5.Effect of Chlormadinone acetate(Prostal.

Based on the theory that benign prostatic hypertrophy may be induced by androgenic effect of testosterone derivatives, especially 5-alpha - dihydrotestosterone, on prostatic tissue, Chlormadinone acetate(CMA), potent oral synthetic antiandrogen was investigated in the treatment of benign prostatic hypertrophy. Twenty-two patients of prostatic hypertrophy were studied over six months period with a special reference to uroflowmetry and following results were obtained : 1) Chlormadinone acetate induced improvement of obstructive urinary symptoms in terms of uroflowmetric measurement. 2) It is very worthwhile to initiate medical treatment before undergoing any surgical intervention or when surgery is contraindicated.

6.A Case of Surgically Extracted Cysticercus Cellulosae from the Anterior Chamber and Three Cases of Subconjunctival Cysticercosis Cellulosae.

The authors experienced a case of anterior uneitis associated with secondary glaucoma caused by a cycitsercosis cellulosae in the anterior chamber of the left eye of a 39-year-old Korean male. The cyst appeared from the posterior surface of the iris through the pupillary margin of 10-o'clock position and grew larger and larger with automatic active movements, filling finally the nasal upper one third of the anterior chamber, but the cyst did not change the position to the 6-o'clock chamber angle. The cysticercus grown for almost four months was extracted successfully with a teethless, serrated, curved forceps with some resistance after opening the anterior chamber from 7~11-o'clock position without iris prolapse and any damage to the anterior lens capsule. The extracted cyst with the size of 8 mm X 6 mm X 3 mm was confirmed as a cysticercus cellulosae by parasitological examinations (Fig. 2,3). After the extraction of the cysticercus the uveitis and the increased intraocular tension subsided completely, remaining some brownish fine keratic precipitates, pigment masses on the anterior surface of the lens and irregularly somewhat dilated pupil due to partial posterior synechia. The naked vision O.S., however, was 20/25 using pin hole, and the eye has been still quiet during last 3 and half years following surgical extraction. In addition the authors have recently seen three cases of subconjunctival cystiticercosis cellulosae. In the first case the cysticercus with the size of 8 mm X 7 mm X 4 mm appeared at the nasal superior part of the bulbar conjunctiva O.D. of a three-year-old Korean male. In the second case the cyst with the size of 7 mm X 5 mm X 3 mm was seen at the nasal inferior part of the bulbar conjunctiva O.S. of a 12-year old Korean female. In the last case the cyst with the size of 10 mm X 8 mm X 5 mm was located beneath the plica semilunaris of 3-o'clock position O.D. of a 58-year-old Korean female. All the cysts were extracted successfully without any rupture, and they were confirmed as the cysticercus cellulosae histologically.

7.Effect of Enoxacin(Flumark) in Urinary Tract Infection - Clinical and Antimicrobial Susceptibility Evaluation-.

Effect of Enoxacin (Flumark), an oral pyridone carboxylic acid, in the urinary tract infection was evaluated clinically and susceptibility of 292 strains of microorganism isolated from urine to the various antimicrobial agents was tested and compared. Thirty six patients with urinary tract infection were given with 300 to 600mg of Enoxacin daily for 7 to 14 days according to severity of urinary tract infection. The clinical symptoms and urinary findings improved in 30 patients (83.3%) and no clinical response was achieved in 6 patients (16.7%), but revealing 100% response in 9 patients of acute uncomplicated cystitis. In the antimicrobial susceptibility test, Enoxacin was much superior to aminoglycoside, especially against Pseudomonas aeruginosa. Serratia spp., Enterobacter spp. and other Pseudomonas spp.. Enoxacin is a very effective and safe antimicrobial agent in treatment of the urinary tract infection.

8.A Nationwide Epidemiological Study of Mental Disorders in Korea(XVIII): Cross-national Epidemiology of Social Phobia.

The nationwide epidemiological study of mental disorders including social phobia in Korea using DIS-III Korean Version was conducted in the Capital City of Korea(Seoul) and 20 rural areas and compaired to the other 3 countries. The results were as follows: 1) Total number of study of Korea was 5.100, USA 18571, Canada 3258 and Puerto Rico 1513. 2) The highest response rate was Puerto Rico and Korea, USA and Canada in order. 3) Female ratio was highest in the USA and Canada, next is Puerto Rico and Korea. 4) The lifetime prevalance of social phobia of the USA was 2.6%, Canada 1.7%, Puerto Rico 1.0% and Korea 0.5%. The Highest ratio was the USA and Canada, Puerto Rico and Korea in male and in female in order. 5) Mean age of onset, Canada was earliest(14.6yrs. old) and USA(15.8), Pueno Rico(19.8) and Korea(24.3). In male the earliest was USA and Canada, Puerto Rico and Korea. Onset of social phobia before onset of other psychiatric disorder, the highest was Puerto Rico and USA Korea and Canada. 6) Comorbidity was the highest in the USA and Canada, Puerto Rico and Korea. 7) The effect of social phobia of the risk of suicide attempts in persons with any other psychiatric disorder, in case of comorbid with social phobia was much more frequent than not comorbid with social phobia. 8) Eating fear in front of people was the highest in the USA and Canada, Korea and Puerto Rico in order. Fear of speaking in front of group was the highest in Puerto Rico and USA Canada and Korea. Fear of speaking to strangers was the highest in Korea and Canada, USA and Puerto Rico.

The nationwide epidemiological study of mental disorders including suicide ideation and suicide attempts in Korea using DIS-III Korean Version was conducted in the Capital City of Korea(Seoul) and 20 rural areas and compared to the other 8 countries. The results were as follows : 1) The lifetime prevalence of suicide ideation were 18.51% in New zealand, 15.62% in Munich, Germany, 14.20% in France, 11.25% in Canada, 11.18% in the USA, 9.5% in Puerto Rico, 5.28% in Taiwan and 2.09% in Lebanon, respectively 2) The lifetime prevalence of suicide ideation was higher in female than male Divorced and separated persons were at the highest risk and next were never married. Married persons were at the lowest risk. 3) The lifetime prevalence of suicide attempts were 5.93% in Puerto Rico, 4.95% in France, 4.43% in New Zealand, 3.82% in Canada, 3.44% in Munich Germany, 3.20% in Korea, 3.13% in the USA, 0.75% in Taiwan and 0.72% in Lebanon, respectively. 4) The lifetime prevalence of suicide attempts were highest in female and divorced or separated. Next were never married and lowest were married.

10.Efficacy of Norfloxacin(Neutam 400.

Forty five patients with urinary tract infection were treated with 100mg Norfloxacin twice daily for 3 or 7 days according to the seriousness of infection. Of the 19 patients with uncomplicated urinary tract infection including 5 patients with acute bacterial prostatitis or acute epididymitis, none had bacteriuria 7 days after therapy and 1 patient was reinfected 14 days after therapy discontinuation. Of the 26 patients with complicated urinary tract infection, 17 patients(65%) were uninfected 7days after therapy and among them, 3 patients were reinfected I4 days after discontinuation of therapy. Of the 9 patients with persistent infection, 4 patients had resistant bacteriuria and 5 patients had bacteriuria different from that found before therapy. Eradication rate of E. coli, Enterobacter spp., Proteus spp., Pseudomonas spp., and Serratia spp. were 100%,, 100%,, 100%,, 80% and 40%, respectively. Further clinical envaluation seems worthwhile especially in pseudomonas infections since it offers one of the very few alternatives for oral treatment of pseudomonas infections which today may require hospitalization only for the purpose of injectible antibiotics.
