1.The Effect of Music Therapy on the Elderly People's Depression and Life-Satisfaction.

PURPOSE: This research was carried out to examine if music therapy is effective in decreasing elderly people's depression level and in increasing their life satisfaction level. METHODS: This study used the quasi-experimental design with nonequivalent control group and the pre-test and post-test design, and was conducted from the 26th of July to the 26th of September in 2004. Data were collected using a questionnaire developed by Y.esavage et al. (1983) to measure depression level and one developed by Yoon (1982) to measure life satisfaction level. The subjects were 80 elderly people who were the members of an elderly welfare center in G city. The subjects were divided into an experimental group (n=40) and a control group (n=40). RESULTS: The first hypothesis' The depression level will be lowered in the experimental group who received music therapy' was supported (t=3.856, p=.000). The second hypothesis' The life satisfaction level will be raised in the experimental group who received music therapy' was supported (t=2.040, p=.045). The results above suggest that music therapy can be an effective nursing intervention for reducing the depression level and increasing the life satisfaction level of elderly people.

2.Multiple Trichoepithelioma Associated with Milia.

We present a case of multiple trichoepithelioma associated with milia. A 12-year-old girl developed symmetrically distributed, numerous, skin-colored papules on her face, some of which were studded with milia-like whitish materials. Histopathological findings of these lesions revealed the typical findings of trichoepitheliomas with overlying milia in the upper dermis.

4.A Case of Chondrodermatitis Nodularis Chronica Helicis.

A 51-year-old man had small, tender, skin colored firm nodule on the helix of the both ears for eight months. He was found to have classic features of chondrodermatitis nodularis chronica helicis, namely, tender nodule less than 1 cm in length with central crusting, localization to helix, age, men, and histologic findings. The histologic examination of the biopsy specimen showed the presence of thinned epiderrnis on center. acanthosis on the adjacent epidermis, with hyperkeratosis and parakeratosis, and cleft between epidermis and dermis. In the dermis, highly vascularized connective tissue and chronic inflarnmatory infiltrate were visible with perichondrial fibrosis. Elastic tissue stain showed degeneration of elastic fibers. After complete excision, no recurrence was obsered until now.

6.Multifocal Reticulum Cell Sarcoma of Bone: A Case Report

A Case of multifocal reticulum cell sarcoma in a 61 year old male is reported. The patient had multiple foci of sarcomatous lesion in the olecranon of the right ulna, medial candyle of the left tibia and in both ankle joints. His initial complaints on admission were pain and swelling in his both ankles and the right elbow of three months duration. Biopsy was carried out on his right ankle joint and diagnosis of reticulum cell sarcoma was made. Surgical excision of the right olecranon and below Knee amputation of the right lower extremity were performed. Following the surgical intervention, Cobalt 60 irradiation therapy was given for 20 days with total amount of 4,000 rads. The follow-up study three months after surgery revealed much improvement and no evidence of recurrence.

8.Closed Locked Intramedullary Nailing for Humeral Shaft Fractures

Intramedullary nailing has become the most popular fixation method in the treatment of the shaft fractures of long bones especially of tibia and femur because it affords most rigid fixation of fracture via closed method which makes early rehabilitation possible. Nowadays intramedullary nailing of the humeral shaft is being performed by many surgeons with good results. Newer implants and better surgical techniques are being developed, which suggests application of this method can be widened. Purpose of this study was aimed to define whether intramedullary nailing could be an effective method or not in the treatment of humeral shaft fractures by documenting the practical points in the application of intramedullary nailing of humerus, effectiveness in fracture healing, any complications or obstacles. We managed thirteen humeral shaft fractures with closed locked intramedullary nailing with distal fanning devices(Seidel nail) from March 1993 to April 1994. Average follow-up period was 14 months(12 months-18 months). The results were as follows; 1, Union of the fracture was obtained at average 12.9 weeks(9 weeks-20 weeks) postoperatively except one case of nonunion, in which case, union was obtained at postoperative 10 months finally. 2. Among the 13 cases, painful limitation of shoulder motion remained in 6 cases. This complication was more common in the cases with protruded proximal end of the nail. Above results suggest that locked intramedullary nailing seemed to be one of the useful method in the treatment of the humeral shaft fractures. However, destruction and irritation of shoulder by the nail was found to be a grave problem, and we found that further investigations should solve this problem.

9.Endoscopec Assisted Ultrasonic Aspiration for Axillary osmidrosis.

Surgical excision of the subcutaneous tissues, with or without skin excision in the axillary hair-bearing area, has been the treatment of choice for Treating axillary a osmidrosis for the several decades. However, long periods of postoperative immobilization of a shoulder joint, partial necrosis of skin flaps or hematoma and the possibility of unsightly scars are frequent complications. For the purpose of reducing these complications, we performed ultrasonic aspiration of subcutaneous fat of the axilla, including the apocrine gland, using ultrasonic liposuction technique under confirmation of endoscopy via one small skin incision. From November 1997 to December 1998, a total of 134 patients (93 women and 41 men) received surgery for bilateral axillary osmidrosis on an outpatient basis. Sixty patients were evaluated more than 6 months after surgery. Among these patients, 6patients complained of a persistent foul odor(10%). Five patients received secondary ultrasonic aspiration for persistent foul odor and were then cured. We concluded that our method has several advantages such as 1) preservation of skin flap vascularity for the prevention of flap necrosis and axillary hair, 2) minimal scarring and bleeding, 3) shorter operation time and postoperative immobilization of the shoulder joint, 4) increased patient comfort, and 5) a safer operative method for recurred cases as a secondary method.

10.A Case of Malignant Acanthosis Nigricans Associated with Gastric Adenocarcinoma.

A 51-year-old female had disseminated, symmetrically distributed, brownish black pigmentation and papillary hypertrophy on the intertriginous and flexural areas for 6 months, and gastric adenocarcinoma was confirmed during the evaluation of internal malignancy, The cutaneous finding of acanthosis nigricans was a significant cutaneous marker of internal malignancy in this case.
