1.An investigation on the life quality and mental health status of the disabled people in Zhejiang Province

Objective To learn the quality of life (QOL) and mental health status of the physically disabled people in Zhejiang Province,in order to provide evidence for improving the quality of life and mental health of the physically disabled people.Methods Using questionnaires and interviews to make household surveys of 950 physically disabled people.The questionnaire included the brief quality of life scale (SF-36) and the general health questionnaire (GHQ-28) in the anxiety and depression scale,physical and mental health status.The basic information and other living conditions of physically disabled people were collected through interviews.Results A total of 907 valid questionnaires were recovered,and the effective response rate was 95.47%.Scores of QOL of 907 physically disabled people was 60.89,and anxiety and depression scores were more than 4 points and there was a high score on anxious and depression.The scores of quality of life,anxiety and depression were not statistically significant between males and females(P >0.05).There were significant differences in all dimensions of SF-36 and anxiety and depression in different levels of physically disabled people(P < 0.01).There was a significant negative correlation between the quality of life and anxiety and depression in physically disabled people(rs =-0.626、-0.639,P < 0.01).The multiple linear regression showed that physiological function,role physical,physical pain and general health were the influencing factors of anxiety of the physically disabled people,and role physical,physical pain and general health were the influencing factors of depression of the physically disabled people.Condusion The quality of life of physically disabled people were comparably poor,and presented anxiety,depression and other psychological problems.Physical function status directly affects the quality of life and mental health status.
2.Investigation of current situation of nurse career pressure source and the analysis of relating factors

Objective To learn today's nurse career pressure and identify relating factom that affectnurses'pressure.so as to provide some adequate measures for supervisors to reduce the career pressure of nurses to the highest degree.Methods The clinical nurses in Medicine and Surgical Departements were investigated with questionnaire.Results The career pressure of nurses mainly came from several aspects such as their work amount,schedule,nursing care of patients,working environment and resources.There were also other relating factors,for instance,age,marital status,educational level,title and health condition.Conclusions Supervisors should pay attention to release the career pressure of nurses and take nurses'mental health into account.Supervisors should also try to create a free and pleasant working atmosphere and give more understand and support to our nurses.
3.Effects of bone marrow mesenchymal stem cells transfected by vascular endothelial growth factor gene on lung development of rats with bronchopulmonary dysplasia

Objective To explore the influence of vascular endothelial growth factor(VEGF) gene-transfected bone marrow mesenchymal stem cells (MSCs) on pulmonary alveolar structure and microvascular in rats with bronchopulmonary dysplasia.Methods Bone marrow MSCs were isolated from SD rats by way of density gradient centrifugation,purified,and transfected with pcDNA3.1 (-)/VEGF164 and blank plasmid pcDNA3.1 respectively using the liposome mediated method.After placing newborn SD rats in the oxygen box of 950 mL/L for 14 days,they were randomly divided into the transfected group(MSCs/VEGF group),the control group(MSCs group),and the blank group(serumfree medium group),with 10 rats in each group respectively,and they were injected respectively with 1 × 105 MSCs transfected by VEGF,MSCs and the same amount of simple serum-free medium by airway.After transplantation for 1 week and 4 weeks,lung tissue was observed by means of hematoxylin eosin staining to study lung structure and radial alveolar counts(RAC),and VEGF protein expression and angiogenesis densities were evaluated by immunohistochemistry,and finally VEGF164 protein was detected using Western blot.Results After transplantation for 1 week,4 weeks,the RAC,VEGF expression,vascular density by immunohistochemistry in transfected group were significantly more than those in the control group and the blank group(all P <0.05).After transplantation for 1 week,4 weeks,the VEGF164 protein level in transfected group was significantly more than that in the control group and the blank group (all P <0.05).Conclusions Eukaryotic expression vector pcDNA3.1 (-)/VEGF164 can effectively be expressed in MSCs.Transplantation of vascular endothelial growth factor gene by means of transfected MSCs brings better improvement in pulmonary alveolar structure and microvascular regeneration.VEGF is closely related to lung development in newborn rats.

The experiences of Shanghai Jiaotong University Medical Library Alliance in integrating and optimizing the allocation of resources in hospital library alliance, improving the comprehensive service ability, providing the excellent service, working out the regulations for rational use of literature resources, innovating the technology and management were summarized by investigating the library holdings and service in its member libraries. The large scale library holdings and large scale circulation can thus be realized in the member libraries of Shanghai Jiaotong University Medical Library Alliance, its group advantages can be brought into full play, and its supporting ability for the multiple needs of its users can be effectively improved.
5.Hybrid operation for acute left leg deep venous thrombosis secondary to left iliac vein compression syndrome: analysis of 36 cases.

