Objective To evaluate the efficacy of transversus abdominis plane (TAP) block for postoperative analgesia in the pediatric patients undergoing operation on the inguinal region.Methods Sixty-four pediatric patients of both sexes, aged 1-3 yr, weighing 8.5-23.6 kg, of American Society of Anesthesiologists physical status Ⅰ , undergoing elective unilateral high ligation of the hernia sac or high ligation of the processus vaginalis, were equally randomized into either intravenous analgesia group (group VA) or TAP block group (group TAP).In group TAP, after induction of anesthesia, the pediatric patients received ultrasound-guided TAP block with 0.2％ ropivacaine 1 ml/kg injected locally.After the laryngeal mask airway was inserted, the pediatric patients were mechanically ventilated, and then inhaled 2％-3％ sevoflurane for maintenance of anesthesia.In group VA, postoperative analgesia was performed with fentanyl 0.35 μg · kg-1 · h-1 until 24 h after operation.The level of pain was rated using the FLACC pain scale.When FLACC score＞5, fentanyl 0.25 μg/kg was injected intravenously as rescue analgesic.The occurrence of TAP block-related adverse events, time for removal of laryngeal mask airway, and occurrence of fentanyl-related nausea and vomiting, respiratory depression, and emergence agitation were recorded.Results No TAP block-related adverse events were observed in group TAP, and no respiratory depression was found in the two groups.Compared with group VA, the time for removal of laryngeal mask airway was significantly shortened, and the requirement for rescue analgesics and incidence of vomiting and emergence agitation were decreased in group TAP (P＜ 0.05).Conclusion TAP block with 0.2％ ropivacaine 1 ml/kg provides good efficacy for postoperative analgesia with good safety in the pediatric patients undergoing operation on the inguinal region.

OBJECTIVETo investigate the role of perindopril for prevention and treatment of glucocorticoid-induced osteoporosis (GIOP) in rabbits.

METHODSA total of 45 male New Zealand white rabbits (10 months old, weight 3.0 to 3.5 kg) were randomly divided into 3 groups involving normal control group (muscle injection of saline solution, n = 15, group NC), model group (muscle injection of dexamethasone, n = 15, group GIOP), and treatment group (muscle injection of dexamethasone combined with oral perindopril, n = 15, group GIOP+ACEI). All rabbits put to death after 12 weeks' treatment. The changes of bone mass and strength were observed and analyzed by bone histomorphology, biomechanics, metabolic bone related serological indexes and mRNA expression.

RESULTSAt 12 weeks, the analysis of bone histomorphology and biomechanics results showed that the bone mass and bone strength of group GIOP were significantly lower than that of group NC (P < 0.05); after perindopril treatment, the bone mass and bone strength of group GIOP+ACEI were higher obviously than that of group GIOP (P < 0.05). Mineralizing surface,mineral apposition rate and serum osteocalcin in group GIOP decreased than group NC; however, osteoclast number, osteoclast surface, eroded surface, and urinary deoxypyridinoline in group GIOP increased than group NC (P < 0.05); these changes were inhibited after perindopril treatment (P < 0.05). Quantitative RT-PCR revealed that after dexamethasone treatment, the ratio of SOST mRNS expression and RANKL/OPG mRNA expression obviously increased than that of group NC (P < 0.05); and Runx2 expression decreased significantly (P < 0.05); while the changes of mRNA expression were improved by perindopril treatment.

CONCLUSIONPerindopril can promote bone formation and inhibit bone resorption to deduce glucocorticoid-induced osteoporosis. This study provides a new method for prevention and treatment of GIOP.

Objective To observe the shape and the hemodynamic changes of the left heart after transcatheter closure of atrial septal defects(ASD) using Amplatzer occluder.Methods Sixty-five subjects with secondum ASD underwent transthoracic echocardiography(TTE) at pre-operation,48～72 h post-operation and follow-up performed more than 3 months after operation in 45 patients.The dimension of ASD,the parameters of the pulse-wave Doppler spectrums of the blood flow at the aotic valves and inlet of the left ventricle were recorded.At each position,systolic,early diastolic velocities and intervals were measured,the mitral annular movement spectrum was acquired at the lateral site of mitral annulus in the apical four-chamber view,Tei index and Tei_(TDI) index were derived from these parameters.Results All the patients had been implanted one Amplatzer occluder under the guide of echocardiography successfully.The preload of the left ventricular volume in the end of the diastolic period was increased and the low hemodynamic situation of the left heart and systemic circulation were corrected after the operation immediately.Accordingly,at 48～72 h after the operation,the end diastolic diameter,the stroke volume and the ejection fraction of the left ventricle were significantly improved continuously.However,the movement of the posterior wall decreased significantly at 48～72 h post-operation.The peak velocity of systolic period and the early diastolic velocities of the left ventricle were increased,but the ejection time had no changes follow-up.The systolic function of the left ventricle highlighted,the whole diastolic time prolonged after the 3 months of the operation.Tei index,Tei_(TDI) index and the heart rate had no difference between pre-and post-operation.Conclusions The anatomic deformation was corrected by transcatheter closure of ASD with Amplatzer occluder,the pre-load of left ventricular increased and had significant effect on the morphology and hemodynamic situation quickly,the left ventricular systolic and the diastolic function improved.The pre-load change of left heart had no effect on heart rate.Tei index and Tei_(TDI) index were independent of the pre-load and present the heart-self function.

In total, 607 bedridden patients aged 60 years and over at Tianlin community hospital and four other hospitals or at home-care in Shanghai were investigated with questionnaire, physical examinations and related laboratory tests. Contents of investigation include general situation, medical conditions, 24-hour recall of diet, intake of nutritional supplement preparation, and so on. As compare to the reference for nutrient intake of the elderly set in the Guide to Diet for Chinese Residents (2007), total nutritional intake was obviously insufficient and three nutrients in diet were imbalanced for most bedridden elderly patients. Intake of most nutrients (including proteins, dietary fiber, calcium, iron, zinc, selenium,copper, magnesium, phosphor, retinol equivalent, vitamin B1 , vitamin B2, vitamin C) was obviously low in bedridden elderly patients, with a larger gap from the requirements of the Dietary Guide.

Diffusion-weighted imaging (DWI) is sensitive to identification of cervical lesions and lymph node metastasis.DWI can be used to predict and evaluate the therapeutic effect of cervical cancer.Using magnetic resonance imaging and DWI scans in the process of diagnosis and treatment of cervical cancer,which may contribute to the personalized treatment program and improve prognosis for patients.

[ ABSTRACT] AIM: To study the autophagy of prostate cancer PC-3 cells induced by CD147 in vitro.ME-THODS:Themethod of amino acid starvation to induce autophagy was used.The expression of CD147 was detected by Western blotting.To study the functional effects of CD147 on autophagy in prostate cancer PC-3 cells, the down-regulation of CD147 expression was induced by the technique of RNAi.The conversion of autophagic marker protein LC3-I to LC3-II was determined by Western blotting.The cell death after starvation-induced autophagy was analyzed by trypan blue exclu-sion assay.RESULTS:The CD147 expression gradually increased in starvation-induced autophagy.The down-regulation of CD147 significantly increased the expression of autophagy-related protein LC3-II compared with control group.Mean-while, the cell death rates increased from (19.3 ±3.1)%and (22.3 ±3.5)%in control groups to (38.4 ±3.1)%in si-lencing the expression of CD147 in the PC-3 cells (P<0.05).CONCLUSION:CD147 inhibits starvation-induced auto-phgy and autophagy death in the prostate cancer PC-3 cells.
