Objective To evaluate the efficacy of tibial bone-skin flap grafts in the management of se- vere traumatic osteomyelitis complicated with bone and skin defect in leg to avoid amputation.Methods From March 1998 to Aug.2004,12 cases of the traumatic osteomyelitis complicated with bone and skin defect in leg were treated with vascularized tibial bone-skin flap graft.The longest flap was 17cm,widethest 10cm, The longest bone flap was 12cm.They were followed up for 0.6 to 5 years.Results All the tibial bone-skin flaps survived completely,2 cases of osteomyelitis recurred.The followed-up,from 0.5 to five years,showed good bone union in all cases,averageing 15 weeks.The infection was under control.The leg function and con- tour were satisfactory.Conclusion The tibial bone-skin flap has the advantages of having distinguished sign of anatomy,highly vascularized,easy to obtain,simply and flexible procedure,improving circulation,short- ens hospitalization and suitable for treatment of traumatic osteomyelitis complicated with bone and skin defect in leg.
4.Eye model for inspecting the functional vision of IOLs

· AIM: To inspect and compare the functional vision of an aspheric intraocular lens (Tecnis) with those of conventional monofocal silicone and acrylic intraocular lens and multifocal intraocular lens (Array).· METHODS: The IOLs were tested in the eye model, which was designed to be optically equivalent to the theoretical eye model. The eye model is a combination of a spherical photographic lens with 35mm focal length ( IOL put in a water cell)and a charge coupled device (CCD) camera. The images constructed by the lenses are observed on a monitor of personal computer and the contrasts of the images are analyzed by using commercial image processing software.· RESULTS: The modulation transfer function of the eye model equals the scale produced by the theoretical eye model. The images constructed by changing the diameter of aperture stop and IOL.· CONCLUSION: The proposed eye model is useful for testing functional vision and for inspecting differences of intraocular lens.
5.Local application of different doses of tranexamic acid without drainage reduces blood loss after total knee arthroplasty

BACKGROUND: Total knee arthroplasty (TKA) is an effective method for the end-stage knee osteoarthritis, which can obviously relieve pain and improve function. But a large amount of postoperative blood loss is a serious complication, how to reduce postoperative blood loss is still a difficulty. OBJECTIVE: To explore the efficacy of local application of different doses of tranexamic acid without drainage on the METHODS: 150 patients with osteoarthritis who underwent TKA were randomly divided into three groups (n=50 per group). Group A received the intra-articular injection of tranexamic acid (2 g) plus 50 mL of normal saline; group B received the intra-articular injection of tranexamic acid (1 g) 50 mL of normal saline; group C received the intra-articular injection of 50 mL of normal saline. The drainage tube was not used in all patients. The number of hemoglobin, the number of patients undergoing blood transfusion, intraoperative blood loss, external blood loss, hidden blood loss, total blood loss and postoperative 3 hour-related blood coagulation indexes were recorded and compared among groups. Additionally, the incision infection and deep vein thrombosis were observed. RESULTS AND CONCLUSION: (1) The difference of hemoglobin among groups was significant (F=7.218-7.516, P=0.000).(2)The number of blood transfusion in the groups A,B and C was 2,5 and 9,respectively,which had significant differences (χ2=25.753, P=0.000). (3) The intraoperative blood loss had no significant difference among groups (F=1.206, P=5.283). The difference in the postoperative external blood loss, postoperative hidden blood loss and total postoperative blood loss among groups was significant (F=14.389, P=0.000; F=7.158, P=0.009; F=6.752, P=0.012). Moreover, the hemostatic effect was in a dose-dependent manner. (4) There were no significant differences in the postoperative 3-hour-related blood coagulation indexes among groups (P > 0.05). (5) There was no deep vein thrombosis at 1 week postoperatively, and no deep vein thrombosis or pulmonary embolism occurred within postoperative 3 months in the three groups. (6) To conclude, topical application of tranexamic acid without drainage can significantly reduce perioperative blood loss, blood transfusion rate, and does not increase the incidence of complications, and the hemostatic effect is in a dose-dependent manner.

