2.Ideal Surgical Period of Children with Tongue-tie: Analysis of 517 Cases

ObjectiveTo investigate the ideal surgical period of children with tongue-tie.MethodsThe data of 517 children with tongue-tie treated by simply clipped surgery or horizontal-to-vertical plasty were analyzed retrospectively to evaluate the effect of operation and the level of phonologic prognosis.ResultsThe sex ratio was approximately 10∶4 (male∶female) in all groups. Two cases occurred postoperative hemorrhage. The follow-up compliance was better in children <1 year old, but higher levels of noncompliance occurred in children >1 years old. One year after surgical treatment, there were no phonologic disorders caused by tongue motion in children <1 year old, but the levels of phonologic prognosis in children >1 years old were better, moderate, and insignificant respectively.ConclusionThe ideal surgical period of children with tongue-tie is age of 6~12 months.
3.N-terminal of ?-Synuclein Involved in Regulation of Mitochondrial Function

Objective: To identify the functional domain of ?-Synuclein in affecting mitochondrial function and how the function to be impaired,especially,the mitochondrial membrane potential and the release of Cytochrome c.Methods: Harvest of ?-Syn-N and ?-Syn-△N by PCR,then subcloned into the pCMV-Myc mammalian expression vector.The recombinant plasmids were transfected into HEK293T cells by Lipofectamine 2000.After detecting the protein expression by Western blot,the functional domain was detected by co-immunoprecipitation.The mitochondrial membrane potential through flow cytometry and immunofluorescence,at the same time,the release of Cytochrome c through flow cytometry to detect.Results: The recombinant plasmids were constructed successfully.CO-IP has proved that N-terminal may be the functional domain of ?-Synuclein in affecting mitochondria.Over-expression of N-terminal could depolarize the mitochondrial membrane potential and induce the Cytochrome c releasing in MN9D cells.Conclusion: N-terminal may be the functional domain of ?-synuclein and over-expression of N-terminal could decrease mitochondrial activity.
4.A new flavone C-glycoside from leaves of Lophatherum gracile.

Several kinds of column chromatography method were used to investigate the chemical constituents of the leaves of Lophatherum gracile. The structures of the isolated compounds were identified based on their physicochemical properties and spectral data. A new flavone C-glycoside was isolated and its structure was identified as 3'-methoxyl-luteolin 6-C-beta-D-galactopyranosiduronic acid (1 --> 2) -alpha-L-arabinopyranoside (1).

5.Influencing factors of mental health of medical students in China.

This study investigated the mental health status of medical students in China, and analyzed the influencing factors in order to provide evidence for mental health education for medical students. A stratified cluster sampling method was used to recruit medical students from Huazhong University of Science and Technology, China. The questionnaire survey on general information and Symptom Checklist 90 (SCL-90) were used for investigation and analysis. The results showed among the 1137 valid questionnaires, 278 (24.45%) participants had SCL-90 score ≥ 160. The top three mental problems of medical students were obsessive-compulsive disorder, interpersonal sensitivity and depression in terms of the factor score ≥ 2.5 and the number of participants who reflected on the diseases. The third-year medical students had the worst mental health status, and fifth-year medical students had the best mental health status. Students from rural area had more psychological problems than those from urban area; furthermore, students with high professional satisfaction, those who were the single child of the family, non-poor students, and those whose parents had high education level had better mental health status. It was concluded that the mental health of medical students is not optimistic in China. Medical students have some mental health problems of different degrees. Factors that influence the mental health of medical students include academic pressure, professional satisfaction level and family environment.
6.Diagnosis of X-Linked creatine transporter deficiency in a patient from Northeast China

Background: Creatine transporter (CRTR) deficiency is the most common creatine deficiency syndrome,
of which the final diagnosis relies on mutation in the X-linked CRTR gene. To date, more than 90
mutations in the SLC6A8 gene have been reported. This paper discusses a novel mutation detected via
the thorough sequencing of all the X-chromosome-specific exons investigated in a four and a half year
old boy with an intellectual disability, speech and language delay and motor disturbance. Methods:
A brain magnetic resonance imaging (MRI) and a proton magnetic resonance spectroscopy (MRS)
were carried out, the creatine and creatinine concentrations in the urine were checked and all exons
were sequenced. Results: A detailed clinical investigation revealed a reduction in the cerebral creatine
levels in the brain by the MRS, elevated creatine and creatinine concentrations in the urine and signal
abnormalities in the left frontal cortex of the brain by the MRI. A novel change was identified in the
heterozygosity of the exon 10: c.1395-c.1401 deletion.
Conclusion: The use of a combination of powerful new technologies, such as thorough exome-nextgeneration
sequencing and a brain MRS, should be considered, in order to determine any neurometabolic
diseases, especially when the signal abnormalities in the brain MRI cannot be explained by any other
factors. This mutation results most likely in a dysfunction of the creatine transport and synthesis,
hence causing central nervous system symptoms.

