2.Assessment and Management of Non-alcoholic Fatty Liver Disease
The Singapore Family Physician 2019;45(2):24-26
Non-alcoholic fatty liver disease (NAFLD) has become the commonest chronic liver disease in the world. Overall improvement in public health, active screening of blood products, and universal vaccination of hepatitis B have led to a drop in incidence of hepatitis B and C worldwide. NAFLD is strongly associated with metabolic syndrome. With the rise in overweight status and obesity worldwide, it is not surprising that NAFLD is on the rise. Diagnosis of NAFLD requires confirmation of fatty infiltration in liver, as well as liver damage such as elevated liver enzymes and presence of fibrosis. Currently, the best treatment for NAFLD is weight loss, and the proven method would be dieting with regular exercises. Vitamin E and pioglitazoles are promising medications for treating NAFLD, but each medication has their shortcomings. Until more studies are conducted, lifestyle modification remains the only reliable way to treat NAFLD. Family physicians ought to look out for cardiovascular diseases, as well as being vigilant in cancer screening, as NAFLD is associated with higher risks of ischemic heart disease and cancer.

Objective To introduce the donor heart preservation techniques and study the clini- cal results in 26 patients undergoing orthotopic heart transplantations with long donor ischemia time (DIT) in our center. Methods From Sept.2004 to Aug.2006,26 patients underwent orthotopic heart transplantation at our center with DIT longer than 360 min.Crystalloid cardioplegia was infused through aortic root after aorta was cross clamped.UW solution was used when donor heart was removed. All patients underwent bicaval anastomotic cardiac transplantation.NO was inhaled through ventilator during and after operation in patients with pulmonary hypertension. Results DIT was 360- 560 (mean 401?43) min. There was no operative death in this group.Echocardiography and X-ray examinations in one month were normal. One case of abnormal coronary orifice was subjected to re- anastomosis of aorta. One case of renal failure post-operation was subjected to blood dialysis.Conclu- sion With proper donor heart procurement and preservation, recipient selection, anastomotic technique, efficient postoperative surveillance and pulmonary hypertension treatment, transplanted patients with long DIT can obtain the satisfactory short-term outcome.

Objective: This study aimed to observe the long-term effect of Pseudomonas aeruginosa preparation used in peritoneal injection of advanced colorectal cancer patients during surgery. Methods: A total of 83 colorectal cancer patients who received surgery between September 2006 and March 2008 were enrolled in this study. The patients were divided into two groups. Palliative resection and a 10 ml P. aeruginosa peritoneal injection were performed in 30 of 83 patients in the treatment group. Simple palliative resection was conducted in the other 53 patients, which comprised the control group. Both groups were then treated by regular chemotherapy and radiotherapy. Results:The follow-up visit was completed in 79 of 83 patients, with a high follow-up rate of 95.2%. No significant difference was found in the five-year overall survival time between the two groups (P=0.403). However, the five-year median survival time in the control group was only 13.9 ± 2.14 months, whereas that in the treatment group was 17.2 ± 2.12 months. Conclusion: Within a short period, peritoneal injection of P. aeruginosa during surgery could confer certain survival advantages for advanced colorectal cancer patients. However, the long-term effect of this therapy remains unknown.

Chinese Journal of Anesthesiology 2013;33(9):1145-1147
Objective To evaluate the effect of continuous renal replacement therapy (CRRT) on extracorporeal membrane oxygenation (ECMO)-induced expression of inflammatory factors in pig kidney. Methods Twenty-four adult pigs of either sex, weighing 25-32 kg,were randomly divided into 4 groups (n =6 each) using a? random number table:control group (group C),sham operation group (group S),ECMO group and CRRT group.Anesthesia was induced with ketamine, diazepam and atropine and maintained with ketamine and diazepam. The pigs were tracheotomized, intubated and mechanically ventilated. The left femoral arteries were cannulated for MAP monitoring.Heparin 150 U/kg was injected intravenously.Right femoral artery and left internal jugular vein were cannulated for blood-letting and fluid infusion. In ECMO and CRRT groups,ECMO was performed for 24 h starting from 1 h after cannulation,in addition CRRT was performed for 24 h simultaneously in ECMO group. The pigs were then sacrificed and kidney specimens were obtained for determination of the content of interleukin-1β (IL-1 β),IL-6 and tumor necrosis factor-α (TNF-α) by ELISA.Results There was no significant differ-ence in the content of IL-1β,IL-6 and TNF-α between C and S groups (P > 0.05).Compared with C and S groups, the content of IL-1β,IL-6 and TNF-α was significantly increased in E and CRRT groups (P < 0.01).Compared with group E,the content of IL-1β,IL-6 and TNF-α was significantly decreased in group CRRT (P <0.01).Conclusion CRRT can decrease ECMO-induced expression of inflammatory factors in pig kidney to some extent, indicating that it can alleviate the inflammatory responses in kidney.

Objective To study the change of interleakin-18 (IL-18) in patients of pregnancy induced hypertension syndrome(PIH) and its correlation with endothelint-1 (ET-1). Methods Serum levels of IL-18 and plasma levels of ET-1 in 47 PIH patients and 30 normal pregnancy women were measured with ELISA and RSA respectively. Results (1)Serum IL-18 of PIH group (M=43.76 ng/L) was significantly higher than that of normal pregnancy group (M=37.975 ng/L) , H=42.845,P

A giant basilar artery aneurysm of young woman with endocrine disturbance was misdiagnosed as a large pituitary adenoma and treated surgically via a trans-sphenoidal approach was planned. But the neurosurgery was finally aborted because of massive bleeding during the procedure. One year later, a cerebral angiography confirmed this basilar artery aneurysm was obviously regressed and then endovascular coiling was successfully performed. No neurological complication occurred post-procedure and the endocrine dysfunction symptom was obviously improved.

OBJECTIVE: To analyze recent developments in image texture research both from methodology and from medical image analysis. DATA SOURCE: With the key words of "medical image, texture research, image analysis, application", we retrieved PubMed database (http://www.ncbi.nlm.nih.gov/sites/entrez/), ScienceDirecr database (http://www.sciencedirect.com/) from 1983 to 2009 and CNKI database (http://www.cnki.net/) from 2004 to 2009. DATA SELECTION: Original research thesis, and reviews with clear opinion, sufficient data and reliable conclusion were included. Repetitive studies and studies concerning unrelated to the objective were excluded. MAIN OUTCOME MEASURES: A total 104 literatures were selected, including 10 Chinese literatures and 94 English literatures. These literatures were primarily collected by reading titles and abstracts. A total of 33 literatures with unrelated objective, 18 literatures with repetitive studies were excluded. Finally, 53 Chinese and English literatures were included for further analysis. RESULTS: Primary methods used in texture analysis are structural, statistical, model-based and transform-based-method. When we are interested in identifying texture primitives and their distribution to analyze regular texture, structural approaches are suited. Characteristics of texture like smoothness and coarseness are well analyzed by statistical approaches. Model-based-method is based on the construction of an image model that can be used not only to describe texture, but also to synthesize it. Digital features of texture are got by using some signal processing theories in transformation domain. Texture applications have been widely used in medical imaging domain. CONCLUSION: Because of the specific and complication of medical image and texture, not all texture measure can be used for medical image analysis. One of the development directions of medical image texture research is how to integrate and educe advantages of different methods to fully extract texture features and exactly attribute medical image texture and the relation between its changes and pathological state, resulting in an important component of computer-aided diagnosis.
