1.Computed tomographic evaluation on ossification of posterior longitudinal ligament and ligamentum flavum ofspine

Ossification and calcification of the spinal ligaments frequently cause pressure upon the spinal cord andnerve roots. Authors reviewed 150 cases of C-spine CT, 80 cases of T-spine CT, and 725 cases of L-spine CT whichwere carried out a Pusan Natina University Hospital for 2 years from May 1983 to May 1985. We analysed 34 caseswhich showed ossification of posterior longitudinal ligament (OPLL) and ligamentum flavum (OLF) of these 955cases. The results are follows: 1. The male ot female ratio of spinal ligamentous ossification was 26:8. The 5thdecade group (24.41%) was the most prevalent age group. 4th decade(26.4%), 6th decade(23.52%), 7th decade(11.76),3rd decade (8.8%), and 8th decade(2.94%) in that order. 2. Of 955 cases of spine CT, the incidence of OPLL was 25cases(21.51%) and that of OLF was 10 ases(1.05%). 3. Regional incidence of spinal ligamentous ossification was asfollows. 1) In case of OPLL, cervical area was 19/150(12.67%) and lumbar area was 6/725 (0.83%). 2) In cases ofOLF, Thoracic area was 3/80(3.75%) and lumbar area was 8/725(1.10%). 4. The most frequent length of OPLL was 4body length (32%) and the most frequent locations are C4 and C5(68%). The types fo OPLL were 15 cases(60%) ofsegmental type, 8 cases(32%) of continuous type, and 2 cases(8%) of mixed type in that order. All segmental typeswere degree 1 or 2 and most continuous and mixed type (80%) were degree 2 or 3 dural sac effect. 5. The number ofinvolved interlaminar spaces in OLF was 1 to 5 interspaces and most of OLF were found at low thoracic and lumbararea. 6. There could be noted high correlation between the spinal ligamentous ossification and degenerative discdisease, The incidence of associated disc disease was 18/25(72%) in OPLL and 8/10(80%) in OLF.

2.MRI findings of cryptococcal infection of CNS: The long term follow-up: case report.

Authors presented the serial changes of brain MRI findings in two cases of cryptococcal meningitis. The MRI findings of the first patient (53-year-old female) consisted of dilated Virchow-Robin spaces, leptomeningeal enhancement, cryptococcomas and hydrocephalus. Dilated Virchow-Robin spaces were noted on the first MR obtained 33 days after symptom onset. The size and number of dilated Virchow-Robin space gradually decreased since 119 days after symptom onset (62 days after treatment). Faint leptomeningeal enhancement was found at cerebellar region on the first MRI, which became more distinct and intense on the day 119, probably due to improper treatment. The leptomeningeal enhancement decreased on the day 156. Mild hydrocephalus was noted on MRI obtained 70 days after symptom onset, which markedly aggravated on the day 119. It decreased after steroid therapy alone. Enhancing crytococcomas of variable size were noted at the right temporal lobe and cerebellum on MRI of the day 119, which decreased in size and number on the day 156. On the day 295 MRI showed nearly disppearance of the dilated Virchow-Robin space, but faint leptomeningeal enhancement, cerebellar crytococcomas and hydrocephalus still remained. The MRI findings of the second patient (36-year-old female) showed the findings similar to those of the first patient. Initial MRI obtained 18 days after symptom onset showed no abnormal findings. Dilated Virchow-robin spaces were noted on the day 36 (13 days after treatment onset), which nearly disappeared on the day 109. Enhancing cryptococcomas in both basal ganglia and cerebral cortex and leptomeningeal enhancement were noted on MRI of the day 136. Both cryptococcomas and leptomeningeal enhancement decreased in size and enhancing degree on the day 157. Hydrocephalus was noted on the day 109.

4.Clinical Usefulness of T2 Relaxometry in Temporal Lobe Epilepsy.

