1.Research on the clinical efficacy and safety of atropine with short covering for amblyopia in children

Abstract?AIM: To discuss clinical efficacy and safety of 10g/L atropine with short covering for children with amblyopia.?METHODS: Eighty -eight children ( 88 eyes ) with amblyopia, staying in hospital from February 2011 to February 2014 for treatment, were divided into control group ( n =44 ) and observation group ( n =44 ) . The control group only given short covering therapy was observed.Observation group was given 10g/L atropine treatment besides covering.Clinical efficacy, treatment compliance, visual acuity, corrected spherical degree of amblyopia eye and adverse events were observed and compared.?RESULTS:1) After treatment, total effective rate of the observation group was 95% ( 42/44 ) , significantly higher than that of control group ( 80%, 35/44, P<0.05 ); 2 ) excellent compliance rate of the observation group was 95% ( 42/44 ) , significantly higher than that of control group (82%, 36/44, P<0.05);3) visual acuity of the two groups when the disease was first diagnosed was not significantly different (P>0.05), but increased number of lines of vision and corrected spherical degree of amblyopia eye in the observation group were significantly higher (P<0.05);4) in the observation group total rate of adverse events was 9% ( 4/44 ) , significantly lower than that in the control group (23%, 10/44, P<0.05).?CONCLUSION: The combined therapy, 1% atropine with short covering, is effective and safe for amblyopia in children.
2.Effects of electroacupuncture preemptive intervention on postoperative pain of mixed hemorrhoids.

OBJECTIVETo evaluate clinical efficacy of electroacupuncture at Changqiang (GV 1) and Chengshan (BL 57) 30 min before surgery on postoperative pain and discomforts in patients with mixed hemorrhoids.
METHODSOne hundred and twenty cases of mixed hemorrhoids who received Milligan-Morgan operation were randomly divided into an electroacupuncture group, a sham electroacupuncture group and a blank group, 40 cases in each one. At the same time of basic treatment, the electroacupuncture was applied at Changqiang (GV 1) and Chengshan (BL 57) 30 min before surgery in the electroacupuncture group, while shallow needling without electrical stimulation at sham acupoint (1 cm next to acupoint) was applied 30 min before surgery in the sham electroacupuncture group, while no treatment was given before the surgery in the blank group. The total dose of painkillers in the first 24 h after operation, the number of cases who received additional anesthetic in the operation, the self-score of most severity pain in the first 24 h after operation and sleeping time in the night of surgery were observed.
RESULTSThe differences of the numbers of cases who received additional anesthetic in the operation had no statistical significance among the three groups (all P > 0.05), but compared with the sham electroacupuncture group and blank group, the total dose of painkillers in the first 24 h after operation was reduced in the electroacupuncture group [(2.43 +/- 1.08) tablets vs (3.23 +/- 1.33) tablets, (3.10 +/- 1.22) tablets], and the score of most severity pain was also decreased (6.65 +/- 1.00 vs 7.48 +/- 0.96, 7.25 +/- 1.19), besides, the sleeping time in the night of surgery was increased [(220.63 +/- 85.50) min vs (162.00 +/- 92.69) min, (151.50 +/- 80.01) min, all P < 0.05].
CONCLUSIONThe electroacupuncture at Changqiang (GV 1) and Chengshan (BL 57) 30 min before surgery has effects of preemptive analgesia on postoperative pain for patients with mixed hemorrhoids.

Objective To evaluate X-ray,CT and MRI findings of soft tissue infections in AIDS. Methods Three cases of soft tissue infections with AIDS were retrospectively analyzed by comparing the imaging findings with pathological results.All patients were performed MRI,X-ray was in 1 case,CT was in 1 case.Results Cellulitis was in 1 case:MRI showed extended thickening of subcutaneous tissues, ill-defined hypointense areas on T_1WI and hyperintensity on T_2WI,and reticular pattern on GRE. Necrotizing fasciitis was in 1 case:MRI showed obvious thickening of subcutaneous tissues and deep fasciae, abnormally increased signal intensity on T_1 and T_2WI.Fluid collections were within muscles and muscles interval on fat-suppressed T2 WI.Tuberculosis was in 1 case:CT demonstrated multiple low density areas in the subcutaneous tissues and clear peripheral rim enhancement.MRI appeared hypointense on T_1WI and hyperintensity on T_2WI,and peripheral rim enhancement following gadolinium injection.Conclusion Infections of soft tissue are common complication in patients with AIDS,radiology is important in early diagnosis and treatment planning in this population.
5.Transcaliosum interfornix approach to microsurgically remove the tumors of the third ventricle

