1.Analysis of poisoning cases from Chinese National Injury Surveillance System, 2006-2008

Objective To study the preventive strategies through analyzing the poisoning cases from the National Injury Surveillance System (NISS), from 2006 to 2008. Methods Data of poisoning cases was descriptively analyzed from Chinese NISS, from 2006 to 2008. Results The proportion of poisoning cases to all injuries cases from NISS were 2.57%, 2.48% and 2.52% from 2006 to 2008, which ranked sixth in all the injuries causes. Most people being poisoned had junior middle school education and most of them were agriculture/animal husbandry/fishery/water producers or commercial/service personnel. Most of the poisoning incidents were happened at home, always occurred in leisure time - around 8 PM, every day. The common types of poisoning were alcohol,clinical drugs, pesticide and carbon monoxide. Unintentional injuries were the main causes. Self-harm/suicidal cases in the rural areas were more than in the urban areas, with women more than men. The main type of self-harm/suicide related poisoning cases were through drugs or pesticide. ≥65, 15-29and 30-44 year-olds were most commonly seen. Conclusion Alcoholism was the primary type of poisoning injuries which is the highest in young adults ( 15-29 years and 30-44 years). It's important to promote civilized drinking habits and limit access to alcohol for youth. Self-harm/suicide had close relationship with clinical drugs and pesticide. The key points to prevent pesticide and clinical drugs poisoning were safe storage of pesticides, universal security of pesticide, and the supervision on drug producing and marketing. Children and the elderly were the high risk people for carbon monoxide poisoning. Monitoring and intervention must be strengthened.
2.The role of AT1 receptor in the changes in TH immunoreactivity of LC induced by brain cholinergic stimulation.

AIM AND METHODSIn the present study, we investigated the TH immunoreactivity and the expression of angiotensin AT1 receptor in locus coeruleus after intracerebroventricular (i. c. v.) injection of carbachol in conscious SD rats with immunohistochemistry. Meanwhile the effects of blocking AT1 receptor were also observed.
RESULTSBoth mean optical density and number of TH and AT1 immunoreactive positive neurons were markedly increased in locus coeruleus after 40 minutes of i.c.v. injection of carbachol (0.5 microg). The enhancement was significantly reduced by i. c. v. injection of losartan.
CONCLUSIONThe results above suggest that i. c. v. injection of cholinergic agonist carbachol can enhance the activity of adrenergic neurons and the expression of AT1 receptor in locus coeruleus. The blockade of AT1 receptor may down regulate the above action induced by carbachol in locus coeruleus.

3.Tumor conditioned medium regulates the proliferation, adhesion and migration of human umbilical vein endothelial cells.

The aim of the present study was to investigate the effects of tumor conditioned medium (TCM) on the proliferation, adhesion and migration of human umbilical vein endothelial cell (HUVEC). Cells were divided into control group, TCM stock solution group, TCM monodilution group and TCM didilution group (n = 6), and MTT assay was used to determine endothelial cells proliferation level after being stimulated with TCM for 24 h. Coating culture dishes with Fn was used to detect the adhesion ability of endothelial cells to basement membrane in control group and TCM stock solution group (n = 6). Wound closure model was used to test the migration ability of endothelial cells in control group and TCM stock solution group at 12 h and 24 h (n = 6). The results showed that TCM at different concentrations influenced proliferation of endothelial cells in distinct degree. Compared to the control group (absorbance value: 0.58 ± 0.04), the stock solution had no effect on proliferation of endothelial cells (absorbance value: 0.55 ± 0.01). However, the TCM monodilution and TCM didilution enhanced the proliferation of endothelial cells significantly (absorbance value: 0.66 ± 0.03, P < 0.01 and 0.70 ± 0.02, P < 0.001 separately). TCM downregulated the adhesion of endothelial cells to basement membrane. Moreover, compared to the migration distance of control group at 12 h and 24 h [(14.05 ± 6.25) µm and (48.75 ± 16.37) µm separately], TCM upregulated the basement membrane migration [(68.25 ± 26.20) µm and (119.70 ± 34.90) µm at 12 h and 24 h, both P < 0.05 separately]. These results suggest that TCM downregulates the adhesion ability of endothelial cells and upregulates their migration ability, and thus facilitates tumor metastasis.

