1.Clinical study of 23G vs 20G vitreous surgery combined phacoemulsification and IOL implantation for macular epiretinal membrane with cataract with

AIM: To evaluate the effects of 23G vs 20G pars plana vitrectomy ( PPV ) combined with internal limiting membrane peeling, phacoemulsification and intraocular lens implantation for macular epiretinal membrane with cataract. ·METHODS: Totally 45 eyes of 45 patients with macular epiretinal membrane and cataract were enrolled in this retrospective non-randomized controlled clinical study. All eyes were treated with PPV combined with internal limiting membrane peeling, phacoemulsification and intraocular lens implantation. There were 20 eyes in 23G PPV group, and 25 eyes in 20G PPV group. The best corrected visual acuity ( BCVA ) , intraocular pressure (IOP), counting of corneal endothelial cells ( CEC) and central retinal thickness ( CRT ) were examined before surgery. BCVA results were converted to the logarithm of the minimum angle of resolution ( LogMAR ) visual acuity. All operations were performed by the same doctor. Operation time for vitrectomy and membrane peeling, average ultrasound energy ( AVE) and effective phacoemulsification time ( EPT ) were recorded. BCVA and CRT were observed postoperatively at 30d and 90d, counting of CEC was observed postoperatively at 90d. IOP was observed postoperatively at 1d and 7d. ·RESULTS:The mean operation time for vitrectomy were 12. 57± 1. 35min in 23G group and 17. 30 ± 1. 19min in 20G group. The difference was statistically significant ( t =-12. 488, P<0. 01). There were no statistical significances in operation time for membrane peeling, AVE and EPT between 23G and 20G groups ( t=-0. 68,-1. 186,-0. 737, P=0. 500, 0. 242,0. 465). On 1d after surgery, IOP in 23G group was lower than that in 20G group, the difference was statistically significant (t= -2. 345, P=0. 024). The BCVA and CRT of the two groups both improved after operations. There were no statistically significant differences between two groups in terms of IOP, BCVA, and CRT ( F = 0. 465, 1. 895, 0. 689; P = 0. 499, 0. 176, 0. 411). IOP, BCVA and CRT were significant statistical different in different time-point within each group ( F=291. 245, 103. 06, 665. 402, P<0. 01 ). Different surgical methods of 23G and 20G had interactive effects on IOP with different time points ( F = 13. 245, P<0. 01 ), but different surgeries had no interactive effects on BCVA and CRT with different time points (F=1. 212, 2. 293;P=0. 283, 0. 129). The counting CEC in 23G group was more than that in 20G group postoperatively at 90d, the difference was statistically significant (t=2. 049, P=0. 048). ·CONCLUSION: The 23G PPV combined with internal limiting membrane peeling, phacoemulsification, intraocular lens implantation for macular epiretinal membrane with cataract is effective. Compared with 20G PPV, 23G PPV has advantages in operation time for vitrectomy and counting CEC. But lower IOP is likely in 23G PPV on 1d after surgery

The RT-ABCDE strategy, a novel concept and an essential method, should be a routine strategy for disease control and prevention. It should be proposed and applied in both clinical and preventive medicine.

4.The rat′s cartilage ultrastructure change in abnormal stress by condylar neck fracture and osteotomy

Objective To investigate whether the abnormal stress could cause the ultrastructural change of the rats′condylar cartilage in vivo .Methods Fifty‐four SD male rats were randomly divided into condylar neck osteotomy operation(OO)group ,con‐dylar neck fracture operation(FO) group ,and control group ,with 12 rats in each group .Then the OO group rats ,FO group rats and control group rats were executed and sampled at 1st week ,3rd week and 5th week separately to observe the structure of condyle with light microscope(LM ) ,scanning electron microscope (SEM ) and transmission electron microscope (TEM ) .Results LM showed that the general condylar thickness of OO group and FO group were thinner than that of control group at 3rd and 5th week . SEM showed that the collagen and fibre were exposed in the OO group and FO group at 3rd week and 5th week ,the fibre was bro‐ken in the OO group at 5th week .TEM showed that the hypertrophic cell in OO group was more mature than those of FO group and control group at 3rd week .The arrange of fibre was disorder in OO group at 1st week ,the fibre was more disorder and there was even leak in OO group at 5th week .Conclusion The abnormal stress in vivo could result in the ultrastructural change of the rats′condyle ,and this change relates with the stess change degree and affects the growth and development of condyle .
5.The Progress on Electron Transport Pathway and Catalytic Mechanism of Copper-containing Nitrite Reductase

