1.AN EXPERIMENTAL STUDY ON THE ETTECTS OF FIBRIN SEALANT IN BONE REGENERATION.

Recent development of maxillofacial surgery has taken an interest in the regeneration of facial bone defects. For the reconstruction of facial bone defects, autogenous bone grafts have been widely used. However, there are some clinical problems : the morbidity of donor sites, resorption of autogenous bone graft and, the availability of the proper form and size. The purpose of this study is to find the effective solutions to above problems. The present study was designed to evaluate the roles and regenerating capacity of fibrin sealant. Fibrin sealant is a newly developed substance for hemostasis, tissue sealing and wound healing. I made the experimental study on the effects and roles of bibrin sealant in sutogenous bone graft procedure to minimize the above complications and problems. Twenty-five adult white rabbits were used as the experimental material. Three trephine defects were made by drilling on the parietal bone of each rabbit. Defect size was 8x8mm. First group consisted of the rabbits which had the artificial defect on the parietal bone as control. The defect in second group was sealed up with autogenous boon chip only. Last group comprised the rabbits autografted by mixing the bone chip with the fibrin sealant. The results were carefully examined grossly and microscopically from the 3rd day to the 28th day postoperatively. The following results were obtained ; 1. The mixture of fibrin sealant and bone chip was useful for regenerating the bone defects. Most of the defects were substituted by new bone. 2. In the case of the graft with bone chip only, most defects were also filled with new bone, but less than fibrin group. 3. In the control group, new bone was formed around the peripheral area only. A mixture of fibrin sealant and autogenous bone chip plays an effective role in forming new bone. find judging from the results of this experiment, it may be expected the much use of the fibrin sealant in maxillofacial orthopedic surgery.

3.Erythema Nodosum Associated with Chronic Myelogenous Leukemia.

Erythema nodosum has been known to be frequently associated with some kind of drug, infections with streptococci, mycobacteria, lymphogranuloma venerum and certain unknown pathogenic processes such as sarcoidosis, iilcerative colitis and regional enteritis. However, the association of erythema nodosum with leukemia has rarely been reported. Erythema nodosum with leukemia has been divided into leukemia cutis with specific infiltration of atypical leukemic cells and leukemid with non specific infiltration of normal infilammatory cells. We report herein a case of the very rare but typical erythema nodesum showing nonspecific infiltrative panniculitis as an initial manifestation of chronic myelocytic leukemia.

It is known in general that neoplasms of smooth mucsle appear very rarely in the oral region, probaly because in the oral region with histopathologic investigations have been reported. This report presents an unusual case of angioleiomyoma that developed on the lower lip of a 48-year the course of one year and the immunohistochemical findings of the angioleiomyyoma are described.

5.Got target?: computational methods for microRNA target prediction and their extension.

MicroRNAs (miRNAs) are a class of small RNAs of 19-23 nucleotides that regulate gene expression through target mRNA degradation or translational gene silencing. The miRNAs are reported to be involved in many biological processes, and the discovery of miRNAs has been provided great impacts on computational biology as well as traditional biology. Most miRNA-associated computational methods comprise the prediction of miRNA genes and their targets, and increasing numbers of computational algorithms and web-based resources are being developed to fulfill the need of scientists performing miRNA research. Here we summarize the rules to predict miRNA targets and introduce some computational algorithms that have been developed for miRNA target prediction and the application of the methods. In addition, the issue of target gene validation in an experimental way will be discussed.

7.The Study on the Hospital - Based Relative Frequency , and Clinical and Laboratory Findings of Atopic Dermatitis.