BACKGROUND: Quantitative measurement of hippocampal T2 relaxation time is an objective means of determining the frequency and severity of signal abnormalities. To evaluate the diagnostic properties of T2 relaxometry in temporal lobe epilepsy(TLE), we measured T2 relaxation time of bilateral hippocampi in pathology-proven TLE patients and normal controls. METHODS: We investigated 10 TLE patients who had temporal lobectomy with MR T2 relaxation mapping. All patients underwent in phase I or II studies, and had pathologic diagnosis. Also we measured T2 relaxation time in 10 normal volunteers. RESULTS: The pathologic findings of 10 TLE patients were followings: 8 hippocampal sclerosis (including dual pathology of necrotic granuloma), 1 calcified fibrous nodule, and 1 normal hippocampus. The mean T2 relaxation time of normal controls is 67.5msec, which is lower value than previous reports. All patients with hippocampal sclerosis in pathology showed increased T2 time greater than 2 SD of mean value of normal controls. But, the T2 values are upper normal range in non-hippocampal sclerosis. The lateralizing value of T2 relaxometry is 50% in TLE patients, and 62.5% in pathology-proven hippocampal sclerosis groups. CONCLUSIONS: There is a clear distinction of T2 relaxation time between the patients of hippocampal sclerosis and normal controls or non-hippocampal sclerosis. These findings suggest that the T2 relaxation time is a reliable objective measurement of hippocampal pathology, especially hippocampal sclerosis in TLE.

Carbamazepine (CBZ) may alter the secretion of prolactin (PRL), however, there is no report about hyperprolactinemia with galactorrhea induced by CBZ medication. A 30-year-old woman presented with galactorrhea. She had been treated with CBZ for three years and hormonal study revealed increased serum PRL levels. Sella MRI showed no evidence of pituitary lesion. Two months after changing CBZ to oxcarbazepine, galatorrhea was improved and the serum PRL level was normalized.

6.The Post-operative Outcomes of Spinal Epidural and Subdural Hematoma Patients Without Spinal Fracture.

OBJECTIVE: Spinal epidural hematoma (SEDH) and spinal subdural hematoma (SSDH) are rare diseases and they may have various causes such as trauma, lumbar puncture, anticoagulant therapy, tumor, blood dyscrasia and vascular malformation. In general, SEDH and SSDH are regarded as surgical emergency. We reviewed our cases with patients with SEDH or SSDH. They were surgically treated or conservatively treated. We examined the relationship between the surgical timing and the neurological outcome. METHODS: Twelve patients (8 cases for SEDH, 4 cases for SSDH) were included in our study. There were seven male and five female patients (mean age: 50.2 yrs, ranged from 18 to 87 years). Ten patients were surgically treated (7 cases for SEDH, 3 cases for SSDH) and two patients were conservatively treated (1 case for SEDH, 1 case for SSDH). We checked preoperative Frankel grade, time interval between onset of symptom and operation and post-operative neurologic change of each case. We investigated relationship between surgical timing and neurological outcome and also relationship between pre-operative Frankel grade and post-operative outcome. RESULTS: In seven cases (70%) of operated cases, there were postoperative improvements in clinical symptoms. Two cases had time interval within 6 hours from onset of symptoms to operations and their neurologic deficits were not improved. There were 2 cases with time interval of more than 6 hours and within 12 hours. And there were 6 cases with time interval over 12 hours. Seven of eight cases with time interval over 6 hours were improved after surgical treatments. CONCLUSION: There are various factors which may affect post-operative neurologic prognosis. Surgical timing is generally regarded as one of the most critical factors. However, in our study, preoperative neurologic status of patients functioned as the most important factor in clinical outcomes. We thought that the initial neurologic status of patient, as well as the surgical timing, may predict the prognosis.