7.Trial study on design of irradiated fields of radiotherapy in cervical and upper thoracic esophageal cancer

Objective To compare three kinds of irradiation treatment plans for cervical and upper thoracic esophageal cancer,in order to arrived at proper decision for the patient.Methods From February 2001 to June 2004,43 such patients were studied with three different simulated treatment plans made including conformal plan,conventional four-field plan and conventional two-field plan for every one.All plans were evaluated with iso- dose curve and dose-volume histogram.Results GTV on 95% isodose curve was 99.5%,98.2% and 87.4% in conforaml plan,conventional four-field plan and conventional two-field plan,respectively;PTV_1 and PTV_2 on 95% isodose with 97.8%,97.2%,94.8% and 95.8%,86.6%,73.7%.The volume of>20 Gy dose of left lung accepted was 18.6%,17.2% and 32.3%,in conformal plan,conventional four-field plan and conventional two-field plan,respectively;the right lung received 20.5%,19.9% and 35.5%.Conclusions Conformal plan is the best in radiotherapy,as it can provide ideal dose distribution of irradiated target with adequate protection of the normal tissues.Conventional four-field plan,being easy to carry out,can replace the conformal plan in most situations.Conventional two-field has the most uneven dose distribution and largest lung volume irradiated.
8.Effect of pressing Hegu acupoint and Neiguan acupoint in preventing PONV in patients after cardiothoracic surgery by general anesthesia

Objective To observe the preventive and therapeutic effect of pressing Hegu acupoint and Neiguan acupoint in patients after general anesthesia in cardiothoracic surgery who got postoperative nausea and vomiting (PONV). Methods 60 cases of patients with a history of general anesthesia cardiothoracic surgery were randomly divided into 2 groups, 30 cases in either group. Control group received routine anesthesia nursing care; care intervention group, routine anesthesia nursing care basically meanwhile pressing bilateral Hegu and Neiguan acupoints 2 days after operation. Postoperative nausea and vomiting symptom in the two groups were observed and compared. Results The incidence of postoperative nausea and vomiting in the care intervention group was significantly lower than the control group (P <0. 01). Conclusions Pressing the acupoints of Hegu and Neiguan will get obviously preventive and therapeutic effects in patients of postoperative nausea and vomiting symptom after general anesthesia cardiothoracic surgery.
9.Epidemiology and treatment analysis of children with out-of-hospital cardiac arrest.

OBJECTIVETo study the weak loop in the treatment of children with out-of-hospital cardiac arrest (OHCA) and the treatment strategy for improvement.
METHODData of a total of 133 patients with OHCA who were rescued by Wuxi pre-hospital care center during the 2005 - 2011 were analyzed.
RESULTThe main causes of pediatric OHCA were drowning (52/133) and accidental injury disease (30/133). The cases of OHCA were mainly located in public places (60/133) and the majority occurred in winter and summer. The emergency rescue response time was (13.21 ± 8.09) min, the rate of first witness treatment was 3.91%, intubations was performed in 11.11%, opening of vein access was 23.15% and one case got restoration of spontaneous circulation (ROSC).
CONCLUSIONThe rate of ROSC of pre-hospital cardiac arrest in children was significantly lower than that of hospital cardiac arrest. Preventive interventions on children's accidents and the skills of pre-hospital staff on pediatric advanced life support (PALS) need to be urgently improved.

10.Liver histopathological features of chronic HBV carriers and inactive HBsAg carriers.

OBJECTIVETo study the liver histopathological features of chronic HBV carriers and inactive HBsAg carriers.
METHODSLiver biopsies were performed on 189 chronic HBV carriers and 30 inactive HBsAg carriers (219 cases in total). All of them had a normal serum ALT value; they were then followed-up for more than 6 months. HBsAg and HBcAg were detected by immunohistochemistry. The circulating HBV DNA loads and serologic markers of HBV were examined at the same time. Grades of liver necrosis/inflammation and fibrosis were compared between the patients regarding their HBV DNA positivity or negativity. The relationships between the HBeAg positivity and degrees of liver histological changes were evaluated. The grades of liver necrosis/inflammation and fibrosis were compared between three age groups: younger than 18 years, 18-40, and older than 40 years.
RESULTSTwo hundred eight carriers of the total 219 (95.0%) had histological liver changes. Fifty percent (104/208) of them had mild histological changes (G0-1/S0-1), while more severe changes (G3-4 and/or S3-4) were found in 18 out of the 208. There were no significant differences in the grades of liver necrosis/inflammation and fibrosis between the chronic HBV carriers and the inactive HBsAg carriers. Among the serologic HBV DNA positive carriers, hepatic fibrosis was more severe in the HBeAg negative group than in the positive group (chi2 = 9.551, P = 0.008), but no differences of the necrosis/inflammation grades were seen between the two groups. The rate of severe fibrosis (S3-4) was 21.1% in those carriers older than 40 years but was 7.7% in patients younger than 18 years. However, no statistically significant differences in degrees of liver inflammation and fibrosis were found among the three age groups. HBcAg positive rate was 100% in the liver tissues of all the chronic HBV carriers, but only in 33.3% in the inactive HBsAg carriers.
CONCLUSIONSThe majority of our HBV carriers have liver inflammation and fibrosis. More severe histological changes were found in 8.65% of them. Liver fibrosis existed in the carriers with negative HBeAg and in those older than 40 years. HBcAg was found in hepatic tissues while their serological HBV DNA was negative.