Background : Atopic dermatitis(AD) is a common disease, and population-based studies indicate that the frequency of AD has increased substantially during recent decades. However, there have been few studies on the hospital based relative frequency, and clinical and laborat,ory findings of AD in Korea. OBJECTIVE: The presenl study was aimed to elucidate the relative frequency of AD on the outpatient base, and to investigate the clinical manifestations and immunological laboratory findings. METHODS: Our study was designed by analysing outpatients with AD via physical examination, questionnaires and immunological investigations such as skin prick tests, Multi test CMI, and measurernent of eosinophil counts, total IgE, Derrnatophagoides pteronyssinus-specific IgE and eosinophil cationic protein(ECP). RESULTS: The results were as follows : 1. The relat,ive frequen:y of AD on the hospital base in 1995-1996 was 8.6%. 2. The mean age of onset was 3.36 years. 57.3% of the patients had their onset of eczema by the age of 1 year, 65.5% by 2 years and 83% by 5 years. The most common site at the onset was the face followed by antecubital area, popliteal area, neck, and trunk. 3. The predilection site on physical examination were the face, neck, antecubit,al area, popliteal area and trunk in decreesing order. 4. Concomitant allergic rhinitis, keratoconjunctivitis and asthma were present in 2~3.4%, 14.1%, and 9.5% of patients with AD, respect.ively, whereas pure AD was present in 34.1%. 5. Family history of atopy was observed in 60.5% of AD patient.s. 6. The most frequent ninor fea~ture of AD was xerosis followed by perifollicular accentuation, allergic shiner, cheilitis, imitation allergy, palrnar hyperlinearity and molluscum cantagiosum. 7. Seasonal aggravation was found in 86.8% of patients with AD, especially in summer and winter, The rnost significant aggravating factors were wools followed by sweating, heating, dryness, excessive bat.hing and emotional stress. 8. Abnormal levels of .osinophils, total IgE and ECP were seen in 44.8%, 78.9% and 31.7% Of patients with AD, respec.ively. 9. Cell mediated immunty was decreased in 68.2% of patients with AD. 10. Positive reactivity( at least rnore than one allergen) in the skin prick t,est was noted in 76% of pat,ients with AD and the most popular 8 allergens were D. pteronyssinus, D. farinae, cat fur, M4 mixed threshing dust., dog hair, mugwort pollen, H3 tree pollen and birch pollen. 11. The specific IgE aritibodies to D. pteronyssinus(>+1) were found in 72.3% of AD patients and correlated with the r esults of the skin prick test in 76%. CONCLUSION: The hospi al-based relative frequency of AD investigated in our study tends to be increased. As the clinica features and laboratory findings of AD might be influenced by genetic, environrnental and psychological factors, the systernic evaluation of patients with AD will be needed.

8.Dual X-ray Absortiometry(DXA) in the Detection of Loosening in the Total Hip Replacement Arthroplasty' Preliminary Study.

PURPOSE: Algorithms to get cross-sectional bone density pattern(transverse histogram) to predict the loosening of hip prosthesis using DXA(Dual X-ray Absorptiometry) have been developed. We performed this study to analyze the correlation between radiologic findings and densitometric pattern of the THRA(Total Hip Replacement Arthroplasty) patients. MATERIALS AND METHODS: Thirty-six hips of 32 THRA patients were evaluated. The duration between THRA and DXA was from 1 year 7 months to 15 years. On transverse histogram, the periprosthetic bone density patterns were classified as 3 types;type I, rigid fixation in 17, type II, definite loosening in 8, and type III, partial loosening in 11 cases. Surgical findings, plain X-ray findnings and transverse histogram using DXA were correlated. RESULTS: Among 14 cases performing revision for acetabular prosthesis loosening, 5 cases revealed loosening of fernoral stems while 9 cases revealed rigid fixation of femoral stems. Sensitivity was 100% for either plain X-ray or DXA. Specificity was 88%, 77% for plain X-ray and DXA respectively. CONCLUSION: This preliminary study reveals that periprosthetic bone density pattern on transverse histogram on DXA may be useful in the evaluation of the loosening. However, further study will be needed for clinical application.

9.A Case of Pregnancy Using Recombinant Follicle Stimulating Hormone and Gonadotropin Releasing Hormone Antagonist.

OBJECTIVE: To report the pregnancy which was made by in vitro fertilization using recombinant follicle stimulating hormone and gonadotropin releasing hormone antagonist. MATERIAL AND METHOD: Case report. RESULTS: Six oocytes were retrieved and all were fertilized by intracytoplasmic sperm injection. Six embryos were transferred and the pregnancy was confirmed. CONCLUSION: It is envisaged that the availability of recombinant gonadotropins and gonadotropin releasing hormone antagonists will ultimately lead to shorter, cheaper and safer treatments, using reduced dosages.

10.Multiple Imputation Technique Applied to Appropriateness Ratings in Cataract Surgery.

Missing data such as appropriateness ratings in clinical research are a common problem and this often yields a biased result. This paper aims to introduce the multiple imputation method to handle missing data in clinical research and to suggest that the multiple imputation technique can give more accurate estimates than those of a complete-case analysis. The idea of multiple imputation is that each missing value is replaced with more than one plausible value. The appropriateness method was developed as a pragmatic solution to problem of trying to assess "appropriate" surgical and medical procedures for patients. Cataract surgery was selected as one of four procedures that were evaluated as a part of the Clinical Appropriateness Initiative. We created mild to high missing rates of 10%, 30% and 50% and compared the performance of logistic regression in cataract surgery. We treated the coefficients in the original data as true parameters and compared them with the other results. In the mild missing rate (10%), the deviation from the true coefficients was quite small and ignorable. After removing the missing data, the complete-case analysis did not reveal any serious bias. However, as the missing rate increased, the bias was not ignorable and it distorted the result. This simulation study suggests that a multiple imputation technique can give more accurate estimates than those of a complete-case analysis, especially for moderate to high missing rates (30 - 50%). In addition, the multiple imputation technique yields better accuracy than a single imputation technique. Therefore, multiple imputation is useful and efficient for a situation in clinical research where there is large amounts of missing data.
