2.The application of somatostatin during the perioperation of porta-azygous disconnection for the patients with portal hypertension

Objective:To study the effects of somatostatin on portal pressure and operative complications in patients of portal hypertension treated with porta-azygous disconnection. Methods:Twenty patients with portal hypertension were treated with azygoportal disconnection. The severity of liver disease according to the Child-Pugh's grade was B in all patients. The patients were divided into two groups. The patients in group A were treated with intravenous infusion of somatostatin (6 mg/24 h) one day before operation and five days after operation. The patients in group B were treated with azygoportal disconnection without the use of somatostatin. The portal pressure, the volume of bleeding during operation, and the ascites and complications after operation were evaluated. The serum concentration of GLU, NO and ET-1 were determined. Results:The portal pressure and GLU concentration in patients of group A were lower than those in patients of group B. There were no differences of NO, ET-1 in patients between group A and group B. The volume of bleeding, ascites and complications in patients of group B were higher than in patients of group A. Conclusion:The use of somatostatin before and after porta-azygous disconnection can decrease portal pressure. It is beneficial in reducing the volume of bleeding in operation, ascites and complications after operation.
6.Correlation of GGT with AFP and diagnostic value of GGT for hepatocellular carcinoma

Objective To analyze the correlation of gamma-glutamyl transpeptidase (GGT)level with alpha-fetoprotein (AFP)level and to re-evaluate the diagnostic value of GGT for hepatocellular carcinoma (HCC).Methods Four hundred and seventy-two patients with HCC or liver cirrhosis,who were hospitalized in China-Japan Friendship Hospital from January 2003 to June 2009,were included in the study.The correlation between GGT and AFP was analyzed by Spearman nonparametric test.The cut-off values for the two parameters were determined based on their receiver operating characteristics (ROC)curves,areas under the ROC curve (AUCs),sensitivity,and specifici-ty,and the diagnostic values were presented using their sensitivity,specificity,and correct index.Statistical analysis was performed using SPSS 17.0.Normally distributed continuous data were analyzed by independent-samples t test,while non-normally distributed continuous data were analyzed by Mann -Whitney U test.Categorical data were analyzed by Pearson chi -square test,continuity-corrected chi -square test,or Fisher’s exact test.Results Among 472 patients,224 were diagnosed with HCC,and 248 with liver cirrhosis.Compared with cirrhotic patients,HCC patients had a significantly higher GGT level (113 (58-254)U/L vs 38 (22-72)U/L,Z=-11.037,P<0.001)and a significantly higher AFP level (429.5 (15.7-1210.0)ng/ml vs 5.7 (3.4-18.2)ng/ml,Z=-10.157,P<0.001).A significant correlation was found between GGT and AFP (r=0.449,P<0.001).The AUC was 0.784 for GGT and 0.788 for AFP.The cut-off value was 60 U/L for GGT and 20 ng/ml for AFP.The sensitivity was 74.1%for GGT,71.8%for AFP,and 90.7%for a combina-tion of the two parameters,the specificity was 70.2%,77.6%,and 58.7%,respectively,and the correct index was 0.443,0.494,and 0.494,respectively.Conclusion GGT may be regarded as one biomarker for HCC,and its level is significantly correlated with AFP level. The diagnostic value of AFP may not be improved when used in combination with GGT.
8.KL-6 as a useful serum marker for interstitial lung disease in patients with idiopathic inflammatory myopathies

Objective To determine the serum level of KL-6 in patients with polymyositis(PM)and dermatomyositis(DM),and to investigate the possible diagnostic value for interstitial lung diseases(ILD)in pa- tients with PM/DM.Methods Serum KL-6 concentrations were measured by ELISA in 53 adult PM/DM pa- tients,the control groups of 22 patients with infectious lung disease,and 50 healthy subjects.The association with clinical features and serum KL-6 levels was analyzed.Results The serum levels of KL-6 were(1543?761)、(429?106)、(336?196)and(289?105)U/ml in PM/DM patients with ILD and without ILD,patients with infectious lung disease and healthy controls,respectively.Serum KL-6 levels in the PM/DM patients with ILD were significantly higher compared with PM/DM patients without ILD,patients with infectious lung disease and healthy controls(all P<0.01 ).However,no significant differences of serum KL-6 levels was found among PM/ DM patients without ILD,patients with infectious lung disease and healthy controls(P>0.05).Significant cor- relations were found between the elevated levels of serum KL-6 and the presence of ILD in patients with PM/ DM(P<0.01).The sensitivity and specificity of serum abnormal KL-6 levels for ILD in patients with PM/DM were 90.9% and 80.6%.Additionally,follow-up study showed the mean serum levels of KL-6 in six patients who died were significantly higher than that in other PM/DM patients with ILD(P<0.01).Conclusion Serum KL-6 level is a reliable serum marker for ILD in idiopathic inflammatory myopathies,which may contribute to early differentiate ILD from lung infectious disease.Increased serum level of KL-6 may predict a poor out- come.

Objective To investigate the anti-fibrogenesis property of imatinib mesylate in a rat model of liver fibrosis induced by carbon tetrachloride/olive oil and its effect on the expression of trans- forming growth factor(TGF)-?1.Methods Rat liver fibrosis was induced by intraperitoneal administra- tion of carbon tetrachloride and olive oil mixture twice a week for eight weeks.Imatinib mesylate was given 20 mg/kg daily by oral lavage.The control rats received saline by oral iavage.Liver collagen depo- sition was evaluated by immunohistochemistry with Masson staining.The activation of hepatic stellate cells was detemined by the immunohistoehemistry staining of?-smooth muscle actin.The mRNA expres- sions of TGF-?1,c-Abl and TIMP-1 were measured by RT-PCR.While protein expressions of TGF-?1, phosphorylated platelet-derived growth factor receptor and c-Abl were detected by Western blot and im- munohistochemical staining.Hepatic hydroxyproline content was also quantified.Results The collagen deposition[(16.23?1.01)%vs(25.61?0.92)%]and the number of activated HSCs(10.52?1.33vs 13.10?1.21)were reduced in the imatinib mesylate treatment group compared with the control group by 35% and 20%,respectively(P
10.MeaSurement of Relative Affinity and Specificity of Monoclonal Antibodies

To measure the relative affinity and type Specificity of neutralizing monoclonal antibodies against the hexon antigen of adenovirus type 7,a simple ELISA double antibody binding system has been developed.By this method,ralative affinity of nine MAbs was estimated from the antibody concentrations at approximately 50% of plateau binding and were ranked.The theoretical basis for this method was discussed. Hybridomas secreting antibodies of desired affinity can be selected at an early stage after fussion by measuring relative affinity of hybridomas supernatants.The type specificity was judged by the difference between end-point concentration against adeno- virus type 7 and type 3.And the relation between affinity and specificity of MAbs was discussed.
