The transarterial embolization has been widely used to control bleeding. It has a variety of clinical utility; to reduce bleeding on the surgical field, to reduce the size of malignant tumor as a preopearative treatment, to treat arteriovenous malformation or arterial aneurysm as a curative method and to promote life qua' ity of patient with diffuse or multiple hepatocellular carcinoma as a palliative treatment, etc. With the advance of modem technology, various embolic materials have been also developed. IIowever, it has not- been -fully investigated of histopathologic changes of the embolized organs according to the embolic materials used. This study was undertaken to investigate the histopathologic changes of embolized renal artery in rabbit by various embolic materials, according to each embolic material and to time passed by after embolization. Of the 5 arteries embolized by ethylene vinyl alcohol copolymer(EVAL), one showed abscess formation in embolized kidney. The other 4 allowed to perform further pathologic study: within a week after embolization there was no any specific change in vessels, however, r,unimal endothelial hypertrophy was observed following 2 weeks of embolization. Of the 8 renal arteries embolized by N-buthyl-2-cyanoacrylate(Histoacryl), 4 showed total occlusion of the main renal arteries as well as renal infarction, which reflects the strong adhesiveness of Histoacryl to vascular wall. We report a case of morphea profunda in a 21-year-old male who had diffuse brown sclerotic plaques on his extremities. Laboratory findings showed peripheral eosinophilia and an increased titers for anti-DNA and anti-nuclear antibodies. Histopathologic findings showed diffuse fibrosis and a thickening of the lower dermis and subcutaneous tissue. He has been treated with hydroxychloroquine 400mg per day and the sclerosis of the skin improved.

A clinical observation of cutaneous complications was made on 200 patients receiving cancer chemotherapy at the Department of Internal Medicine, Seoul National University from January through May, 1986. The results were as follows: 1. Among the 200 patients, 191 case(95. 5%) showed cutaneous complications 2. The cutaneous complications included the following; hyperpigmentation(14l cases, 70. 5%), alopecia(138 cases, 69.4%), nail change(118 cases, 59.0%), mucositis(47 cases, 23.5%), dryness of the skin(40 cases, 20.0%), seborrheic dermatitis(24 cases, 12. 2%), increase of seborrheic keratosis(11 cases, 5.6%), folliculitis or acneiform eruptions(9 cases, 4,5%), melasma(6 cases, 3.0%), gynecomastia(3 cases, 1.5%), vessel hardening or dimpling(3 cases, 1.5%), radiation recall(2 cases, 1.0%), hyperhydrosis(2 cases), photosensitivity(1 case, 0.5%), tissue necrosis(1 case), facial flushing(1 case), purpura(1 case) and obesity(1 case), 3 Steps were taken to determine the chemotherapeutic agents causing these cutaneous complications, though in some cases it was difficult in determining exaetlr which chemotherapeutic agent was the cause of the observed cutaneous complication.

BACKGROUND: The high concentration of plasma total homocysteine is recently considered an independent risk factor for atherosclerosis. The purpose of this study was to provide reference ranges for plasma homocysteine levels and to investigate the relationship between plasma homocysteine and cardiovascular risk factors in healthy Korean men. METHODS: Anthropometric parameters, alcohol intake, cigarette use and nutrient intake were determined in 166 healthy men within a wide age range(30-69 yr). Serum levels of lipids, glucose and insulin levels during oral glucose tolerance test(OGTT), plasma amino acid concentrations and levels of antioxidant nutrients and enzymes were also measured. Hyperhomocysteinemia was defined as plasma homocysteine levels above the 90th percentile(> or =15micromol/L) of respective plasma homocysteine distribution in study subjects. Characteristics of hyperhomocysteinemic men(n=16) were compared to normohomocys- teinemic men(n=16) matched for age and body mass index. RESULTS: Plasma total homocysteine values ranged from 2.4 to 38.1micromol/L, a skewed, right-tailed distribution. The homocysteine levels of 25th, 50th and 75th percentile were 7.02, 9.61 and 12.4micromol/L, respectively. The mean concentration of plasma total homocysteine was 10.7micromol/L. Plasma total homocysteine level was positively correlated to body mass index, serum cholesterol and triglyceride levels and alcohol intake, but negatively correlated to serum bata-carotene concentration. In multivariate analysis, serum triglyceride level was the strongest determinant of plasma total homocysteine concentration. There were no significant differences between two groups in waist to hip ratio, alcohol intake, cigarette use, blood pressure and serum levels of glucose and insulin during OGTT. Hyperhomocysteinemic men had significantly higher mean values of serum triglyceride(258mg/dl), total cholesterol(226mg/dl), and LDL-cholesterol(140mg/dl) than normohomocysteinemic men. Hyperhomocysteinemic men showed a decrease in lipid corrected values of serum bata-carotene and alpha-tocopherol and plasma concentrations of serine and taurine, when compared to normohomocysteinemic men. The mean intakes of vitamin B6, folate, vitamin B12 and bata-carotene tended to decline by 25-30% in hyper- homocysteinemic group, when compared to normohomocysteinemic group. CONCLUSION: Our results indicate that healthy Korean men with hyperhomocys- teinemia show signs of hyperlipidemia and decreased antioxidants nutrients and these factors increase risk for coronary artery disease. In addition, hyperhomocysteinemia may affect amino acid metabolism related to homocysteine.

The retinoic acid and retinoid(Ro 11-1430) were alternately applied topically on forty patients with acne vulgaris. Therapeutic effects and side reactions of this regimen were compared with our previous studies of the treatment with retinoic acid alone and with retinoid alone. Compared with the treatment with retinoic acid alone, the therapsutic effect of the alternating treatment was relatively superior as indicated by the markedly reduced severity of local side reactions.

A clinical observation of the nail pigmentation change was made on 200 patients receiving cancer chemotheray who were seen at; the Department of Internal Medicine, Seoul National University Hospital, from January through May, 1986. The results were as follows: l. Among the 200 patients, 118 cases(59%,) showed nail pigmentation changes. 2. The patterns of nail pigmentation change were as follows: Parallel transverse band(70 cases 59.3%), longitudinal pigmentated band(32 cases, 27. 1%), brown arc(25 cases, 21. 2%), Proximal black pigmentation(10 cases, 8. 5%), diffuse pigmentation(5 cases, 4.2%), parallel transverse white line(5 cases, 4.2%), half and half nail(3 cases, 2. 5%), pigmentations with transverse white band(1 case, 0. 8%). 3. Various cambinations of nail pigmentation pattern were found in 33 patients (28. 0%). 4. 11 cases of the nail dystrophy with nail pigmentation change were observed: Longitudinal ridge(7 cases), transverse groove(3 cases), wavy transverse fissure (1 case).
