1.Effect of photochemotherapy on expression of gelatases

Objective To determine the effect of photochemotherapy on the expression of gelatinases. Methods Reverse transcription-polymerase chain reaction and gelatine zymography were employed to detect the effect of photochemotherapy on the expression and bioactivity of gelatinases at mRNA and protein levels respectively. Results Combination of ultraviolet A(UVA,0.8-2.0 J/cm2) and 8-methoxypsoralen(8-MOP,100 ng/mL) resulted in a decrease in the expression and bioactivity of gelatinases. Conclusion Photochemotherapy can inhibit the angiogenesis of endothelial cells through downregulating the expression and bioactivity of gelatinases.
2.Arbitrary position in the early period after laparoscopic cholecystectomy

Objective To evaluate the feasibility and safety of taking arbitrary position in the early period after laparoscopic cholecystectomy.Methods 186 patients who underwent laparoscopic cholecystectomy from Jun.2010 to Oct.2010 were randomly divided into two groups,observation group (n =94) and control group ( n =92) ; the former group was instructed to freely mobilize 2 hours after the operation,while the latter group was administered with traditional nursing.Abdominal distension,pain,urinary retention of both groups after laparoscopic cholecystectomy were compared.Results The observation group happen abdominal distension 4 cases,urinary retertion in 3,lumbar muscle ache 15 cases and control group happen abdominal distension 13 cases,urinary retention in 12 cases,lumbar mascle ache 60 cases,and muscle soreness after laparoscopic cholecystectomy were obviously lower in the observation group,and the differences between both groups were statistically significant ( x2 =5.460,6.087,46.885 ; P < 0.05 ).Conclusions Taking arbitrary position in the early period 2 hours after laparoscopic cholecystectomy is safe and feasible,it can also reduce the occurrence of abdominal distension,urinary retention and muscle soreness,mitigate pain,dizzy,palpitation and lack of energy,and facilitate patients' early recovery.
3.The study of FTY720 on inducing apoptosis and autophagy in multiple myeloma cell line U266.

OBJECTIVETo investigate the effects of FTY720, a new immunosuppressive agent, on apoptosis and autophagy in multiple myeloma(MM) cell line U266 and to clarify its molecular mechanism.
METHODSU266 cells were treated with 0, 2.5, 5.0, 10.0 and 20.0 µmol/L FTY720 for 24 hours, and the cell viability was assayed by CCK-8 method. Then U266 cells were treated with 20.0 µmol/L FTY720 for 0, 2, 6 and 24 hours, the cell viability was tested. The apoptotic rates induced by different doses and time points of FTY720 were tested by flow cytometry separately. The expression of LC3B was detected by Western blot after U266 cells treated with different doses of FTY720 to see autophagy. U266 cells were treated with FTY720 ± Bafilomycin A1, an inhibitor of autophagy, for 24 hours, then the cell viability and apoptotic rates were tested. Meanwhile the expression of survivin, anti-apoptotic factors, were tested by Western blot.
RESULTSThe cell viability and the apoptotic rates were inhibited significantly by FTY720 (P < 0.05) in time-dependent and dose-dependent manner. The expression of LC3B-II increased significantly in a dose-dependent manner, it indicated that the autophagy was induced by FTY720. Bafilomycin A1 could rescue the cell viability and apoptotic rates in U266 cells treated with FTY720, and it could also rescue the expression of survivin decreased by FTY720.
CONCLUSIONSFTY720 can cause apoptosis and autophagy of U266 cells. The autophagy promote the apoptosis, which maybe due to the degradation of anti-apoptotic factors such as survivin or their upstream factors in lysosomes through autophagy.

4.Morphological characteristics of mandibular symphysis in adult skeletal class II and class III malocclusions with abnormal vertical skeletal patterns.

OBJECTIVETo figure out the differences of the morphological characteristics of mandibular symphysis between Class II and Class III adult skeletal malocclusions with different abnormal vertical skeletal patterns.
METHODS109 Chinese female adults of skeletal Class II and Class III were chosen and divided into four groups according to vertical and sagittal skeletal pattern: Class II--vertical-growth-pattern group (n=30), Class III--vertical-growth-pattern group (n=25), Class II--horizontal-growth-pattern group (n=29), Class III--horizontal-growth-pattern (n=25). Lateral cephalograms were taken. The symphyseal widths and heights, along with lower incisor positions were evaluated. Observation and statistics analysis were done to clarify the morphological characteristics of the symphyseal region of different skeletal patterns.
RESULTSThere were morphological differences of symphyseal region between Class II and Class III skeletal malocclusions, but not significant in width and total height. With a vertical-growth-pattern, Class II malocclusions had higher alveolus than Class III, but smaller chin prominence and lower basal bone (P < 0.01). With a horizontal-growth-pattern, Class II malocclusions had higher alveolus (P < 0.05) and larger alveolar top width (P < 0.001). With a same sagittal skeletal pattern, vertical-growth-pattern group had thinner but higher symphyseal region and bigger chin prominence (P < 0.001 in Class II while P < 0.05 in Class III). Besides, a vertical-growth-pattern malocclusion was prone to have a cucurbit-morph chin, of which Id width was larger than basal width (P < 0.01).
CONCLUSIONThere are morphological differences in symphyseal region between Class II and Class III skeletal malocclusions with different abnormal vertical skeletal patterns. The influence of abnormal vertical skeletal pattern to symphyseal morphological characteristics is greater than that of abnormal sagittal skeletal pattern. There is a risk of orthodontic movement of low incisors in vertical-growth-pattern skeletal malocclusion.

