2.Investigation of Microorganisms Contamination in Bathing Beach

Objective To know the present sanitary status of bathing beach in Haikou,and provide the scientific data for improving the assessment of microorganisms contamination of bathing beach. Methods The routine water quality data and related meteorological data of the bathing beach were collected from 2007 to 2009. In July 8th-10th 2009,a high grade bathing beach and an ordinary one were selected,four sampling sites were selected where the contamination by feces were often seen,the number of swimmers was the largest,the samples were collected from the deep-water areas and sand beach respectively,and fecal coliform and enterococcus were examined. Results The concentration of fecal coliform was(1 085.8?538.8)cfu/L in water and(120 000.0?32 659.9)cfu/1 000 g in sand,and the E.faecalis was(32.5?19.1)cfu/L in water and(2 425.0?689.8)cfu/1 000 g in sand. The numbers of the two kinds of bacteria in water decreased with the increase of distance from the land. The concentration of bacteria was positively correlated to the day's rainfall(P
3.Protective effects of acutobin on cerebral ischemia-reperfusion injury in rats

Aim To further study the protective effects of acutobin on focal cerebral ischemia -reperfusion injury in rats. Methods Reversible middle cerebral artery occlusion (MCAO) models were produced by intraluminal suture technique, and reperfusion was begun 3 hours after occlusion and lasted 24 h. The extent of neurological deficits was evaluated by Longa' method; The cerebrovascular morphology was observed by electron microscope. The infarct area of brain was measured by 2,3,5-triphenyltetrazolium chloride (TTC) staining technique;The 2~4 U?kg~(-1) doses of Acutobin were administrated i.v. at the beginning of ischemia or reperfusion respectively. Results ① After 3-h occlusion and 24 h reperfusion, the neurological syndromes and the infarct area were showed, the change of cerebrovascular morphology were appeared and the ratio of TXB_2/6-Keto-PGF_(1?) in plasma was increased. ② After different doses of acutobin were administrated at different time respectively, the neurological syndromes were alleviated; the infarct areas of brain were diminished; the hurt of cerebrovascular endothelial cell was lessened and the ratios of TXB_2/6-Keto-PGF_(1?) in plasma were reduced. Conclusion Protective effects of acutobin on cerebral ischemia-reperfusion injury in rats may be related to lessening cerebrovascular injury and balancing the ratio of TXB_2/6-Keto-PGF_(1?)
4.Relative factors of the compliance in schizophrenic out-patients

ObjectiveTo explore the relative factors of the compliance in schizophrenic out-patients.Methods118 schizophrenic out-patients chosen randomly were investigate with the compliance and assessed with Clinical Global Impression-Severity of Illness (CGI-SI), Social Disability Screening Schedule (SDSS), Brief Psychiatric Rating Scale (BPRS) and Treatment Emergent Symptom Scale (TESS) by telephone. Logistic regression analysis was used to confirm the relative factors of the compliance.Results56.8% of all patients were of full compliance. The risk factors of compliance in schizophrenic out-patients were the frequency of medicine taken, the scores of CGI-SI and the insight of the illness. ConclusionIt is important to choose effective and convenient antipsychotic drugs and improve the insight of the illness to enhance the compliance of the schizophrenic out-patients.
5.Application of role-play test in caring nursing service

Objective To make nurses understand the essential factors of caring nursing service and satisfy the requirement of in-hospital patients. Methods To establish the plan of role-play test(RPT).The roles of patients were played by nurses. They experienced the caring requirement of patients, true feeling, the state of physiology and psychology, etc. Then we discuss and analyze the collected information to consummate measures of caring nufing service. Results The RPT deeped the nurses' understanding about essential factors of caring nursing service. The nursing quality and patients' satisfaction degree were improved compared with those of before(P<0.05).The nursing complaints and service quality were also increased. Conclusion Application of RPT proved to be an effective way to deepen the caring nursing service.
6.NPM1 mutation in acute myeloid leukemia with normal karyotype:a clinical analysis

