were taken, and the 3-D image was rotated in three axes to visualize the omovertebral bony connection. In the trunk posterior view, the amount of superior displacement of the affected scapula was measured using the glenoid level as reference, and the rotational deformity by the tilting of the base of scapular spine. Scapular dysplasia was evaluated in the scapular true posterior and medial views. The presence, size, and anchoring points of omovertebral bone were assessed in various view points. There was a tendency of inverse correlation hetween superior displacement and rotational deformity of scapula. In most cases, the affected scapulae were convex at their medial borders and concave at their lateral borders, with increased width/height ratio. The anchoring point of omovertebral connection appeared to determine the scapular shape, level, and amount of rotation. Three-dimensional CT was helpful in preoperative planning.

8.Total Knee Replacement Arthroplasty Without Patellar Resurfacing.

We analysed the results of total knee replacement arthroplasty without patellar resurfacing in 20 patients(28 knees) retrospectively. The patella was not resurfaced when it maintained relatively healthy cartilage in osteoarthritis or the patella was too thin to be replaced in rheumatoid arthritis. The diagnoses were osteoarthritis in 11 and rheumatoid arthritis in 17 cases. The follow up period was 30.3 months in average(12-70 months). A lateral retinacular release was performed in 22 knees(78.6%). The mean knee society score improved from 45.4 preoperatively to 84.1 postoperatively and the functional score improved from 21.4 preoperatively to 63.3 postoperatively. The modified Kujala score was 43.2 points in osteoarthritis and 43.6 points in rheumatoid arthritis. We conclude the total knee replacement arthroplasty without patellar resurfacing shows good results, without high risk of postoperative peripatellar complication.

9.Evaluation for the sensitivity of LISS in antiglobulin test.

The association rate of antibody with antigen has been reported to be greatly increased by lowering ionic strength. Low-ionic strength salt solution(LISS) method has been used for the detection of various alloantibodies. To investigate the sensitivity of LISS in indirect antiglobulin test, a comparison study of LISS with saline and albumin methods was conducted. A total of 32 patients' samples requested for indirect antiglobulin test were evaluated. Out of 32 patients with clinical immune hemolytic anemia, 11(34.3%) were positive in 37 degrees C saline antiglobulin test, 18(56.2%) in albumin antiglobulin test, 23(71.8%) in LISS antiglobulin test respectively. These results were statistically analyzed using non parametric Page's test for ordered alternatives. LISS method is more sensitive than 37 degrees C saline method or albumin method significantly (p<0.01).

10.A Case of Multiple Bowen' s Disease in a Leprosy Patient.

We report a case of multiple Bowen's disease associated with leprosy. A 68-year-old man was seen because of slightly pruritic multiple erythematous scaly patches and plaques on the trunk and the both extremities for 5 years. He has been treated for leprosy with many kinds of drugs consisting of the herb medications for over 40 years. He also showed decreased cell-mediated immunity in skin tests. It is considered that depressed cellular immunity may be an important predisposing factor in the development of mutiple Bowen's disease in this case, .although arsenics can not be completetly excluded.