OBJECTIVETo investigate the effects of monofluorphosphate and alphaD3 on the bone mineral density of mandibular of ovariectomy rats.
METHODS39 female Sprague-Dawley rats about 90 days of age were randomly divided into the following treatment groups and treated for 12 weeks: SHAM group, OVX group, MFP group, MFP+alphaD3 group. Rats of MFP group received monofluorphosphate by gastric feeding. Rats of MFP+alphaD3 group received monofluorphosphate and alphatD3 by gastric feeding. The femur and the mandibles were collected for histomorphometry and bone mineral density measurement.
RESULTSThe bone area, bone thick, and bone mineral density of femur in the OVX group were significantly lower than those of other groups, while the trabecular separation of the OVX group was significantly higher than that of other groups. The bone mineral density of mandible was increased in OVX group and MFP+alphaD3 group compared with SHAM group.
CONCLUSIONMonofluorphosphate and alphaD3 can obviously increase the mass of mandible. The bone mineral density of femur decreased after ovariectomy while it increased in mandibular.

8.Efficacy of lamivudine on acute-on-chronic liver failure in patients with chronic hepatitis B.

To observe the therapeutic effects of lamivudine treatment in patients with early- to mid-stage hepatitis B virus-related acute-on-chronic liver failure (HBV-ACLF). Clinical data of 73 hospitalized patients with HBV-ACLF were retrospectively analyzed. Prothrombin time (PT, active coagulation), HBV DNA, and model for end-stage liver disease (MELD) score data from treatment weeks 4, 8, 24, and 48 were collected and analyzed using the statistical t-test. During the treatment duration, the complete virologic response rates were 57.5% (42/73) at 4 weeks, 71.0% (44/62) at 8 weeks, 83.1% (49/59) at 24 weeks, and 86.5% (45/52) at 48 weeks. The partial virologic response rates were 30.1% (22/73) at 4 weeks, 25.8% (16/62) at 8 weeks, 17.0% (10/59) at 24 weeks, and 13.5% (7/52) at 48 weeks. At week 48, the survival rate was 71.2% (52/73) and the probability of survival was higher in the complete virological response rate (VRR) group than in the partial VRR group [45/73 (61.6%) vs. 7/73 (30.1%), respectively; P = 0.000]. In addition, there were significant improvements in the serum normalization rate of HBV DNA, alanine aminotransferase, aspartate aminotransferase, albumin, total bilirubin, PT and MELD score in surviving patients compared to baseline (P less than 0.05) and in the complete VRR group compared to the partial VRR group (P less than 0.05). Antiviral therapy using lamivudine may be an effective therapeutic option for patients with HBV-ACLF.

9.Lack of association between three single nucleotide polymorphisms in the PARK9, PARK15, and BST1 genes and Parkinson's disease in the northern Han Chinese population.

BACKGROUNDParkinson's disease (PD) is an autosomally inherited neurodegenerative disease in elderly people. The etiology of PD has long been thought to be associated with both genetic and environmental factors. To explore potential genetic risk factors for PD in the northern Han Chinese population, we investigated three single nucleotide polymorphisms (SNPs) (rs4538475, rs11107 and rs12564040) in the BST1, PARK15 and PARK9 genes.
METHODSGenomic DNA from 215 PD patients and 212 matched controls was amplified in two independent PCR systems and subsequently genotyped by digestion with the endonuclease PstI. Genetic parameter and association studies were carried out with SPSS 13.0 and PLINK 1.07 software.
RESULTSWe could accurately detect all genotypes in the three loci with the PCR-RFLP or mismatched PCR-RFLP techniques. The observed heterozygosities of the rs4538475 and rs11107 loci in PD and control groups ranged from 0.460 - 0.481 and 0.410 - 0.441, in BST1, PARK15 respectively, while we detected no heterozygosity at the rs12564040 locus in PARK9. The similar distributions of genotypic frequency between both groups suggest that the three SNPs investigated in this study are unlikely to play roles as common risk factors or pathogenic mutations for PD in northern Han Chinese.
CONCLUSIONThe SNPs investigated in the BST1, PARK15 and PARK9 genes associated with PD susceptibility are not associated with PD in the northern Han Chinese population.

The accurate location of acupoints, comprehensive understanding of the theory and features of the eight-diagram are held as the prerequisites of the application of the therapy. The combination of the theories of five wheels, five zang organs, and mother and son relation among the five elements are taken as the theoretical foundation of the treatment. And clinical analyses on three cases treated with the theory are provided as well.
