Objective:To investigate the expression of wild type estrogen receptor(wER)and the ex-on-5 deleted ER(variant ER,vER)in human hepatocellular carcinoma(HCC)samples,and thereafteranalyze the possibility of HCC treatment by endocrine therapy.Methods:The mRNA expressions of wERand vER were analysed from 28 cases of HCC by RT-PCR.The expression of ER at the protein level wasdetected by immunohistochemistry(IHC).Results:IHC results showed that 39.3% of the HCC speci-mens expressed ER.The mRNA of wER was detected in 89.3%(25/28)of the HCC specimens whilethat of vER was detected in 96.4%(27/28).Twenty four out of 28 HCC cases(85.7%)expressedboth wER and vER.One out of 28 patients(3.5%)expressed only wER whereas 3 patients out of 28(10.7%)expressed vER only.Conclusion:Ninety six percent(27/28)of the HCC patients expressedvER,which suggests that the expression of vER is an important event in the development of HCC.

ObjectiveTo examined how autoregulation is affected by vasospasm after subarachnoid hemorrhage (SAH) by using transcranial Doppler. MethodsThe moving correlation coefficient between slow changes of arterial blood pressure and mean or systolic flow velocity (FV), termed Mx and Sx, respectively, was used to characterize cerebral autoregulation. Vasospasm was declared when the mean FV increased to more than 120 cm/s and the Lindegaard ratio was more than 3. This occurred in 15 of 32 SAH patients.On the basis of the bilateral transcranial Doppler recordings of the middle cerebral artery in vasospastic patients, Mx and Sx were calculated for baseline and vasospasm. ResultsMx increased during vasospasm (0.46±0.32) and was significantly higher (P=0.03) than at baseline (0.21±0.24) Sx was also increased (0.22±0.26 vs 0.05±0.21,P=0.03). Mx correlated with mean FV (P=0.577, P=0.006) and the Lindegaard ratio (r=0.672,P<0.01).Mx(P=0.006) and Sx (P=0.044) were higher on the vasospastic side compared with the contralateral side.ConclusionThe increased Mx and Sx during cerebral vasospasm demonstrate impaired cerebral autoregulation. Mx and Sx provide additional information on changes in autoregulation in SAH patients.

Objective To explore the effect of Melatonin(MT) on CD4+CD25+ regulatory T cell (CD4+CD25+Tr)and airway inflammation in asthmatic rat.Methods Thirty-two SD rats were randomly divided into 4 groups,8 rats in each group.Asthmatic group:rats were immunized on day 1 and 7 by intraperitoneal inject of mixture of ovalbumin(OVA) and aluminumhydroxide.From day 14,the animals were allenged with aerosolized OVA for 20 min per day for 7 consecutive days.MT group:OVA-sensitized rats were injected intraperitoneally with 0.1 mg/kg MT 30 min before each OVA challenge.Dexamethasone group:OVA-sensitized rats were injected intraperitoneally with 0.5 mg/kg Dexamethasone 30 min before each OVA challenge.Control group:OVA for inhalation and MT for intraperitoneal injection was replaced with saline.After the last challenge,peripheral blood was stained to count the percentage of eosinophil(EOS).Then the rats were lavaged and total leukocytes counts in bronchoalveolar lavage fluid(BALF) were performed after staining with Wright-Giemsa staining.The EOS counts around the airway was counted after the histological section of lung staining with hematoxylin and eosin staining.The serum level of immunoglobulin E(IgE) was detected by immunoenhancement.The change of CD4+CD25+Tr was assessed with flow cytometry.SPSS 10.0 software was applied to analyze data. To systematically review the incidence rate of adverse drug reaction/event by Qingkailing injection. Such databases as the PubMed, EMbase, the Cochrane library, CNKI, VIP WanFang data and CBM were searched by computer from foundation to July 30, 2015. Two reviewers independently screened literature according to the inclusion and exclusion criteria, extracted data and cross check data. Then, Meta-analysis was performed by using the R 3.2.0 software, subgroup sensitivity analysis was performed based on age, mode of medicine, observation time and research quality. Sixty-three studies involving 9,793 patients with Qingkailing injection were included, 367 cases of adverse reactions/events were reported in total. The incidence rate of adverse reaction in skin and mucosa group was 2% [95% CI (0.02; 0.03)]; the digestive system adverse reaction was 6% [95% CI(0.05; 0.07); the injection site adverse reaction was 4% [95% CI (0.02; 0.07)]. In the digestive system as the main types of adverse reactions/events, incidence of children and adults were 4.6% [0.021 1; 0.097 7] and 6.9% [0.053 5; 0.089 8], respectively. Adverse reactions to skin and mucous membrane damage as the main performance/event type, the observation time > 7 days and ≤ 7 days incidence of 3% [0.012 9; 0.068 3] and 1.9% [0.007 8; 0.046 1], respectively. Subgroup analysis showed that different types of adverse reactions, combination in the incidence of adverse reactions/events were higher than that of single drug, the difference was statistically significant (P < 0.05). This study suggested the influence factors of adverse reactions occur, and clinical rational drug use, such as combination, age and other fators, and the influence factors vary in different populations. Therefore, clinical doctors for children and the elderly use special care was required for a clear and open spirit injection, the implementation of individualized medication.

