2.Clinical study of pigmemted Nevi on the palms,soles,and genitalia in Korean.

BACKGROUND: There are reviews with regard to the treatment of pigmented nevi present on the palms, soles, and genitalia. One advocated that pigmented nevi on thesa locations should be prophylactically removed to avoid their tuture development into malignant melanoma, while the other believes that this is not practicable. OBJECTIVE: Our purpose was to determine the incidence rates and clinical characteristics and to discuss the management of pigrnented nevi, developed on these areas of the Korean men. METHOD: We examined 1,537 Korean men, between 19 and 21 rears old, to evaluate the pigmented nevi on their palms, soles, and genitalia. RESULTS: Atleast 1 pigmented nevus appeared in 314(20.4%) men on the palms, 189(12.3%) on the soles, and 117(7.6%) on the genitalia. The sizes(mean diameter) of nevi on genitalia, soles, and palms were 4.5mm, 3.1mm, and 2.3mm respectively. The color of the nevi was darkest on the penile shaft(4.3 point) and then glans penis(3.9 points), soles(3.2 points), palms(2.5 points) in decreasing orders of the points. Most of the nevi was consistent with type 1 by Levers clinical classification and their incidence were 357 on palms, 211 on soles, and 128 on genitalia. Among the 21 nevi of type 2, 13 nevi appeared on the soles, and noine of type 3 and type 5 were found. The distribution of the nevi on the palms was of a rather uniform and scattered pattern, whereas, on the soles, they were distinctly concentrated in the midfoot. Tkie nevi on the glans penis appeared larger and more irregular in outline than those on the penile shaft. CONCLUSION: The plantar nevi were larger, darker, and had greater incidence of elevated type compared with the palmar nevi. From this observation, particular attenticn should be paid to the presence of pigmented nevi on the soles.

5.Patch Test to Silk Industry Workers.

Hand dermatitis of silk industry workers had been a troublesome problern for a long time. It was said the cause was due to toxic reaction by denatured pupae and physical reaction from high water temperaturc. During Augnst and September 1973, patch test was performed with pupa and cocoon to 680 workers in 4 separate silk factories in order to persue the factors which may cause their hand dermatitis. Among 475 workers who had performed first patch test, only 12 workers in one of four factories showed positive reaction in pupa and we came to know that only that factory had used formalin in order to protect denaturation of pupae and cocoons. Second patch test with pupa and 1% formalin was repeated to 130 subjects including 6 of 12 positive cases in the first pateh test. Five Subjects shosved positive reaction in pupa, also 5 in 1% formalin and 2 of them showed positive reaction in both pupa and 1% formalin. In the second patch test, 3 of 6 who had been positive in pupa at first patch test were positive at pupa. It seemed to be that toxic or allergic theory is not enough to explain clearly the cause of hand dermatitis in silk industry workers. Further stucly is necessarv to elucidate how the silk fiber, water temperature and parasites in the pupa and cocoon contribute to the hand dermmatitis.

6.Five Cases of Microsporum Gypseum Infection in Seoul Area.

Microsporum gypseum, long considered to be of animal origin, has recently been found to be soil inhabitating fungus and abundant in soil throughout the world. M. Gypseum infections tend to increase nowadays in Korea. Nineteen cases of ringworm caused by M. gypseum were reported in Korean literature until 1980 to the best of our knowledge. We present 5 cases of M. gypseum infection confirmed by culture findings on Sabouraud agar media and microscopic examination. The first case was 4 year-old boy with solitary coin sinzed pruritic annular erythernatous plaque ontaining papules, pustules and scales on the right cheek of on month duration. The second case, 7 year-old boy, had solitary walnut sized ovoid plaque consisted with papulopustules, oozing and peripheral scaling on right cheek of one month duration. The third case, 5 year-old boy, had solitary egg sized pruritic erythematous plaque with papuIes, scales and crust on the right lower leg of three weeks duration. The fourth case, 63 year-old female, had solitary egg sized sharply demarcated erythematious scaly patch with central clearing on nose and. nasolabial area of 10 days duration. The last case was 2 year-old girl with solitary coin sized annular erythematous scaly pIaque on left lower leg of two weeks duration.