5.Study on the posterior teeth mesiodistal tipping degree of normal occlusion subjects among different facial growth patterns.

OBJECTIVETo study the relationship between the mesiodistal tipping degree of maxillary and mandible posterior teeth and different vertical facial skeletal types of subjects with normal occlusion.
METHODS163 subjects with normal occlusion were selected, lateral cephalograms were taken and divided into three different facial skeletal types. The difference of the mesiodistal tipping degrees and intersection angles of upper and lower posterior teeth between the three different facial skeletal types were analyzed.
RESULTSAmong 163 subjects, vertical growth pattern, average growth pattern and horizontal growth pattern were 24, 96 and 43 respectively. There were statistic differences of mesiodistal tipping degrees of the first and second maxillary and mandible premolar and the first permanent molar between vertical growth and horizontal growth pattern, horizontal growth and average growth pattern (P < 0.05). But there were no statistically significant differences between vertical growth and average growth pattern (P > 0.05). The differences of the maxillary and mandible posterior teeth's intersection angle among three vertical facial skeletal types had no statistic significance (P > 0.05).
CONCLUSIONDifferent vertical facial skeletal type has its own normal and coordinated posterior teeth mesiodistal tipping degrees.

6.Re-operation for thyroid carcinoma.

OBJECTIVETo evaluate the results of re-operation of thyroidectomy for patients of thyroid carcinoma.
METHODS268 patients of thyroid carcinoma received completion thyroidectomy were retrospectively reviewed from 1984 to 2000. There were 59 males and 209 female cases. 256 cases had received nodule enucleating or partial thyroidectomy in other hospitals and 12 cases had unilateral subtotal thyroidectomy in this hospital. The types of reoperation were: Total thyroidectomy for bilateral thyroid carcinoma (6 cases); expand isthmectomy for isthmus carcinoma (1 case). Lobectomy plus isthmectomy for unilateral disease (261) Simultaneous neck dissections were performed in 196 cases. Among them, 94 cases had classical neck dissection, 102 cases had modified neck dissection.
RESULTSPathological results confirmed that there were 78 cases with residual thyroid carcinoma. The rate of residual carcinoma was 29. 1% (78/268). There were 95 cases with lymphnode metastasis. The rate of lymphnode metastasis was 48.5% (95/196). The rate of recurrent laryngeal nerve injury was 1.1% (3/268). The 5-year and 10-year survival rates of patients were 94. 0% (251/267) and 85. 2% (127/149) respectively.
CONCLUSIONSThe residual carcinoma of completing thyroidectomy was high. Therefore re-operations of thyroid in selected cases were necessary.

7.Level VI and II-V cervical lymph node metastasis in differentiated thyroid carcinoma.

OBJECTIVETo explore the characters of the cervical lymph node metastasis in differentiated thyroid carcinoma, and to provide evidence for proper surgery of differentiated thyroid carcinoma.
METHODSFrom 1984 to 2000, 99 cases with differentiated thyroid carcinoma were performed thyroidectomy and neck dissection. The patients were followed up. A retrospective analysis was performed. Results In 99 cases with differentiated thyroid carcinoma, there were 61 papillary carcinoma, 13 papillary and follicular mixed carcinoma, 25 follicular carcinoma. According to 2002 UICC TNM classification, 60 cases were staged I, 1 case staged II, 5 cases staged III, 33 cases staged IV. Lobectomy and isthmectomy was performed in 80 cases, lobectomy was resected and opposite subtotal lobectomy in 15 cases, total thyroidectomy in 4 cases. One hundred and four neck dissection were performed in 99 cases (5 cases were bilateral neck dissection ). Among them, 66 (68 sides) were radical neck dissection, 33 (36 sides) were modified neck dissection. Pathological results showed that lymph nodes were positive in 86 sides of 83 cases. The rate of cervical lymph node metastasis was 83.8% (83/99). The positive rates of lymph node were 37.5% (39/104) in level VI and 76.9% (80/104) in II-V, which was statistically different (chi2 = 33.01, P < 0.01). The cervical lymph node metastasis in lateral area (level II-V) and that in VI had not relationship (chi2 = 2.08, P > 0.05). Ten and 15 year survival rates of all 99 cases were 88.3% and 84.5% respectively.
CONCLUSIONSThe occurrence of lymph node metastasis in level VI and level II was different and no relationship .One can not judge whether lateral neck metastasis by the lymph node statue in level VI only . Although they all had good prognosis, patients with positive nodes in level VI were not worse than that in lateral neck (II-V).