OBJECTIVETo evaluate the surgical techniques for acute left deep venous thrombosis (LDVT) secondary to left iliac vein compression syndrome (IVCS).
METHODSThirty-six patients with acute LDVT secondary to IVCS received inferior vena cava filter placement, and in 2 of the cases, stent implantation was canceled for acute episode of obsolete DVT. The remaining 34 patients underwent left femoral venotomy for iliofemoral thrombectomy with Fogarty catheter and distal femoral vein thrombus removal by sequential compression of the legs, followed by implantation of stent-graft (2 cases) or bare-metal stents (32 cases) in the left common iliac veins. With routine anticoagulation and thrombolytic treatments, the patients were regularly examined for postoperative blood flow in the affected limb.
RESULTSIn 2 of the cases undergoing bare-metal stent implantation, the residue thrombi were squeezed into the stent by balloon, which was managed subsequently with local thrombolysis. One patient with bare-metal stent implantation received a secondary stenting for posterior stent displacement. Three patients had self-limited bleeding due to decreased serum FBG. Significant improvements were achieved at 3, 6, 30 and 180 days postoperatively in the circumferences of the affected limb (P<0.05) and in the levels of D-dimer (P=0.011), and FBG level showed no significant variations (F=1.163, P=0.345). The total rate of excellent outcomes was 83.3% (26/34) with a total effective rate of 91.2% (31/34) in these cases.
CONCLUSIONSThrombectomy to revascularize the inflow tract and stent implantation to enlarge stenosed iliac veins are key issues in treatment of acute LDVT secondary to IVCS.

6.Cervical spondylosis of vertebral artery treated with thermosensitive moxibustion with different dosages: a randomized controlled trial.

OBJECTIVETo compare the difference in the clinical efficacy on cervical spondylosis of vertebral artery type (CSA) treated with thermosensitive moxibustion at different dosages.
METHODSSixty cases of CSA were randomized into a saturated moxa dosage group and a regular moxa dosage group, 30 cases in each one. The thermosensitive moxibustion was adopted in the two groups. The mild suspended moxibustion was applied at two acupoints with the strongest thermosensitization. In the saturated moxa dosage group, the moxibustion time was determined by the disappearance of thermosensitization. In the regular moxa dosage group, 15 min was required on each acupoint. The treatment was given twice a day for first 4 days in the two groups. Since the 5th day, the treatment was given once a day, continuously for 10 times, and totally 14 days were required. The score of symptoms and function and clinical efficacy were compared between the two groups before and after treatment as well as 6-month follow-up after treatment.
RESULTSThe curative and effective rate was 56.7% (17/30) after treatment and 60.0% (18/30) in 6-month follow-up after treatment in the saturated moxa dosage group, which were superior to 26.7% (8/30) and 30.0% (9/30) in the regular moxa dosage group respectively (P < 0.01, P < 0.05). The scores of clinical symptoms and function after treatment and in follow-up were improved apparently as compared with those before treatment in both groups (all P < 0.01). The scores of clinical symptoms and function after treatment and in follow-up in the saturated moxa dosage group were increased much more apparently than those in the regular moxa dosage group (after treatment: 22.32 +/- 4.64 vs 17.43 +/- 3.21; in follow-up: 23.01 +/- 4.76 vs 18.32 +/- 2.13, both P < 0.01).
CONCLUSIONThe thermosensitization moxibustion of saturated dosage achieves the superior short-term and long-term efficacies in the treatment of CSA as compared with the regular moxibustion dosage.

Objective To investigate the relationship between APE1, XRCC1 gene polymorphisms and hepatocellular carcinoma(HCC) susceptibility and to explore the correlation of APE1, XRCC1 gene polymorphisms with the sensitivity to platinum-based drugs .Methods Seventy-eight HCC patients and 80 controls were selected .By PCR and RFLP , the single nucleotide polymorphism of APE1 Asp148Glu and XRCC1 Arg194Trp genes and the susceptibility of HCC or platinum drug sensitivity were analyzed.Results The Glu/Glu genotype of APE1 could increase in the risk of HCC by 7.21 times (95%CI:1.325-29.109) (P<0.05).APE1 and XRCC1 gene polymorphisms could also affect the platinum drug resistance of HCC patients.Conclusion APE1 Asp148Glu is correlated with the susceptibility to HCC .APE1 and XRCC1 genes can be considered a target for therapy to improve the sensitivity of HCC platinum drugs .
8.Polymorphism of apolipoprotein E gene and natural longevity in the Xinjiang Uighur people: an association study.