Nitrite reductases (NiRs) are the key enzymes in the denitrification pathway of the nitrogen cycle. By the catalysis of NiRs, the nitrites are turned into nitric oxides and the nitrogen pollution is decreased in water body. NiRs are divided into two different types based on their prosthetic groups, namely heme-containing nitrite reductases (cd1-NiRs) and Copper-containing nitrite reductases (Cu-NiRs). As all know, Cu-NiRs have trimeric structures, in their each monomer, there exist two types of Cu centers that play pivotal roles as the components of electron transfer pathway in the process of catalysis. Furthermore, some residues alteration of Cu-NiRs would contribute to the catalytic reaction. In this review, the latest progresses about the construction features, the process of electron transfer and catalytic mechanism of Cu-NiRs were discussed.
6.Influence of long-time illumination of monochromatic light on density of cones and opsin expression in guinea pig

Background The visual system of animal have to optimally adjust in various environmental conditions in order to obtain stable and effective visual funetion.However,the color vision system of animals which encounter uncertainty of spectral signals should be plastic.Whether the densities of various cones and expression of opsins change with long-time spectral deprivation is unclear.Objective This study was to investigate the changes of cone density as well as the expression of corresponding opsin and mRNA following the long-term illumination of monochromatic light.Methods Thirty 3-day-old guinea pigs were randomized into 3 groups and exposed tO the 530 nm green light,400 nm purple light and white light for consecutive 8 weeks respectively.The flat-mounted retinal sample was prepared and divided into dorsal zone,ventral zone and mixed zone anatomically according to the distribution of difierent light-sensitive cone.The changes in density of cone cells sensitivited to different colored light were detected by single-1abel or double-label immunocytochemistry.The levels of opsin and its mRNA were determined using Western-blot and real-time PCR respectively.Results The density of green-sensitivity cones was significantly different in the dorsal zone of retina among green light group,purple light group and white light group (F=234.28,P<0.01).Compared with white light group,the density of green-sensitive cones in dorsal retina of green light group was obviously higher but that of purple light group wag evidently lower(q=389.68,P<0.01;q=67.11,P<0.01).No significant difference was found in the density of purple-sensitive cones in the ventral zone of retina among green light group,purple light group and white light group(F=3.14,P>0.05).The density of coexpression of the mixed cone cells was increased in green light group(q=157.55,P<0.01)but decreased in purple light roup (q=254.85,P<0.01)in comparison with white light group.The expression levels of green-opsin and green-opsin mRNA in green light group was significantly elevated(q=184.45,P<0.01;q=4.71,P<0.05),but those of purple light group were evidently declined(q=5.87,P<0.05;q=346.66,P<0.01)in comparison with white light group.There was no statistically significant differences were found in the expression of purple-opsin and its mRNA among all the groups(F=1.24,P>0.05;F=3.27,P>0.05).Conclusion After the exposure of long-time monochromatic light illumination,monochromatic cones density and its opsin in guinea pig occur the corresponding alteration to gain good spatial vision as a compensatory reaction.These outcomes imply that there is some plasticity during the development of color vision.The increase of green-sensitive cones might be from the differentiation of coexpression cones in transition region.
7.Research progress of drug toxicity mechanism based on the gut microbiota

