2.A Study on the Design of High-Frequency Jet Ventilator Using PLL system.

This paper describes to design and to examine the mechanical characteristics of high frequency jet ventilator. The device consists of Phase lock loop (PLL) system, solenoid valve driving control part and Air regulating system. This study is carried out by changing several factors such as endotracheal tube (E.T. tube) diameter, injector cannula diameter, 1%, and frequency (breaths/min.) having direct effects on the gas exchange as well as parameters of the entrained gas by venturi effect, so as to measure the tidal volume and minute volume. This system characteristics were as follows: 1) Frequency: 6-594 bpm 2) Inspiration time: 1-99% 3) Variance of input air pressure: 1-30 PSI

4.Extramammaty Paget's Disease: Electron Microscopic Study for Observation of Bleomycin Effect.

Extramammary Paget's disease is a dermatosis which pathologically and clinically resembles Paget's disease of the breast. The disease occurs in regions with apocrine gIands; most often in the anogenital area, seldom in the axilla. umbilicus, mucous membrane of the mouth, nose, ear and Moll's gland of tne eyelid. In this work the effect of bleomycin for extramammary Paget's disease was studied on ultrastructural aspect. The material was a case of extramammary Paget's disease of 7 years standing occuring in the scrotum and the pubic area by serial sections after IV injection of 60mg, 120mg and 240mg of bleomycin, and examined under the electron microscope. The results obtained are summarized as follows: 1. The size of Paget's cell and its nucleus was gradually decreased. 2. The numbers of lysosome, dense body, autophagic vacuolization and tonofilament was decreased. From these fidings, it is suggested that bleomycin induces retrograde degenerative change of Pagct's cell, and cell necrosis finally.

5.Echocardiographic Studies on Mitral Valvular Heart Disease.

Echocardiography was done with Ekoline 20 model of Smith Kline Instrument in 91 patients of mitral valvular heart disease, 46 with mitral stenosis, 10 with mitral insufficiency, 38 with mitral stenoinsufficinency and 115 normal controls. The results obtained are as follows. 1. Right ventricular dimension was 13.61+/-5.61 mm/sq m. in normal controls, 16.50+/-5.06mm/sq m. in mitral stenosis, 20.17+/-3.28mm/sq m. in mitral insufficiency, 18.61+/-8.48mm/sq m. in mitral stenoinsufficiency revealing significant increment in all mitral valvular disease groups but there was no significant difference btw. each disease groups. 2. Left ventricular end-diastolic dimensions was 29.83+/-7.06mm/sq m. in normal controls, 41.57+/-7.77mm/sq m. in mitral insufficinency, 38.48+/-10.22mm/sq m. in mitral stenoinsufficiency revealing significant increment but there was no significant difference btw. mitral stenosis and normal controls. 3. Left atrial dimension was 17.57+/-4.96mm/sq m. in normal controls, 33.22+/-5.32mm/sq m. in mitral stenosis, 35.96+/-7.35mm/sq m. in mitral insufficiency, 35.00+/-8.22 in mitral stenoinsufficiency revealing significnat increment in whole mitral valvular disease. 4. E-F slope was 90.76+/-30.20mm/sec in normal controls 18.08+/-10.3mm/sec in mitral stenosis, 29.52+/-19.3mm/sec in mitral stenoinsufficiency revealing marked decrement. In mitral insufficiency, it was 65.3+/-45.4mm/sec revealing significant decrement but not so severe as previous. 5. There was no significant difference between normal controls and mitral valvular disease in valve excursion. 6. Left atrial dimension to aortic root size was 1.13+/-0.31 in normal controls, 1.77+/-0.63 in mitral stenosis, 1.56+/-0.60 in mitral regurgitation and 1.82+/-0.56 in mitral stenoinsufficinecy revealing significant increment in whole mitral valvular heart disease.

6.Dermatitis Herpetiformis Resembing to Atopic Dermatitis: Report of a Case.

A case of dermatitis herpetiformis resembling atopic dermatitis is reported. The patient is 45 year-o1d male, who 6 months ago developed the pruritic erythematous vesicular eruptions on the left side of back and it gradually spread to the margin. He was treated with steroid but the lession had undergone exacerbation and remissions. Biopsy was performed on the vesicular lesion and revealed non-acantholytic subepidermal bulla with mild inflammatory cells infiltration and then he was treated with DDS for 2 months with good response.

7.Tuberous Sclerosis Occuring through 3 Generations.

Tuberous sclerosis is a complex neurocutaneous disease inherited as an autosomal dominant pattern, which is characterized by facial angio-fibroma, mental retardation and seizures. Many cases have been reported in the literatures, but familial cases are relatively rare. We present 9 cases of tuberous sclerois occuring througb 3 generations, which are 3 cases of angiofibroma, mental retardation and seizures, 2 cases of facial angiofibroma and seizures, 1 case of mental retardation and seizures and 3 cases who showed facial angiofibroma only. Two brothers in this family reveald icthyosis vulgaris without tuberous sclerosis.

8.A Case of Isolated Congenital Double-Orifice Mitral Valve.

Double-orifice mitral valve is a relatively rare congenital abnormality, usually discovered at autopsy or surgery. In most cases, the double-orifice mitral valve causes no hemodynamic effects, sometimes it is regurgitant, and rarely is stenotic. Appreciation of this echocardiographic abnormality is important because double orifice mitral valve is often associated with other congenital anomalies and this echocardiographic findings may be confused with other cardiac abnormalities. The authors report a case of isolated congenital double-orifice mitral valve in a 42-year-old woman. Data from the literature are reviewed and the echocardiographic images of the malformation are described.

A xanthoma is a localized collection of tissue histiocytes containing lipid and is usually associated with hyperlipidemia. Tendinous and tuberous xanthomatosis have been found in association with familial hypercholesterolemia, Type III hyperlipidemia, beta-sitosterolemia and cerebrotendinous xanthomatosis. Tendinous and tuberous xanthomatosis without hyperlipidemia have been reported very rarely. Especially tendinous xanthomatosis without hyperlipidemia have not been reported at all in korea. We are reporting a patient with tendinous and tuberous xanthomatosis accompanied by normal plasma lipids

10.Abnormal Imposition of hands as a Possible Cause of Syphilis.

Syphilis is a communicahble disease caused by the motile microaerophilic spirochete Treponema pallidum, which is only a netural pathogen for human. Prevalence of syphilis and other sexually transmitted diseases has traditioially fluctuated with changes in sccia, conditions and sexual behavior. Although sexual contact is the main route of transmission, T. pallidum may also be infected through direct contact with syphilitic lesions, blood transfusion, ingestion of menstrual blood or vaginal secretions, or trsnsplacental transemission. Fomites as the means of transfer is only hypothetical and account for very few, if any, infection. Prosectors, blood handlers and laboratory technicians are at risk for accidental inoculation with infected materials. In extremely unusual circumstances, infection by means of contact with a skin lesion and human bite have been reported. We report two cases of eyphilis following the abnormal imposition of hands, which caused unnecessary erosive trauma with fingernails,