OBJECTIVETo explore the distribution of polymorphisms of apolipoprotein E(apoE) gene and its relation to natural longevity in the Xinjiang Uighur people.
METHODSUsing polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP), the authors obtained data from centenarians(n=42), 90-year-old people (n=102), 65-70-year-old people (n=70) and controls(n=53).
RESULTSThe frequencies of apoE genotypes epsilon 3/4, epsilon 3/3 and epsilon 2/3 were 2.4%, 69.0% and 23.8% respectively in the centenarian group, and the frequencies of their epsilon 4, epsilon 3 and epsilon 2 alleles were 1.2%, 82.1% and 16.7%. The frequencies of genotype epsilon 3/4 and epsilon 4, epsilon 3 allele in the centenarian group were significantly lower than those in controls(P< 0.01), whereas the frequencies of genotype epsilon 2/3 and epsilon 2 allele in the centenarian group were significantly higher than those in controls(P< 0.01). Longevity was noted to be positively correlated with epsilon 2 and negatively correlated with epsilon 4.
CONCLUSIONThis relationship reflects a genetic influence on differential survival and may point to pleiotropic age-dependent effects on longevity.

9.Changes of serum N-terminal pro-brain natriuretic peptide levels in patients with cardiovascular diseases and its clinical significance.

OBJECTIVETo detect the changes in serum N-terminal pro-brain natriuretic peptide (NT-proBNP) levels in patients with cardiovascular diseases and explore its clinical significance.
METHODSSerum NT-proBNP concentrations were measured by electrochemiluminescent immunoassay (ECLIA) in 460 patients with cardiovascular diseases and in 50 normal controls, and echocardiographic examination was performed to determine the left ventricular ejection function (LVEF). Analysis of NT-proBNP was performed for its correlation to New York Heart Association (NYHA) functional classifications LVEF, and risk factors of cardiovascular diseases.
RESULTSThe serum LgNT-proBNP concentrations was 3.74 in patients with cardiovascular diseases, significantly higher than that of normal controls (1.42, P<0.001). NT-proBNP concentrations also varied significantly among patients with different cardiovascular diseases as shown by one-way ANOVA analysis (F=17.761, P<0.001). The NT-proBNP levels increased with the severity of heart failure according to NYHA functional classifications (P<0.001), and varied significantly in patients suffering different cardiovascular diseases with the same NYHA functional class. Multivariable regression analysis indicated there were significant correlations of NT-proBNP levels with the patients' age (r=0.152, P<0.001), NYHA functional classifications (r=0.725, P<0.001), LVEF (r=-0.634, P<0.001), and clinica outcomes (r=-0.581, P<0.001). Logistic regression analysis identified NT-proBNP level as a strong indicator for cardiovascular events (HR=2.763, P<0.01) with close correlation to the treatment results.
CONCLUSIONSSerum NT-proBNP level varies significantly with the severity of heart failure and can be indicative of the patients' cardiac function in close correlation to the clinical prognosis, but its value for diagnostic stratification of cardiovascular diseases awaits further investigation.

10.Efficacy of regional administration of urokinase and argatroban via small saphenous vein catheter for treatment of acute deep venous thrombosis in the lower limb.

OBJECTIVETo investigate the clinical value of local regional administration of urokinase and argatroban through small saphenous vein (SSV) catheter in the treatment of acute deep venous thrombosis in the lower limb (LDVT).
METHODSFifty-six patients with acute LDVT were prospectively randomized into the study group (21 cases, 24 limbs) and control group (35 cases, 36 limbs) for treatment with urokinase and argatroban regionally administered via the SSV catheter and with the same agents given via the peripheral vein, respectively. The patients were examined for changes in serum fibrinogen (FBG) and D-dimer and the perimeter of the affected limbs, and the complications in relation to the agents were observed.
RESULTSBy corrected Chi-square test, the incidence of complications was significantly lower in the study group than in the control group (1/21 vs 4/36, χ(2)=1.92, P≤0.05). Wilcoxon's sign rank test suggested no statistically significant difference between the two groups in the total effective rate (95.8% vs 94.4%, V=0.52, P>0.05), but the total excellent rate differed significantly between them (83.3% vs 55.6%, V=2.36, P≤0.05). Serum FBG underwent no significant variations in the study group during thrombolysis (P>0.05), but decreased significantly in the control group (P≤0.05). The decreases in serum D-dimer and perimeter of the affected limbs occurred earlier in the study group than in the control group (P≤0.05).
CONCLUSIONRegional administration of urokinase and argatroban via small saphenous vein catheter can promote the thrombolytic effect and reduce the risk of hemorrhage in the treatment of LDVT.
