1.A Case of Ureteral Intussusception with Giant Ureteral Polyp.

A case of intussusception of a ureter due to a ureteral polyp was observed. Ureteral intussusception have been exceedingly rare, and very few reference to this condition can be found in the medical literature. The author have report a 39 years old female of a case of intussusception of the ureter associated with a benign polyp. The author have performed satisfactory conservative treatment. The patient was entirely recovery and has remained in excellent health since.

2.Clinical Observation On Urinary Vaginal Fistula.

A vesicovaginal fistula is a distressing complication which may follow hysterectomy and other extensive pelvic operation. Maligntncies of the uterin cervix or bladder may predispose to erosion and vesicovaginal fistula or such fistulas may occur as a result of extensive radiation therapy. Childbirth, particularly involving prolonged labor with potential necrosis of the vesico vaginal septum or complicated delivery with trauma may also induce a vesicovaginal fistula. In this experience the most common cause of vesicovaginal fistula resulting from surgery was total abdominal hysterectomy. Diagnosis of vesicovaginal fistula is usually made when appearance of methylene blue in the vagina after instillation of the dye into the bladder and further confirmed the presence and definitive localization of the fistula with cystoscopic examination. The operative procedures, the authors utilized were suprapubic transvesical closure in 8 cases, vaginal closure in two cases, and transabdominal approach in one case. The bladder was opened at the dome and then stay sutures were made at 1.0cm from the fistula margin. Then, the fistulous tract was excised making a lateral margin wide enough to leave viable tissue for subsequent closure. Vaginal wall and bladder wall closure were carried out in whole layer using interrupted 2 zero chromic suture. In one case of large fistula with high opening, the patient was underwent transabdominal method interposing a peritoneal flap between vagina and bladder wall. A urethral catheter was placed for 10 to 12 days postoperatively. In 5 cases of ureterovaginal fistula, one patient required the nephrectomy because of pyonephrosis, 2 patients refused further medication, one had ureteral reimplantation and another patient had psoas hitch and Boari operation. The following conclusions were obtained. 1. The most common cause of vesicovaginal fistula was the complication of total hysterectomy. 2. The diameter of fistulous tract was ranging from 1 to 10 mm. 3. For the repair of vesicovaginal fistula transvesical approach were 8 out of in 13 case, vaginal route in 2 cases, intraabdominal approach using a peritonal flap in one case. 4. Urethral catheter had been placed postoperatively was removed on the 12th postoperative day in most cases. Ureteral splint catheter was also placed for 8 to 10 days postoperatively. 5. 12 out of 14 cases, urinary vaginal fistulas were successfully closed on the first surgical attempt and one case healed on the second attempt. One case failed because of infection at the area of closure.

3.Evaluation of 51Cr labelled In-vivo Crossmatching Test.

For the evaluation of the 51Cr labelled in-vivo crossmatching test as a transfusion safety test, We examined the viability of the thransfused autologous and heterologous 51Cr labelled red cell at 1 hour and 24 hour in 6 normal volunteers and 4 patients whose RBC had been stored in CPDA-1 media from 1 day to 30 day. In both autologous transfusion control group(n=3) and allogeneic transfusion control group(n=3), the in-vivo RBC survival rate were more than 70% at 1 hour and 24 hour compared to that of basal 3 minute result(percent counted as 100%), and the eluted free radioisotope activity was less than 5% compared to that of whole blood. The crossmatch incompatible patient group also satisfied the safe transfusion criteria. The mean labelling efficiency which was tested in three patients was more than 95% and also satisfactory to reliable test. We concluded that in-vivo crossmatching test using 51Cr radioisotope would give us valuable informations about transfusion safety, especially in serologically incompatible patients.

6.A Study on the chemotherapy in clonorchiasis. Report 1. An experimental study on chemotherapy with Dithiazanine iodide and Bithionol sulfoxide in clonorchiasis.

Authors carried out experimental study on chemotherapy with oral administration of Dithiazanine iodide (D.I.) and Bithionol sulfoxide(B.S.) in rabbit clonorchiasis. And the following result was obtained. In change of the E.P.G (eggs per gram feces) by D.I. administration, it was rather increased in early stage of the administration than prior to administration, and thereafter decreased gradually. In the change of the C.S. worm body by D.I. administration, there was not only prominent change of supporting tissue but also the change of reproductive organ was found. In considering the wormicidal effect of D.I. from detecting rate of survival worms, the effect was slight in group of 100 mg(80 mg/kg) per day dosage, but the effects were very excellent and almost complete by proper times of administration in groups of 200 mg (130 mg/kg) per day or higher dosage. But the side effect and intoxication sign of D.I. were appearent in groups of 200 mg or higher dosage. By B.S. administration, E.P.G. was decreased gradually. In changes of the C.S. worm body by B.S. administration, prominent inhibitory chnnge was seen in egg formation ability. Slight wormicidal effect of B.S. was observed in groups of 140 mg (100 mg/kg) per day or higher dosage. Side effect and intoxication sign of B.S. were found little in groups of 140 mg or lesser dosage.

7.A Case of Priapism.

Priapism is a condition in which persistent erection occurs unaccompanied by sexual excitation or desire. A case of priapism who underwent corpus-saphenous shunt procedure is in here described. Although attempts to relieve the erection were tried by ice water enema, evacuation of the sludged blood by needle aspiration and heparin injection, all conservative therapy failed. Prompt subsidence of erection was followed by corpus-saphenous shunt procedure.

8.Two Cases of Megaloureter.

Two megaloureter wee by extensive resection of the ureter to shorten it's length and reduce it's caliber. A nephrostomy tube was inserted for temporary drainage of the kidney in one case. In both cases an operation on tire lower ureter sufficed. Results were satisfactory, with restoration of ureteral function and improvement in kidney drainage.

10.Chronic Subdural Hematoma Due to Cumulative Trauma to the Head.

Chronic subdural hematoma usually occurs as a consequence of minor trauma. But, chronic subdural hematoma of occupational origin has not been reported yet in Korea. We experienced a case of chronic subdural hematoma induced by repeated trauma to the head. The patient was a 45-year-old male. He complained of repeated headache and nausea. By the computed tomogram, he diagnosed as chronic subdural hematoma and took the operation. He was not an alcoholism and had no intracranial disease nor coagulopathy. We studied all possibilities through working environment and personal environment survey. As a result, we decided the case as an example of cumulative trauma disorder to the head by occupational origin. We report a case of chronic subdural hematoma in the worker exposed repeated head trauma of occupational origin.
