1.Neurofibromatosis: Report of 2 cases.

Bilateral acoustic neurofibrornatosis or neurofibrornatosis-2 is characterized by bilateral acoustic neurornas and it is thought to be genetically distinct from the neurohbrornatosis-1. Also called von Recklinghausen's neurofibrornatosis. We report 2 patients with neurofibrornatosis-2. Who showed progressive bilateral hearing loss, unsteady gait and headache.Neuroirnaging studies revealed bilateral cerebellopontine angle rnasses and biopsies confirmed the diagnosis.

2.The Effect of Alcohol on the Blood Lipid Level of Korean Adult Men.

This study was conducted to evaluate the relations between alcohol intake and blood lipid level in a group of 1,138 Korean adult men, ages 20 to 69 years. Total number of each of a variety of drinks in the previous two weeks was obtained by questionnaire and converted into grams of alcohol consumed in a week. The levels of blood lipid such as Cholesterol, HDL-Cholesterol, Triglyceride and LDL-Cholesterol were examined by enzyme method. We also observed the effects of various variables such as age, body mass index, smoking, exercise and blood pressure on blood lipid level. The results obtained were as follows; 1. Average weekly alcohol intake was 129.0+/-167.4 gm and that of 30-39 age group was the highest as 149.3+/-170.4gm. 2. Levels of Cholesterol, Triglyceride and LDL-Cholesterol tended to increase with increasing age, but level of HDL-Cholesterol showed no significant relationship with age. 3. The positive linear regressions of alcohol intake on HDL-Cholesterol and Triglyceride were noted statistically significant. 4. Multiple regression analysis demonstrated that the effect of alcohol on HDL-Cholesterol and Triglyceride was statistically significant and the contribution rates were 5.0% and 0.8%, respectively. And, in the case of HDL-Cholesterol, the alcohol intake was the most significant independent variable.

3.Congenital coronary arteriovenous fistulae

Six cases of congenital coronary arteriovenous fistulae were diagnosed radiographically and confirmed afteroperation at Seoul National University Hospital in recent three and a quarter years since 1979. The clinical andradiological findings with the emphasis on the cinecardiographic findings were analysed. The summaries of theanalysis are as follows; 1. Among 6 cases, 2 cases were male and 4 cases were female. Age dsitribution was from 3months to 19 years. 2. In chest x-ray findings, 5 cases had cardiomegaly. Among these 5 cases, increase ofpulmonary vascularity was in 3 cases. Chest X-ray finding of the rest 1 case was within normal limit. 3. Thecoronary arteriovenous fistula orginated from the right coronary artery in 5 cases and from the single left coronary artery in 1 case. In 5 cases, a coronary arteriovenous fistula drained into the right ventricle and in 1case, a fistula from a SA node branch of the right coronary artery into the right atrium. 4. In 5 cases, a mainright coronary artery was dilated. Among these 5 cases, aneurysmal dilatation of main right coronary artery was in2 cases, saccular aneurysmal dilatation of distal portion of right coronary artery was in 2 cases and in 1 case,diffuse dilatation without aneurysm formation of a main right coronary artery. In 1 case, SA node branch of theright coronary artery was dilated with aneurysm formation. 5. For accurate diagnosis of congenital coronaryarteriovenous fistula, biplane cinecardiography is an essential procedure.

5.An Echocardiographic Study of Left Ventricular Functional Change in Pure Aortic Regurgitation Patients after Aortic Valve Replacement after Aortic Valve Replacement.

Twenty-one patients undergoing aortic valve replacement for pure aoritic regurgitation were studied retrospectively to evaluate the left ventricular function and internal dimension change before, 1-6 weeks(early postoperative) and 2-36 months after(late postoperative) aortic valve replacement by serial echocardiography. Postoperatively, NYHA function class improved remarkably (from 3.3+/-0.6 to 1.4+/-0.7). Early postoperatively, left ventricular end-diastolic dimension (EDD), left ventricular end-systolic dimension(ESD), left ventricular fractional shortenting(FS) significantly decreased in all patients(7.6+/-1.2cm vs 5.8+/-1.5cm P<0.001, 5.5+/-1.3cm vs 4.7+/-1.3cm P<0.001, 39+/-12% vs20+/-8% P<0.001 respectively). Interventricular septum thickness(IVS) and posterior wall thickness (PW) were slightly thickened before(1.4+/-0.3cm, 1.3+/-0.3cm respectively) and in the early postoperative period (1.3+/-0.4cm, 1.3+/-0.3cm respectively) without significant interval change. Late postoperatively, EDD and ESD decreased significantly (7.8+/-1.2cm vs 5.1+/-0.8cm P<0.01, 5.1+/-1.1cm vs 3.4+/-0.8cm P<0.001. respectively), and FS increased significantly (25+/-9% vs 34+/-9%, P<0.05). Among 3 patients of so called high risk group mentioned by Henry(22,33), ESD and FS improved to normal range in 2 patients, and ESD decreased to 4.4cm and FS increased to 33% in the other one. EDD and ESD decreased significantly in both group I(preoperative ESD<5.5cm) and group II(preoperative ESD<5.5cm), without no decrement difference between two groups, and there was a significant difference of FS decrement between group I and group II at early postoperative period. Preoperative ESD correlated highly with the early postoperative EDD(r=0.89) and ESD(r=0.87) with statistical significance, and moderately high with late postoperative EDD(r=0.45), ESD(r=0.50) and FS(r=0.42) without statistical signiticance. We concluded that there was significant improvement in left ventricular function in pure aortic regurgitation patients postoperatively. Preoperative left ventricular and systolic dimension above 5.5cm and fractional shortenting below 25% are not so reliabel index of poor postoperative prognosis.

BACKGROUND: Trimethyltin (TMT) is a neurotoxicant which produces a distinct pattern of neuronal cell death in the hippocampus following systemic administration of a single dose. However, the mechanism of selective neuronal death remains unclear. We performed this study to elucidate the underlying mechanism of TMT-induced hippocampal neu-ronal death. METHODS: The effects of trimethyltin (8.0mg/kg, I.p., single dose) on the hippocampal neurons were investi-gated in terms of changes in the neurobehavioral status, histologic, and electron microscopc findings. RESULTS: Behaviorally, TMT treatment caused stereotypic limbic system dysfunction, i.e. tremors, spontaneous seizures, vocaliza-tion, hyperactivity, hyperexcitability, intraspecific aggression as described previously. Morphologically, TMT produced prominent neurodegeneration in the dentate gyrus. Widespread, strong glial fibrillary acidic protein (GFAP) immunore-activity, which was suggestive of reactive astrogliosis, was noted throughout the hippocampal subfields. Many degener-ating neurons were TUNEL positive. Electron microscopic findings revealed characteristic features of apoptosis in the dentate granule cells. NADPH-diaphorase positive cells were spared after TMT exposure. CONCLUSIONS: It is suggested that TMT-induced hippocampal degeneration might be a useful in vivo model for the study of learning and memory, neuronal-glial interactions, and selective neuronal apoptosis.
