1.Intracranial calcified schwannomas: report of two cases.

Calcification is exceedingly rare in schwannomas. In the literatures, we found only three reports of schwannomas with calcification. We report two cases of intracranial calcified schwannomas, one in the anterior cranial fossa and the other in the middle cranial fossa.

2.Rud's Syndrome.

Rud's syndrome (RS), basically composed of ichthyosis, mental deficiency and hypogonadism, is a rare hereditary disease. Some varying dermatologic, neurologic, endocrinologic, ophthalmologic and musculoskeletal abnormalities have coincided with RS. No case of RS has been documented from Asian countries except one from Japan. We describe a 16-year-old girl who presented with lamellar ichthyosis, mental retardation, hypogonadism, short stature, alopecia, sparse eyebrows, strabismus, cataracts, and congenital dislocation of the hip. To our knowledge, RS coexisting congenital dislocation of the hip herein is the first case in English literature.

3.A Case of Tuberous Sclerosis with Psoriasis.

A 10-year-old boy had pin-head sized, discrete, yellowish waxy, translucent papules on the face and also, various sized round, circumscribed, erythematous patches with silvery white scales on the trunk. The clinical and histopathologic findgs were compatable with diagnosis of tuberous sclersis associate with psorlasis. There were strong familial history during 3 generation in psoriasis(Fig. 1) Both psoriasis and tuberous sclerosis are heritable skin diseases. Some heritable dermatologic conditions found in tuberous sclerosis including ichthyosis, hypertrichosis and palmar and plantar keratoses but there is no report of tuberous sclerosis associated with psoriasis in published literatures.

5.Treatment of Alopecoa Areata with Dinitrochlorobenzene.

This study was designed to investigate the therapeutic effect of dinitrochlorobenzene (DNCB) on alopecia areata. Forty patients with alopecia areata were sensitized by applying 0.1ml of 2% DNCB dissolved in aceton solution on the voiar aspect of left forearm. Patients were subsequently chaIlenged at weekly mtervals with 0. 1% DNCB solution appIied to one side of affected area for periods of 2 to 8 months. The other side of affected area was reserved as a contol site. The results were as follows. 1. In 29 out of 40 patients (72.5%), hair regrowth occurred either excIusiveIy on the treated site (18 cases) or considerably faster and denser on treated site(11 cases)(Table 1) 2. In the majority of cases, the regrowth of hair was observed, within eight weeks of the treatment. 3. The response to DNCB depended on the severity of the aIopecia. The response rate were 80.9% (17/21) in moderate degree (<25% bald area) cases, 68.7% (ll/16), in the extensive degree(>25% bald area) caes and 33% (1/3) in alopecia totalis(Tale 1) 4. The rate of response depended on the duration of the alopecia. Satisfaetory results were obtained in 19 out of 25 patients with a history of less than one year(76%), in 7 out of 10 patients with a history of one to three year(70%) and in 3 out of 5 patients with a history of more than three years (60%) (Table 2) 5. The main side reactions were burning sensation (3 cases), severe eczema (2 cases), folliculitis (2 cases) and cervical lymphadenopathy(7 cases). 6. Liver function test and complete blood cell count were in normal range during the course of treatment in 5 randomly selected patients. 7. Although the treatment of alopecia areata with DNCB still remains to be an experimental trial, this therapy could be substituted for corticosteroids in selected cases.

7.Surgical Treatment of Post - traumatic Kyphosis.

Post-traumatic kyphosis is generally recognized as the result of failure of initial treatment in injured spine. The purposes of this study are to find out the surgical indications in residual post-traumatic kyphosis and to analysis the result of operative correction in post-traumatic kyphosis. The authors analyzed 14 cases of post-traumatic kyphosis, operated from Jan. 1992 to Mar. 1996. Their injuries were estimated initially compression fracture in 12 cases, hurst fracture in 2 cases. Their initial treatments were conservative method in 12 cases, anterior fusion in 1case & laminectomy in 1 case. The results of this study are as follows: 1) By standard Cobb lateral measurement, the mean preoperative kyphosis of 32.9degrees was reduced to 14.2 degrees(56.8% correction ratio). 2) Symptomatic improvement was definite, stooping with fatigue pain, mid back pain and accompanied claudication were improved in all cases. And low hack pain due to compensatory lordosis was improved in 5 of 6 cases. 3) We acquired bone union in all cases. We concluded that some thoraco-lumhar fractures could be underestimated in supine X-ray and needed more meticulous evaluation and follow-up. In addition, preexisting symptomatic lumhar degeneration below fracture should be considered in initial surgical decision making of post-traumatic kyphosis.

9.Effect of Additional Epinephrine on Spinal Anesthesia with Bupivacaine.

Background: Bupivacaine was introduced to be a long-acting spinal anesthetic agent. It has been argued about whether the addition of epinephrine prolongs the bupivacaine action or not. The aim of this present investigation was to find out the effect of additional epinephrine on spinal anesthesia with bupivacaine. Methods: 47 patients undergoing an operation on lower extremity were randomly allocated to 2 groups. All patients were anesthetized by subarachnoid block with 0.5% bupivacaine in 8% glucose, which was mixed with 0.2 ml of normal saline in group non-E (n=24) and 0.2 ml of 1:1,000 epinephrine in group E (n=23). We evaluated blood pressure and heart rate, the sensory and motor blockade and voiding time after spinal anesthesia. Results: The systolic blood pressure (SBP) at 8 and 10 min after anesthesia were lower in group non-E than in group E (p<0.05). The trend of decreasing diastolic blood pressure was similar in both groups. The heart rate(HR) at 2 min after anesthesia was lower in group non-E than in group E (p<0.05). The sensory block in T10 was produced faster in group non-E (7 min) than in group E (11 min)(p<0.05). And the maximal sensory block level and its reaching time was T7 and 14 min in group non-E, and T8 and 17 min in group E (p=0.12, p=0.11). Two segment regression time was 124 min in group non-E, and 184 min in group E (p=011). The onset time of motor block to Bromage scale 3 was 12 min in group non-E and 16 min in group E (p=0.06). The recovery time from complete motor block to Bromage scale 1 after maximal motor block was 263 min in group non-E, and 278 min in group E. The time at which patients voided after anesthesia was 469 min in group non-E, and 466 min in group E. Three patients urinated by using a urinary catheter in each group. Conclusions: The addition of epinephrine to bupivacaine for spinal anesthesia can slow the decrease in SBP and increase the HR at early stage of anesthesia, and slow the sensory block.

10.Quantitative Changes of Immunoglobun Levels in the Serum of Allergic Dermatoses Patients.

With development of immunochemical methods for anlysis of serum proteins, quantitative determination of serum immunoglobulin levels and purification of them have been made possible in recent years. There are numerous reports about immunoglobulin levels in various diseases. A search of the literature reveals very few reports of quantitative immunoglobulin determinations in allergic dermatoses. The present study was conducted to find quantitative changes of serum immunoglobulins in a few allergic dermatoses. To quantify the IgG, IgA and IgM, one hundred and two sera were analysed form 30 normal control group, 30 contact dermatitis patients, 12 stopic dermatitis patients and 30 urticaria patients by modified Oudin's capillary tube method. The results are as follows. 1.In contact dermatitis the IgG was significantly increased to compare with control group(p<0.01) 2.In atopic dermatitis the IgA was significantly increased to compare with control group(p<0.01) 3.In urticaria the IgG was increased (p<0.05) 4.The IgM has no differences between allergic dermatoses and control group.
