1.Influence of pathological characteristics on radiotherapeutic target area of esophageal squamous cell carcinoma

Objective To investigation the pathological characteristics of esophageal squamous cell carcinoma to provide reference criteria for delineating the target area in radiotherapy.Methods Fifty-two patients from the Fourth Hospital of HeBei Medical University underwent resection whom all had been proved to have esophageal squamous cell carcinoma before operation.Chest CT was scanned and transmitted to the 3- dimensional conformal planning system for radiotherapy by VRX-16 scanner.The lesion of esophageal carcinoma was delineated in the 3-dimensional rebuild CT image and the lesion volume was computed by digital rebuild program.Every surgically resected specimen was made into pathologic giant section.The actual size of the specimen was obtained by calculating the size under the microscope with the shrinkage ratio.Multicentric carcinomatous lesion,severe dysplasia and direct intramural infiltration were observed in the giant section with a microscope and the order of such pathological characteristics were analysed statistically.Results 1.The tumor length by different method of preparation of operated specimens differed obviously.The longest was shown by CT. 2.Multicentric carcinomatous lesion was found in 15(29%)cases out of 52 patients.Proximal to the tumor,the mean distance between the multicentric carcinomatous lesion and the main lesion plus the length of the multicentric carcinomatous lesion was 3.02?1.45cm.Distal to the tumor,it was 2.60?2.44 cm.Severe dysplasia was found in 28 patients.Proximally,the mean distance between the severe dysplasia and the main lesion plus the length of the severe dysplasia was 2.45?1.30 cm.Distal to the tumor,it was 3.24?2.19 cm.Direct intramural infiltration was found in 41 patients,of which the mean length being 2.80?1.52 cm proximally and 2.02?1.51 cm distally. 3.Tumor thrombus was found in 6 patients and lymphoduct infiltration in 36 patients.Direct intramural infiltration was found at higher incidence in specimens complicated with lymphoduct infiltration(86%)and those complicated with tumor thrombus(91%).There were no apparent factors affecting severe dysplasia.The proximal distance to direct intra- mural infiltration was much longer than distally.Conclusions Multicentric carcinomatous lesion,severe dysplasia and direct intramural infiltration may be observed in esophageal squamous cell carcinoma.Multicentric carcinomatous lesion and direct intramural infiltration are obviously correlated with lymphoduct infiltration.To cover 95% of the microscopic extension,a margin of 5.0 cm is needed proximal to the base of gross tumor volume,and 7.5 cm distal to it.To cover 90% of the microscopic extension,a margin of 4.5 cm is needed proximally,and 5.0 cm distally.

Gastric neurofibroma is rare and its exact incidence is unknown. We experienced a case of solitary gastric neurofibroma. A 49-year-old woman was admitted because of hematemesis and melena. Neither cafe-au-lait spots in skin nor superficial tumor were found. Gastrofibroscopic examination and UGI series showed a 3 * 4 cm sized luminal protruding mass with adjacent bridging mucosal fold and cental ulceration on the anterior wall of lower body. Mass excision was performed. Grossly, the mass was 4.5 * 3.0 * 2.0 cm sized, well circumscribed submucosal tumor with homogenous cut surface, Microscopically, each of the tumor cells had oval shaped nucleus and spindle shaped cytoplasm. Nuclear atypism and frequent mitosis were not observed. We report a rare case of solitary gastric neurofibroma with review of the literatures.

5.Visualization of Coronary Arteries by Color-Coded Transesophageal Doppler Echocardiography.

The visualization of coronary arteries by transthoracic two-dimensional echocardiography has been used for over 10 years. In many cases, the imaging quality is too poor to allow an anatomic evaluation. During the last few years, transesophageal echocardiography has been shown to provide optimal imaging quality in virtually all patients and of all cardiac structures including the coronary arteries. The purpose of this study was to test the ability of transesophageal echocardiography in the visualiation of the coronary arteries and assessment of coronary blood flow by transesophageal two-dimensional pulsed Doppler echocardiography. We Studied 285 patients, 91 men and 194 women, aged 16 to 81 year(mean 50.6 year men, mean 54.2 year women). We have been used a 5-MHz phased array transducer with incorporated color-coded Doppler. The left main coronary artery was visualized 95.1%, left circumflex artery 27.4, left anterior descending artery 21.4% and the main stem of the proximal right coronary artery 45.1%. The time-sequential left anterior descending artery flow pattern generally consisted of a small late systolic component and a large diastolic component. The peak flow velocity in the proximal left anterior descending artery during diastole was 40.8+/-8.0cm/sec(integrity 7.6+/-0.9) and during late systole was 18.5+/-5.5cm/sec(integrity 2.9+/-0.9). There were no complications during and after examination. This study suggests that transesophageal color-coded Doppler two-dimensional echocardiography appers to be a feasible noninvasive technique for imaging the proximal left coronary artery and the left anterior descending artery flow is detectable from the transesophageal approach.

Astragali Radix(AR)is a clinically used herbal medicine with multiple immunomodulatory activities that can strengthen the activity and cytotoxicity of natural killer(NK)cells.However,owing to the complexity of its composition,the specific active ingredients in AR that act on NK cells are not clear yet.Cell membrane chromatography(CMC)is mainly used to screen the active ingredients in a complex system of herbal medicines.In this study,a new comprehensive two-dimensional(2D)NK-92MI CMC/C18 column/time-of-flight mass spectrometry(TOFMS)system was established to screen for potential NK cell acti-vators.To obtain a higher column efficiency,3-mercaptopropyltrimethoxysilane-modified silica was synthesized to prepare the NK-92MI CMC column.In total,nine components in AR were screened from this system,which could be washed out from the NK-92MI/CMC column after 10 min,and they showed good affinity for NK-92MI/CMC column.Two representative active compounds of AR,isoastragaloside Ⅰ and astragaloside Ⅳ,promoted the killing effect of NK cells on K562 cells in a dose-dependent manner.It can thus suggest that isoastragaloside Ⅰ and astragaloside Ⅳ are the main immunomodulatory compo-nents of AR.This comprehensive 2D NK-92MI CMC analytical system is a practical method for screening immune cell activators from other herbal medicines with immunomodulatory effects.
7.Recovery, growth and development of Macroorchis spinulosus in albino rats.