7.Clinical Study of Scabies and Itch Mite.

The clinical and biological study for Sarcoptes scabiei from 66 scabies patients was performed form Nov. 1974 to April 1975. The diagnosis of these scabies patients were confirmed by method proposed by Muller et al. The results were as follows; 1) Male to female ratio is 5. 6: 1 in total patients and predominant age group is in 16~20 in both sexes(Table 1). 2) 50% of cases had family history of similar troubles and the duration of subjective symptom complaind at their first visit was 1~2 weeks in 20 cases, 3~4 weeks in 25, 2 months in 10, and over 3 months and unknown were 12. 3) Suspected sources of infestation were, 22 cases from one of their family members, 15 at hotel, 7 at boarding house, 2 at camping, 1 at military camp, 19 were unknown. 4) Distribution of burrows was hand(79.4%), wrist(12.4%), foot(3.1%), penis(2.6%) scrotum(2.1%) and axillary area(0.5%). Number of burrows was 1~2 in 61.4%, 3~4 in 19.3%, 5~6 in 5.3%, 7~8 in 1.8%, 9~10 in 5.3% and 10 in 7. 6%. 5) 44 patients (66.7%) were confirmed by finding of female mite and among them 50% had 1 female mite, 36.7% had 2, 6.8% had 3, 2.3% had 4 and 4.5% had 6. 6) 69 among total 79 female mites detected were found at burrow, 6 from papule and 4 from vesicle. Total 7 male mites were found from patients and 4 from papules and 2 from veside and 1 from pustule(Table 5) 7) From total 194 burrows examined 69 female mites (36.1%) were found and non of male mite were found. 8) Mean size of female mite(length*breadth) was 359.3u*266.5u, male mite 21.1u*160.9u, larva 131.8ux94.2u, egg with developing larva 167.3*93.6u, egg with developed larva 159.7u * 92.3u and scybala 18.7u x 12.3u. Sizes of egg with developing larva and egg with developed larva had no significant difference, statistically.

8.A Case of Congenital Long OT Syndrome with Pseudo - Atrioventricular Block.

The congenital long-QT syndrome (LQTS) is characterized by recurrent syncope, prolonged QT intervals, QT interval lability, polymorphic ventricular tachycardia, and sudden death. We report a case of congenital long QT syndrome in a 28-day-old male infant who presented with syncope, bradycardia with 2: 1 pseudo-atrioventricular block and a markedly prolonged QT inteval. One episode occured after crying and degenerated into ventricular fibrillation and terminated after cardioversion. A VVI type cardiac pacemaker was implanted. Subsequently, the infant's heart rate was over 110/min and 2: 1 AV block and any other arrhythmia were absent. The infant recovered from the accompanied pneumonia and sepsis and was discharged 47 days after adrnission. However, 13 days after discharge, the infant returned to our hospital ER with syncope. Ventricular fibrillation ceased after cardioversion. Despite medication with propranolol, ventricular tachycardia persisted. The infant expired the day after he was discharged against medical advice.

9.A Case of painful Fat Herniation.

Painful fat hemiation is characterized. by the soft, skin colored painful papules on the heels that appears only when weight is bome and disappears when weight. is taken off the heels. The papules were thought to be caused by hemiation of adipose tissue through connective tissue defect. To our knowledge, such a case or similar one has not been reported previously in Korea. The patient was 30 year-old housewife, who have had soft, skin colored painful papules on the medial aspect of the both heels when weight was placed on the foot. Routine laboratory findings were within normal limit. Skin biopsy revealed nodules of lobulated adipose tissue on the mid-dermis far above the original subcutaneous tissue and large venous channels with thick muscular walls. Hypertrophied peripheral nerve trunks were scattered adjacent to the above vascular structure. Diagnosis was confirmed by clinical and histopathological features. Literatun was briefly reviewed.