Objective To investigate the frequency of NPM1 mutation and its clinical significance in patients with cytogenetically normal acute myeloid leukemia (CN-AML). Methods The data of 190 patients with CN-AML were collected from Department of Hematology, Tongji Hospital between January 2008 and June 2015, and the discrepancies in clinical features and efficacy between CN-AML patients with NPM1 mutation and those without NPM1 mutation were also analyzed. Results Among the 190 CN-AML patients, NPM1 mutation was found in 44 patients (23.16 %). The proportion of bone marrow blast cells and the count of peripheral white blood cells in patients with NPM1 mutation were higher than those in patients without NPM1 mutation (75.82 % vs. 63.87 % , P <0.05; 75.7 ×109/L vs. 60.0 ×109/L, P <0.05). The rate of response (complete remission + partial remission) in patients with NPM1 mutation was also higher than that in patients without NPM1 mutation [70.09 %(22/31) vs. 56.91 %(45/79), P<0.05) ]. Conclusion NPM1 mutation is associated with higher tumor burden and higher remission rate in CN-AML patients.
7.Receptor for advanced glycation end-products may mediate the upregulation of hypoxia-induced early growth response-1 in mouse aorta

Objective To investigate the impact of hypoxia on the expression of early growth response-1 (Egr-1) and monocyte chemoattractant protein-1 (MCP-1) in mouse aorta,and to probe the underlying mechanism involving receptor for advanced glycation end-products (RAGE).Methods 3-month-old C57BL/6 mice were subjected to hypoxia [(6.0±0.5) % oxygen] to establish the global hypoxia model(n=6 rats for each).Aortas were dissected,Egr-1 mRNA and MCP-1 mRNA were detected by real time RT-PCR,Egr-1 and RAGE proteins were tested by Western blot,and Egr-1 DNA binding activity was assayed by electrophoretic mobility shift assay (EMSA).For blockade of RAGE,mice were pretreated with soluble RAGE (sRAGE) for 1 h by intra-peritoneal injection before they were exposed to hypoxia.Mice with normoxia were used as controls.Results After 30 minutes of hypoxic exposure,Egr-1 mRNA in aorta was increased to (28.3±0.9)folds compared with normoxic controls (F=617.17,P<0.01),and the induction persisted for at least 3 hours.After 45 minutes of hypoxic exposure,Egr-1 proteins in aorta was increased to (5.7 ± 0.3) folds compared with normoxic controls (F =57.18,P< 0.01); the enhanced DNA binding activity of Egr-1 by hypoxia was attenuated by pretreatment with anti-Egr-1 lgG.After 4 hours of hypoxic exposure,MCP-1 mRNA expression in aorta was increased to(4.0±0.3)folds compared with normoxic controls (F=30.68,P<0.01).RAGE antigen was increased significantly within 30 minutes of hypoxic exposure,with the peak at 15 minutes; hypoxia-induced Egr-1 mRNA expression was significantly attenuated by pretreatment with sRAGE (3.3 ± 0.2) folds compared with normoxic controls (F =30.20,P<0.01).Conclusions Hypoxia significantly induces Egr-1 and MCP-1 upregulation expressions in mouse aorta,and blockade of RAGE significantly attenuates hypoxia-induced Egr-1 expression.Thcsc findings suggest RAGE signaling is involved in hypoxia-induced vascular inflammatory stress,and highlight this receptor as a potential therapeutic target to protect tissues injured by hypoxia.
8.Evaluation of early groin pain after total hip arthroplasty

