OBJECTIVE: To investigate the immunoregulation and short-term therapeutic effects of super-selective intra-arterial chemotherapy combined with Fuzheng Kang'ai Granules, a compound Chinese herbal medicine, on patients with late gastric cancer. METHODS: Forty patients with late gastric antrum cancer were randomly divided into study group and control group. Patients in the study group were orally administered Fuzheng Kang'ai Granules 48 hours after the first super-selective left gastric artery chemotherapy with high-dose drugs (EAP regime: VP(16) 100 mg/m(2) + epirubicin 60 mg/m(2) + carboplatin 200 mg/m(2)), while patients in the control group were only administered the same local artery chemotherapy as in the study group. RESULTS: The short-term therapeutic efficacy in the study group and control group were 82.5% and 57.5% respectively (P<0.01). The occurrence rates of side effects in the study group were significantly lower than those in the control group (P<0.01). The Karnofsky score of life quality of the study group was obviously higher than that of the control group after treatment (P<0.05). The half survival time and one year survival rate in the study group and control group were (24.9 +/- 1.36) month, 70% (28/40) and (13.7 +/- 0.72) month, 35% (14/40) respectively. They were significantly improved in the study group compared with the control group (P<0.01). The levels of cytokines interleukin 2 (IL-2), tumor necrosis factor-alpha (TNF-alpha), interferon-gamma (IFN-gamma) after treatment in the study group were significantly higher than those before treatment (P<0.05 or P<0.01), as well as than those after treatment in the control group (P<0.01). On the other hand, the level of immune inhibitory factor soluble interleukin-2 receptor (sIL-2R) after treatment in the study group was lower than that before treatment, as well as than that after treatment in the control group (P<0.01). CONCLUSIONS: The super-selective intra-arterial chemotherapy combined with Fuzheng Kang'ai Granules has good short-term therapeutic efficiency and few side effects for patients with late gastric antrum cancer. It can significantly improve the life quality, extend the survival period, and improve the survival rate in patients with late gastric antrum cancer, which may be due to the up-regulation of the immune regulating factors such as IL-2, TNF-alpha, IFN-gamma and down-regulation of the immune inhibitory factor sIL-2R.

BACKGROUNDCervicothoracic junction spinal tuberculosis (CJST) in children is uncommon, especially when accompanied by a huge abscess. However, its consequences can be severe. Because of the special anatomic location of the cervicothoracic junction, surgical treatment is difficult and rarely reported. The aim of this clinical study was to assess the effectiveness of combined anterior and posterior approaches for focal debridement, decompression, allografting and anterior instrumentation in the treatment of CJST in children.
METHODSTen pediatric CJST patients underwent focal debridement and cord decompression through combined anterior and posterior approaches. Then an appropriate allograft and titanium plate were applied to reconstruct the spine. The patients were asked to wear head-neck-chest braces for six months and received regular anti-tubercular drugs therapy for 12 months.
RESULTSThe patients were followed-up for an average of 26 months (range, 15-32 months). There was no recurrent tuberculous infection. The bone grafts incorporated well and the instrumentation was stable. Cervical and thoracic kyphosis was successfully corrected from 40° (range, 30-52°) before the operation to 18° (range, 12-26°) post-operation. Neurological function was improved in all patients.
CONCLUSIONSCombined anterior and posterior approaches for focal debridement, decompression, bone allografting and anterior instrumentation provided an effective means of treatment in children of CJST with a huge abscess in the posterior part of the vertebral body.

Objective To study the related influencing factors involving in vertebrobasilar dolichoectasia (VBD) and changing characteristics of hemodynamics of VBD. Methods Outpatients and inpatients with posterior circulation ischemia (PCI) underwent brain MRA from June 2008 to September 2009 were chosen; 116 patients met the standard of VBD were selected as VBD group and other 130 patients without VBD were as control group. The univariate analysis and multiple factor analysis were applied to study the correlation of hypertension,smoking,diabetes,low-density lipoprotein (LDL) and intracranial vascular variation with VBD.Transcranial Doppler (TCD) ultrasound and digital subtraction angiography (DSA) were applied to observe the hemodynamic changes and arteriosclerosis of these patients. Results Univariate analysis indicated that statistical significance on intracranial vascular variation, hypertension and smoking was noted between the VBD group and the control group (x2=13.530,P=0.000; x2=8.539,P=0.003; x2=3.959,P=0.047),while no statistical significance in diabetes and LDL was obtained between the 2 groups (x2=0.451,P=0.519; x2=0.220,P=0.881).Multiple factors analysis showed that VBD could be obviously related to intracranial vascular variation (OR=21.732, 95%CI:3.02-5.94),hypertension (OR=5.173,95%CI:1.23-3.05) and smoking (OR=3.145,95%CI:1.86-4.85).According to TCD,peak velocity (Vp),mean velocity (Vm) in the VBD group were significantly lower than those in the control group, but pulsatility index (PI) and resistant index (RI) in the VBD group were statistically higher (P<0.05).DSA showed that the blood flow length of the VBD group was obviously shorter than that of the control group in the same time with statistical significance (P<0.05). Conclusion Vascular variation, smoking and hypertension are related to VBD; and VBD leads to slow blood flow and aggravated arteriosclerosis, which may result in high incidence of stroke.

