Objective To investigate the effect of tripterygium glycosides(TG) combined with tanshinone Ⅱ A(Tan Ⅱ A) on the level of tumor necrosis factor-α(TNF-α) ,yon willebrand factor(vWF) in serum in adjuvant ar-thritis(AA) mice. Methods Animal model of adjuvant arthritis was indwecd,after 3 weeks administration the mice were kiued and get serum. TNF-α, vWF were assayed by ELISA. Results TNF-α: model group and Tan Ⅱ A group are significantly higher than control group,TG + TanⅡ A group are lower than the other groups,there is no significant difference between control group, MTX group,TG group; vWF: model group have the highest vWF level, has significant difference between control group ,MTX group, TG group, TG + Tan Ⅱ A group and Tan Ⅱ A group, TG + Tan Ⅱ A group are lower than the other group. Conclusion TG + Tan Ⅱ A can improve cardiovascular damage of rheumatoid arthritis by means of inhibiting inflammatory factor expression.

Objective To explore the mechanism of apeptosis in rheumatoid arthritis synoviocyte induced by dihydroartemisinin. Methods Synovial tissues were cut from rheumatoid arthritis patients when who was under knee prosthesis. Apoptosis was detected with flow cytometry. Western blot was performed to assess ser473-phosphorylated Akt. EMSA (electrophoretic mobility shift assay) was used to ana-lyze NF-κB activation. Results Dihydroartemisin can induce apoptosis in rheumatoid arthritis synoviocyte in a dose-dependent manor from 2.5μmol/L to 10μmol/L. Rheumatoid Arthritis synoviocyte cultured with dihydroartemisinin in 5μmol/L or 10μmoL/L can significantly in-hibit serine 473 phosphorylation in Akt and activation of NF-κB. Conclusion Dihydroartemisinin can induce apoptosis in rheumatoid arthri-tis synoviocyte through Akt signal pathway.

Objective To investigate the prevention and management of portal vein thrombosis following orthotopic liver transplantation.Methods Between May 1995 to September 2005,clinical data of 137 cases subject to orthotopic liver transplantation were analyzed.Among them,there were 10 cases of portal vein thrombosis(5 cases in gradeⅠ,4 cases in gradeⅡand 1 case in gradeⅢ). All patients received an eversion thromboendovenectomy(ETEV)with occlusion of the portal flow u- sing a Forgarty balloon.Ligation of the collateral circulation,especially spontaneous or surgical sple- norenal shunt,was made as approaches to improve portal flow.Heparin or low-molecule-weight hepa- rin as a prophylactic anticoagulation therapy was maintained during and after operation if prothrombin time was less than 18 s.Follow-up Doppler ultrasonography was used daily in the early postoperative period.Results After a follow-up of 2～66 months,overall incidence of portal vein thrombosis was 2.92%(4/137).Surgical thrombectomy and revascularization was carried out in 1 case.Thromboly- sis,balloon angioplasty and stent placement via hepatic artery were performed on 2 cases.No treat- ment was given in 1 patient without hepatic dysfunction and portal hypertention.Mortality related to portal vein thrombosis was 0.Conclusions Portal vein thrombosis might be avoided by performing a complete thrombectomy as often as possible,by ligation of portosystemic shunt during surgery,and by postoperative anticoagulation.Close follow-up by Doppler ultrasonography may make a prompt di- agnosis and reduce portal vein thrombosis-derived loss of grafts.