Objective To discuss causes and corresponding prevention for groin pain occurred early after total hip arthroplasty(THA).Methods A retrospective study was done on 189 cases(193 hips)treated with THA including unilateral procedures in 185 hips and bilateral procedures in eight hips to analyze common causes for early groin pain.Results Groin pain was found in 9.3% hips(18/ 193)during hospital stay,including 1.6%(3 cases)with deep infection,1.6%(3 cases)with incision infection,1%(2 cases)with posterior dislocation,4.1%(8 cases)with leg lengthening and 1%(2 cases)with hematoma.Conclusions Despite of the numerous diagnostic alternatives available to the orthopedic surgeon,detailed history,careful physical examination,necessary laboratory and imaging stud- ies can contribute to a correct determination of causes for groin pain.Meanwhile,appropriate indication, accurate preoperative radiographic measurement,intraoperative standardized surgical procedures and per- fect rehabilitation are necessary to avoid complications.
9.Assessment of a Bathing Beach Environment and Human Health Effect

Objective To investigate the environmental quality and water quality of bathing beach in Haikou,Hainan province and the effects on human health.Methods The investigation of the bathing beach environment and the water quality were conducted according to Bathing Beach Monitoring Regulations(2002).The water samples were collected at PM 3:00-5.00 on May 28th to June 3th in 2007 from three sampling sites,No.1(easily be polluted),No.2(crowded),No.3(less pollution).The questionnaire survey was conducted on the present day and 7 days later,the items included the respiratory,digestive systems infection and eye,ear,nose and skin inflammation,the survey was completed in two days.Results The environment survey of bathing beach suggested that non-point source pollution caused by domestic wastewater emerged on raining days,the salinity was 31‰-33‰,clarity exceeded 30 centimeter and no heat pollution was found.The pH value,chroma,dissolved oxygen,nitrate nitrogen,chemical oxygen demand,inorganic nitrogen,fecal coli group was 8.01-8.10,11.42-15.00 NTU,5.60-6.71 mg/L,0.16- 0.17mg/L,2.70-3.40mg/L,0.19-0.21 mg/L and 35.0-36.0/L respectively.Four hundred and eight questionnaires were eligible, the response rate was over 90%.The results demonstrated that the swimmers were facing to the increased illness risk from the water quality,such as skin pruritus,gastrointestinal allergies,throat sore,eye and ear and nose infections.There was distinct higher proportion in the participants with water contact and complained one or more symptoms than those without water contact(P
10.Adjustment of positive end-expiratory pressure during presence of tonic diaphragm electrical activity in rabbits with acute respiratory distress syndrome

Objective To study the positive end-expiratory pressure (PEEP) adjustment after recruitment maneuver during acute respiratory distress syndrome (ARDS) especially in the presence of tonic diaphragm electrical activity (Tonic EAdi) in order to obtain optimum PEEP and in turn to get successful oxygenation.Methods Rabbit model of ARDS was nade by intratracheal instillation of hydrochloric acid.After sufficient recruitment maneuvers,the rabbits were randomly (random number) divided into two groups,namely Tonic EAdi group ( n =5 ) and maximum oxygenation group ( n =5 ).In Tonic EAdi group,the regulation of PEEP was guided by Tonic EAdi.In maximum oxygenation group,PEEP was adjusted as per maximum oxygenation.The differences in magnitude of PEEP,pulmonary mechanics,gas exchange and hemodynamics were compared between two groups.The t-test was used to compare continuous variables between the two independent samples,and the difference was statistically significant when P < 0.05.Results (1) PEEP:The PEEP was (10.7 + 1.4) cmH2O (1 cm H2O=0.098 kPa) in Tonic EAdi group and (10.0 ± 2.8) cm H2O in maximum oxygenation group (P > 0.05). (2) Pulmonary mechanics:After PEEP adjustment,there was no significant difference in tidal volume ( Vr),peak pressure (Ppeak) and mean pressure (Pmean) between the two groups (P > 0.05 ).(3) Gas exchange:After PEEP adjustment,there was no significant difference in oxygenation index (PaO2/FiO2) and partial pressure of arterial carbon dioxide ( PaCO2 ) between the two groups (P > 0.05).Conclusions Tonic EAdi could be a good indicator for regulating PEEP in ARDS.
