Objective To study the mRNA expression and function of TGF-?1, TGF-?RⅡ and CD105 in the lesional, non-lesional skin of psoriasis and of normal human skin. Methods The RNA of skin tissue was extracted using single-step method of RNA isolation by acid guanidinium thiocyanate. Reverse transcription polymerase chain reaction (RT-PCR) was used to measure the expression of mRNA. 2.Study on the differentially expressed genes in vascular tissue of diabetic gangrene

Objective To study the differentially expressed genes in the vascular tissue of diabetic gangrene toot and the possible mechanism of diabetic gangrene.Methods The differentially expressed genes in the vascular tissue of diabetic gangrene foots were screened by the functional classific gene chip.Reverse transcription polymerase chain reaction(RT-PCR) detection was used to verify the differentially expressed 3 genes.Results In the detected 113 genes,13 of the gene expression level increased more than 2 times,3 of the gene expression level decreased more than 2 times.Three genes were verified with RT-PCR detection,which were results similar to those with chip testing.Conclusions The abnormal expression in a variety of genes in tissues of vascular lesions plays an important role in the course of development and progression of diabetic gangrene.Analysis of differential gene expression can contribute to understanding of the possible mechanisms of diabetic gangrene.
3.Effect of CGRP on Expression of Neuronal Nitric Oxide Synthase in HaCaT Cells

Objective To investigate the regulatory effect of calcitonin gene-related peptide (CGRP) on the mRNA and protein expression of neuronal nitric oxide synthase (NOS), as well as the release of nitric oxide (NO) in cultured human keratinocyte line HaCaT. Methods NO level in the supernatant of cell culture medium was detected with an enzymatic NO detecting kit, the mRNA expression of neuronal NOS was studied with reverse transcription polymerase chain reaction(RT-PCR), and the protein expression of neuronal NOS was studied with immunochemical technique(SP). Results Compared with that in normal culture condition, the mRNA and protein expression of neuronal NOS and the release of NO was significantly upregulated by CGRP in HaCaT cells. Whereas, the expression of neuronal NOS and the release of NO in HaCaT cells induced by CGRP was inhibited by CGRP-8-37, an inhibitor of CGRP receptor. Conclusion The expression of neuronal NOS in keratinocytes and the release of NO from keratinocytes could be upregulated by CGRP.
4.Effect of metabolic syndrome in the bone mineral density in elderly male type 2 diabetics

Objective To explore the impact of metabolic syndrome (MS)in bone mineral density (BMD) in elderly male type 2 diabetics (T2DM). Methods One hundred and fourty cases were divided into MS group and non-MS(NMS)group according to with or without MS. Height,weight were measured, body mass index(BMI) were calculated and the total cholesterol (TC), triglyceride (TG), high-density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), glycated hemoglobin (HbA1c)were tested. The bone mineral density (BMD) was measured by dual-energy X-ray absorptiometry (DEXA). The differences among these groups were compared and the risk factors for MS were analyzed. Results The MD, BMI, TC,TG in MS group was higher than that in NMS group (P < 0.05 vs. P < 0.01),and HDL-C was lower than NMS group (P < 0.05). Correlation analysis showed that BMD was negatively correlated with age , TG , HDL-C , and was positively correlated with BMI.The results of multiple stepwise regression analysis showed that age ,HDL-C,BMI were independent risk factors for BMD. Conclusion The BMD significantly increases in T2DM with MS.Obesity and low HDL-C have positive effects in bone mass.
5.Association between serum level of interleukin-18 and insulin resistance in women with polycystic ovary syndrome

Objective To evaluate the relationship between interleukin-18(IL-18) and insulin resistance, measured by euglycemic hyperinsulinemic clamp, in women with polycystic ovary syndrome (PCOS). Methods Forty-two young women with PCOS and 38 age-and body mass index (BMI)- matched control women were recruited in this study. A complete hormonal assay was performed and serum IL-18 level was determined in each subject. And euglycemic hyperinsulinemic clamp test was completed in 41 of the above subjects. Results Serum IL-18 levels were increased in the PCOS women, as compared with the control (P=0.033). When all subjects were divided into lean and obese groups, the IL-18 levels were slightly increased in the obese subjects (P=0.902). IL-18 levels were negatively correlated with all the clamp parameters [mean glucose infusion rate (M), M-to-insulin ratio (M/I) and glucose metabolic clearance rate (MCRg), r =-0.419,-0.396,and-0.405,P=0.006,0.010 and 0.009 respectively], but were positively associated with HOMA-β index(r=0.334, P=O.035). Conclusion Serum IL-18 level was significantly increased in PCOS women and was strongly associated with the parameters obtained from the euglycemic hyperinsulinemic clamp, indicating that IL-18 may be an important mediator between inflammation and insulin resistance.