8.The relationship between methylenetetrahydrofolate reductase gene polymorphism and microsatellite instability in gastric cancer.

OBJECTIVETo explore the relationship between methylenetetrahydrofolate reductase (MTHFR) gene polymorphism and microsatellite instability (MSI) in patients with gastric cancer.
METHODSMTHFR gene C677T and A1298C polymorphism were detected by polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP) and MSI was examined with PCR.
RESULTSMTHFR gene C677T and A1298C polymorphisms were analyzed on 122 gastric cancers and 110 normal controls The genotype frequencies of MTHFR 677CC, 677CT and 677TT were 47.5%, 39.3% and 13.1% on patients with gastric cancer, and 48.5%, 42.6%, 8.9% in the controls respectively. There was no significant difference of genotype frequency between the two groups (P > 0.05). The individuals with 677CT genotype, 677TT genotype and 677CT + TT genotype exhibited significantly reduced risk (OR = 0.38,95% CI: 0.15-0.98; OR = 0.26,95% CI: 0.03-2.18 and OR = 0.36,95% CI: 0.07-0.98) of developing gastric cardia cancer compared with those harboring the wild-type(677CC). The individuals with 677TT genotype having a 3.03-fold (95% CI: 1.07-8.65) increased risk of developing gastric corpus cancer. The genotype frequency of MTHFR 1298AA, 1298AC and 1298CC were 59.8%, 36.1% and 4.1% in gastric cancer patients, and 57.4%, 7.6%, 5.0% in the controls, respectively. The distribution of MTHFR A1298C genotype was not significantly different between gastric cancer and controls (P > 0.05). The individuals with 1298CC genotype had a reduced risk of developing gastric antrum cancer (OR = 0.41- fold, 95% CI: 0.03-2.18, 0.05-3.72) when comparing with those having 1298AA genotype. Patients with MSI+ gastric cancer had an increased frequency of the MTHFR 677TT genotype when comparing with those suffering from MSI- gastric cancer (P = 0.009) and with controlled subjects (P = 0.008). There was no significant association found between MTHFR A1298C polymorphism and MSI (P>0.05).
CONCLUSIONPolymorphism of MTHFR C677T was associated with increased risk on gastric corpus cancer and reduced risk on gastric cardia cancer. The polymorphism of MTHFR A1298C was associated with reduced risk for gastric antrum cancer while MSI pathway was possibly involved in the development of gastric cancer with MTHFR 677TT genotype.

9.Preliminary construction of risk assessment tool framework for surgical wound infection

Objective To construct a risk assessment tool framework for surgical wound infection(SWI). Methods The assessment system was constructed by referring to expert interviews, relevant domestic and foreign literatures, and the latest SW1 guidelines. After two rounds of Delphi expert consultation, the assessment items were drawn up, selected and modified, and the initial scale was established. Results The positive coefficients of two rounds of expert Delphi methods were 84% and 100% respectively; expert personal authority coefficient were both >75%, the all expert authority coefficient was 0.86; two rounds of Kendall's W coefficients were 0. 450 and 0. 441 respectively (all P く0. 05). The recommendations of two rounds of experts were 26 and 5 items respectively. The final assessment system indexes included 4 first-level indicators, 10 second-level indicators, and 27 third-level indicators. Conclusion The risk assessment tool framework for SWI is preliminarily established, which can provide a scientific basis for the effective evaluation of the risk of postoperative SWI.
10.Mechanisms of ROS in U266 cell death induced by FTY720.

This study was purpose to investigate the role of reactive oxygen species (ROS) in apoptosis and autophagy induced by FTY720 in multiple myeloma cell line U266. U266 cells were treated by different concentrations of FTY720 for 24 h, the apoptotic rates were detected by flow cytometry, and the expression of LC3B was detected by Western blot. The results indicated that apoptosis and autophagy were induced by FTY720 in U266 cells. Autophagy induced by FTY720 could lead to cell death. Bafilomycin A1, the inhibitor of autophagy, could enhance the cell viability in U266 cells treated with FTY720. NAC or Tiron, ROS scavenger, could decrease the FTY720 induced apoptosis and the expression of LC3B-II was reduced in combination of FTY720 with NAC or Tiron as compared with treatment with FTY720 only. It is concluded that FTY720 can induce U266 cell apoptosis and autophagy. ROS is the mediator that regulates both the apoptosis and autophagy in multiple myeloma cells.
