BACKGROUND: One of the main disadvantages of caudal block is the long latent period before a satisfactory blockade is obtained. Many investigators have used various preparations of local anesthetic solutions to improve the speed of onset. This study was performed to evaluate the effectiveness of prewarming of lidocaine HCl for caudal block. METHODS: Fifty healthy young patients (ASA I) were allocated into two groups, A and B. In group A, the local anesthetic solution were injected at room temperature (25 degrees C), while in group B, they were injected at 37oC. All the caudal block were performed using 2% lidocaine HCl 20 ml with fentanyl 100ug and epinephrine 1:200,000. The onset time was defined as the period from completion of injection until the patient first noticed loss of sensation to pin-prick on perianal region. Assessment of sensory loss was made at 15 seconds interval. We have compared the onset of sensory blockade between groups. The duration of analgesia and any significant side effects were also recorded. RESULTS: The onset of sensory blockade was significantly faster in group B (3.5 +/- 0.5 minutes) than group A (6.2 +/- 0.9 minutes). The duration of analgesia were not significantly changed between groups. Side effects of urinary retention, pruritus and nausea were noted between both groups but the difference was not significant. CONCLUSIONS: We have found that the onset time was 44% faster with warm lidocaine-fentanyl mixture (37 degrees C) than with the room temperature (25 degrees C). The improved clinical usefulness was achieved with no increase in side effects. The technique is a safe and effective method to reduce the latency of onset.

A 25-year-old female was seen because of rnultiple, asymptornatic papular eruptions on the vulvar area since 1 year ago. She had been suffered from palmar hyperhidrosis for 3 years. A skin biopsy specimen from the vulval lesion revealed characteristic findings of syringoma.

A 72 years old man had an 1 x 1cm sized, solitary glomus tumor on the right knee joint and had suffered from sharp, episodic pain and tenderness, radiating to the right thigh and the buttock for about 20 years. Simple surgical excision was performed for removal of the tumor mass and the subjective symtoms. There is no recurrence until now.

Fractures of the femoral head associated with hip dislocation are uncommon injury and all traumatic fracture-dislocation of the hip must be treated as surgical emergencies. But, there was no uniformity of treatment of fracture-dislocations of the hip were treated by operative and conservative methods in Chonbuk National University hospital were observed between March 1983 and January 1987; Operative treatment is included screw fixation method and fixation with bone pegs. And so, the followings were summsrized: 1. The most common cause was dash-board injury and patients sex was msle in majority. 2. Frequency of the type of femoral head fracture was the most in Pipkin type I, the next was type IV. 3. The better result was noted in operative treatment than in conservative method as above good. 4. Bone pegging method was relatively not superior to screw fixation. 5. The following complications were encountered, 1 case of sciatic nerve palsy, 1 case of traumatic arthritis.

Glomus tumor is a kind of vascular tumor that arises from the glomus body, which regulates skin temperature and is placed in the skin and the subcutaneous area. It is a benign tumor that usually presents in the subungal area. It is relatively common in areas other than the fingers, but its occurrence in peripheral nerves is known to be comparatively rare. We report our experience with a case of glomus tumor arising from the brachial plexus, a rare site of occurrence for glomus tumors.

PURPOSE: Cataract and refractive surgical procedures are now among the most common and successful surgeries in medicine. Since 1995, we have annually surveyed the members of the Korean Society of Cataract and Refractive Surgery(KSCRS) on cataract surgery technique, anesthesia, intraocular lens and refractive surgery to understand the current situation and future trends in these fields. METHODS: Fifth annual survey forms consisted of 80 multiple-choice questions mailed in January 2000 to 234 ophthalmologists of the KSCRS. Approximately 47% of the questionnaires mailed were returned by the February cutoff date. Current data were compared with previous annual survey and data from Japan and USA. RESULTS: Hospitalized period is decreased annually and use of topical anesthesia(38%) increased steady. Self sealing wound construction was the main wound closure technique in phacoemulsification accounted for 41%. Most preferred intraocular lenses for small incision cataract surgery are silicone(60%), acrylic (33%), and PMMA(7%). Especially, acrylic intraocular lens had been used highly. Viscoelastics(38%) produced by Korean medical company becomes widely used compared with previous year. 61% of the respondents were performing excimer laser keratomy for refractive surgery. Its complications are the regression of visual acuity(39%), halo or glare(36%), and irregular astigmatism(15%). 52% of the respon-dents performing LASIK had the experience of thin or perforated flap formation, and 48% had free cap formation. Also, legal problems on excimer laser and LASIK have been increased highly. CONCLUSION: From this survey, we found current trend and change in cataract and refractive surgery in Korea are similar to those of Japan and USA.

OBJECT: This cross-sectional study was performed in order to evaluate the prevalence of tardive dyskinesia among the hospitalized schizophrenic patients. METHODS: Four hundred nineteen hospitalized schizophrenic patients(male=263, female=156) were recruited for this study. They were treated with antipsychotics for more than 3 months. The prevalence of tardive dyskinesia was assessed by the Abnormal Involuntary Movement Scale. RESULTS: The prevalence of tardive dyskinesia was 35.6%(Male=36.9%, Female 33.3%). There were no significant differences in the prevalence of tardive dyskinesia among male and female schizophrenic patients. The prevalence of tardive dyskinesia among the patients over 30years old was much higher than those below 30years old. There were no significant correlations between the prevalence of tardive dyskinesia and the duration of hospitalization, the total amount of antipsychotics. The frequently involved parts of the body in the schizophrenic patients who have tardive dyskinesia were tongue, upper extremity, lips and perioral area, jaw, lower extremity, muscles of facial expression trunk, respectively. CONCLUSIONS: There was significant correlation between the age and the prevalence of tardive dyskinesia in the antipsychotic-treated schizophrenic patients.
