1.Direct Immunofluorescent Studies in the Scabies.

There are many reports about the deposition of immunoglobulins and complement by direct immunofluorescence(DIF) in the lesion of scabies. The authors investigated the immunoglobulins and C, for the purpose of studying the immunologic reactivity of the host to scabies mites by DIF. The results are summerized as follows: Total number of the cases investigated were 20 (18 males, 2 females), of whom 11 were in their third decade of age. The clinical forms of the tested lesions were papular type (7 cases), nodular type (12 cases), and Norwegian type (1 case). The durations of the patients were mostly within 2 months, but was 9 months in 1 case of nodular type, Deposition of immunoglobulin and C, was demonstrated by DIF in 10 of 12 cases (83. 3%) of nodular type, and 1 of 7 cases (14. 3%) of papular type. The Norwegian type showed negative results. There seemed to be no relation between the duration of scabies and positive rates. Of 20 cases tested, immunoglobulin and C, were deposited in the vascular wall in 11 lesions (55%), and at the dermoepidermal junction in 3 lesions (15%). While in 1 of 7 cases of papular type both IgM and C, were observed, IgM and C, were noted in 7 and 9, respectively, in 12 cases of nodular type.

3.A Case of Kaposi's Varicelliform Eruption Occuring in Pemphigus Foliaceus.

This report presents a case of a 17-year-old-girl who developed a, Kaposis varicelliforrn eruption(eczema herpeticum) during oral prednisolone therapy in order to control pemphigus foliaceus. Diagnosis was made by the history, clinical feature and histologic examination. Within 24 hours after the initiation of therapy with ribavirin(Viramid), new lesions had ceased to develop.

4.A Case of Sea Urchin Granuloma.

Sea urchin granuloma is a rare skin disorder, considered to be allergic or foreign body reaction caused by stick of spines of sea urchin. However, detection of the spine in the granuloma is quite difficult. A 27 year-old marine was seen with several asymptomatic hard nodules on the dorsum of hands, fingers, forearrns and knees at the injured site of the spines after a latent period of 6 rnonths. Histolagical examination revealed dermal granulomatous inflamation with hyperkeratotic and acanthotic epidermis. On examination with polarized microscopy, sea urchin spines in the granuloma were seen as bright white fragments in the dark field. Those nodules were regressed with intralesional injection of triamcinoIone.

5.Distribution of Causative Fungi in Home Environment of Parients with Tinea Capitis Caused bu Microsporum Canis.

Isolation of Microsporum canis from home environment was carried out for 30 tinea capitis patients infected by M, canis. M. canis was isolated in 84.2-100% of patients caps, mattresses, clothes and pillows and families' clothes. The greatest mean number of M. canis colonies per petri dish was found in the patients pillows and followed by caps, mattresses, outerwear, and underwear. Mean number of M. canis colonies in the clothes of mothers, grandmothers, brothers, sisters and roommates of patients was smaller than that of the M. canis colonies in the clothes of patients but greater than that isolated from the clothes of other family members. Frequency of isolation and mean number of colonies decreased drarnatically after 2 weeks therapy and rarely isolated after 6 weeks therapy. M. canis was isolated in 100% of the house dust of patients. Isolation index was greater in the house dust of patients with concomittant infection of other family members and with lesions of scaly and inflammatory type. M. canis was isolated from cats and goat of patients by the hair brush technique.

6.A Clinical Analysis on 186 Cases of Glaucoma.

The authors analysed statistically 186 cases of the glaucoma patients among 20,785 out-patients who visited to our eye clinic from Jan. 1957 to Dec. 1969. The incidence of the glaucoma was 0.895% which was remarkably lower than the other reports. The following results were obtained. 1) Among the 186 cases, there were 85 male and 101 female patients. 2) There was no significant difference in left and right eye. 3) 51-60 year age group was prevalent (49 cases). 4) The age group over 40 occupied 61.3%. 5) The open angle glaucoma was the highest incidence.

7.A Clinical and Pathologic Study on Becker' s Melanosis.