Objective To study insulin sensitivity and first-phase insulin secretion in obese subjects with impaired glucose regulation (ICR) in Shanghai. Methods A total of 129 subjects [38 lean controls and 91 obese subjects with ICR including 64 isolated impaired glucose tolerance (IGT) ,8 isolated impaired fasting glucose (IFC) and 19 IFC + ICT] underwent 75 g oral glucose tolerance test and insulin-modified reduced sample number (n = 12) of Bergman's minimal model method with frequently sampled intravenous glucose tolerance test (FSIGTT). Insulin resistance was determined from the insulin sensitivity index (S1) of the FSIGTT. Insulin secretion was determined during the FSIGTT by the acute insulin response to glucose (AIRg). Objective:To investigate the understanding of cancer pain by medical doctors and its therapeutic status in Shanghai. Methods:We recruited 1 982 medical physicians from 106 secondary and tertiary hospitals in Shanghai city from January to March in 2007. The understanding and the therapeutic status of cancer pain were investigated by conducting a questionnaire survey and the results were statistically analyzed.Rusults:There were 52.3% physicians who believed that they understand the treatment protocol of the Three-step Analgesic Ladder regimen. The rate increased by 21.6 percentage point compared with the results in 1999. The understanding degree of oncologists was significantly better than non-oncologists. There were 16.8% physicians giving the correct answers on the three main principles of the dosage titration in analgesic management. The correct rate of oncologists was significantly higher than that of non-oncologists. Opium with strong analgesic effect had become the first choice for severe cancer pain, and the use of pethidine (Dolantin) for severe cancer pain decreased significantly.The pain was significantly relieved in 74.8% patients. The pain-relieving rate increased by 25.4 percentage point compared with the results in 1999. There were 81.1% medical doctors confirming the effect of pain treatment and among them oncologists were more optimistic to therapeutic outcome than others. There were 93.3% physicians who agreed that it was necessary to carry out the standardized training of cancer pain treatment and the requirement of non-oncologists were much emergent. The over-strict controls by the hospitals and the regulations were the main obstacles for using analgesic drugs.Conclusion:The understanding of medical doctors and the diagnostic and therapeutic levels of cancer pain were improved significantly compared with those in 1999. But more training of cancer pain treatment are still required to publicize the standardized therapy and overcome the obstacles of using analgesic drug.
8.Retinol-binding protein 4 rises significantly in women with gestational diabetes mellitus

To investigate the relationship between serum retinol-binding protein 4(RBP4) and gestational diabetes mellitus (GDM) in Chinese Han pregnant women.195 (23-42 years) pregnant women were recruited (July 2005 to December 2007) from the Department of Gynecology and Obstetric in Ruijin Hospital during their visiting for routine prenatal examination.99 subjects belonged to GDM group,and 96 belonged to the group with normal glucose tolerance (NGT).65 non-pregnant healthy women served as control.Serum RBP4 was measured using sandwich enzyme linked immunosorbent assay (ELISA).Pregnant women had higher level of serum RBP4 than that of non-pregnant control.The concentration of serum RBP4 was significantly increased in GDM group as compared with NGT group[(43.04±1.85 vs 33.84±2.17) rag/L,P<0.01].Multiple stepwise regression analysis showed that triglycerides and homeostasis assessment for insulin resistance (HOMA-IR) were independent variables of RBP4 (r2 =0.165) in pregnant women.The results suggest that serum RBP4 level is significantly increased in pregnant women.Women with GDM had even higher RBP4 level than that of normal pregnant women,and RBP4 levele was positively correlated with triglycerides and HOMA-IR.
9.Survival analysis of liver cancer between 2002 and 2006 in Shanghai

Background and purpose:China is a high incidence area of liver cancer. The latest monitoring data in Shanghai show that liver cancer is one of the most common cancers with very high disease burden. This study aimed to describe and analyze the population-based survival rates of patients with liver cancer in Shanghai.Methods:Data of liver cancer cases diagnosed between 2002 and 2006, including follow-up information and death report, were collected from Shanghai Cancer Registry. Life table and Ederer Ⅱ were used to calculate observed survival (OS) and relative survival (RS). The related demographic characteristics and status were also analyzed to relfect the survival situations of the liver cancer survivors in major areas of Shanghai.Results:In this study, 20 702 liver cancer cases were included in analysis. Five-year OS rate for liver cancer was 11.72%, whereas RS rate was 15.45%. The OS of male liver cancer patients was higher than that of female patients. Patients whose age ranged from 0-34 years had the highest survival rates than patients from other age groups. The survival of patients with hepatocellular carcinoma was higher than that of patients with other histologic types of liver cancer. Signiifcant difference in survival had also been found among patients with various stages of liver cancer. The survival rate of patients with stageⅠliver cancer was much higher than that of patients with stageⅢ andⅣ liver cancers. There was no signiifcant difference in the survival of liver cancer patients between urban and rural residents. Over the past 3 decades, the 5-year OS increased dramatically in Shanghai. Conclusion:The survival of patients with liver cancer in Shanghai is improved signiifcantly. The prognosis is poor compared with other common malignant tumors. It is necessary to strengthen the risk factors and high-risk population control and intervention in the future.
10.The status and related factors of health promotion behavior of cancer patients with PICC during tube indwelling period

Objective To investigate the status of health promotion,influencing factors,and health promotion level of cancer patients with PICC during tube indwelling period.Methods A total of 125 PICC cancer patients were investigated by using general information questionnaire and Health Promoting Lifestyle Scale (HPLP-Ⅱ R) according to convenience sampling.Results The average score of health promotion lifestyle in cancer patients was(100.70 ± 18.42)scores during PICC,and the overall level was at the middle level.There was a negative correlation between health promotion behavior and age in cancer patients during PICC detention (P < 0.01),and positively correlated with education,family income and catheterization days (P < 0.05).Multivariate analysis showed that age,education level,diagnosis and catheterization days were influencing factor of health promotion behavior level of cancer patients during PICC indwelling.Conclusion The health promotion behavior of cancer patients during PICC stay is in general level.In clinical nursing work,these patients with elder age,low education level and short timne of catheterization should be paid much attention,so as to improve their health promoting behavior.