The developmental features, growth and organogenesis of Macroorchis spinulosus were observed in albino rats. Globular and thick walled metacercariae, possessed a stylet, Y-shaped excretory bladder and extracecal testes. In albino rats, M. spinulosus showed habitat shifting. The majority of M. spinulosus reside in the jejunum for the first four days post infection (p.i.) and migrate to the duodenum at the later stage of infection. M. spinulosus grew rapidly during the first four days and reached full maturity at 14 days p.i. and later reduced in size. The ovary was separated from the genital primodium at one day p.i. The seminal vesicle appeared on the third day and divided into two sacs on the fourth day p.i. and intrauterine eggs and sperm mass were produced on the fourth day. Organogenesis and enlargement of reproductive organs governed the growth of M. spinulosus. The similarity of related species of the genus Macroorchis to M. spinulosus was discussed in consideration to developmental features.

8.Studies on intestinal trematodes in Korea XVII. Development of egg lying capacity of Echinostoma hortense in albino rats and human experimental infection.

The worm development and egg laying pattern of Echinostoma hortense(Trematoda: Echinostomatidae) were studied in albino rats and the brief clinical course was observed in human volunteers. A total of 21 rats were infected with 20-69 metecercariae each and two humans were with 7 and 27 metacercariae, which were collected from loaches. For recovery of worms, the rats were sacrificed at irregular intervals from the 6th to 150th day after infection and the human volunteers were treated with praziquantel and purged with magnesium salt on the 26-27th day. The stools of the rats and humans were examined for the eggs. The results were as follows: The worm recovery rate from the rats was not affected by the increase of infection time but varied individually; 9.1-50.0% (31.1% in average). From humans, 14.3% and 37.0% (32.4% in average) of challenged were recovered. In the rats, it was revealed that the worms rapidly grew for the first 14 days to become 7.59 mm in average length and 1.17 mm in average width but the growth became much slower thereafer until the 150th day; 7.95 mm in length on the 21th day, 9.04 mm on the 28th day, 10.21 mm on the 49th day and 12.62 mm on the 150th day. During the early stage of infection, the growth of genital organs(male or female) was expressed as sigmoid curves whereas non-genital organs(such as suckers) was simply as straight lines. The prepatent period of this fluke was 10-12 days in the rats and 16-17 days in men. After the start of oviposition, the egg production by the worms remarkably increased, reached maximum on the 32-33th day, followed by decrease thereafter. The maximum value of E.P.G./worm was 390. The major subjective symptoms in human volunteers were abdominal pain and diarrhea during the early stage of infection. The results show that human is as susceptible as the rats to E. hortense infection and the amount of egg production in the rats is greatly affected by the age of worms.

9.Classifications and surgical treatments of venous malformations in parotid region.

OBJECTIVETo summarize clinical and pathological characteristics and surgical experiences of venous malformations in parotid region.
METHODS39 cases of venous malformations in parotid region were divided into peripheral, honeycomb-like and reticular three classifications according to clinical manifestations and histological characters. Surgical treatments were correspondingly selected as overall excision, overall excision and superficial lob of parotid excision after facial nerve dissection, overall excision and superficial lob of parotid excision combined with pingyangmycin injection.
RESULTS26 cases were cured, 9 cases with excellent result and 4 cases with effective result.
CONCLUSIONSThe new effective classification of venous malformations in parotid region can be a guide to the selection of the therapy on them.

10.Clinical observations on the effect of Tuigua manipulation combined with quadriceps exercise for the treatment of degenerative gonarthritis.

OBJECTIVETo investigate the effect of Tuigua manipulation combined with quadriceps exercise for treating degenerative gonarthritis.
METHODSIn the study, 48 patients (60 knees) meeting diagnostic criteria of degenerative gonarthritis were randomly divided into two groups: treatment group and control group. The patients in the treatment group were treated with Tuigua manipulation combined with quadriceps exercise; while the patients in the control group were treated with manipulation for osteoarthritis in Tuina Xue. All the patient in the control and treatment group were treated for 3 courses of treatment, and 10 days were one course of treatment. The index such as pain, joint range of motion, function and muscle power were observed and compared before and after treatment.
RESULTSAfter treatment, in the treatment group, 7 knees were cured, 21 knees got a good result and 2 knees had no effect; while in the control group, the above data were 8, 19, 3 respectively. There was no statistically differences between the therapeutic effects of the two groups. Both methods were effective to improve clinical symptoms of degenerative gonarthritis, and the index such as the pain, joint range of motion, function and muscle power of the patients in two groups were improved after the treatment (P < 0.05). Comparison between the two groups, the improvement of knee function and quadriceps muscle power of patients in treatment group was better than that of control group (P < 0.05); but the improvement of pain and joint range of motion in two groups had no statistically differences (P > 0.05).
CONCLUSIONThe method of Tuigua manipulation combined with quadriceps exercise is effective to improve clinical signs and symptoms of patients with degenerative gonarthritis, to improve quality of life of patients, slow down degenerative of arthrodial cartilage, prevent the disease aggravating,which is worth spreading.