The molecular mechanism underlying muscular atrophy and gravisensing during spaceflight is still unknown. The major effects of spaceflight on body-wall muscles of Caenorhabditis elegans (C. elegans) in the structures and functions wore examined, and five important muscle-related genes and three proteins were studied after nearly 15-day spaceflight. The changes for the wall-muscles were observed in situ. Decreased muscle fiber size was observed with myosin immunofluorescence and duller dense-body staining in flight samples, which suggested that muscular atrophy had happened during spaceflight. However, F-actin staining showed no differences between the spaceflight group and ground control group. Otherwise, after returning to the earth the C eleganu displayed reduced rate of movement with a lower ratio (height/width) in crawl trace wave, which indicated a functional defect. These results demonstrated that C. elegans muscular development was changed in response to microgravity, and changes also occurred at the level of gene transcription and protein translation. Expression of dys-I increased significantly in body-wall muscles, while hlh-1, myo-3, uric-54 and eg1-19 RNA levels decreased after spaceflight. Dystrophin (encoded by dys-1) is one of important components in dystrophin-glycoprotein complex (DGC). Increased dys-I expression after flight implied that the muscular cell would accept more gravity signals by DGC in mierogravity in order to keep mechanical balance within the cells. It is concluded that DGC was involved into the mechanical transduction in body-wall muscles of C. elegans when gravity varied, which potentially played a vital role in gravisensing. The changes ofhlh-l, myo-3, tmc-54 and egl-19 suggested that they had the effects of promoting microgravity-induced muscular atrophy in strcture and function aspects. Result of Western blotting showed that the level of myosin A in spaceflight group decreased, further confirmed that atrophy happened during flight.

To search for potent drugs against non-small-cell lung cancer(NSCLC),a series of hybrids(9a-9e, 10a-10e and 11a-11e﹚ from anilinopyrimidines and diazeniumdiolates were designed and synthesized. The MTT assay was employed to evaluate their antiproliferative activity against H1975 cells harboring epithelial growth factor receptor(EGFR)L858R/T790M mutation. The results showed that compounds 9a-9e displayed remarkable inhibitory activity on H1975 cells.Among these compounds, the most potent was compound 9b(IC50=0.65 μmol/L),which was superior to the positive control gefitinib.Additionally, molecular docking study indicated that 9b could bind with EGFR T790M by forming hydrogen bond, electrostatic interactions, et al, suggesting that compound 9b may be a potential anti-NSCLC agent for further investigation.

OBJECTIVEThe pathologic slides and clinical data of 119 patients with mucoepidermoid carcinoma (MEC) of salivary glands were reviewed. All patients underwent surgical therapy in Department of Oral and Maxillofacial Surgery, College of Stomatology, Fourth Military Medical University. The postoperative overall observed survival rates and specific survival rates were statistically caculated, and the correlative clinicopathologic factors influencing the prognosis were evaluated.
METHODSStatistical analysis was performed using the SPSS 10.0 software. The survival rates were obtained by the Kaplan-Meier method. The clinicopathologic parameters were evaluated using log rank test (univariate analysis) and Cox proportional hazards regression model (multivariate analysis).
RESULTSThe overall observed survival rates of this group were 92.53%, 87.52% and 85.39% at 5, 10 and 15 years respectively after surgical therapy. Univariate analysis demonstrated that the factors correlated with lower in survival rates were patient's age (> or = 40 years), advanced clinical stage (TNM III, IV stage) and the poorly differentiated tumors (P < 0.05). The survival rates of male patients with MEC in the major salivary glands were significantly lower than those of female patients (P = 0.008). The degree of differentiation, TNM stage and preoperative symptoms were three important prognostic factors that were selected into the Cox proportional hazards regression model.
CONCLUSIONSMost patients with highly differentiated MEC in salivary glands had a favorable outcome after receiving adequate surgical treatment. The degree of differentiation and the TNM stage of MEC in the salivary glands are two useful factors to evaluate the prognosis of these neoplasms.

OBJECTIVETo investigate the expression variation and significance of Skp2 and p27(kip1) during the proliferation of lymphoma cell line Jurkat cells.
METHODSThe binding of p27(kip1) and Skp2 in Jurkat cells were detected by immunoprecipitation. Jurkat cells were treated with serum starvation and release synchronization. The expression variation and subcellular localization of p27(kip1) and Skp2 were detected by subcellular fractionation, Western blot and double immunofluorescence labelling.
RESULTSThe results of immunoprecipitation suggested that p27(kip1) and Skp2 could bind each other in Jurkat cells. During the proliferation of Jurkat cells, the protein expression of p27(kip1) decreased and intranuclear p27(kip1) decreased significantly, while the Skp2 protein increased and cytoplasmic Skp2 increased significantly.
CONCLUSIONDuring the proliferation of Jurkat cells, the increased cytoplasmic synthesis of Skp2 may speed up p27(kip1) degradation via the ubiquitin-proteasome pathway, then intranuclear p27(kip1) decreases significantly, leading to an increased cell cycling activity.