Objevtive To explore the microsurgical methods for resecting the tumors of the third ven- tricle and discuss the anatomic foundation of mircrosurgical methods.Methods Twelve patients with tumors of third ventricle were operated on via approach of transcallosum interfornix.Pathologic diagnoses included eight craniopharyngiomas intruding the third ventricle,two gliomas in medial thalamus,one ependymoma and one teratoma.Results Total removal in microscope of the tumors were achieved in 10 cases and subtotal re- moval in 2 cases.The percentage of total resections achieved to 83.3%.Seven of eight craniopharyngiomas were totally resected and the postoperative MR indicated no remanent or recurrent tumors.No death occurred in all cases and the recent complications which were disappeared after one or two weeks' management mostly included polydipsia,diuresis and electrolyte disturbances.Long termed complications included two hydren- cephalus and two spontaneously absorbed subdural fluidifies ventricle-abodminal shunt was performed in one case.The time of follow-up continued were between two months to two and a half years.One subtotal resected craniopharyngiomas was relapesed in eight months,one glioma in thalamus was relapesed in one year and in the remanent ten cases nine can live normally and one can live with self-care.Conclusion Transcallosum interfornix approach for microsurgically removing the third ventricle tumors reached the third ventricle through the rudimental tissue space of embryonic tissue.It can provide a slight trauma,a quite large operative field, euthyhoria for operating.We can use this appoach to resect the tumors located in all directions in the third ven- tricle.The percentage of total resection was large and few complications occurred.
6.Detection of TERC gene amplification by fluorescence in-situ hybridization in cervical intraepithelial lesions.

OBJECTIVETo explore the feasibility and practical value of fluorescence in situ hybridization (FISH) detection of TERC gene amplification in cervical intraepithelial lesions (CIN) and squamous cell carcinoma (SCC).
METHODSTissue microarray was constructed to cover 150 cases of various cervical conditions, including 24 cases of normal cervical mucosa, 78 cases of CINs (CINI, 25 cases; CINII, 21 cases and CINIII, 32 cases) and 48 cases of SCC. FISH was used to detect TERC gene amplification.
RESULTSTERC gene amplification was detected in 8% (2/25) CINI, 47.6% (10/21) CINII, 71.9% (23/32) CINIII and 87.5% (42/48) SCC. There were significant differences among these groups (P < 0.05). The amplification rates of TERC gene in SCC, CINIII and CINII were significantly higher than those of normal cervical epithelium and CINI (P < 0.05). Significant differences were also observed among CINI and CINII, CINIII and SCC (P < 0.05), and between CINII and SCC (P < 0.05). There were no significant differences between normal cervical epithelium and CINI, CINII and CIN III, and between CINIII and SCC (P > 0.05). FISH detection of amplification of TERC gene in CINI and CINII-III demonstrated the following statistics: sensitivity of 62.3%, specificity of 92.0%, accuracy of 71.8%, positive and negative predictive values of 94.3% and 53.5%, respectively.
CONCLUSIONSFISH detection is a reliable method in detecting TERC gene amplification using paraffin tissue sections. When histological evaluation becomes difficult, TERC amplification detectable by FISH may offer a diagnostic distinction of CINI from CINII. Moreover, TERC amplification may be used as a biomarker in predicting CIN progression to invasive cancer.

7.Consciousness and speech evaluation of 96 patients with craniocerebral injury.

OBJECTIVETo explore the effective methods for evaluating the consciousness and speech status of patients with special types of cerebral injuries.
METHODSA total of 96 patients with injury in the language center and in coma were treated with operative and correlated conventional therapies. Then their recovery status of consciousness and speech was observed.
RESULTSAll the patients were recovered to consciousness. Sixty-nine patients with aphasia were cured completely, but 7 patients were complicated with incomplete ataxic aphasia, 15 with incomplete sensory aphasia, and 5 with incomplete mixed aphasia.
CONCLUSIONSFor the patients with injury in the language center, evaluation of the conscious state with GCS scoring system has certain limitations and conscious behaviours are advantageous evidences to evaluate the consciousness recovery of the patients. The patients with conscious disturbance and injury in the language center should be considered to have aphasia.

8.Ulinastatin attenuates lung injury in rats with hemorrhagic shock.

