1.Differences on the parameters of the optic disc between the amblyopic eye and non-amblyopic eye in adolescent with anisometropic amblyopia

AIM: To compare the peripapillary retinal nerve fiber layer ( RNFL ) thickness and peripapillary topographic map's parameters between amblyopic eyes and non -amblyopic eyes in adolescent with anisometropic amblyopia by optical coherence tomography(OCT).
METHODS: Thirty - four juveniles with anisometropic amblyopia were selected. Peripapillary RNFL thickness and peripapillary topographic map were measured by frequency domain OCT with both eyes in all participants, and the differences between amblyopic eyes and non -amblyopic eyes were compared.
RESULTS: There was no significant difference in average thickness of peripapillary RNFL and in any other region of peripapillary RNFL. The disc area in amblyopic eyes was bigger than that in non - amblyopic eyes ( t =2. 8054,P= 0. 0263). The disc area in amblyopic eyes were significantly related to the thickness of nasal RNFL and the rim area(r= 0. 7592,0. 7501;P= 0. 0289,0. 0321).
CONCLUSION: There existed some difference in peripapillary structure between amblyopic eyes and non-amblyopic eyes in adolescent with anisometropic amblyopia.
2.PCNA, Bcl-2 and TERT expression in epiretinal membrane of rat traumatic proliferative vitreoretinopathy

· AIM: To explore the dynamic expression and correlation among telomerase catalytic subunit (TERT), proliferating cell nuclear antigen ( PCNA) and antiapoptosis protein Bd-2 which relate to cell proliferation in epiretinal membrane of rat traumatic proliferative vitreoretinopathy(PVR).· METHODS: S-P technique was applied for immunohistochemical staining of epiretinal membrane of traumatic PVR with TERT, PCNA and Bcl-2 antibody. HE staining was also carried out. The staining results were analyzed with image analysis system.· RESULTS: The positive rate and average A of PCNA protein were upregulated at first and then down-regulated, with the peak value in 14d Group, which was significantly different from those in 7d Group and 28d Group.The positive rate and average A of TERT and Bcl-2 were also upregulated at first and then down-regulated, with the peak value in 14d Group and 21d Group, which were significantly different from those in 7d Group. There was significant correlation among PCNA, Bcl-2 and TERT protein expression (P≤0.01).· CONCLUSTON: TERT and Bcl-2 take part in the regulation of proliferative cells in epiretinal membrane of traumatic proliferative PVR, with high correlation with the dynamic changes of cell proliferation.

4.Clinical significance of detecting CXC chemotatic factor in early diabetic retinopathy

BackgroundDiabetic retinopathy (DR) is the result of the cytokine network disorders,the imbalance of angiogenic factor and vascular inhibitory factor is the start factor.ObjectiveTo analyze the levels of CXC chemotatic factors of type 2 diabetes mellitus patients,evaluate the clinical application value of them in different clinical types of DR using receiver operating characteristic (ROC)analysis and to approach the new way of individualized treatment.Methods This was a prospective research.The gold standard was ophthalmolscope and fundus fluorescein angiography.The levels of CXC chemotatic factors and multiplicaiton factors were measured in 96 cases with type 2 diabetes mellitus (66 cases with retinopathy and 30 cases without retinopathy as control).The assessment tasks were performed for these index and courses of DR with ROC curve.Results The expression of age,course of disease has significant difference in different courses of DR ( F =8.507,P =0.001 ; F =28.143,P =0.000).Compared with the control group,the expression of growth-related oncogene-α ( GROα ) ( t =- 2.172,P =0.035,AUC =0.625 ),whole blood viscosity 200 ( t =- 3.724,P =0.001,AUC =0.904 ) and neutrophilic leukocyte (t=-2.562,P =0.013,AUC =0.577 ) has significant difference in the group of mild NPDR.Compared with the control group,the expression of interferon-γ-inducible protein 10 ( IP-10 ) ( t =-3.591,P =0.001,AUC =0.592 ),platelet derivation growth factor-BB ( PDGF-BB ) ( t =- 3.233,P =0.003,AUC =0.735 ),vascular endothelial growth factor(VEGF) ( t =- 3.617,P =0.001,AUC =0.776 ),C peptide ( t =- 3.366,P =0.002,AUC =0.962 ),leukocyte ( t=-3.201,P =0.003,AUC =0.852) and neutrophilic leukocyte(t =-4.201,P=0.000,AUC =0.852) has significant difference in the group of moderate and severe NPDR.ConclusionsCXC chemotatic factors may act as reactivator in the pathogenesis of DR,GROα and IP-10 may be useful for clinical monitoring of the severity of DR,and evaluating the imbalance state of chemotatic factors maybe a new approach to clinical monitoring and prognosis of DR.
5.Treatment of atrophic rhinitis by transplantation of pediculated bone-suberiosteal muscle flap

Objective To explore a better method for treatment atrophic rhinitis.Methods 56 patients with atrophic rhinitis(96 lateral)were treated by nasal submucou pediculated bone-suberiosteal muscle flap extracted from anterior wall of sinus maxillaries.Results All patients were followed 2 to 10 years,total effective rate was 100 %, with 49 cases(87.5 %)showing prominent effect.Conclusion The grafted flap cannot be assimilated,felled off and necrosis,because the flap has rich blood supply.This methods has obvious short-term effective and stable long-term effective.No complications were found.
6.The relationship between platelet endothelial cell adhesion molecule-1 and paraquat-induced lung injury in rabbits

