3.Clinical Experience with the Palmaz-Schatz Coronary Stent: Initial Results and 7 Month Follow-up.

BACKGROUND: Although percutaneous transluminal coronary angioplasty(PTCA), first reported in 1977, represents a major advance in cardiovascular therapeutics, acute closure and restenosis are major limitations of PTCA. The focus of this report is to describe the procedural results and short-term follow-up obtained at Yonsei Cardiovascular center. METHODS: We implanted 12 balloon expandable Palmaz-Schatz stents in 12 patients with significant stenosis of coronary artery. Five patients presented as candidates for primary stenting and the remainder presented with restenosis. Clinically, there were unstable angina in 4, stable angina in 5, old myocardial infarction with stable angina in 2, and old myocardial infarction in 1. The stented vessels were the right coronary artery in 4 and left anterior descending artery in 8. All patients received a single stent. The sizes of stents were 3.0mm in 9, 3.5mm in 2, and 4mm in 1. The lesion morphology according to AHA/ACC classification were type A in 1, type B in 10, and type C in 1. The extent of coronary artery disease was 1-vessel in 4, 2-vessel in 6 and 3-vessel in 2. RESULTS: Successful delivery was accomplished in all patients and complications including acute and subacute thrombosis bleeding requiring transfusion, myocardial infarction, and death were absent. Vessel patency after mean follow-up of 7 month showed restenosis in 4 out of 7 patients : 50% in 1, 60% in 1., 0% in 1 and total obstruction in 1 patient. PTCA was done in the patient with 80% restenosis successfully. CONCLUSION: Balloon-expandable intracoronary stenting is a feasible method for treating the acute complication of balloon angioplasty. It seemed to reduce the rate of restenosis for single stent implantation, but long-term results and indications should be evaluated more extensively.

4.Geometrical design of bilobed flap for nasal reconstruction(5 cases).

The nose has number of characteristics that make it unique when planning or reconstruction of surgical defects. The topography of nose is complex with multiple adjacent convex and concave surfaces that must be preserved. The free margins of the alar rims are mobile and easily distorted in case of inadequate planning. The skin over the lower one third of the nose id liss mobile and, therefore, cannot be easily recruited for closure of small defects. In addition, the texture and color of this skin are so unique that neither distant nor nearby skin can always provide a good match. Finally, the function of the nose must always be considered by preserving or replacing the bony and cartilaginous framework, mucosal linig, and never compromising a patent airway. The bilobed flap is particularly suited for reconstruction of small sized or medium sized nasal defects up to 1.5 cm in diameter. By definition, it is a double transposition flap and it is designed to move more skin over a longer distance than would be possible with a single transposition flap in the same location. On the lower one third of the nose where the skin is the least mobile, the bilobed flap allows the surgical site to be covered with nearby skin matched for color and texture and then allows for repair of that secondary defect with another well-matched flap whose donor site can finally be closed primarily. This flap also results in little or no distortion of the nose since the flap efficiently recruits skin from distant and more lax sites. While the standard design often results in tissue protrusions or pincushioning effect, improvements in the design are outlined herein to achieve the best results for defects of the nose.

5.Non-Hodgkin Lymphoma Containing Low Attenuation Area at Enhanced CT: Correlation with Histopathologic Typing.

PURPOSE: To evaluate the frequently of low ahetenuation area in enhanced CT scans of non-Hodgkin lymphoma(NHL) and to find out if there is any pertinent relationship between this and the histopathologic classification. MATERIALS & METHODS: The authors reviewed CT scans in the newly diagnosed 53 patients with NHL. We defined the-low attenuation area as the one with CT attenuation value lower than that of the muscle, surrounding lesion, or other lymph nodes after contrast enhancement. NHL with the low attenuation areas were correlated with the histopathologic findings according to the classification based on the Working Formulation and the frequency of the lesion was evaluated. RESULT: Of the 53 patients, the low attenuation area was found in 13 patients (25%) at CT. The histopathologic classification could be made in 12 patients, among whom one patient was classified as low grade, six as intermediate grade, and five as high-grade. Concerning the specific cell typing, the diffuse large cell type was most common in intermediate-grade NHL seen in five patients and the large cell, immunoblastic type was most common in high-grade NHL seen ih three patients. CONCLUSION: The authors concluded that the low attenution area within lymphoma is not an infrequent finding at CT, and there was no stastistically significant correlation between this finding and the prognostic grading of the Working Formulation.

9.The radiation effect on healing of surgical wound in mouse skin

Remarkable improvement in control of malignant tumor was achieved by combined surgery and postoperative radiation therapy. In Past, radiation therapy had been recommended after 4-6 weeks from operation because of worry about increased complication rate of surgical wound by post-operative irradiation. Nowadays, early surgical extirpation and early post-operative irradiation is recommended for better therapeutic effect. To evaluate the relationship between surgery-radiation interval and healing of surgical wound, an experimental study was undertaken using a total of 132 mice. A single dose of 2000 rads irradiation was delivered immediate after and on 1,3,5,10,14days after incision and suture on the skin of hind limbs of mice. Tensile strengths of the wounds were measured after removal of stitches on 1st, 3rd, 5th, 7th, 10th, 14th and 21st post-operative days. The results are summarized as follows; 1. Wound healing was delayed by irradiation delivered within 3 days from operation. 2. No significant delay of wound healing was observed by irradiation on 5 or more days after operation. 3. Normal wound strength was attained at 21st post-operative day in any surgery-radiation interval. 4. More severe delay of wound healing by irradiation at 24 hrs after operation than by immediate post-operative irradiation although statistical significance is not confirmed. In conclusion, early post-operative irradiation delays healing of the surgical wound though ultimately tensile strength reaches the value of non-irradiated wound.

10.Whitening Effect of Cosmetics Containing Magnesium L-Ascorbyl-2-Phosphate(VC-PMG, Vitamin C Derivatives) Assessed by Colorimeter.

BACKGROUNDS: An inhibitory effect of magnesium L-ascorbyl-2-phosphate (VC-PMG, a stable derivative of ascorbic acid) on melanogenesis has been described. Furthermore, glabridin in licorice is known to have inhibitory effects on melanogenesis and widely used for raw materials for depigmenting agents. OBJECTIVE: The purposes of this study are to provide objective data by measuring the visual clinical effects of VC-PMG with the colorimeter and to promote the development of de-pigmenting agents. METHODS: 20 volunteers joined the study. With an artificial UVB irradiation, eight tanned areas were made on the inner side of the forearm. During two months, each tanned area was treated with five whitening cosmetics with 3% VC-PMG and increasing concentration of licorice from 0% to 3%. Darkness degree of each area was measured weekly by the colorimeter and the visual assessment. RESULTS: For all cosmetics, whitening effect was measured by colorimeter and visual as-sessment. The cosmetic containing VC-PMG 3% and licorice 1% had more whitening effect than any other cosmetics of different concentrations. Moreover, VC-PMG 3% alone also had whitening effect in some volunteers. CONCLUSION: VC-PMG was clinically found to have whitening effect.