The exploration of drug toxicity and mechanisms is a vital component in ensuring the safe use of drugs in clinical practice, as this topic has attracted widespread concern. The intestinal flora holds great significance for drug metabolism, efficacy and mechanism, and is an instrumental metabolic organ that facilitates material information transfer and biotransformation. However, an increasing number of studies have shown that intestinal bacteria are closely related to the toxicity of specific drugs. On the one hand, drugs are transformed into toxic metabolites under the influence of intestinal bacteria, thus inducing direct drug toxicity. On the other hand, the composition and function of the intestinal flora are altered under drug influence, resulting in disruption of endogenous metabolic pathways. Consequently, this disruption compromises the intestinal barrier and affects other organs, leading to indirect drug toxicity. This review meticulously compiles recent examples of drug toxicity attributed to intestinal bacteria, explores in depth the contention that metabolic enzymes of gut microbiota may be of great influence on oral drug toxicity, and outlines prospective avenues for future research on gut microbiota and drug toxicity and mechanisms. This not only provides novel perspectives for the judicious clinical utilization of drugs but also offers insights for the safety assessment of innovative pharmaceuticals.
8.Analysis on disease burden of occupational coal workers′ pneumoconiosis in Anhui Province

Objective
To analyze the disease burden caused by occupational coal workers' pneumoconiosis (CWP) in Anhui
Methods
Province. A total of 1 649 patients with occupational CWP diagnosed in Anhui Province from 1950 to 2019 were
selected as the study subjects using a retrospective study method. Age, gender, survival time, location, working age of disease
onset, age of death, stage and date of diagnosis of CWP, comorbidities at the time of investigation, hospitalization and outpatient
expenses in the past year, cost of absence from work, cost of absence from work of caregivers, and cost of nutrition and
transportation were investigated. The years of life lost, years lived with disability (YLDs), disability adjusted life years (DALYs)
Results
and economic losses were calculated. Among these patients, 1 405 cases survived and 244 cases died. In the age range

of 20.0 70.0 years, the YLDs of surviving patients were 2.12 22.20 (8.19±3.59) person years. The YLDs of patients with disease

complications was higher than that of patients without complications [(8.55±3.95) (7.87±3.21) person years, <0.01]. The total

DALYs of the patients was 14 031.59 person years, and the average per capita DALYs was 8.51 person years. Its YLDs
accounted for 82.0 % of the total DALYs. The total economic loss caused by CWP in the 1 405 surviving patients was 354.903 0
Conclusion
million yuan, and the average per capita economic loss was 252 600 yuan. The disease burden caused by CWP is
relatively high in Anhui Province. In addition to early detection, diagnosis and treatment, it is necessary to focus on prevention
and treatment of CWP complications to reduce the disease burden.
9.A novel dipeptidyl peptidase IV inhibitors developed through scaffold hopping and drug splicing strategy.

Though all the marketed drugs of dipeptidyl peptidase IV inhibitors are structurally different, their inherent correlation is worthy of further investigation. Herein we rapidly discovered a novel DPP-IV inhibitor 8g (IC50 = 4.9 nmol.L-1) which exhibits as good activity and selectivity as the market drugs through scaffold hopping and drug splicing strategies based on alogliptin and linagliptin. This study demonstrated that the employment of classic medicinal chemistry strategy to the marketed drugs with specific target is an efficient approach to discover novel bioactive molecules.

10.Principles of safety, efficacy and stability in gene therapy--review.

Somatic gene therapy based on nonviral and viral vectors is an attractive approach for treatment of human diseases. It has developed from preliminary animal experiments to clinical trials. Vectors and target genes used in gene therapy are mainly focused on viral, nonviral vector and single target gene or monogene and each vector system has a series of advantages and limitations. However, the application of gene therapy to human disease is currently hampered by potential hazards of methods of gene delivery, the relatively low efficiency and intracellular stability of target gene. Safety, efficacy and stability are three problems that people often met and also three principles that people should be followed in gene therapy.
