1.An Experimental Study of Chemexfoliation Using Trichloroacetic Acid.

BACKGROUND: Trichloroacetic acid chemexfoliation has gained popularity and is widely used cosmetically. The depth of the wound caused by the various dilutions of TCA is paramount to the therapeutic efficacy. Therfore rational use of TCA chemexfoliation requires a clear understanding of the expected depth of destruction caused by a given dilution. OBJECTIVE: The purpose of this stuely is to quantitate the wound deptlw produced by varying concentrations of TCA. We have stirdied takie effects of consecutive applications, occlusion and tretinoin pretreatment on wound depth in TCA clinimical peeling. METHODS: Various concentrations of TCA and distilled water were aplied to two pairs of test sites of a guinea pigs back in a uniform fashion with cotton-tip applicators and then we have measured the mean depth of epidermal necrtsis mieroscopically by using Visual Imagc Data Analysis System. RESULTS: 1. Depths of wounds createdly threre consecutive applications of 20, 35% and 50% TCA were deeper than those of wounds by a single application of TCA. 2. Depths of wounds created by application of TCA with the occlusive method were deeper than those of the open method. Therefore the effect of occlusion is to enhance the penetration of TCA. 3. Depths of wounds pretreatwi with 0.05 % tretinoin daily for 7 days are deeper than those of the control in 50% TCA chemical peeling. Therefore the effect of 0.05% retinoin pretreatment is to enhance the penetration of TCA. CONCLUSION: This study showed that consecutive applications, oeclusion and tretinoin pretreatment in TCA chemexfoliation enhance the peietration of TCA.

2.A Case fo Infantile Digital Fivromatosis.

3.A Case of Granuloma Annulare.

Granuloma, annulare is a chronic dermatosis characterized by skin colored, firm intradermal papules or subutaneous nodules, which may be discrete or arranged in an annular configuration. Histologically the dermis shows well define8 areas of necrobiosis of collagen tissue surrounded. by palisading histiocytes and lymphocytes. There may be lobules of epithelioid cells ani multinucleated giant ce.1s surrounding the necrobiotic areas. Many factors have been implicated in the genesis of this reaction. Recent studies have indicated that delayed hypersensitivity reactions rnay be involved in the pathogenesis of granuloma annulare. We present a typical granulorna annulare on a 16 year-old boy who had flesh colored, hyperkeratotic small nodules arranged in ring fashion on the dorsum of hands and right ear auricle.

5.A Case of Cutis Marmorata Telangiectatica Congenits.

Cutis marmorata telangiectatica congenita, which was first described by Van Lehuizen in 1922, is characterized by the presence at birth of a persistent cut;is marmorata pattern, phlebectasia, spider nevus-like telangiectasia, and areas of ulceration and steady improvernent with time. We reported a case of cutis mar.morata telangiectatica. congenita occuring in a 70 days old female infant showing the reticulated mottling and atrophy of the skin on the both lower extremites, right upper extremity, abdomen and buttock muscle weakening of right upper extremity and left lower extremity compare with the opposite side. Skin lesions were much improved during the observation period of 10 months without any other specific treatment except hermiatrophy of right upper extremity.

6.Multiple Neurilemomas Associated with Neurofibromatosis.

Multiple neurilerv,omas occurred i.n a 14-year-old boy with neurofibromatosis and right eye blindness. His father succumbed to neurofibromatosis at the age of 42. Histologically-proved neurilemornas and neurofibromas were noted on the face, neck, trunk and extremities, but no cafe au-Iait spots ohserved. There were nevus anemicus, hairy nevus and pigmented moles. The associated features in this patient includc muscular atrophies, weakness of the left hand, parest,hesia of the medial side of left forcarm and Dupuytrens contracture of left fourth finger.

7.Pachyonychia Congenita.

A case of pachyonychia congcnita occurring in a 8 year-old boy is described. A few weeks after delivery, his mother noticed thickening of all the finger and toe nails of her baby. Family history was denied. In addition to characteristic features of pachyonychia congenita of all the nails, follicular keratotic papules were noted on the knees, elbows, buttocks and legs, and also tender thick calluses, on the soles with palnioplantar hyperhidrosis. There were three cornu cutaneurn on the popliteal and left antecubital fossae. Other associated fea.tures include a painful tense bulla at the left fourth finger, irnpetiginous patches on the anterior thighs, hoarseness and leukokeratosis linguae, sparseness of the scalp hairs, and granulosis rubra nasi accompanied by facial hyperhidrosis.

8.Multiple Trichoepithelioma Occurring in a Son and his Mother.

A 23-year-old man had multiple small, skin-colored firm papules on the both sides of the nasolabial folds since the age of approximately 17. His mother had a similar skin lesions on her face. The histopathological findings of skin biopsy specimens from patient(son) showed horn cysts consist of a fully keratinized center surrounded by basophilic cells and tumor islands composed of basophilic cells arranged in a lacelike or adenoid network and occasional solid aggregates and there are also a foreign body giant cell reaction in the vicinity of the ruptured horn cysts.

9.Nail Avulsion by Potassium Iodide Treatment in Onychomycosis.

The effect of 50 per cent potassium iodide (KI) ointment with occlusive dressing on onychomycosis of the fingers and toes was studiei in 54 patients with 176 diseased nails. The results obtained were as follows. 1. Of a total of 76 fingernails and 100 toenails in 54 patients, all fingernails and toenails showed an excellent response with easy removal of the diseased nails. 2. The duration that the occluded KI aintment remained in place before successful avulsion was 6. 6 (range, 3 to 18) days for fingernails, and 6. 8 (range, 3 to 20) days for toenails with onychomycosis. 8. Of 16 nails of control group which were treated with vaseline, none showed the significant improvement. 4. The problems during this treatment were; mild irritation and/or itching (9 cases), and pin-point bleeding on curettage of nail bed (18 cases). It is suggested that the KI treatment has many advantages and seemed to be an ideal method of treatment of onychomycosis.

10.Effect of Various Fungi on the Aflatoxin Productivity in the Culture of Asp. Flavus.

This study was attempted to know that the interactions of various fungi, and methionine and MgSO4 introduced as the substrate of culture media for fungi were affected to produce aflatoxins by Asp. flavus. 5 different fungi were isolated from the fermented soybean mash and were cultured in Chemically Defined medium (C.D. media) and soybean mash at 25 degrees for 10 days. (1) It was confirmed that Asp. flavus produced aflatoxins in the C.D. medium and soybean mash, but that Asp. niger, Asp. oryzae, Asp. awamori and Asp. terreus did not produced them respectively. (2) Asp. flavus cultured with Asp. niger did not produce aflatoxins in C.D. medium, but produced in soybean mash, in other hand, Asp. flavus with other fungi except Asp. niger produced aflatoxins in C.D. medium and soybean mash. (3) The growth of fungi were more prosperous in the separate culture than in the mixed culture. (4) In the C.D. medium added 20% of cultured medium of Asp. niger, Asp. flavus did not produce aflatoxins but other cultured medium did not prohibit the production of aflatoxins by Asp. flavus. (5) On the contrary, MgSO4 increasing the productivity of aflatoxins by Asp. flavus in the C.D. medium methionine known as one of precurser of aflatoxins did not affected the increasing productivity with significance.