BACKGROUND: Becker's mealanosis is not an uncommon entity, and many cases are probably not reported because the diagnosis is usually made clinically and the condition is benign with cosmetic significance only. the etiology remains obscure. OBJECTIVE: The purpose of this study is to establish the clinicopathologic characteristics of Becker's melanosis in Korean. METHODS: Thirty-six patients with Becker's melanosis were retrospectively studied. RESULTS: The clinical and histopathologic findings observed in our cases were similar to previous reports except for the following. Nine patients(25%) had abnormal findings in their hair follicles : dilatation of the infundibular portion filled with abnormal keratin, Pityrosporum orbiculare and bacteria(19.4%); formation of intrafollicular cyst(5.6%). Three patients(8.3%) had granulomatous infiltration. Twenty-two patients(61.1%) had increased smooth muscle bundles. CONCLUSION: The findings associated with dermal hair follicles may be related to the pathogenesis of Becker's melanosis.

8.Clinical Studies on Spondylolisthesis

Spondylolisthesis is characterized by forward displacement of the vertebral body with bilateral defects of the partes interarticulares. Because the progression of slipping occurs in young adults, difficult problems are present in the treatment of this condition. Watkins(1953) described the technique of posterolateral fusion between transverse processes, which was accepted in the dynamics of spine motion. In this procedure, grafted bone is placed close to the center of axis of the spine motion and so, bony fusion is not affected by flexion motion but distracted by a Iateral shearing force. The role played by the Norton-Brown back brace is in the restriction of lateral bending. However it pormits somewhat flexion motion of the spine. It is suggested that early ambulation may be possible without the interference of bony union after the posterolateral fusion operation when this brace is applied instead of longterm cast immobilization. In clinical analyses and follow-up studies after operative treatments in 55 patients with spondylolisthesis, the following results were obtained: 1. 30 patients (54.6%) were distributed between 21 and 40 years of age. 2. Males (52.7%) were slightly more than females (47.3%). 3. Among clinical symptoms and signs, low back pain was most common (87.3%). Sciatica was present in 45.5% 4. Duration of symptoms at admission was variable. 5. 15 patients (27.3%) had a previous history of trauma. 6. 5th lumbar vertebra was most commonly involved (74.5%) followed by the 4th lumbar vertebra(23.6%). There was a case(1.9%) in which the 5th and 4th lumbar vertebrae were involved. 7. Degrees of slipping were divided into 4 grades (Grade I–IV) by Meyerdings method. Grade 1 was most common (72.2%), Grade II 22.2% and Grade III 5.6%. 8. In follow-up studies of the operative treatment, laminectomy & H-graft were satisfactory in 84.9% and posterolateral fuion in aII cases (13 patients). 9. Early ambulation was preferred without healng disturbance in a case in which the Norton-Brown back brace was applied after posteroIateraI fusion and also, other advantages were expected as follws: a) Elimination of prolonged cast immobiIization b) Financial aid c) Early return to work d) Mental comfort.

9.Studies for Korean women breast carcinoma using combined imaging diagnosis

Since Oct. 1, 1983 until April 15, 1986, authors analyzed variable combined imaging diagnosis andhistopathologic correlation for confirmed 100 breast carcinoma among 2773 patients whom authors took care of them.1. Incidence of female breast carcinoma was 3.6% among 2773 patients who visited Yong Dong Severance Hospital, andoverall breast carcinoma occupied 29.7% among 337 confirmed breast diseases. 2. Prevalent cancer age for Koreanwomen breast carcinoma was ages between 40-49 years old(42%) however authors experienced 5% breast carcinoma amongtwentieth, and 22% breast carcioma among thirtieth. 3. Most common histopathologic type for breast carcinoma wasinfiltrating ductal carcinoma, scirrhous type(65). And most common breast parenchymal pattern related to breastcarcinoma was DY pattern(42%) followed by N1 pattern(25%). 4. Common film mammographic findings of breastcarcinoma were: ill defined bordered mass(68.4%), spiculation of mass(82.9%), vessel dilatation (59.8%) and thencalcification (44.4%). 5. Frequently visible ultramammographic findings were; irregular mass contour(79.6%),nonuniform internal echoes(87.0%), and posterior wall shadowing (86.0%). 6. Trials of above combined imagingmodalities for breast carcinoma resulted in his sensitivity of diagnosis(93.3%).
