Chinese Journal of Postgraduates of Medicine 2013;(18):21-23
Objective To evaluate the increased carotid artery pulsatility index(PI) and resistance index (RI) in young obese males by Doppler ultrasound. Methods All of the volunteers without local and systemic disease included 48 young obese males (observation group) and 100 healthy males with the same age(control group).Common carotid artery had to come into practice in all of the volunteer by pulsed Doppler ultrasound to calculate the PI and R[.Results The average of RI,PI,body mass index (BMI),systolic pressure, diastolic pressure, fasting plasma insulin, fasting C-peptide,triglyceride,low density lipoprotein cholesterol and high sensitivity C reactive protein in observation group were significantly higher than those in control group [0.70 ± 0.10 vs.0.60 + 0.10,1.48±0.30 vs.1.00 ± 0.10,(36.6 ± 3.4) kg/m2 vs.(23.6 ± 1.6) kg/m2,(127.5 ± 14.3) mm Hg(1 mm Hg =0.133 kPa) vs.(110.5 ± 11.0) mm Hg,(78.8 ±9.8) mm Hgvs.(69.9 ±7.6) mm Hg,(27.5 ± 19.6) mU/L vs.(8.8 +8.8) mU/L,(3.7 ± 1.6) pmol/L vs.(1.8 ± 1.2)pmol/L,(3.9 ± 2.0) mmol/L vs.(2.1 ± 1.3) mmol/L,(3.1 ± 0.7) mmol/L vs.(2.3 ± 0.7) mmol/L,(4.0 ± 4.4)mg/L vs.(1.3 ± 1.2) mg/L,P < 0.05].Conclusions The characteristic of vascular properties in young obese males with normal blood pressure and without other cardiac risk factor is the decrease of blood wall compliance and the increase of resistance. The change of artery in the incipient properties in young obese males is the vascular function.Further studies will be required to prove thai whether early intervention will improve vascular function such as diet and practice.
2.The prediction of SIRS criteria and C-reactive protein to severe infection in children with fever in pediatric emergency room
Chinese Pediatric Emergency Medicine 2017;24(7):517-522
Objective As the diagnosis criteria of sepsis, the systemic inflammatory response syndrome(SIRS) has the predictive effect on severe infection.Groups abroad have reported that SIRS criteria is not suitable for the severe infection in emergency room. This article aimed at discussing whether the SIRS criteria and C-reactive protein(CRP) are suitable for the prediction of severe infection in our country. Methods We collected the patients in fever(T>37.5℃)arriving at Shengjing Hospital of China Medical University from October to December in 2015 and recorded the symptoms, signs and treatments, then we discussed the sensitivity and specificity of SIRS criteria in evaluating the severe inflammation. Results Three hundred and ninty-two patients met the SIRS criteria out of 784 patients involved, and 9 of them were in hospital,SIRS criteria had no relevance with the treatments(χ2=4.714,P=0.194),which showed that the SIRS criteria had low sensitivity and specificity in evaluating the severe infection.Taking CRP>8mg/L as unusual,363 patients had abnormal CRP,216 of them met the SIRS and 9 of them were in hospstal.CRP had relevance with the treatment of patients in fever(χ2=46.750,P<0.01),and SIRS combined with CRP had statistics meaning in choosing treatment of infectious patients(χ2=41.783,P<0.01).Conclusion SIRS may fail to diagnose the severe infection. But SIRS criteria combined with CRP has the predictitive effect on infection in pediatric emergency room to a certain extent.
3.Clinical manifestations and pathogenesis of mycoplasma pneumoniae-associated extrapulmonary complications in children
Chinese Pediatric Emergency Medicine 2021;28(1):7-11
Mycoplasma pneumonia is the smallest free-living and self-replicating prokaryotic microbe being devoid of cell wall, which can not only lead to respiratory system diseases in human, but also play an important role in a wide variety of extrapulmonary diseases, involving cardiovascular, neurological, digestive, skin, hematological systems, and so on.However, the pathomechanisms of mycoplasma pneumonia-related extra-pulmonary diseases(MpEPDs) remain largely unknown.Yet there are three main possible pathomechanisms of mycoplasma pneumonia causing MpEPDs: direct type, immune type and vascular occlusion. They are connected with each other and cooperate on promoting MpEPDs, especially in children and the young.Comprehensive therapeutic strategies cover microbiology, immunology and anticoagulants, while supportive treatments are necessary. Most patients recover from MpEPDs have a favorable prognosis.