4.Application of iterative model reconstruction iterative reconstruction in cardiac CT imaging--an animal experimental study

Objective To evaluate the value of iterative modal reconstruction (IMR) for reducing radiation dose and controlling image quality in cardiac CT. Methods Ten pigs were included. All pigs were scanned on a 256?slice prospectively ECG?gated cardiac CT utilizing routine dose (group A) and tube current reduced by 30%(group B), 50%(group C) and 70%(group D), respectively. Filtered back projection (FBP), hybrid iterative reconstruction (iDose4) and IMR were used for all data, respectively. Image noise and contrast?to?noise ratio (CNR) of ascending aortic root were measured, while overall image quality and coronary artery image quality was rated (five point scale). All results reconstructed by FBP, iDose 4 and IMR were compared. Objective measurements were compared with one?way analysis of variance, and subjective assessments were compared with Kruskal?Wallis H test andχ2 test. Results Compared with that of FBP and iDose4, image noise of IMR was(15.1 ± 6.1),(18.8 ± 5.5),(22.1 ± 4.8)and(33.0 ± 4.0)HU, respectively in group A, B, C and D with significant reduction (F=82.77, 90.71, 96.59, 95.51 respectively, all P<0.01). Using IMR, groups A, B, C, D had higher CNR (42.0±11.1, 37.2±10.4, 31.4±8.7, 23.7±7.0;F=50.65, 53.55, 76.60, 57.36, all P<0.01) and overall image quality (5.0 ± 0.0, 4.8 ± 0.4, 4.6 ± 0.5, 4.5 ± 0.5;H=20.96, 15.63, 18.66, 23.56, all P<0.01) than FBP and iDose4. Using IMR, group A (100%, 40/40) and group B (100%, 40/40) had no significant difference (P>0.05) in the diagnosis rates of proximal coronary arteries compared with that using FBP and iDose4, while group C (100%, 40/40) and group D(92%, 37/40) had significantly increased diagnosis rates (χ2=20.05, 45.72, both P<0.01). The diagnosis rates of distal coronary arteries of IMR reconstruction which were 100%(50/50), 98%(49/50), 90%(45/50), 78%(39/50), respectively in groups A, B, C, D had significant increase compared with that of FBP and iDose4 reconstruction (χ2=7.39, 16.75, 34.62, 81.33, all P<0.05). Conclusions IMR can significantly reduce image noise, improve CNR and image quality compared with iDose4. Application of IMR can reduce radiation dose but without compromising image quality.
5.Assessment of longitudinal segmental myocardial strain and displacement of left ventricle in patients with dual chamber pacing by velocity vector imaging

Objective To assess longitudinal segmental myocardial strain and displacement of left ventricle (LV) in patients with dual chamber pacing (DDD) using velocity vector imaging (VVI) and to explore the clinical application value of VVI in assessing left ventricular motion.Methods A total of 23 patients before and after DDD pacemaker implantation were enrolled in this study.The peak systolic strain and displacement of ventricular segments were measured with VVI and compared before and after pacemaker implantation.Results Compared to the values at the baseline,the strain and displacement of lateral,interventricular septum,anterior and posterior wall of LV in patients with pacing were significantly decreased.The strain,displacement of basal and mid segments with pacing were decreased as well(P<0.01).The left ventricular strain and displacement in patients with pacing were significantly decreased than those of patients at the baseline(P<0.05).Left ventricular longitudinal displacement before and after DDD pacing was significantly decreased from basal,mid to apical segments.But no significant difference was found in longitudinal strain from basal,mid to apical segments of LV.Conclusions (1) Left ventricular strain and displacement in patients with pacing were different between those before and after DDD pacing.(2) VVI can accurately assess left ventricular strain and displacement in patients with DDD pacing,and can become a powerful mean to assess the regional myocardial function.

OBJECTIVE To investigate the risk factors,clinical characteristics and resistance of Staphylococcus aureus nosocomial infections so as to guide the treatment of S.aureus infection.METHODS To collect clinical materials of S.aureus nosocomial infection and analyze risk factors and clinical characteristics and detect sensitivity of isolated strains to antibacterial agents.RESULTS Severe underlying diseases existed among 73 cases of S.aureus nosocomial infections,82.19 percent of patients had received invasive interventions.Lower respiratory tract was the most common infective site.Seventy-nine strains of S.aureus were isolated,including 66 meticillin-resistant S.aureus(MRSA) and 13 meticillin-sensitive S.aureus(MSSA).S.aureus showed general resistance to many kinds of antibiotic drugs.The resistant rates of MRSA were much higher than those of MSSA(P
7.Neuropeptide Y promotes TGF-beta1 production in RAW264.7 cells by activating PI3K pathway via Y1 receptor.

OBJECTIVETo examine the effect of neuropeptide Y (NPY) on TGF-beta1 production in RAW264.7 macrophages.
METHODSEnzyme linked immunosorbent assay (ELISA) was used to detect TGF-beta1 production. Cell counting kit 8 (CCK-8) was used to assay the viability of RAW264.7 cells. Western blot was used to detect the phosphorylation of PI3K p85.
RESULTSNPY treatment could promote TGF-beta1 production and rapid phosphorylation of PI3K p85 in RAW264.7 cells via Y1 receptor. The elevated TGF-beta1 production induced by NPY could be abolished by wortmannin pretreatment.
CONCLUSIONNPY may elicit TGF-beta1 production in RAW264.7 cells via Y1 receptor, and the activated PI3K pathway may account for this effect.

