3.Association of Job Stress with Health-promoting Behaviors and Health Status in Clinical Nurses.

PURPOSE: This study examined association of job stress with health-promoting behaviors and objective health status in 129 clinical nurses working at a university hospital. METHODS: A cross-sectional and descriptive design was used. Job stress and health behaviors were measured with Korean Occupational Stress Scale and Heath Promoting Lifestyle Profile-II, respectively. Health status measured with afternoon plasma cortisol, C-peptide, and lipid profiles. RESULTS: The level of job stress among clinical nurses was moderate with 51.41 on average. The mean for health-promoting behavior in the low stress group was significantly higher than that in the high or moderate stress groups (p<.001). The proportions of nurses with high C-peptide and cortisol levels, or low high-density lipoprotein levels, ranged from 14.0% to 35.7%. In particular, the percentage of nurses with high C-peptide levels was significantly higher in moderate and high stress groups than in the low stress group (24.1% versus 11.6%, p<.05). CONCLUSION: The study findings affirmed the associations of job stress with health-promoting behaviors as well as selected health status indicators such as C-peptide in clinical nurses. Job stress management intervention can help clinical nurses to improve their health-promoting behaviors and health status.

It is well known that the computed tomography(CT) is very useful i the evalutaion of obstructive jaundice. Wehave studied 55 cases of obstructive jaundice with whole body scanner from Jun. 1980 to Jun. 1981. The resultswere as follows; 1. The sex distribution was 36 males and 19 females, and 40 cases of obstructive jaundice wereseen in fifth, sixth, and seventh decades. 2. Causes of obstructive jaundice were 25 cases of pancreas cancer, 8cases of common duct cancer, 4 cases of gall bladder cancer, 4 cases of ampulla vater cancer, 12 cases of commonduct stone, and 2 cases of common duct stricture. 3. Levels of obstruction were 8 cases of hepatic portion, 15cases of suprapancreatic portion, 28 cases of pancreatic portion, and 4 cases of ampullary portion. 4. In tumorouscondition, CT demonstrated metastasis of other organs, 9 cases of the liver, 1 case of the lung, 3 cases of thepancreas, 3 cases of the common bile duct, 1 case of the stomach,and 12 cases of adjacent lymph nodes. 5.Associated diseases were 12 cases of intrahepatic stone. 4 cases of clonorchiasis, 2 cases of pancreas pseudocyst,1 cases of hydronephrosis, and 1 case of renal cyst.

8.Effect of a Physical Activity Promoting Program Based on the IMB Model on Obese-Metabolic Health Outcomes among Obese Older Adults with Knee Osteoarthritis

Purpose:
This study examined the effects of a physical activity promoting program based on the Information-Motivation-Behavioral Skills (IMB) model on physical activity and health outcomes among obese older adults with knee osteoarthritis.
Methods:
This study utilized a randomized controlled trial with a convenience sample of 75 obese older adults with knee osteoarthritis in a university hospital. The older adults in the intervention group participated in a 12-week program involving weekly group sessions and monitoring calls with education booklets and video clips for exercise dances, while those in the control group received an usual care. Outcomes were measured using self-report questionnaires, anthropometrics, and blood analyses. The intervention effects were analyzed using Mann-Whitney U test and ANCOVA.
Results:
The mean age of participants was 74.9 years with 84.0% women. The intervention group at 12 weeks showed significantly greater improvements in self-efficacy for physical activity (F=81.92, p <.001), physical activity amounts (Z=-2.21, p =.044), knee joint function (F=15.88, p <.001), and health-related quality of life (F=14.89, p <.001) compared to the control group. Among obese-metabolic outcomes, the intervention group at 12 weeks showed a significant decrease in visceral fat mass (F=7.57, p =.008) and improvement in high-density level cholesterol (F=9.51, p =.003) compared to the control group.
Conclusion
Study findings support the need for an IMB based physical activity program for promoting physical activity, knee function and health outcomes in obese older adults with knee osteoarthritis. Longitudinal studies are warranted to confirm the persistence of obese-metabolic effects in clinical settings.
9.Extraskeletal Ewing`s Sarcoma on the Duodenum.

Extraskeletal Ewing's sarcoma is a rare primary malignant soft tissue tumor that has the same pathological characteristics of skeletal Ewing's sarcoma. It is known that the differential diagnosis of Ewing's sarcoma from undifferentiated rhabdomyosarcoma, neuroblastoma, reticulum cell sarcoma, and malignant lymphoma by the histologic studies only are not easy because their histologic findings are nearly similar each other. Therefore, immunohistochemical method, electron microscopic examination, cytogenetic study, or molecular gene study are used together to confirm the diagnosis. It chiefly involves the lower extremity, soft tissues of the paravertebral region, and adjacent chest wall. We recently experienced a case of extraskeletal Ewing's sarcoma of duodenum in a 21-year old female. The patient underwent Whipple's operation and postoperative combined chemotherapy were done.

10.The Prevalence of Proteinuria and Hematuria at a Tertiary Referral Hospital.

Urinalysis is one of the basic laboratory tests in patient care and routine examinations. But few studies have been carried out about the prevalence of proteinuria and hematuria in Korea, especially in tertiary referral hospital, and the meanings of them are not clear until now. So we performed a cross- sectional study to find out the prevalence of proteinuria and hematuria at a tertiary referral hospital, Seoul National University Hospital. From Jan. 1. 1995 to Dec. 31. 1995, there were 35,361 adults (17,123 males and 18,238 females) in whom urinalyses were carried out. The age of them ranged from 15 to 96 years and the average was 49 years with the distribution of 2.3% in 15-19 years, 10.6% in 20-29 years, 16.0% in 30-39 years, 19.6% in 40-49 years, 24.3% in 50-59 years, 19.2% in 60-69 years, and 8.0% in 70 years or older. The degree of proteinuria was classified to '-', '+/-', '+', '++', '+++' by dipstick method. In males, 80.0% were '-', 6.3% '+/-, 6.8% '+', 3.7% '++', 3.2% '+++', and 5.0% '+/-', 5.1% '+', 2.8% '++', 2.1% '+++' in females. Proteinuria of degree '+/-' or higher was defined as significant and found in 19.9% of males and 14.9% of females. According to the age groups, 17.0% in 15 to 19 years, 17.3% in 20 to 29 years, 16.3% in 30 to 39 years, 15.2% in 40 to 49 years, 16.8% in 50 to 59 years, 18.6% in 60 to 69 years, 23.6% in 70 years or older had significant proteinuria. Hematuria was classified by the number of erythrocytes per 400X high-power field, to grade 1 (<1), 2 (1-4), 3 (5-9), 4 (10-19), 5 (>30), 6 (many). In males, 62.9% were grade 1, 23.5% grade 2, 5.3% grade 3, 3.9% grade 4, 1.9% grade 5, 2.6% grade 6, and 47.4% grade 1, 32.6% grade 2, 8.8% grade 3, 5.3% grade 4, 2.4% grade 5, 3.4% grade 6 in females. Hematuria of grade 3 or higher was defined as significant and found in 13.6% of males and 20.0% of females. According to the age groups, 11.1% in 15 to 19 years, 13.7% in 20 to 29 years, 16.8% in 30 to 39 years, 17.4% in 40 to 49 years, 16.7% in 50 to 59 years, 18.0% in 60 to 69 years, 19.9% in 70 years or older had significant hematuria.