7.Traditional Korean diet can alter the urine organic acid profile, which may reflect the metabolic influence of the diet

Purpose:
To determine the metabolic influence of the traditional Korean diet (K-diet), which has been regarded as a healthy diet, we investigated the profile of urine organic acids that are intermediates of various types of metabolism including energy metabolism.
Methods:
Ten women aged 50–60 years were recruited and randomly divided into 2 diet groups, K-diet and control diet, the latter of which is a Westernized Korean diet that is commonly consumed by Koreans nowadays. Before and after the 2-week intervention, 46 urine organic acids were determined using LC/MS/MS, along with clinical parameters.
Results:
The average concentrations of succinate (4.14 ± 0.84 μg/mg creatinine vs. 1.49 ± 0.11, p = 0.0346) and hydroxymethylglutarate (3.67 ± 0.36 μg/mg creatinine vs. 2.97 ± 0.29, p = 0.0466), both of which are intermediates of energy metabolism, decreased in the K-diet group after the 2-week intervention, but these were not observed in the control diet group. In particular, the average concentration of succinate in the K-diet group was lower than that in the control group (3.33 ± 0.56 μg/mg creatinine vs. 1.49 ± 0.11, p = 0.0284) after 2 weeks. The concentrations of two tryptophan metabolites, 5-hydroxyindolacetate (3.72 ± 0.22 μg/mg creatinine vs. 3.14 ± 0.21, p = 0.0183) and indican (76.99 ± 8.35 μg/mg creatinine vs. 37.89 ± 10.06, p = 0.0205) also decreased only in the K-diet group. After the 2-week intervention, the concentration of kynurenate, another tryptophan metabolite, was lower in the K-diet group than that in the control diet group (3.96 ± 0.51 μg/mg creatinine vs. 2.90 ± 0.22, p = 0.0356). Interestingly, the urine level of kynurenate was positively correlated with BMI (r = 0.61424, p = 0.0003) and total cholesterol (r = 0.46979, p = 0.0088), which decreased only in the K-diet group (239.40 ± 15.14 mg/dL vs. 198.20 ± 13.25, p = 0.0163).
Conclusion
The K-diet alters the urinary excretion of organic acids involved in energy metabolism and tryptophan metabolism, suggesting the influence of the K-diet on these types of metabolism. Urine organic acids changed by the K-diet may serve as biomarkers in future studies.
8.Human Urine-derived Stem Cells Seeded Surface Modified Composite Scaffold Grafts for Bladder Reconstruction in a Rat Model.

We conducted this study to investigate the synergistic effect of human urine-derived stem cells (USCs) and surface modified composite scaffold for bladder reconstruction in a rat model. The composite scaffold (Polycaprolactone/Pluronic F127/3 wt% bladder submucosa matrix) was fabricated using an immersion precipitation method, and heparin was immobilized on the surface via covalent conjugation. Basic fibroblast growth factor (bFGF) was loaded onto the heparin-immobilized scaffold by a simple dipping method. In maximal bladder capacity and compliance analysis at 8 weeks post operation, the USCs-scaffold(heparin-bFGF) group showed significant functional improvement (2.34 ± 0.25 mL and 55.09 ± 11.81 microL/cm H2O) compared to the other groups (2.60 ± 0.23 mL and 56.14 ± 9.00 microL/cm H2O for the control group, 1.46 ± 0.18 mL and 34.27 ± 4.42 microL/cm H2O for the partial cystectomy group, 1.76 ± 0.22 mL and 35.62 ± 6.69 microL/cm H2O for the scaffold group, and 1.92 ± 0.29 mL and 40.74 ± 7.88 microL/cm H2O for the scaffold(heparin-bFGF) group, respectively). In histological and immunohistochemical analysis, the USC-scaffold(heparin-bFGF) group showed pronounced, well-differentiated, and organized smooth muscle bundle formation, a multi-layered and pan-cytokeratin-positive urothelium, and high condensation of submucosal area. The USCs seeded scaffold(heparin-bFGF) exhibits significantly increased bladder capacity, compliance, regeneration of smooth muscle tissue, multi-layered urothelium, and condensed submucosa layers at the in vivo study.