BACKGROUND: Platelet endothelial cell adhesion molecule-1 (PECAM-1), also known as CD31, is mainly distributed in vascular endothelial cells. Studies have shown that PECAM-1 is a very significant indicator of angiogenesis, and has been used as an indicator for vascular endothelial cells. The present study aimed to explore the relationship between the expression of PECAM-1 and the degree of acute lung injury (ALI) and fibrosis in paraquat (PQ) induced lung injury in rabbits. METHODS: Thirty-six adult New Zealand rabbits were randomly divided into three groups (12 rabbits in each group) according to PQ dosage: 8 mg/kg (group A), 16 mg/kg (group B), and 32 mg/kg (group C). After PQ infusion, the rabbits were monitored for 7 days and then euthanized. The lungs were removed for histological evaluation. Masson staining was used to determine the degree of lung fibrosis (LF), and semi-quantitative immune-histochemistry analysis to determine the expression of PECAM-1. Pearson's product-moment correlation analysis was performed to evaluate the relationship between the expression of PECAM-1 and the extent of lung injuries expressed by ALI score and degree of LF. RESULTS: Rabbits in the three groups showed apparent poisoning. The rabbits survived longer in group A than in groups B and C (6.47±0.99 days vs. 6.09±1.04 days vs. 4.77±2.04 days) (P<0.05). ALI score was lower in group A than in groups B and C (8.33±1.03 vs. 9.83±1.17 vs. 11.50±1.38) (P<0.05), and there was statistically significant difference between group B and group C (P=0.03). LF was slighter in group A than in groups B and C (31.09%±2.05 % vs. 34.37%±1.62% vs. 36.54%±0.44%) (P<0.05), and there was statistically significant difference between group B and group C (P=0.026). The PEACAM-1 expression was higher in group A than in groups B and C (20.31%±0.70% vs. 19.34%±0.68% vs. 18.37%±0.46%) (P<0.05), and there was statistically significant difference between group B and group C (P=0.017). Pearson's correlation analysis showed that the expression of PECAM-1 was negatively correlated to both ALI score (Coe=–0.732, P=0.001) and degree of LF (Coe=–0.779, P<0.001). CONCLUSIONS: The PECAM-1 expression significantly decreases in New Zealand rabbits after PQ poisoning, and the decrease is dose-dependent. The PECAM-1 expression is negatively correlated with ALI score and LF, showing a significant role in the development of lung injuries induced by PQ.

METHODSepearing the chemical constituents by means of chromatography and identifying their structures on the basis of MS and NMR spectra and TLC with authentic samples.
RESULTEight compounds were isolated as 4-[(4-hydroxyphenyl) methoxy]benzenemethanol(I), 4,4'-dihydroxydiphenyl methane(II), 4,4'-dihydroxydibenzyl ether(III), gastrodin(IV), 4-hydroxy benzenemethanol(V), 4-hydroxybenz aldehyde(VI), beta-sitosterol(VII) and beta-daucosterol(VIII).
CONCLUSIONAll the compounds were obtained from genus Coeloglassum for the first time. The compound I is a new natural product.

AIMTo study the chemical constituents of the rhizomes of Coeloglossum viride (L.) Hartm. var. bracteatum (Willd.) Richter.
METHODSThe compounds were mainly isolated with normal phase and reverse phase column chromatographic methods and HPLC. Their structures were elucidated on the basis of physical constants and spectral analysis (UV, IR, EI-MS, positive and negative FAB-MS, APCI-MS, 1HNMR, 13CNMR, DEPT, 1H-1H COSY, HMQC and HMBC).
RESULTSEight compounds were obtained from the ethanolic extract of the rhizomes of this plant, dactylorhin B (I), loroglossin (II), dactylorhin A (III), militarine (IV), coelovirin A (V), gastrodin (VI), thymidine (VII) and quercetin-3,7-di-O-beta-D-glucopyranoside (VIII).
CONCLUSIONAll the compounds were obtained from this plant and genus Coeloglossum for the first time. V named coelovirin A, is a new compound.

9.Clinical trial of tolvaptan tablets in the treatment of heart failure with systolic dysfunction

Objective To compare the clinical efficacy and safety of tolvaptan tablets and furosemide injection in the treatment of heart failure with systolic dysfunction. Methods One hundred and four patients of heart failure with systolic dysfunction were randomly divided into control group (n = 49 cases) and treatment group (n = 55 cases) . Control group received furosemide 40 mg per time, qd, intravenous bolus. Treatment group received tolvaptan 15 mg per time, qd, orally. The clinical efficacy, heart rate, pulmonary artery pressure, pulmonary capillary pressure and cardiac output, and adverse drug reactions were compared between two groups. Results After treatment, the total effective rates of treatment and control groups were 89. 09% (49 cases/55 cases) and71. 43% (35 cases/49 cases) with significant difference (P < 0. 05) .After treatment, the main indexes of treatment and control groups were compared: heart rates were (80. 15 ± 10. 04) and (84. 71 ± 9. 66) beat·min-1, pulmonary artery pressure were (21. 85 ± 4. 49) and (28. 47 ± 4. 46) mm Hg, pulmonary capillary pressure were (11. 24 ± 1. 61) and (15. 18 ± 2. 76) mm Hg, cardiac output were (1. 94 ± 0. 30) and (2. 16 ± 0. 25) L · min-1· m-2, the differences were statistically significant (all P < 0. 05) . The adverse drug reactions of treatment group were increased blood sodium, urinary frequency and dry mouth, which in control group were fatigue, thirst and muscle soreness. The total incidences of adverse drug reactions in the treatment and control groups were 12. 73% and 12. 24% without significant difference (P> 0. 05) . Conclusion Tolvaptan tablets have a definitive clinical efficacy in the treatment of heart failure with systolic dysfunction, which can effectively improve the heart rate, pulmonary artery pressure, pulmonary capillary pressure and cardiac output, without increasing the incidence of adverse drug reactions.