Objective To observe of the effects of tacrolimus on blood glucose,insulin secretion and the expression of phosphorylated AKT in rats in order to study the mechanism of diabetogenic effects of tacrolimus.Methods 40 male SD rats were randomly divided into two groups.The rats in tacrolimus group were delivered tacrolimus at a dose of 4mg/kg· d.The rats in the control group were given the same amount of saline solution in the same way.The body weights,fasting blood glucose levels and blood concentrations of tacrolimus were measured monthly.After 5 months,all rats were killed.Pancreas and liver tissue were stored in 4％ paraformaldehyde solution.Serum insulin levels were detected by radioimmunoassay method.The expression of phosphorylated AKT in liver were measured by immunohisto-chemical method.Results ①The body weights in tacrolimus group in the 3rd,4th,and 5th month were significantly lower than those in the control group (P＜0.01).②The blood glucose levels in tacrolimus group in the 3rd,4th,and 5th month were significantly higher than those in the control group (P＜0.05).③The insulin secretion and insulin sensitivity index in tacrolimus group were significantly lower than those in the control group (P＜0.01).④The rats in tacrolimus group showed varying degrees of damage in pancreatic duct and pancreatic islet cells.⑤The expression of phosphorylated AKT in liver cells in tacrolimus group were significantly lower than those in the control group (P＜0.05).Conclusions Tacrolimus can induce pancreatic islet cells necrosis,decrease the number of islet cells,reduce insulin secretion and insulin sensitivity,which lead to blood hyperglycemia in rats.In addition,we also find that tacrolimus can reduce expression of phosphorylated AKT in hepatic tissue,which indicates that tacrolimus results insulin resistance through interfering PI3K/ AKT signal transduction pathways.

Objective To make nurses understand the essential factors of caring nursing service and satisfy the requirement of in-hospital patients. Methods To establish the plan of role-play test(RPT).The roles of patients were played by nurses. They experienced the caring requirement of patients, true feeling, the state of physiology and psychology, etc. Then we discuss and analyze the collected information to consummate measures of caring nufing service. Results The RPT deeped the nurses' understanding about essential factors of caring nursing service. The nursing quality and patients' satisfaction degree were improved compared with those of before(P＜0.05).The nursing complaints and service quality were also increased. Conclusion Application of RPT proved to be an effective way to deepen the caring nursing service.

BACKGROUND:The key of vertebroplasty and percutaneous kyphoplasty to success is whether the puncture needle can accurately reach the vertebral body through pedicle. Therefore, it is important to identify the correct point and direction of needling in the X-ray fluoroscopy. Among many methods published in present reports, the puncture point and the puncture angle are not fixed. Few reports concerned whether the puncture needle perforated pedicle medial wal . OBJECTIVE:To seek safe, effective puncture point and the puncture angle of percutaneous pedicle from the perspective of anatomy and radiography. METHODS:The best entry point during percutaneous vertebroplasty in the X-ray fluoroscopy:dissection was performed on thoracic, lumbar skeletal samples (T 6-L 5 ) to find the position of pedicle axis leading to the rear of the vertebral body, and this position is the best entry point of percutaneous vertebroplasty. It was fixed with mini-screw. The relationship of the best entry point and pedicle developing position in the X-ray fluoroscopy was analyzed to find the best entry point in the X-ray fluoroscopy. The best entry angle during percutaneous vertebroplasty:The average included angle of pedicle axis and vertebral sagittal line was measured using autopsy and CT scanning on adult thoracic and lumbar skeletal samples (T 6-L 5 ). The best entry angle during percutaneous vertebroplasty was found. RESULTS AND CONCLUSION:During percutaneous vertebroplasty, the best entry point in the X-ray fluoroscopy was the left pedicle projection 9 area and right pedicle projection 3 area. The optimal needle angle during percutaneous vertebroplasty:5°-10° in lumbar vertebra L1-L4;20° in L5, not more than 25°;about 5° in thoracic vertebra T6-T12 .

Objective:To evaluate the effect of different polishing instruments on the microstructure of composite resin.Methods:Specimens of 5 kinds of resins were randomly divided into three groups and were polished with Astropol(group A),Sof-Lex(group B) and Super-snap(group C)respectively(n=3 for each resin).Specimens of natural enamel surface were used as the controls(group D).The surface roughness(Ra)and microstructure of the specimens were evaluated by an atomic force microscope(AFM).Results:Ra value in group A was lower than that in group B(P=0.015);Z350 XT resin obtained the lowest Ra value among the 5 kinds of resin specimens(P<0.05).All of the resin specimens had similar average Ra after polishing procedure(P>0.05).Conclusion:Astropol polishing instrument is better than Sof-Lex;Z350 XT has better polish performance.The three polishing instruments are ef-fective in polishing the 5 composite resins.