OBJECTIVETo investigate the effects of ulinastatin on lung injury in hemorrhagic shock rats.
METHODSTwenty-four normal SD rats were randomly divided into 3 groups (n=8), namely the control group, hemorrhagic shock group (group H) and ulinastatin group (group U). In group H and group U, blood was drawn from the femoral artery over a period of 10 min until a mean arterial pressure of 40 mmHg was obtained. Controlled hypotension was then maintained at 40-/+5 mmHg for 60 min by blood drawing or infusion when necessary. All the blood drawn and an equivalent volume of Ringer lactate solution were subsequently infused for resuscitation. Four hours after the resuscitation, the activity of superoxidedismutase (SOD), content of malondialdehyde (MDA), expression of heme oxygenase-1 (HO-1), wet to dry weight ratio (W/D), and pathologic changes of the lung tissues were measured or observed.
RESULTSCompared with those in the control group, the content of MDA, expression of HO-1 and W/D increased significantly in both group H and group U (P<0.05); these indexes in group U were significantly lower than those in group H (P<0.05). The activity of SOD in group U was significantly lower than that in the control group (P<0.05) but higher than that in group H (P<0.05). Optical microscopy demonstrated milder inflammatory cell infiltration and interstitial edema in the lung tissues in group U than in group H.
CONCLUSIONUlinastatin can lower the content of MDA, W/D and the expression of HO-1, increase the activity of SOD, and reduce histological lung injury in rats with hemorrhagic shock.

9.Expression of recombinant human IFNa-2b/IgG4 Fc fusion protein in a baculovirus insect cell system.

To investigate a baculovirus insect cell system for expressing an interferon alpha 2b (IFNa2b)/immunoglobulin G-4 (IgG4) Fc fusion protein, which has long-acting antiviral effects. Human IFNa2b and IgG4 Fc cDNAs were generated by molecular cloning and inserted into a baculovirus shuttle vector, which was then transposed into the DH10 Bac strain to form recombinant Bacmid-IFN/Fc. The Bacmid-IFN/Fc was transfected into High five insect cells, and expression of the IFN/Fc fusion protein was detected by Western blotting and its biological activity was assessed by the cytopathic effect inhibition method. The IFNa2b and IgG4 Fc cDNA fragments were successfully amplified by RT-PCR using human peripheral lymphocytes. After cloning into the baculovirus shuttle vector, pFastBac1, and transforming into DH10 Bac competent cells, screening identified positive clones carrying the recombinant Bacmid-IFN/Fc. A Bacmid-IFN/Fc clone was successfully transfected into the High five insect cells and packaged into the baculovirus for expression of the IFN/Fc fusion protein. Western blotting revealed that the fusion protein expression was specific, and yielded a protein of 45 kD in size. The in vitro antiviral activity of the IFN/Fc fusion protein was 580 IU/mL. A novel IFN/Fc fusion protein was successfully generated using a baculovirus insect cell system, which may prove useful for providing future experimental data for development of a new long-acting interferon to treat chronic viral hepatitis.

10.Operative treatment of collapse separation fractures of lateral tibial condyle.

OBJECTIVETo investigate the treatment method for collapse separation fractures of lateral tibial condyle.
METHODSFrom January 2003 to October 2008, 45 patients with collapse separation fractures of lateral tibial condyle were treated by surgical method, including 27 males and 18 females,ranging in age from 15 to 76 years,with an average of 37 years. The duration from injury to operation ranged from 1 to 14 days. All of the patients preoperatively underwent X-ray examinations in order to accurately understand the extent of fracture and fracture collapse. All of the 45 fractures with collapse more than 2 mm and widen more than 5 mm were selected for surgical treatment. The lateral surgical approach was used to expose the lateral margin of the lateral meniscus first, and then the reduction of the lateral meniscus and lateral condyle were achieved. The results after operation were evaluated by the Rasmussen criteria, as well as the complications of the knee joint pain, the joint activity, the infection, deep venous thrombosis and recurrent fracture collapse.
RESULTSPostoperative X-ray showed anatomical reduction was achieved in 31 patients, 14 patients got nearly anatomical reduction. The plate and screw position were satisfactory. Forty-two patients were followed up,and the duration ranged from 6 to 12 months, with an average of 10 months. Fracture re-displacement,postoperative infection and deep venous thrombosis did not occur after the lifting of external fixation. All the patients had pain disappeared about 10 days after operation,and had no vertical axis percussion tenderness at 3 weeks after operation. The X-ray showed fractures was healed in all patients at 3 months after operation. Joint activity Rasmussen assessment criteria results: 37 patients got an excellent result, 7 good, 1 poor.
CONCLUSIONThe incarceration of lateral meniscus and the collapsed bone fragments are two factors which make the reduction of the lateral tibial condyle difficult.
