1.Multifocal Reticulum Cell Sarcoma of Bone: A Case Report

A Case of multifocal reticulum cell sarcoma in a 61 year old male is reported. The patient had multiple foci of sarcomatous lesion in the olecranon of the right ulna, medial candyle of the left tibia and in both ankle joints. His initial complaints on admission were pain and swelling in his both ankles and the right elbow of three months duration. Biopsy was carried out on his right ankle joint and diagnosis of reticulum cell sarcoma was made. Surgical excision of the right olecranon and below Knee amputation of the right lower extremity were performed. Following the surgical intervention, Cobalt 60 irradiation therapy was given for 20 days with total amount of 4,000 rads. The follow-up study three months after surgery revealed much improvement and no evidence of recurrence.

6.A Case of Oculopharyngeal Muscular Dystrophy.

Oculopharyngeal muscular dystrophy, one of the variants of chronic progressive external ophthalmoplegia, is a disorder characterized by progressive bilateral ptosis, immobility of the eyes and dysphagia. We have experienced a 46-year-old woman complaining of bilateral ptosis, mild dysphagia and immobility of the eyes. Oculopharyngeal muscular dystrophy was confirmed by clinical, electromyographic and histopathologic examination. Therefore, we report a case of oculopharyngeal muscular dystrophy with review of literatures.

8.A case of incontinentia pigmenti.

Down syndrome is the most common autosomal chromosomal abnormality characterized by mental and growth retardation, and by various typical features including prominent epicanthal fold, oblique palpebral fissure, flat nasal bridge, short and broad hand, wide toe interspace, etc. The overall incidence has been shown to be 1:800 deliveries, increasing with advancing maternal age. However, twin cases are extremely rare, and thus far only 500 cases were reported worldwide. We have recently observed 10-day-old male monozygotic twins with Down syndrome, born to a mother of 30 years of age with one normal child. Both have VSD confirmed by 2D-echocardiography, in addition to various typical features. Cytogenetic examination revealed that both have karyotypes of 47, XY, +21. This is the first report in Korea as the authors are aware of.

9.Clinical Evaluation of Myasthenia Gravis Using Electromyography.

A clinical study was performed on 27 patients with myasthenia gravis diagnosed at the Department of Ophthalmology, St. Mary's Hospital, Catholic University Medical College from January 1986 to December 1989, including electromyography, chest CT scan and thyroid function test. The results were as follow: 1. The patients were classified as group I;14 patients(51.9%), most common, group II(A);8 patients(29.6%) and group II(B);5 patients(18.5%) based on the modified Osserman's classification. 2. Ocular symptoms were ptosis in 25 patients(92.6%), diplopia in 10 patients(34. 4%), limitation of ocular movement in 4 patients(14.8%). Systemic symptoms were dysphagia and weakness of extremeties in 6 patients(22.2%) and general weakness in 5 patients(18. 5%). 3. In EMG study, among the 27 patients, 17 patients(63%) revealed positive response. Positive rate according to clinical classification was 50%(7 of 14 patients) in group 1,62.5%(5 of 8 patients) in group II(A) and 100%(all 5 patients) in group II(B). The more severe the symptoms, the more positive were the findings. 4. The associated thymic disorders were thymoma in 4 patients(14. 8%), thymic hyperplasia in 2 patients(6.9%) and malignant thymoma in 1 patient(6.9%).

10.Clinical Analysis for Complications of Nd-YAG Laser Posterior Capsulotomy in Uveitis Patients.

Nd-YAG laser is known as a safe and effective treatment for posterior capsular opacity after cataract surgery. We studied the ocular complications after Nd-YAG laser treatment in uveitis patients. From January 1991 to December 1996, we retrospectively investigated the complications of uveitis in 22 persons(25 eyes) after Nd-YAG laser posterior capsulotomy, and also investigated that of nonuveitis in 563 persons(635 eyes) as a control group. The uveitis cases consisted of six anterior uveitis, eight intermediate uveitis, seven Behcet`s syndromes, three panuveitis, and one posterior uveitis. In the uveitis group, the complications(18 eyes) consisted of three retinal detachments, one giant retinal tear, three glaucoma, two aggravations of uveitis, two vitreous opacities, one retinal emorrhage, one proliferative vitreoretinopathy, one hyphema, and four transient high intraocular pressures(IOP), and in the control group, it consisted of two subluxations of intraocular lens(IOL), three glaucomas, one retinal tear, three transient high IOPs, two vitreous opacities, two retinal hemorrhages, three macular holes, two retinal detachments, and five cystoid macular edemas. From our experience, the incidence of complications after Nd-YAG laser posterior capsulotomy in uveitis was higher than that in nonuveitis. It was also noted that it is necessary to follow up thoroughly the complications after Nd-YAG laser posterior capsulotomy in uveitis patients.