Objective To discuss the application of cephalocervical CT angiography (CTA) and ABCD2 score in evaluating arterial characteristics and interventional curative effect for patients with transient ischemic attack (TIA). Methods During the period from Jan. 2010 to Jan. 2012, a total of 64 patients with clinically-diagnosed TIA were admitted to authors’ hospital. ABCD2 score evaluation and cephalocervical CTA examination were performed in all patients. The correlation of stenotic degree and distribution of the cephalocervical arteries with the ABCD2 score was analyzed. According to the stenotic degree of the cephalocervical arteries, the patients were divided into low-risk group and mid-to-high-risk group. Twenty-two patients in mid-to-high-risk group received stenting angioplasty treatment. Follow-up was made at 3, 6 and 12 months after the treatment. Results Of 64 TIA patients, cephalocervical artery stenosis was found in 52(81.25%) A total of 149 arterial segments were involved, including intracranial segment (n=81, 54.36%) and extracranial segment (n = 68, 45.64%). Patients in low-risk group mainly suffered slight or moderate arterial stenosis, while patients in mid-to-high-risk group had moderate or serious arterial stenosis (χ2 =10.126, P=0.018). The preoperative ABCD2 score was (6.41 ± 0.50). The ABCD2 scores determined at 3, 6 and 12 months after the treatment decreased to(2.88 ± 0.69), (2.82 ± 0.63) and(3.00 ± 0.71), respectively. The differences were statistically significant (F = 86.657, P < 0.05). Conclusion Cephalocervical CT angiography and ABCD2 score can reliably evaluate the arterial stenosis and interventional curative results in TIA patients.

Objective To investigate the influence of aging on motor function, binding activity and protein expression level of striatum dopamine transporter(DAT)of rats. Methods Rolling-bar test was performed to assess motor function.Western blot and 131I-FP-β-CIT up-take ratio were used to evaluate DAT protein 1evel and striatum DAT binding activity respectively. Results There were an age-related decline of rolling-bar latency and striatum 131I-FP-β-CIT up-take ratio in rats older than6 months. There was a significant difference between 6-month-old rats and older rats(12-month-old rats,16-month-old rats,20-month-old rats),and rolling-bar latency was correlated with striatum 131I-FP-β-CIT up-take ratio(r=0.656,P<0.01).There was no change in DAT protein 1evel during aging process. Conclusions The age-related decline of motor function of rats was correlated with the decreasing of striatum DAT binding activity. The synaptic membrane expression of striatum DAT may decrease in aged rats.

A thermal desorption ( TD) device was developed and coupled to gas chromatography ( GC) or gas chromatography-mass spectrometry ( GC-MS ) for the qualitative and quantitative analysis of semi-volatile organic compounds on atmospheric particulate matters ( PM ) . The TD was operated by direct heating and placed on the GC injector, leading to high heating rate and easy transfer of analytes to GC without focusing of analytes by cold trap. For establishing the TD-GC method, the materials used for supporting PM samples, temperature and time of thermal desorption, and types of sample injection were investigated for detection of sixteen polycyclic aromatic hydrocarbons ( PAHs) and nine n-alkanes. The limits of detection of the proposed TD-GC method were in the range of 0. 014-0. 093 ng for PAHs, and 0. 016-0. 026 ng for n-alkanes, respectively, with the correlation coefficients of correlation above 0. 9975. The TD-GC method was applied to the determination of trace PAHs and n-alkanes on PM10 samples from three cities. The recoveries were in the range of 95%-135% ( PAHs) and 95%-115% ( n-alkanes) , respectively. Finally, the TD was coupled to GC-MS for comparison of the contents of PAHs and n-alkanes on PMx with different particulate size ( x=10 , 5, 2, 1, 0. 5, 0. 25, 0. 1).