Objective To investigate the association between head fat and metabolic parameters in Chinese obese patients. Methods The data of 99 obese patients were collected from the outpatient department, the Tenth People's Hospital of Shanghai from September, 2011 to December, 2014. The accumulated regional fat was measured by dual-energy X-ray absorptiometry (DEXA). The body weight, body mass index (BMI), neck circumference (NC), waist circumference (WC), hip circumference (HC), visceral index, basal metabolism (BM), liver function, fasting plasma glucose (FPG), fasting insulin, fasting C peptide, blood lipid, uric acid (UA) and C-reactive protein were also obtained for analysis. Results (1) The participants consisted of 35 men and 64 women, with a mean head fat of (1.67±0.34) kg. (2) The head fat was positively correlated with body weight, BMI, NC, WC, HC, visceral index, BM, head fat percentage, arm fat, arms and legs fat, trunk fat, total fat and total muscle in both male and female participants (male: r=0.620, 0.616, 0.670, 0.678, 0.663, 0.635, 0.518, 0.635, 0.488, 0.458, 0.588, 0.618, and 0.579; female: r=0.624, 0.593, 0.610, 0.672, 0.482, 0.657, 0.647, 0.616, 0.639, 0.336, 0.467, 0.544, and 0.762, repectively; all P<0.05). In the female participants the head fat was positively correlated with fasting insulin and UA (r=0.348, 0.349,all P<0.05). For all the participants, the head fat was positively correlated with body weight, BMI, NC, WC, HC, visceral index, BM, UA, head fat percentage, upper arm fat, arms and legs fat, trunk fat, total fat, total muscle, and trunk/peripheral fat (r=0.664, 0.598, 0.685, 0.716, 0.575, 0.703, 0.610, 0.288, 0.526, 0.542, 0.346, 0.527, 0.564, 0.688, and 0.207,respectively; all P<0.05). (3) NC and WC were predictors of head fat for all subjects (β=12.802 and 29.138, both P<0.05); WC was a predictor of head fat for males (β=21.108, P<0.001); and total muscle, upper arm fat, and BM were predictors of head fat for females (β=0.046, 0.116, and-0.914, all P<0.05). Conclusion Head fat accumulation is closely related to the content and distribution of total fat and UA. Head fat may be used as an indicator for metabolic disorder risk for Chinese obese patients.

Objective To evaluate the effects of meal replacement (Glucerna SR, Abbort) on weight loss and fat redistribution in patients with simple obesity. Methods Twenty-two simple obesity patients received dinner replacement with Glucerna for five weeks (6 spoons, about 52 g per time, 220.48 kcal [1 772 kJ]), once a day at night replacing dinner. The weight, blood pressure, BMI, lipid, total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), fasting plasma glucose, and insulin were all observed before and after dinner replacement. The fat distribution was detected by DEXA (dual energy X-ray absorptiometry). The study was done by before-after design in the same patients. Results The body weight (P<0.001) and BMI (P<0.001) were significantly decreased after dinner replacement in our group. Dinner replacement also led to significant decrease of total fat mass (P=0.035), fat mass percentage (P=0.033) and trunk fat mass (P=0.002), while the decreases of peripheral fat mass and arm fat/leg fat mass were not significant. Lipid profiles showed that TG was significantly decreased (P=0.028)and HDL was significantly increased (P<0.001) after dinner replacement. Dinner replacement with Glucerna also resulted in a reduced HOMA-IR, though without statistical significance (P=0.372). Conclusion Dinner replacement can greatly reduce the weight of obese patients, improving fat distribution and regulating blood lipid and glucose. Dinner replacement is tolerable and safe, and it can improve the metabolism of obese patients.
