3.Comparison of the effect of different methods of anesthesia on cerebral autoregulation in patients undergoing neurosurgery

Objective To compare the effect of different methods of anesthesia on cerebral autoregulation in patients undergoing neurosurgery.Methods Sixty-nine ASA Ⅱ orⅢ patients with brain tumor, aged 23-62 yr, scheduled for neurosurgery under general anesthesia, were randomly divided into 3 groups ( n = 23 each) : propofol-remifentanil group (group PR), sevoflurane-remifentanil group (group SR) and propofol-sevoflurane-remifentanil group (group PSR) . Anesthesia was induced with target-controlled infusion (TCI) of propofol (target plasma concentration3 μg/ml, PR and PSR groups) or inhalation of 8% sevoflurane (group SR) and iv injection of remifentanil 1 mg/kg and atracurium 0.5 mg/kg. The patients were mechanically ventilated after tracheal intubation. PETCO2 was maintained at 32-35 mm Hg. Anesthesia was maintained with TCI of propofol (target plasma concentration 2.0-3.5 μg/ml) in group PR, with inhalation of 1.5%-2.5% sevoflurane in group SR, with TCI of propofol (target plasma concentration 1.5-3.0 μg/ml) and inhalation of 1% sevoflurane in group PSR, and with TCI of remifentanil (target plasma concentration 2.0-4.5 ng/ml) and iv infusion of atracurium at 6 μg · kg-1 · min-1 in all groups. Auditory evoked potential index was maintained between 40-45. The middle cerebral artery time-average peak flow velocity was recorded before induction (baseline) , immediately after intubation, immediately before craniotomy and at the beginning of skin suture. The unilateral carotid artery was compressed for 7 s at the corresponding time points mentioned above. The transient hyperemic response ratio (THRR) was calculated to reflect cerebral autoregulation. Results Compared with the baseline value at T0, THRR was significantly increased at T2in group PR and decreased at T2,3 in group SR (P <0.05) ,while no significant change was found in THRR at T1-3in group PSR (P >0.05). The THRR was significantly lower in SR and PSR groups than in group PR, and higher in group PSR than in group SR ( P < 0.05). Conclusion Propofol-remifentanil anesthesia can improve cerebral autoregulation, sevoflurane-remifentanil anesthesia can reduce cerebral autoregulation, and propofol-sevofluraneremifentanil anesthesia exerts no effect on cerebral autoregulation in patients undergoing neurosurgery.
4.Relationship between obesity levels and adipocytokines in children

Objective To explore the relationship between obesity levels and adipocytokines like leptin,adiponectin and resistin in children.Methods One hundred and ten patients aged 2-6 years with simple obesity were selected into obesity group,which included 40 cases of slight obesity (slight obesity group),40 cases of medium obesity (medium obesity group) and 30 cases of severe obesity (severe obesity group).Forty cases of healthy children with normal bodyweight were included in control group.Their serum leptin,adiponectin and resistin levels were tested and compared.Results Serum leptin and resistin levels in obesity group were remarkably higher than those in control group (P < 0.01),whereas adiponectin level was much lower than that in control group (P< 0.01).Serum leptin and resistin levels in medium obesity group were remarkably higher than those in slight obesity group [(14.3 ±1.9) μg/L vs (7.8 ± 1.5)μg/L and (14.6±2.2) μg/Lvs (13.1 ±3.0) μg/L](P< 0.01),whereas adiponectin level was much lower than that in slight obesity group[(6.7 ± 3.0) mg/L vs (9.1 ± 3.5) mg/L](P< 0.01).Serum leptin and resistin levels in severe obesity group [(20.4 ±4.7),(18.3 ±3.2) μg/L]were remarkably higher than those in medium obesity group(P< 0.01),whereas adiponectin level [(5.1 ± 1.2) mg/L]was much lower than that in medium obesity group (P< 0.01).Conclusion The levels of adipocytokines are closely correlated with obesity level in children,especially for abdominal obesity.
5.Application effect of recombinant human epidermal growth factor derivative Eye drops in treatment of dry eye

Abstract? AlM: To investigate the application effect of recombinant human epidermal growth factor derivative eye drops ( Jinyinshu) in treatment of dry eye.?METHODS:Sixty cases ( 87 eyes ) of dry eye patients were randomly divided into control group and observation group, 42 eyes and 45 eyes respectively. The control group received Chondroitin sulfate eye drops treatment, and the observation group were used for treatment of Jinyinshu. The changes of clinical efficacy, correlation index (symptom score, BUT, S l t, FL) before and after treatment, quality of life after treatment and the incidence of adverse reactions were compared.? RESULTS: ln the control group, the total clinical effective rate was 71%, which was significantly lower than that in the observation group ( 91%) , the difference was statistically significant ( P<0. 05 ); There were statistical differences of symptom scores, BUT, S▏t and FL level of the two groups before and after treatment (P<0. 05), and there were statistical differences of the above indexes after treatment between the two groups ( P<0. 05 ); The quality of life score was 142. 16 ± 10. 32 of the observation group after treatment, which was significantly higher than that in the control group after treatment (115. 24±7. 34), the difference was statistically significant (P<0. 05); The incidence of adverse reaction of the control group was 13%, 10% in the observation group, and there were no statistical differences (P>0. 05).?CONCLUSlON:Jinyinshu is significantly effective in the treatment of dry eye, and its clinical efficacy is better than chondroitin sulfate.

9.Effect of Perforin Antibody on the Expression of Granzyme B in Viral Myocarditis

Objective To explore the effect of perforin(PFP) antibody on the expression of Granzyme B (GzmB) in viral myocarditis.Methods Forty-five 4-week-old BALB/c male mice were randomly divided into 3 groups,including normal control group (n=15),viral control group (n=15) and PFP antibody therapy group (n=15).The mice in normal control group were inoculated with 0.15 mL Eagle reagent,and the mice in viral control group and PFP antibody therapy group were moculated with 0.15 mL TCID501012 L-1 coxsackievirus B3.The mice in PFP antibody therapy group were inoculated with PFP antibody (0.1 mg/kg) on 6 h and 3 d post inoculation.The mice in 3 groups were sacrificed on day 10 post inoculation.Extracted their hearts,then fixed,dehydrated,embeded and sliced the myocardial tissues.The expressions of GzmB proteins in myocardium were determined by immunohistochemistry.The areas of GzmB were measured by the Leica Qwin V3 system.SPSS 11.5 software was used to analyze the data.Results There were 4 mouse dead in viral control group,and 2 mouse dead in PFP autibody therapy group,no mice dead in normal contol group.The expressions of GzmB were not found in myocardial tissues of normal control group,but there were lots of expressions of GzmB in myocardial tissues of viral control group[(67.13?1.82)%].Athough the expressions of GzmB in PFP antibody therapy group[(6.98?1.34)%] were significantly decreased compared with those in myocardial tissues of viral control group(q=66.180 P