8.Analysis of curative effect of discectomy for lumbar intervertebral disc herniation.

OBJECTIVETo evaluate the short and long term results of discectomy for lumbar intervertebral disc herniation.
METHODSFrom 2000 to 2007, 400 patients (male 220 and female 180, the age was from 16 to 73 years old with an average of 42.3 years) with lumbar intervertebral disc herniation underwent discectomy by posterior mini-incision less than 5 cm and vertebrae plate was ectomized in 2 cm x 2 cm winder,and nerve root was compressied. The short and long term clinical result were analyzed with SPSS 10.0 software.
RESULTSThree hundred and eighty patients were followed up in the short term (less than 2 years after operation), 308 cases obtained excellent result, 48 good, fair 24, the excellent and good rate was 93.7%. Three hundred and forty-eight patients were followed up in the long term (more than 3 years after operation), 244 cases obtained excellent result, 48 good, fair 56,the excellent and good rate was 83.9%. There was significant difference in follow-up between the short and long term (P < 0.05).
CONCLUSIONThe clinical effect of discectomy for lumbar intervertebral disc herniation decreased with time lapse.

9.Clinical apprehension on application of Tri-lock BPS total hip arthroplasty.

OBJECTIVETo study short-term results and clinical application of Tri-lock BPS in total hip arthoplasty.
METHODSFrom May 2010 to July 2011, 32 hips in 31 patients (18 males and 13 females, ranging in age from 50 to 77 years old, with an average of 60.5 years old) were treated by total hip arthroplasty with Tri-lock BPS, including 8 patients with osteonecrosis (ON), 13 patients with fresh femoral neck fracture, 10 patients with developmental dysplasia of the hip (DDH). The therapeutic effects were evaluated by self assessment form, preoperative and postoperative Harris hip score, radiographs, Engh score and bone in growth of femoral side described by Gruen. Based on the short-term results,its design characteristic and clinical properties were analyzed.
RESULTSAll the incisions healed well and there were no complications such as femoral fracture, infection, dislocation and neurovascular injuries. All the patients were followed up with an average time of 12.2 months (ranged, 10 to 14 months). All the joints had good or excellent clinical results. The Harris score increased from preoperative 38.3 +/- 4.9 to 92.5 +/- 11.2 at the latest follow-up (t = 27.53, P < 0.01). Radiographically, the positions of the prostheses were normal,the average limbs length and femoral eccentricity recovered to normal. X-ray of the hips showed that the femoral stem prosthesis was in line with good initial fixed standard. At 3 months after surgery, X-ray of the hips showed that bone in growth in Gruen II and VI of femoral side.
CONCLUSIONShort-term results show that the design of Tri-lock BPS is more in line with human anatomy, and has the advantages in rapid recovery of hip function and retains more bone mass. It offers a newly valuable technology for the treatment of osteonecrosis, femoral neck fracture and DDH and so on.

10.Suture anchor for acute injuries of medial collateral ligament of knee at degree III.

OBJECTIVETo evaluate clinical efficacy of suture anchors in treating acute injuries of medial collateral ligament (MCL) of knee at degree III.
METHODSTwenty-seven patients with degree III acute MCL injuries of knee were treated with suture anchors from January 2007 to June 2011. There were 15 males and 12 females, aged from 19 to 56 (averaged 32.6) years old. The time from injury to operation was 3 to 10 days, averaged 6 days. Symptoms and physical signs before and after treatment were observed, Lysholm scoring were used to evluated clinical efficacy.
RESULTSAll patients were followed up from 16 to 30 months with an average of 21.6 months. The stability of knee joints was good in all patients. Abduction stress test was negative when the knee joint was straightened at 0 degrees and flexed at 30 degrees. The average degree of flexed knee (67.00 +/- 5.80) degrees preoperatively was lower than that of postoperatively (136.50 +/- 6.30) degrees at 1 year. According to Lysholm scoring, preoperative scores ranged from 30 to 43 points, averaged 36.46 +/- 1.48; 1 year after operation ranged from 87 to 100 with an average of 91.50 +/- 3.80 and higher than postoperative. Twenty patients got an excellent results, 5 good and 2 fair.
CONCLUSIONSuture anchors in treating acute injuries of medial collateral ligament of knee at degree III has following advantages: small range of tissue dissection, easy to operate, reliable fixation and less complications.
